Clinical Characteristics, Virulence Profile, and Molecular Epidemiology of Klebsiella pneumoniae Infections in Kidney Transplant Recipients.

BACKGROUND: Carbapenem-resistant Klebsiella pneumoniae (CRKP) infections may increase the potential for mortality in kidney transplant (KT) recipients. This study aimed to investigate the clinical features, molecular epidemiology, virulence, and antimicrobial resistance of KP strains from KT patients. METHODS: Strains isolated from KT patients were collected, and antimicrobial susceptibility analysis was verified via the Vitek2 compact instrument and the disc diffusion method. In gene expression analysis, carbapenemase genes (KPC-2, OXA-48, IMP, VIM, NDM), capsular genes (K1, K2, K5, K20, K54

Should Older Patients Accept a High Kidney Donor Profile Index Kidney or Wait? A National Data Analysis of Matched Cohorts.

OBJECTIVE: To address whether kidney transplant (KTx) candidates, ages 60+, should accept a high Kidney Donor Profile Index (KDPI) kidney and differences when considering a high (>85%) versus low (<85%) KDPI KTx. BACKGROUND: To date, there is limited survival data to guide decision-making for patients aged ≥60 years who are faced with the choice of accepting a high KDPI kidney or remaining on the waitlist. METHODS: Propensity-matched cohort study using data from United Network on Organ Sharing (2014-2021). Cox proportional hazard models and competing risk models allowed comparison of (1) pat

Living kidney donation from a death row inmate.

We report on living kidney donation from a prison inmate on death row. Thirty years ago, we were involved in an unusual set of circumstances that we assumed would never arise again. That assumption is incorrect as a current death row inmate wishes to be a living kidney donor and we speak now to describe the lessons learned and to help others. This report is focused on living donation, excluding posthumous donation, and discusses the ethical, legal, medical, and logistic considerations underpinning prisoner donation for both the general prison population and death row inmates. We believe inmate

Using Unsupervised Clustering to Characterize Phenotypes Among Older Kidney Transplant Recipients: A Cohort Study.

BACKGROUND: Older kidney transplant recipients have inferior outcomes compared to younger recipients, and this risk may be compounded by donor characteristics. OBJECTIVE: We applied an unsupervised machine learning clustering approach to group older recipients into similar phenotypes. We evaluated the association between each cluster and graft failure, and the impact of donor quality on outcomes. DESIGN: This is a nationally representative retrospective cohort study. SETTING AND PATIENTS: Kidney transplant recipients aged ≥65 years identified from the Scientific Registry of Transplant Recip

Alternate Choice Organ Counselling in Altruistic Non-Directed Solid Organ Donation: An Ethical Analysis.

INTRODUCTION: Currently, recruitment of non-directed altruistic (NDA) kidney and liver lobe donors in the UK regards these individuals as potential NDA donors of the particular organ type they initially express an interest in donating. Conceptualising these individuals instead as potential NDA donors of either a kidney or a liver lobe would require them to be counselled on both kidney donation and liver lobe donation. This can be referred to as 'alternate choice organ counselling'. METHODS: This paper conducts an ethical analysis of alternate choice organ counselling using the ethical framewo

Effectiveness of a Mobile Health System on Compliance With 2-Year Living Kidney Donor Follow-Up in the United States.

BACKGROUND: Organ Procurement and Transplantation Network (OPTN) policy requires 2 years of follow-up for living kidney donors (LKDs); however, many transplant hospitals struggle to meet this requirement. We developed and tested a mobile health (mHealth) system for LKD follow-up in a pilot randomized-controlled trial (RCT). METHODS: LKDs were randomly assigned to either the intervention (mHealth + standard of care) or control arm (standard of care). We assessed OPTN policy-defined completeness and timeliness of 6-month, 1-year, and 2-year follow-ups. Four hundred LKDs were enrolled in the stu

A Prognostic Index for Deceased Donor Kidneys and Criteria for Identifying Suitable Candidates for Kidney Transplantation from Expanded Criteria Donors with Prolonged Waiting Times.

INTRODUCTION: The kidney donor profile index (KDPI) is a valuable prognostic tool in deceased donor kidney transplantation (DDKT), while its optimization for each country using local data is essential. It remains unclear which patients derive survival benefits from expanded criteria donor (ECD) DDKT compared to waitlist or standard criteria donor (SCD) DDKT, particularly in the context of long waiting times. This study aimed to develop a prognostic index for donor kidneys and propose criteria to identify suitable candidates for ECD DDKT in Korea. METHODS: Two prediction models were developed

Robotic Assisted Laparoscopic Donor Nephrectomy: An Update.

PURPOSE OF THE REVIEW: To review the outcomes of robotic-assisted laparoscopic donor nephrectomy in the published literature. RECENT FINDINGS: Robotic-assisted laparoscopic donor nephrectomy has demonstrated to be a safe, efficient and effective technique of minimally invasive surgery, that offers multiple advantages to the surgeon, and good outcomes for the kidney donor and recipient. Although still a recent technique, it has been adopted by multiple centers worldwide. Robotic donor nephrectomy studies demonstrated consistent perioperative outcomes with low complication rates. Mean operative

Enhanced recovery after surgery in laparoscopic kidney donor nephrectomy: the role of quadratus lumborum and transversus abdominis plane block.

