PURPOSE OF REVIEW: This manuscript highlights key areas of recent pediatric nephrology research that may be relevant to clinical practice.
RECENT FINDINGS: Recent reviews of registry data have emphasized the long-term harms of even limited dialysis exposure in children, while studies of 'nonideal' donor kidneys in children have shown positive outcomes. New observational research suggests a benefit to thromboprophylaxis and aggressive fluid management immediately posttransplant, though the best prescriptions remain unclear. Belatacept and letermovir are emerging options for immunosuppression a

RATIONALE & OBJECTIVE: Nearly 20% of deceased donor kidneys in the United States are placed "out-of-sequence" (ie, outside of standard allocation rules). The rationale for out-of-sequence placements is to expedite placement of kidneys at risk of nonuse. We aimed to (1) develop machine learning (ML) models to predict the risk of kidney nonuse over time during the allocation process and (2) use the ML predictions to assess current out-of-sequence placements.
STUDY DESIGN: Retrospective cohort study using Organ Procurement and Transplantation Network data.
SETTING & PARTICIPANTS: Deceased donor

The nonuse rate of deceased donor kidneys has increased significantly. The National Organ Transplantation Act requires sequential organ allocation to candidates, which is inefficient for medically complex organs because it increases cold ischemic time and contributes to organ decline. A novel Sliding Scale AdaptiVe Expedited rescue algorithm for deceased donor kidney transplantation was developed that uses the nonuse rate to determine a rescue trigger threshold. Once triggered, allocation occurs in sequential batches, sized proportionately to the nonuse rate. Using the validated organ allocati

Living donor kidney transplants (LDKTs) are vital for access to transplantation among patients with kidney disease. Little research has been conducted on LDKT behaviors after the coronavirus disease 2019 (COVID-19) pandemic. Our aim was to analyze donor and recipient patterns in contemporary LDKT trends after the health system shock from the pandemic. Using national Scientific Registry of Transplant Recipients and Census data, we calculated monthly rates of adult LDKTs per 10 million population overall, by donor relationship, and by recipient characteristics from January 2015 to December 2024.

BACKGROUND: Diabetes mellitus (DM) is an increasing comorbidity in the kidney donor population, but there are limited data on allograft outcomes from donors with DM. We investigated outcomes after kidney transplantation from deceased diabetic donors.
METHODS: We undertook a retrospective observational study of adult patients who underwent kidney alone transplantation from deceased donors at a single center between 2012 and 2022 in the United Kingdom. We stratified transplants according to the diabetic status of both the donor and recipient. We compared clinical characteristics of diabetic don

RATIONALE & OBJECTIVES: There are growing concerns over inequities and inequality in deceased-donor kidney access. The current allocation system emphasizes distance from the kidney donor, resulting in geographic disparities in kidney transplantation. Increasing the priority radius for deceased-donor kidneys may improve access; however, increasing the distance a kidney travels increases cold ischemia time and may result in worse allograft outcomes. We examined whether donor kidney travel distance is associated with posttransplant outcomes.
STUDY DESIGN: Retrospective cohort study.

BACKGROUND: Iohexol clearance (mGFRioh) is a marker of choice for measuring glomerular filtration rate (GFR) in potential living kidney donors.
OBJECTIVES: We studied (1) the determinants of mGFRioh variation among kidney donors before and 1 year after donation and (2) the impact of donor mGFRioh change on the gain in estimated GFR 3 months after transplantation in kidney transplant recipients.
METHODS: We performed a retrospective, monocentric study in couples of living kidney donors/transplant recipients. In donors, mGFRioh was measured before and 1 year after donation and calculated using

RATIONALE & OBJECTIVE: Increasing access to kidney transplantation requires patients to be active on the waiting list and empowered to consider donor offers that are consistent with their preferences. We sought to develop and refine patient-centered communication tools for shared decision-making conversations within dialysis facilities about active status on the waiting list, consent for hepatitis C and high-kidney donor profile index organ offers, and living donation. Conversations may help patients improve their probability of kidney transplantation.

