Donor Warm Ischemia Time Modifies Susceptibility to Cold Ischemia in Donation After Circulatory Death Kidney Allografts Under Hypothermic Machine Perfusion.

BACKGROUND: Donation after circulatory death (DCD) kidneys are exposed to donor warm ischemia time (dWIT) in addition to cold ischemia. Although porcine studies suggest synergy between these insults, evidence in clinical transplantation is limited. METHODS: Adult DCD kidney-only transplants performed using hypothermic machine perfusion (n = 28 698) in the United Network for Organ Sharing database (January 1, 2015, to December 31, 2023) were included. We performed 1:1:1 propensity score matching across 3 cold ischemia time (CIT) categories (short, <18 h; intermediate, 18-23 h; and long, ≥24 h)

Impact of donor's acute kidney injury on graft outcomes of deceased donor kidney transplantation.

Deceased donor kidney transplantation (DDKT) has a serious problem of donor organ shortage, particularly in Asia. Consequently, donor kidneys with acute kidney injury (AKI) have been utilized for transplantation. This study aimed to evaluate the prognosis of DDKT using AKI kidneys. We analyzed the data of 8,523 adult patients who underwent DDKT between 2008 and 2022 using the Korean nationwide DDKT database. The association between donor AKI and death-censored graft failure was assessed using competing risk analysis, with adjustment through inverse probability of treatment weighting (IPTW). Th

"Rethinking High-KDPI Kidneys: A Multidomain Approach to Predicting Success".

BACKGROUND: The allocation and acceptance of deceased-donor kidneys in the United States is influenced by theKidney Donor Profile Index (KDPI). We conducted a national analysis of high-KDPI kidney transplants performed from 2014 to 2021 to identify key predictors of post-transplant outcomes beyond those incorporated in KDPI. METHODS: This retrospective cohort study used data extracted from the Scientific Registry of Transplant Recipients (SRTR). Adult, first-time recipients of kidney-only deceased-donor transplants with KDPI greater than 85% were included. Regression models were used to ident

Clinical Impact of Preoperative Obesity on Living-Donor Kidney Transplant Recipients in Japan: A Multicenter Experience.

Background: Obesity is increasingly prevalent among kidney transplant candidates; however, its impact on graft outcomes in Asian populations is not well characterized. We evaluated the association between preoperative obesity and living-donor kidney transplantation outcomes in Japan. Methods: We analyzed 623 living-donor kidney transplants performed from 1998 to 2021 at six centers in northern Japan. Recipients were categorized by body mass index (BMI) at transplant, and multivariable Cox regression was employed for assessing graft outcomes. Results: Obesity (BMI, ≥30 kg/m2; n = 27 [4.3%]) was

Pregnancies in Living Kidney Donors.

BACKGROUND AND HYPOTHESIS: Although the majority of kidney donors are women, data on pregnancy-related maternal and fetal outcomes remain scarce. Understanding these risks is essential for evidence-based counselling. Our aim was to assess whether pregnancies in living kidney donors were associated with adverse outcomes. We hypothesized that kidney donation increases the risk of pregnancy complications, particularly pre-eclampsia and related adverse outcomes. METHODS: In this nationwide registry-based cohort study, Swedish women ≤45 years with births before and/or after kidney donation between

Pediatric Kidney Transplant Outcomes After Kidney Donor Profile Index-Based Organ Prioritization.

INTRODUCTION: Following implementation of the U.S. Kidney Allocation System (KAS) in 2014, deceased donor kidneys with a kidney donor profile index (KDPI) < 35% are prioritized for allocation to pediatric candidates. Early post-KAS data suggested this prioritization may have led to more frequent delayed graft function compared to pre-KAS, when pediatric allocation priority was based on donor age < 35 years. We sought to understand the impact of this allocation change on longer-term pediatric kidney transplant outcomes. METHODS: We used SRTR data to identify all deceased donor kidney transplan

Outcomes of Kidney Transplants From Donors With Stage 3 Acute Kidney Injury Requiring Dialysis: A National Cohort Study.

