Popularized by Tessier in 1982, the calvarial bone graft has been used extensively in reconstructive techniques in the cranio-facial region. The proximal portion of the femur was evaluated as a source of autogenous cancellous bone in dogs. Bilateral oval cortical defects were created in the lateral subtrochanteric area of the femur in 16 dogs. Cancellous bone was removed and the weight recorded. Cancellous bone was similarly harvested from the proximal portion of the humerus in 7 of these dogs. The capacity of fresh murine allogeneic bone to induce a specific immune response in vitro was studied. T-cells stimulated by allogeneic bone in vitro were collected and were characterized for state of activation, cell-surface phenotype, and antigen specificity. The stimulating antigens were determined by genetic mapping with use of recombinant inbred strains of mice and by blocking of mixed lymphocyte cultures with use of neutralizing antibodies. Purified T-cells were cultured alone or with allogeneic or syngeneic bone. In some experiments, the bone marrow was removed before in vitro culture.

Patients requiring cancellous bone grafting of an extensive deficit or multiple grafting procedures often lack a sufficient quantity of autogenous cancellous donor bone. Most health professionals are aware of the criteria that need to be met to secure organ donation. Bone and soft tissue procurement is another issue, however. Even though allografts have been used since the 1880s, new techniques and an increase in the possible uses for allografts have recently increased the demand for bone and soft tissue. The criteria for choosing a bone donor is similar to organ donors with one major exception. A bone and soft tissue donor does not need the special circumstances of brain death.

In conventional reconstruction of the facial skeleton, bone grafts are usually harvested from distant sites such as the ilium or ribs. Because of the morbidity associated with the use of these sites, the calvarium was studied as an alternate donor site. Twenty-three patients underwent bone grafting using autogenous calvarial bone.
