Expanding the living kidney donor pool: outcomes of robotic donor nephrectomy in obese versus non-obese donors.

BACKGROUND: Robotic donor nephrectomy (RDN) offers a minimally invasive alternative to open and laparoscopic techniques for living kidney donation. As obesity rises among potential donors, data comparing RDN outcomes between non-obese and obese donors remain limited. This study evaluates the safety and feasibility of RDN in obese living kidney donors. METHODS: A single-center retrospective analysis included 212 consecutive donors who underwent RDN between August 2020 and April 2025; after excluding one donor with incomplete covariate data, 211 (137 low BMI [< 30 kg/m2], 74 high BMI [≥ 30 kg/m

Urine inflammatory protein levels decrease following kidney transplantation.

BACKGROUND: Kidney transplant recipients are subjected to several types of immunosuppressants in order to prevent rejection of the graft. Over-immunosuppression increases the risk of infectious complications and thesepatients require a delicate balance. Inflammatory profiles from the urine can help identify which biomarkersmay guide clinicians in balancing over vs. under-immunosuppression. METHODS: Urine samples were obtained from 10 complete kidney donor-recipient pairs at a pre-operative visit ~1 weekprior to or at time of transplant and then for the recipients, ~3 months following transpla

Top-Down Proteomics in the Assessment of Kidney Donor Quality: a proof-of-concept for increased organ utilization.

A persistent barrier to optimizing the utilization of deceased donor kidneys, nearly 30% of which are discarded, is the absence of objective, molecularly resolved metrics of organ quality. To address this gap, we integrated high-resolution proteoform imaging mass spectrometry (PiMS) of donor kidney biopsies with deep LC-MS-based intact proteoform profiling of recipient peripheral blood mononuclear cells (PBMCs) and standard clinical assays. This approach enables direct delineation of proteoform landscapes and reveals immunological and cell stress signatures. Across donor types, living donor (L

Hypothermic Machine Perfusion Modifies the Impact of Cold Ischemia Time on Kidney Transplant Outcomes: A Single-Center Analysis.

INTRODUCTION: Preservation strategies are critical for mitigating ischemia-reperfusion injury in kidney transplantation, particularly as the utilization of marginal grafts and prolonged cold ischemia times (CIT) increases. Although hypothermic machine perfusion (HMP) is increasingly used in kidney graft preservation, the comparative effectiveness of continuous HMP (cHMP), hybrid HMP-static cold storage (HMP-SCS), and static cold storage (SCS) alone remains incompletely defined. We evaluated the impact of preservation strategy, CIT, and HMP exposure on posttransplant outcomes. METHODS: We perf

Consent in the dark: Can living kidney donation proceed amid marital infidelity?

Living kidney donation requires rigorous medical, psychosocial, and ethical evaluation to safeguard donor welfare, ensure valid informed consent, and prevent wrongful exploitation. In Singapore, the Human Organ Transplant Act mandates review by a transplant ethics committee (TEC) for all living-donor transplants. However, ethical judgment is not reducible to TEC approval or procedural compliance; transplant programs themselves carry a broader responsibility to evaluate relational dynamics that may affect voluntariness and authenticity of consent. This paper examines three cases involving conce

Quantifying Center-level Aggressiveness in Transplanting Suboptimal, Deceased Donor Kidneys in the United States.

BACKGROUND: Understanding center-level decision-making for suboptimal kidney (SOK) offers is critical to ensure utilization of all transplantable kidneys. METHODS: We quantified center-level variation in accepting SOK deceased donor kidney transplant (DDKT) offers using 2021-2023 national registry data. SOK subtypes included: donor age >60, ultimate cold ischemia time >24 h, hepatitis C positive, terminal serum creatinine >2.0 mg/dL, donation after circulatory death, kidney donor profile index >85%, and public health service increased risk donors. Gini coefficient (Gini) was used to analyze i

The Detrimental Outcomes Associated With Kidney Delayed Graft Function Persist Beyond the Short Term.