INTRODUCTION: Changes in national allocation policy and regulatory practices have led to increases in cold ischemia time (CIT) and out-of-sequence allocation (AOOS) or expedited placement (EP) of deceased donor kidney transplants in the United States. The aims of this study were to gauge the clinical relevance of the increased CIT that accompanied kidney allocation system (KAS) 250 NM (KAS250), and to measure CIT for AOOS or EP kidneys.
METHODS: The United Network for Organ Sharing (UNOS) data from 2007 to 2023 were analyzed across pre-KAS, KAS, and KAS250 eras. The impact of CIT on kidney gr

RATIONALE & OBJECTIVE: Children in the United States receive priority for kidneys with a kidney donor profile index (KDPI) < 35%, although their priority falls below that of certain adult recipients. Recently, the KDPI was revised to include 8 donor characteristics instead of 10. We sought to (1) describe the allocation of these high-quality kidneys and (2) compare the characteristics of donors with a KDPI < 35% using the original KDPI (KDPI-10) versus the revised KDPI (KDPI-8) calculation.
STUDY DESIGN: Retrospective cohort study.

BACKGROUND: Identifying the impact of living organ donation on donor quality of life (QoL) is crucial for tailored health care support. However, specific and validated instruments for this purpose remain scarce.
OBJECTIVE: We aimed to develop and validate a modular-design QoL scale for living solid organ donors.
METHODS: We recruited liver and kidney donors from 2 Chinese hospitals. After generating items via literature review and donor interviews, item selection employed the Delphi method, cognitive interviews, and item analysis. Psychometric properties were assessed through exploratory and

A substantial proportion of living kidney donor candidates are hypertensive or diagnosed with hypertension during the evaluation process. Furthermore, many donors develop hypertension after nephrectomy as they age, similar to nondonors. In this article, we review the data available from the general population and donor cohorts relevant to the risk of development of hypertension after donation, and the risk associated with pre-existing hypertension. Large long-term data show that treatment and control of hypertension mitigate the cardiovascular mortality risk associated with hypertension. Given

BACKGROUND: Large language models (LLMs) are undergoing exploration as clinical decision support tools. However, their role in complex, high-stakes transplant nephrology decisions, including potential living kidney donor candidate (pLKDC) evaluation, remains unclear.
METHODS: In this prospective pre-post pilot study, 14 physicians (seven fellows and seven early-career nephrologists [consultants]) evaluated 30 standardized, deidentified complex pLKDC vignettes, first unaided and then with OpenAI ChatGPT-4o assistance. The primary outcomes were diagnostic accuracy (scale, 0-1), justification qu

PURPOSE: Rural patients who qualify for kidney transplantation are less likely to undergo transplant compared to urban patients. Multi-listing, or being added to the waiting list at more than two centers, has been shown to improve kidney transplantation rates, yet it remains a subject of debate and is understudied in rural populations. We sought to compare demographic, clinical, and donor characteristics among rural kidney transplant recipients who were on the waiting list at two or more centers (i.e., multi-listed) versus those who were on the waiting list at one center (i.e., single-listed)

BACKGROUND: Post-donation care is variable in Canada.
METHODS: A virtual workshop including donors and health providers was conducted to advance recommendations for a new national standard of living donor follow-up. Attendees participated in facilitated breakout groups to deliberate pre-set questions on a draft care model informed by a survey of 685 previous donors. Deliberations were audio-recorded, transcribed, and imported into HyperRESEARCH software to facilitate inductive, systematic and auditable coding of the data. Final meeting recommendations were approved by attendees and then prese

Nephrolithiasis remains an important consideration in living kidney donor evaluation, but long-term outcomes in carefully selected donors with asymptomatic stones remain incompletely defined. We evaluated renal functional trajectories and postdonation stone recurrence among donors with incidental nephrolithiasis identified during predonation assessment. In this single-center retrospective cohort study, all living kidney donors who underwent nephrectomy between March 2007 and March 2017 under a standardized age-stratified protocol were analyzed. Donors with asymptomatic nephrolithiasis on predo
