PURPOSE: Quantitative Rubidium-82 (82Rb) PET allows assessment of myocardial blood flow (MBF) and flow reserve (MFR), yet clinically applicable normal reference ranges remain incompletely defined across age and sex. This study aims to establish age- and sex-specific reference ranges for resting MBF (rMBF), stress MBF (sMBF), and MFR using 82Rb PET in healthy adults.
METHODS: Data from four cohorts of 277 carefully screened healthy adults (143 women, 134 men) aged 19-79 years were examined with an identical 82Rb PET protocol on the same scanner. Global rMBF, sMBF, and MFR were quantified with

BACKGROUND: Most living kidney donors recover 60-70% of baseline renal function, yet some show suboptimal compensation despite a favorable baseline eGFr. The remnant kidney volume to weight ratio (rKV/w) reflects anatomical reserve relative to metabolic demand, but its predictive value has not been tested in Middle eastern populations, and it is unknown whether volumetric hypertrophy guarantees functional gain.
METHODS: we retrospectively analyzed 368 living kidney donors who underwent left sided laparoendoscopic single site nephrectomy (2017-2024). Preoperative CT volumetry measured rKV/w. s

AIMS: Hand-assisted laparoscopic donor nephrectomy (HALDN) is an internationally accepted technique to safely retrieve kidneys for live kidney donor transplantation. Locally published data regarding outcomes in the donors is lacking. This study aims to assess outcomes after HALDN in the Australian setting.
METHODS: A retrospective review of all patients undergoing HALDN was conducted at Royal Melbourne Hospital and Melbourne Private Hospital between 24 March 2011 and 31 December 2022. Patients were identified and information collected from surgical and renal databases which collect data prosp

BACKGROUND: Pediatric kidney transplant recipients typically receive low-KDPI deceased donor kidneys but often experience higher HLA mismatches. The relationship between KDPI and HLA mismatch and their combined effect on graft and patient outcomes in children has not been well studied.
METHODS: We conducted a retrospective review of SRTR data in evaluating the association between KDPI and HLA mismatch. We included all pediatric kidney transplant recipients under 21 years of age who received a transplant between December 4, 2014, to December 31, 2023.
RESULTS: There was no significant differe

BACKGROUND: Kidney transplantation (KTx) is superior to dialysis but is limited by organ shortages. Kidneys from pediatric donors, whether single or en bloc, have excellent outcomes but are underutilized, with little consensus on splitting pediatric en bloc grafts.
METHODS: A single-center retrospective review of all deceased donor KTx using single kidneys from pediatric donors weighing ≤20 kg between 2005 and 2023 was performed. Recipients were compared based on donor weight: <15 and ≥15 kg. Data are presented as medians (interquartile range) and numbers (percentages).
RESULTS: Seventy-two

KEY POINTS: Combining donor urinary biomarkers with Kidney Donor Profile Index improved the ability to predict paired recipient outcomes. Donor urinary biomarkers provided additional information on donor kidney quality, thus informing the kidney transplant allocation process.
BACKGROUND: Kidney transplant allocation currently relies on the Kidney Donor Profile Index (KDPI), a measure that has limitations. This study evaluated the potential of using KDPI and deceased donor urinary biomarkers to improve the assessment of organ quality and the prediction of allograft outcomes compared with KDPI

INTRODUCTION: Although genetic testing is increasingly used in evaluating living kidney donor (LKD) candidates and recipients, objective data on genetic testing practices are limited. This study aimed to describe current genetic testing practices in LKD evaluation using data from the international living donor genetic registry.
METHODS: A research electronic data capture (REDCap) registry was developed to collect cross-sectional, deidentified information on LKD candidates and genetic test results. Participating centers registered candidates meeting at least 1 of the following criteria: (i) un

KEY POINTS: This qualitative study explored patient, caregiver, and provider perceived barriers to retransplantation after kidney allograft loss. We identified four medical and nonmedical barriers to retransplantation and four additional barriers to pursuing living donor retransplantation. System-, transplant center-, provider-, and patient-level approaches to address these barriers are highlighted.
BACKGROUND: Many kidney transplant recipients experience graft loss, and retransplantation offers a significant survival benefit when compared with waitlisted patients. Yet, trends related to pree