BACKGROUND: Whether severe donor acute kidney injury (AKI) requiring dialysis affects posttransplant outcomes remains debated and may influence center-level acceptance decisions. METHODS: This retrospective cohort study of United States deceased-donor kidney transplants (2015-2020) used OPTN data with follow-up through September 2024. Donor AKI was staged by KDIGO 2012 criteria, and dialysis was identified using a validated rule-based natural language processing (NLP) algorithm applied to DonorNet records. The primary outcome was all-cause graft failure (ACGF). Secondary outcomes included del

Outcomes Following Alemtuzumab Induction in Deceased Donor Kidney Transplantation According to Recipient Age Categories: A Single Center Report.

INTRODUCTION: The effects of induction therapy with the anti-CD52 monoclonal antibody alemtuzumab on age-specific outcomes following kidney transplantation (KT) remain inadequately studied. This study aimed to analyze the comparative safety and efficacy of single-dose alemtuzumab induction in deceased donor (DD) KT recipients according to different recipient age groups. METHODS: Single-center retrospective nonrandomized cohort review of DDKT recipients stratified by age (30-49/50-69/ ≥ $ \ge $ 70 years). All patients received a single 30-mg intra-operative dose of alemtuzumab. Standardized m

Immunosuppressive regimens and long-term kidney transplant outcomes: A dual survival modeling framework.

Optimizing immunosuppressive therapy remains central to improving long-term outcomes after kidney transplantation. Both induction and maintenance therapies are widely used, yet their comparative effectiveness across diverse recipient populations requires further evaluation. This national retrospective cohort study analyzed 228,855 deceased-donor kidney transplant recipients using data from 2000-2024. Multivariable Cox proportional hazards (PH) models were used for clinical inference, and four machine learning (ML) survival models: random survival forest (RSF), support vector machine (SVM), pen

Surgical Approach to Redo Open Living Donor Nephrectomy via the Same Flank Incision: A Case Report.

Living donor nephrectomy is performed with donor safety as the highest priority, and repeat donor nephrectomy through the original incision is rarely reported in transplantation literature. We present the case of a 33-year-old male living kidney donor and a 48-year-old female recipient who underwent scheduled kidney transplantation. The initial procedure was aborted before vascular ligation because the recipient developed intraoperative anaphylactic shock after anesthesia induction. Three weeks later, transplantation was reattempted using the same donor flank incision. Dense retroperitoneal ad

The combined impact of HLA and non-HLA mismatch between donors and recipients on kidney transplant survival: a genomic analysis in a prospective cohort.

BACKGROUND: Kidney transplantation outcomes are strongly influenced by immunological compatibility between donor and recipient. While genetic mismatches in the human leukocyte antigen (HLA) region have long been recognised as key determinants of graft survival, increasing evidence, including our own previous work, suggests that non-HLA alloimmunity also plays a critical role. METHODS: We sequenced exomes of deceased kidney donor and recipient pairs in the prospective kidney transplant cohort at the Vienna General Hospital, recruited between January 1, 2012, and June 15, 2023. Out of 1209 pair

Living Kidney Donor Transplantation: A Single-Center Analysis of Acceptance Rate, Donor Exclusion Reasons, and Evaluation Timeline.

OBJECTIVES: With limited deceased donor availability, living kidney donor transplantation remains the primary source of grafts in Tunisia. Strict donor selection criteria are essential to ensure donor safety and optimize recipient outcomes. We investigated the acceptance rate of living donor kidney transplant among eligible recipients and potential donors, enumerated the primary causes of donation discontinuation, and characterized the donor evaluation process at a single Tunisian transplant center. MATERIALS AND METHODS: We conducted a retrospective review of 70 patients with end -stage rena

Long Term Outcomes in Living Kidney Donors Over 30 Years.