PURPOSE: Living donor kidney transplants (LDKTs) represent nearly 35% of all kidney transplants, with laparoscopic living donor nephrectomy (LLDN) being the standard surgical approach. Enhanced Recovery After Surgery (ERAS) protocols have demonstrated better perioperative outcomes in LLDN. This study evaluates the impact of an ERAS protocol on postoperative opioid consumption and length of stay (LOS) compared to standard of care (SC). METHODS: This experimental, prospective study included patients undergoing LLDN at a tertiary care center. The ERAS protocol included preoperative nutritional o

Pilot Randomized Trial Exploring the Impacts of Deceased Donor Kidney Procurement Biopsies on Organ Evaluation and Transplant Outcomes.

BACKGROUND: Kidney biopsies obtained at the time of organ procurement are often used in evaluating deceased donor (DD) kidneys. We conducted a pilot randomized controlled trial (RCT) to help assess the feasibility and impact of deferring procurement biopsy information at the time of organ offer. METHODOLOGY: This pilot RCT involved one Midwestern Organ Procurement Organization (OPO) and two local transplant centers in the United States. Waitlisted transplant candidates were approached between July 01, 2019, and May 31, 2022, for consent to receive a kidney from a randomized DD if an offer aro

Kidney Donor Risk Index and Cardiovascular Complications in a Long-Term Follow-Up Observation.

Background: The suitability of the Kidney Donor Risk Index (KDRI) has not been fully validated in the European population. The aim of this study was to evaluate the value of the KDRI in predicting kidney graft function and cardiovascular events (CVEs) in a Polish cohort of kidney transplant recipients (KTRs). Methods: In this retrospective study kidney graft function and CVEs were analyzed among 1420 patients transplanted between 1999 and 2017 and followed until 2021. The KDRI was calculated according to the formula proposed by Rao. Patients were assigned into quartiles (Qs) of KDRI values. Re

Clinical and Histologic Risk Factors for the Development of Early Allograft Dysfunction in Donation After Circulatory Death Kidney Transplantation.

Transplantation of kidneys from donors after circulatory death (DCD) may help address the severe shortage of donor organs. However, they remain underutilized due to concerns for increased rates of delayed graft function (DGF) and primary nonfunction (PNF), despite similar long-term patient and allograft outcomes compared to kidneys from donors after brain death (DBD). Kidneys that recover from DGF have good long-term outcomes compared to those resulting in PNF. Considering immediate graft function (IGF), DGF, and PNF as a spectrum of renal allograft injury and recovery, we analyzed donor and r

A Novel Visible Method for Monitoring Kidney Rejection: Homologous Donor-Derived Sentinel Skin.

BACKGROUNDS: Rejection in facial transplant patients is always accompanied by changes in skin appearance and can be monitored by observation of the skin. Acute rejection (AR) after kidney transplantation has a significant impact on the function of the transplanted kidney, which is relatively difficult to monitor. In this study, the skin of the same donor was used as a sentinel for monitoring the rejection of kidney transplantation. METHODS: A 3 cm × 5 cm skin was procured from the kidney donor and was grafted to the recipient at the same time as the kidney transplantation, to serve as a senti

Age- and sex-specific reference values for CT-based low skeletal muscle quantity and quality in healthy living kidney donors.

BACKGROUND: Computed tomography (CT) imaging is a useful tool for assessing skeletal muscle mass and quality. The present study aimed to determine age- and sex-specific reference values for CT-based skeletal muscle markers in a healthy population, and to correlate them with serum creatinine and 24-h urinary creatinine excretion (24h-UCE). METHODS: Skeletal muscle index (SMI) - a marker of muscle mass/quantity - and skeletal muscle radiation attenuation (SMRA) and intermuscular adipose tissue index (IMATI) - markers of muscle quality/myosteatosis - were determined using a deep-learning-based m

Utility of Deceased Expanded-Criteria Donors in Kidney Transplantation: A Single-Center Experience.

Purpose: The success of solid organ transplantation and the consequent increase in the patients on the waiting list has led to an increased utilization of donor kidneys with a high kidney donor profile index (KDPI)/expanded criteria. In our study, patients who underwent transplantation based on the standard and expanded donor criteria were compared in terms of factors affecting graft survival. Data of patients who underwent transplantation from cadavers with standard and extended criteria (SCD, ECD) between 01 July 2011 and 30 June 2016 were evaluated retrospectively. Donor characteristics, tr

Donor-Recipient Matching for Kidney Transplantation Using Uncertainty Estimation in Generalized Propensity Score.

Kidney donor-recipient matching is a complex process influenced by various clinical and demographic factors, requiring advanced techniques for effective allocation. This study explores the use of Generalized Propensity Score (GPS) modeling with uncertainty estimation and Sufficient Dimensionality Reduction (SDR) methods to enhance matching strategies. We evaluated several dimensionality reduction techniques using Root Mean Squared Error (RMSE) and Pearson Correlation (PC). The regression method (MLP) showed the best performance with the lowest RMSE (1163) and highest PC (0.23) with single dime