Kidney allocation relies on rapid sequential offers to maximize organ utilization, yet patient-level refusal remains poorly characterized. We conducted a retrospective study of kidney refusal encounters across 3 transplant centers over 2 years. Among 841 deceased donor offers where patients were contacted, 108 resulted in patient-level refusals (13%). Donation after circulatory death kidneys represented 51% of offers and 54% of refusals, whereas donation after brain death kidneys comprised 49% of offers and 46% of refusals. Median kidney donor profile index of refused offers was 50.5, and refu

INTRODUCTION: An information most relevant to patients when deciding to accept a kidney offer is an estimate of its potential longevity. Using a novel machine learning approach, our aim was to develop a model that can output personalized curves estimating kidney graft and patient longevity to support clinical decision-making.
METHODS: We performed a retrospective cohort study in recipients of a first deceased donor kidney transplant aged ≥60 years between 2000 and 2020, using the United Network for Organ Sharing dataset. Outcomes were overall graft survival and patient survival. Independent v

INTRODUCTION: Living kidney donation is essential for expanding transplant opportunities, given the worldwide scarcity of deceased donor organs. Kidney donors experience a slight but detectable long-term increase in risks such as hypertension, diabetes, and end-stage kidney disease, making sustained follow-up essential. However, adherence to post-donation surveillance remains inconsistent worldwide.
METHODS: This descriptive evaluation included living kidney donors enrolled in Qatar's national follow-up program at Hamad Medical Corporation, implemented in January 2017. The program mandates ne

PURPOSE: Deceased donor liver transplantation (DDLT) remains constrained by organ shortages, emphasizing the need for accurate predictive models to optimize allocation and improve outcomes. Although multiple indices, such as Model for End-Stage Liver Disease (MELD), MELD-Na, and donor-specific models, exist to predict recipient and graft survival, their comparative performance remains uncertain.
METHODS: This study compared 8 predictive models (MELD, MELD-Na, Gender-Equity Model for Liver Allocation [GEMA], GEMA-Na, Donor Rejection Organ Procurement Evaluation, Donor Risk Index, Kidney Donor

Psychosocial assessment of living related-kidney donors is important, yet limited prospective data exist on donor-perceived family functioning after transplantation. Identifying donors at high risk for deterioration in family functioning prior to transplantation is crucial for management. This study aimed to identify preoperative psychosocial predictors of post-donation family dysfunction to facilitate early targeted intervention, and to identify clinical predictors of functional change. A total of 106 Japanese LRKT donors were included in the analysis. Family functioning was assessed using th

Expansion of living kidney donation is needed to meet the increasing global need for kidney replacement therapy. Advances in the understanding of postdonation health outcomes have confirmed that living donation in carefully selected donors is associated with a low absolute risk of kidney failure of 50 per 10,000 after 20 years. However, information about lifetime risks, heterogeneity of risk, and the long-term safety of donors with chronic kidney disease risk factors before or after donation remains incomplete. This uncertainty likely contributes to the global stagnation in living-donor kidney

BACKGROUND: Using older living donors is a strategy to expand the kidney donor pool, but concerns remain regarding mid-term graft performance. We evaluated 5-year patient and graft outcomes after living donor kidney transplantation from donors aged 60 years and older versus younger donors.
METHODS: We conducted a single-center retrospective cohort study of 279 living donor kidney transplants. Recipients were grouped by donor age (<60 years, n = 237; ≥60 years, n = 42). To reduce selection bias, inverse probability of treatment weighting was applied. Outcomes included longitudinal recipient es

BACKGROUND: Decisions to accept or refuse kidney organ offers require transplant programs and patients to evaluate a multifaceted set of risk factors. The risks of accumulating more dialysis time on the waitlist must be weighed against offer features including the Kidney Donor Profile Index (KDPI).
METHODS: In this analysis, we use a modern period-prevalent cohort of adult nonpreemptive kidney-alone candidates and recipients from the Scientific Registry of Transplant Recipients. We fit Cox proportional hazards models with time-varying coefficients modeled flexibly with splines to characterize

BACKGROUND AND HYPOTHESIS: Accurate assessment of glomerular filtration rate (GFR) is essential during evaluation of prospective kidney donors. While estimated GFR (eGFR) equations are widely used in clinical practice, evidence regarding their performance in individuals with preserved renal function remains limited. We evaluated the agreement of creatinine, cystatin-C, and combined biomarker eGFR equations against measured GFR (mGFR) calculated using iohexol plasma clearance in healthy prospective kidney donors.
METHODS: We conducted a retrospective observational study of adults undergoing ki
