'Bones of contention': a donor register for temporal bone donation?

INTRODUCTION: Temporal bone dissection is essential training for otological surgery. Organ retention scandals have made it difficult to obtain consent for cadaveric temporal bone removal. The current literature does not address the means of acquiring a steady supply of cadaveric temporal bones for medical education and training. The US national temporal bone registry has established a temporal bone donor bank for medical education and research. Could a similar programme in the UK be an answer to the paucity of temporal bones? OBJECTIVES: (1) To ascertain the degree of interest amongst our reg

Primary stability of simultaneously placed dental implants in extraoral donor graft sites: a human cadaver study.

PURPOSE: To compare the primary stability of dental implants placed in fibula, iliac crest, and scapula of human cadavers. MATERIALS AND METHODS: Straumann Dental Implants (Institut Straumann, Basel, Switzerland) 4.1 mmx10 mm in diameter were placed into bilateral fibula, iliac crest, and scapula of 4 fresh human cadavers. For the assessment of primary stability of implants, installation torque values (ITV) and removal torque values (RTV) were measured using a custom-made strain-gauged torque wrench, and resonance frequency analysis was carried out to quantify the implant stability quotients

Lingual vascular canals of the mandible: the risk of bleeding complications during implant procedures.

PURPOSE: The aim of this article is to investigate the potential risk of a critical life-threatening hemorrhage due to the perforation of the lingual cortical plate and arterial trauma of the terminal branches of the sublingual artery. In fact, in many cases, implants are placed in the mandibular interforaminal region, and this area is also often involved in oral surgery as a bone donor site. MATERIALS AND METHODS: One hundred dry skull Caucasian mandibles and 100 computed tomographies (CTs) of the mandible of Caucasian patients were examined. The entrances of the lingual vascular canals were

Donor exclusion in the National Blood Service Tissue Services living bone donor programme.

National Blood Service (NBS) Tissue Services (TS) operates living donor and deceased donor tissue banking programmes. The living bone donor programme operates in collaboration with 91 orthopaedic departments across the country and collects bone donations, in the form of surgically removed femoral heads (FHs), from over 5,000 patients per annum undergoing total hip replacement. Bone donated via the living programme constitutes approximately 55% of the total bone donated to NBS. Non-NBS tissue banks, primarily in hospital orthopaedic departments, also bank donated bone for the UK. A survey of in

Reverse achilles tendon allograft technique for anterior cruciate ligament reconstruction.

Because of the increasing popularity of allograft anterior cruciate ligament (ACL) reconstructions, the technical difficulties with posterior bone loss in revision ACL surgery, and the limited supply of bone-patellar tendon-bone donor grafts, we have developed a technique using a reversed Achilles tendon allograft to reconstruct the ACL-deficient knee. This technique allows for bony tibial fixation with an interference screw by rotating the graft 180 degrees and optimizing soft-tissue fixation at the femur with an EndoButton CL (Smith & Nephew, Andover, MA). Short-term follow-up (average, 26.4

Use of calcium phosphate cement for bone defect after harvesting radial forearm osteocutaneous flap.

To resolve esthetic and psychologic discomfort of patient due to the lack of radius after harvesting radial forearm osteocutaneous flap, we filled the bone defect and recreated its contour using calcium phosphate cement (CPC; Biopex; Mitsubishi Material Corporation, Tokyo, Japan). Injection of CPC was performed in 5 patients (3 women and 2 men). The patients' age ranged from 61 to 72 years (mean, 65 years), and the follow-up period ranged from 8 to 18 months. The size of the radius harvested varied from 10 x 35 mm to 10 x 50 mm. The width of the harvested bone was approximately 25% to 35% of t

Circumferential fusion of the lumbar and lumbosacral spine using a carbon fiber ALIF cage implant versus autogenous bone graft: a comparative study.