BACKGROUND: Delayed graft function (DGF) is associated with acute rejection (AR) and decreased graft survival, mainly in the early posttransplant period. However, its impact beyond this time is not well known. METHODS: All deceased donor kidney-only transplant recipients at our center between June 2005 and December 2019 were stratified based on DGF status (+ versus -). Adjusted hazard ratios (aHRs) of all-cause graft failure (GF), death-censored GF (DCGF), death with a functioning graft (DWFG), and acute rejection (AR) were analyzed within the short-term (0-6 mo) and medium-term (7-36 mo) pos

OPTN/SRTR 2024 Annual Data Report: Kidney.

In 2024, the field of kidney transplantation faced both notable successes and ongoing challenges. A record 27,660 kidney transplants were performed in the United States, an achievement driven largely by deceased donor kidney transplant (DDKT) growth, although with only a modest increase from 27,351 kidney transplants in 2023. The number of adult candidates listed for kidney transplant rose to 143,982 (approaching the prepandemic peak of 2019), with 10.7% of candidates waiting 5 years or longer. Transplant counts increased across Black, Hispanic, Asian, and Multiracial recipients, with the larg

Effects of End-ischemic Hypothermic Oxygenated Perfusion on 1-y Graft Function in Kidney Transplantation: A Real-world, Single-center Analysis From the United States.

BACKGROUND: Hypothermic oxygenated perfusion (HOPE) supplies oxygen to kidney allografts for resuscitation before transplantation and may improve graft recovery. This study represents the first U.S.-based comparative analysis evaluating the impact of HOPE on kidney transplant (KT) outcomes. METHODS: A retrospective, single-center study of adult, deceased-donor, kidney-only transplants performed between December 1, 2023, and March 31, 2025. Transplants were grouped according to preservation method: HOPE, static cold storage (SCS), or hypothermic machine perfusion (HMP) without oxygen alone. Th

Transmission of Anellovirus From Kidney-Donor to Pediatric Recipient: An Exploratory Study.

BACKGROUND: Among the most abundant viruses in the human blood virome are anelloviruses, including alpha-, beta-, and gammatorqueviruses (TTV, TTMV, and TTMDV). Whether anelloviruses are transmitted through kidney transplantation at a young age and subsequently persist in their new host is unknown. This study investigates the dynamics and composition of the anellome in the blood of six pediatric donor-recipient pairs, with monitoring beginning before transplantation and continuing until 2 years after transplantation. METHODS: Donors were sampled once, before donation, while recipients were sa

Understanding the Relationship Between Severe Donor Diabetes Insipidus, Kidney Donor Profile Index, and Recipient Kidney Function.

BACKGROUND: Severe diabetes insipidus (DI) causes dynamic changes in creatinine before organ donation. It is unknown whether terminal or peak creatinine is the optimal measure of graft quality among donors with severe DI. METHODS: Adult donors after brain death were identified from the United Network for Organ Sharing database, years 2015-2022. Kidney Donor Profile Index (KDPI) was computed using both terminal creatinine (KDPI-Terminal) and peak creatinine (KDPI-Peak). Model fit for the prediction of graft failure and reduced glomerular filtration rate (GFR) was compared. Logistic regression

Machine Perfusion Parameters and 1-Year Outcomes in High KDPI Kidney Transplantation: A Single-Center Cohort Study.

BACKGROUND: Kidney donor profile index (KDPI) was intended as an objective measure of donor organ quality but its utility in kidney allograft assessment has been questioned. High KDPI kidneys (KDPI ≥85) present a source of transplantable allografts. METHODS: A retrospective review was conducted on a single-center database for KDPI ≥85 transplants from 2013 to 2023. Allografts placed on hypothermic machine perfusion (HMP) with a flow ≥100 ml/min and resistance ≤0.2 mm Hg/mL/min (Group A) were compared to those with a flow <100 mL/min and/or resistance >0.2 mm Hg/mL/min (Group B). The cardiovascular evaluation of candidates for living kidney donation.