INTRODUCTION: Living kidney donor transplantation provides major benefits for recipients, but long-term donor safety remains a critical concern. METHODS: We conducted a prospective, multicenter cohort study using data from the Swiss Organ Living-Donor Health Registry. A total of 1863 living kidney donors (1993-2018) with ≥ 5 years of follow-up were followed until death, kidney failure (KF), or last visit (censoring March 31, 2024). Kidney function trajectories from 12 months postdonation onward were analyzed using linear mixed-effects models. A composite secondary end point included early dea

Long-Term Risk for Scrotal Hydrocelectomy After Laparoscopic Living Donor Nephrectomy: A Population-Based Matched Cohort Study.

BACKGROUND: An underappreciated complication of laparoscopic nephrectomy in male living kidney donors is testicular pain, swelling, and hydrocele development. A recent cohort study in Ontario, Canada, found that male donors had a higher rate of hydrocelectomy (7.7 vs. 0.1 events/1000 person-years) and scrotal ultrasounds (25.6 vs. 6.9 events/1000 person-years) than nondonors, when followed for a median of 9 years. OBJECTIVE: To conduct a replication study in Alberta, Canada, examining the long-term risk of hydrocelectomy after laparoscopic living donor nephrectomy, comparing male donors to no

Beyond Living Donor Kidney Transplantation in COL4 Nephropathy - A Real-World Clinical Dilemma in Light of Current Guidelines.

Kidney transplantation is the best choose of modality for treatment of the patients with end-stage kidney disease (ESKD). However, hereditary conditions like COL4 nephropathy introduce significant obstacles in living kidney donor selection. This review presents a real-world clinical dilemma involving a 24-year-old male with autosomal recessive COL4 nephropathy (Alport syndrome) and his 51-year-old mother, a heterozygous carrier. We analyzed the clinical spectrum of COL4 nephropathy in North Macedonia, where 92 patients have been identified via next-generation sequencing (NGS), and COL4A5 mutat

Two Cases of Successful Kidney Transplantation From a 39-Year-Old Male Deceased Donor With Marfan Syndrome: A Case Series.

BACKGROUND Marfan syndrome is a multisystemic, autosomal dominant connective tissue disorder caused by mutations in the fibrillin-1 gene. It is associated with life-threatening complications, especially aortic aneurysms and dissections, and has a global prevalence of 1 in 3000-5000 individuals. Although kidney transplantation is the standard treatment for end-stage renal disease (ESRD), deceased kidney donors with Marfan syndrome remain uncommon due to concerns about connective tissue fragility. Emerging reports, however, indicate that kidneys from such donors may be safely used. This report d

Should All Living Donors Receive Equal Priority in Organ Allocation? An Equality of Opportunity Perspective.

Priority rules in organ allocation confer preferential access to organs for individuals who have previously donated or pledged to do so. Several countries extend such rules to living donors. Yet existing systems do not distinguish between different kinds of living donation. This paper examines whether all living donors should receive equal priority or whether distinctions are justified, using John Roemer's conception of equality of opportunity as the analytical framework. The paper develops a typology of living donation along two axes: interest-based versus altruistic motivation and directed v

Median arcuate ligament compression of the renal artery.

Renal artery entrapment (RAE) is an uncommon form of median arcuate ligament or diaphragmatic crura compression. This case describes a woman in her mid-40s with an incidental finding of right renal artery stenosis by the right crus following renal donor protocol imaging. She was an otherwise healthy former smoker with no history of hypertension or atherosclerotic disease. Imaging demonstrated bilateral anatomical variants within her renal arterial system, including accessory renal arteries for both kidneys. She was deemed unsuitable to become a kidney donor, and her RAE was conservatively mana

Management of brain-heart multimorbidity: a clinical practice guideline.

BACKGROUND: Although brain and heart conditions share overlapping risk factors and commonly co-occur, current cardiac and neurologic clinical guidelines are typically produced within specialty silos. The objective of this guideline from a Canadian Cardiovascular Harmonized National Guideline Endeavour (C-CHANGE) panel is to expand on current cardiovascular guidelines to include evidence from the neurologic and mental health literature, with specific recommendations for providers managing comorbid brain and heart conditions. METHODS: The guideline development panel comprised an Executive Steer