This study reports on 92 patients treated with circumferential lumbar fusions for degenerative conditions. The elected procedure was a circumferential fusion using transpedicular stabilization. Two groups were examined depending on whether they were stabilized anteriorly with autogenous bicortical iliac crest graft (n = 38) or with an ALIF carbon fiber cage implant (n = 54). The patients were evaluated for clinical and radiographic outcome. The minimum follow-up interval was at least 12 months. Fusion rates, postoperative loss of correction, and clinical results, including pain at the bone don

[The "candlestick" technique for the correction of certain types of congenital metacarpal synotosis].

Metacarpal synostosis is a rare congenital hand malformation requiring only occasionally a surgical correction. However in case of divergent epiphyses there is a progressive accentuation of the deformity. In the "Y" type of symmetrical synostosis, the authors propose a trapezoidal osteotomy with upside down relocation allowing realignement of the epiphyses without distant bone donor site.

[Vascularized bone grafts in the treatment of hemifacial microsomia].

A systematic and theoretic review is performed on the vascularised bone donor sites that may be useful in the case of hemifacial microsomia. A review of the literature reveals that the iliac crest and the scapula are the most commonly used vascularised bone grafts. A case study is included in the paper, showing an hemifacial microsomia reconstruction using a composite fibular vascularised bone graft. They highlight the criterias they think fundamental for choosing the vascularised bone graft donor site regarding the aims of the reconstruction of a Pruzansky grade II or III.

Resorbable fixation in facial plastic and head and neck reconstructive surgery: an initial report on polylactic acid implants.

OBJECTIVES: The purpose of this study was to evaluate and report our initial experience with a resorbable fixation system in facial cosmetic and head and neck reconstructive surgery. The specific goals were to determine in which settings the absorbable system could be used, to evaluate the outcome of its use, to detail complications that occurred, and to report our observations on advantages and disadvantages of the system compared with traditional methods of osteosynthesis and fixation. STUDY DESIGN: A retrospective review of the initial 30 consecutive patients at the University of Californi

Comparisons of bone volumes and densities relating to osseointegrated implants in microvascularly reconstructed mandibles: a study of cadaveric radius and fibula bones.

The study was designed to compare the dimensions and densities of two frequently used bone donor sites with regard to placement of endosseous dental implants in microvascularly reconstructed mandibles. A total of 40 radii and of 40 fibulae were investigated. Fifty two percent of the fibulae had adequate bone volume for the positioning of four 10 mm implants, while this figure was 55% for the radii. After using the 'double barrel' technique the placement of four 10 mm implants succeeded in 87% of all the fibulae. Due to the lack of bone length required, this special technique was not possible i

Advantages of percutaneous hollow needle technique for iliac bone harvest in alveolar cleft grafting.

OBJECTIVE: To evaluate the percutaneous hollow needle technique for bone harvest to determine if morbidity from the bone donor site can be reduced significantly. METHODS: A retrospective chart review was performed evaluating all patients undergoing alveolar bone grafting at our institution from January 1992 through December 1996. Patients who underwent additional major procedures were excluded. Group I consisted of 12 patients in whom the percutaneous technique was utilized. The patients had an average age of 11.1 years (range: 8 to 15 years). Six were male and six were female. One had a bila

Bone donor site. Iliac crest or distal radius?

In a series of 18 patients requiring bone-grafting procedures to the wrist, bone structure and bone turnover characteristics were compared between samples of bone harvested from the iliac crest and the distal radius. Histologically and biologically the iliac crest bone was found to be superior to distal radial bone. Axial bone was also shown to be more resistant to the ravages of increasing age and chronic disease. However, in spite of these laboratory findings, clinically, radial bone is equally successful as graft material. This, in all probability, is due to the greater porosity of radial b

Benefit of dynamic over static incubation in the detection of a low-level HBsAg (chronic carrier) bone donor.