Most studies concluding that living kidney donation does not increase cardiovascular risk have been conducted in low-risk cohorts. As transplant programs increasingly encounter medically complex donors, careful consideration of long-term cardiovascular risks is essential. There is significant variation among institutions in the practices regarding the selection of living donors. This comprehensive review examines the existing evidence on post-donation cardiovascular outcomes, and prevalent risk factors in the donor candidate population including older age, hypertension, prediabetes, diabetes,

Trimethoprim/sulfamethoxazole-triggered drug-induced hypersensitivity syndrome in an HLA B*13:01-positive kidney transplant recipient: a case report with implications for HLA-severe cutaneous adverse reaction associations in transplant care.

Drug-induced hypersensitivity syndrome/drug reaction with eosinophilia and systemic symptoms (DIHS/DRESS) is a severe cutaneous adverse reaction (SCAR) with a reported mortality rate of approximately 2-10%. DIHS/DRESS typically develops 2-8 weeks after exposure to an offending drug. An important focus of contemporary SCAR research is the growing evidence that specific human leukocyte antigen (HLA) alleles confer a markedly increased risk of drug-specific hypersensitivity reactions. We report a case of a kidney transplant recipient who developed DIHS/DRESS after prolonged trimethoprim/sulfameth

Unravelling Cholera's Footprints in India: A Data-Driven Investigation of its Burden, Risk, and Strain Variability.

INTRODUCTION: Binge Eating Disorder (BED) has high lifetime prevalence rates, low treatment success rates, and high rates of treatment dissatisfaction, early discontinuation of care, and recurrence. Complementary and integrative health (CIH) interventions (non-mainstream practices used with conventional approaches for whole-person treatment) hold potential to overcome many treatment barriers and improve BED treatment outcomes. Some CIH interventions have empirical support for use in eating disorders. However, little is known about the current state of CIH use in BED. METHODS: This mixed-metho

Anesthetics and cancer biology: Translational evidence from perioperative modulation to drug repurposing (Review).

BACKGROUND: Reports suggest that children with mobility impairment represent a significant proportion of the population living with a disability. Footwear is considered to be the key extrinsic factor affecting children's gait and footwear modifications have been historically postulated to assist with locomotory difficulty. Although therapeutic footwear has been considered within the literature, there is a lack of consistency on terminology and paucity on the overall understanding. A scoping review was performed to chart the key concepts in children's footwear and to establish the range of stud

Accurate prediction of cytomegalovirus disease through optimized viral load cut-offs in intermediate-risk kidney transplant recipients.

BACKGROUND: Cytomegalovirus (CMV) infection remains a major complication in kidney transplant recipients (KTRs), particularly among donor-negative/recipient-positive (D-/R+) individuals, who are at intermediate risk for viral reactivation. Although CMV DNAemia monitoring allows for preemptive therapy, the optimal threshold to predict disease remains uncertain. This study aimed to determine a clinically relevant CMV DNAemia cut-off for predicting CMV disease in D-/R+ KTRs and evaluate associated clinical outcomes. METHODS: A retrospective cohort of 192 D-/R+ KTRs was followed for one year post

Human antibody repertoire among kidney donors with and without HIV.

BACKGROUNDTransplanting kidneys from donors with HIV to recipients with HIV has become standard clinical practice. However, donors with HIV may have higher prevalence of viral and bacterial infections and autoimmunity that could increase allograft rejection in recipients.METHODSWe included deceased kidney donors (60 with HIV and 41 without HIV) who participated in a multicenter prospective study of HIV kidney transplantation between April 2018 and September 2021. Using phage immunoprecipitation sequencing, we compared the human antibody repertoire (allergens, autoantibodies, viruses, and bacte

Long-Term Outcomes of Living-Related Kidney Donors Over 26 Years: A Study of 154 Cases From the Ibn Sina University Hospital Center in Rabat, Morocco.

Introduction Kidney transplantation remains the treatment of choice for end-stage kidney disease (ESKD) because of its excellent outcomes in terms of both graft and recipient survival. However, preserving the long-term health of living kidney donors represents a major challenge in nephrology. Rigorous post-donation follow-up is essential for the early detection of potential renal and cardiovascular complications and for ensuring donor safety. Methods We conducted a retrospective, descriptive, and analytical study over a 26-year period (1998-2024) at the Ibn Sina University Hospital Center in R