BACKGROUND: Hepatitis B infection can be transmitted by organ or tissue transplantation. CASE REPORT: A 40-year-old man received a bone graft from a donor who tested negative for HbsAg with a static-mode assay. The same donor, on a subsequent donation, was found to be HbsAg-positive with a dynamic mode assay. The archival sample from the initial donation also tested HbsAg-positive with the dynamic-mode assay. The recipient of the bone, who had received HBV vaccine in the past, did not develop clinical or laboratory evidence of HBV infection. CONCLUSIONS: This case highlights the importance o

Comparative densitometric study of iliac crest and scapula bone in relation to osseous integrated dental implants in microvascular mandibular reconstruction.

The study was designed to compare dimension and density of four frequently used bone donor regions (the iliac crest, the lateral border of the scapula, the radius and the fibula) with regard to placement of endosseous dental implants in microvascular mandibular reconstruction. The first part of the study reports on the results of bone dimension and density findings for iliac crest and scapula bone. Nearly all of the iliac crests had adequate dimensions for the positioning of four 10 mm implants. In 63% of the scapulae, it was possible to place four 10 mm implants. In the case of the scapula, h

The anthropometry of contemporary commercial cremation.

This study establishes baseline parameters and examines those variables thought to influence cremains weights. Data were collected during the cremation of 100 individuals. A series of measurements was taken to determine relative skeletal robusticity. The weight, stature, sex, and age of each cadaver was recorded prior to cremation. The average cremains weight for the fully developed adults (n = 91) was 2430 g and ranged from 876 g to 3784 g. Male and female means were separated by 1053 g, but there was considerable overlap in the distribution. All cremains weights above 2750 g were male and al

Age-dependent expression of osteoblastic phenotypic markers in normal human osteoblasts cultured long-term in the presence of dexamethasone.

We have previously shown that osteoblasts derived from trabecular bone explants and cultured long term in 10 nM dexamethasone ((HOB + DEX) cells) exhibited properties consistent with a more differentiated phenotype compared with those grown in the absence of dexamethasone ((HOB-DEX) cells). To characterize these two cell models further, we measured the steady-state mRNA levels of the phenotypic markers alkaline phosphatase (ALP), collagen type I (COLL) and osteocalcin (OC), OC production, and the activities of ALP and parathyroid hormone (PTH)-stimulated adenylate cyclase. These findings were

[Bone harvesting from the oral cavity].

Autogenous bone grafts have been used successfully in combination with osseointegration and have shown several advantages as compared with other regenerative techniques in the restoration of bone volume for implant placement. Minor bone defects of periodontal, traumatic, and endodontic origins can easily be corrected by harvesting the bone intraorally. Also, sinus grafting procedures can be performed using inlay bone blocks from the mandibular symphysis. Mandibular and maxillary bone can be used as a whole block or can be ground into particulate size and grafted. The harvesting techniques shou

Use of vascularized corticoperiosteal flap from the femur for reconstruction of the orbit.

The reconstruction of small and thin defects in the orbital area using vascularized cortex is described herein. This vascularized cortex is taken from the medial supracondyle area of the femur because a small, thin, and flexible bone can be harvested. We have treated 5 patients with orbital bony defects that would have been difficult to reconstruct using conventional free bone grafts. All flaps took perfectly, the results were satisfactory, and no functional loss occurred in the donor site. The advantages of this bone flap include rich vascularity, a long vascular pedicle of about 7 cm, easy f

Nonvascularized autogenous bone grafts for craniofacial skeletal augmentation and replacement.

Autogenous nonvascularized bone grafts play an important role in the reconstruction of complex craniomaxillofacial defects. Experimental animal data have demonstrated that grafts from membranous bone donor sites tend to undergo less resorption than grafts from endochondral donor sites, probably because of the different bony architecture of each of these types of grafts. Of all the potential donor sites, the harvest of bone graft from the calvarium is associated with the least overall morbidity. Surgeons should be aware of the biologic basis for the successful application of free autogenous bon
