Sequential Donation: Evaluating Safety and Quality of Life of Living Donors.

One of the most persistent problems in transplantation in the U.S. is the demand for organs outweighs the supply. Sequential living donation may be one strategy to address this problem, however a reluctance to perform these surgeries exists secondary to the lack of research on the medical outcomes and quality of life of sequential post-donation. A medical record review of 34 sequential liver and kidney living donors and matched controls (by sex, age within 7 years, timing of surgery, and last organ donated) was performed. No significant difference between groups were observed on sociodemograph

From Patient Selection to Surveillance: Artificial Intelligence Applications in Radioiodine Therapy - A Systematic Review.

UNLABELLED: Dietary phytochemicals exhibit anticancer activity, and increasing evidence suggests that the combinations of co-occurring compounds may provide greater therapeutic efficacy than individual compounds. Eruca sativa Mill. (rocket salad) contains both erucin and kaempferol; however, their synergistic anticancer activity against non-small cell lung cancer (NSCLC) has not been systematically investigated. Network pharmacology, molecular docking, and molecular dynamics (MD) simulations were performed to identify molecular targets of erucin and kaempferol. Their synergistic anticancer act

Temporal Declines in Accuracy of the Donor Risk Index for Liver Transplantation.

INTRODUCTION: The Liver Donor Risk Index (LDRI) has long served as the principal tool for quantifying donor risk in liver transplantation, integrating donor and graft characteristics to estimate graft failure. However, shifts in donor demographics and the widespread adoption of machine perfusion technologies may have reduced its relevance to contemporary practice. We therefore evaluated the contemporary predictive performance of the LDRI for 90-d graft survival. METHODS: Using the UNOS registry, we examined 49,220 liver transplant recipients across three time periods, replicating the methodol

Posttraumatic growth in parents of children who underwent liver transplantation: a cross-sectional study.

BACKGROUND: Confronted with the traumatic experience of a child’s liver transplantation, parents may experience positive psychological changes alongside distress. This study aimed to: assess the level of posttraumatic growth (PTG) in this population, and identify factors associated with PTG, focusing on deliberate rumination, intrusive rumination, and social support. METHODS: This cross-sectional study involved parent-caregivers of young children (≤ 3 years) after their first liver transplantation at a Chinese tertiary hospital (May 2023–June 2024). Data were analyzed using SPSS 25.0. Group co

Mitochondrial Transplantation in the Eye: A Review and Evaluation of Surgical Approaches.

PURPOSE: Mitochondrial dysfunction contributes to major blinding diseases, including age-related macular degeneration and glaucoma. Although mitochondrial transplantation has shown therapeutic potential in multiple organ systems, translation to the eye remains limited, partly due to uncertainty regarding optimal delivery. We summarize the biologic rationale and preclinical evidence supporting ocular mitochondrial transplantation and present feasibility data evaluating clinically relevant delivery routes. METHODS: We conducted a focused narrative review of ocular mitochondrial transplantation.

Candidates for liver transplant with high-frequency deceased donor offers.

Changes in organ allocation, technology, and regulatory policy have led to significant increases in deceased donor liver offers for transplantation. Expansion of potential donors is associated with inefficiencies, including increased liver nonutilization, resources, and complications. We evaluated the contribution of individual waitlisted candidates to the national allocation of deceased donor liver offers to understand characteristics of candidates with numerous offer turndowns. We performed an observational cohort study of the Scientific Registry of Transplant Recipients, including deceased

Chronic postsurgical pain after living donor hepatectomy: Pay attention to donor features.

Postoperative pain lasting longer than 90 days is defined as chronic postsurgical pain (CPSP) and is known to be a long-term bothersome problem for surgical patients. The study presents the incidence, characteristics, and risk factors of CPSP in living donor hepatectomy (LDH). This was a single-center, retrospective study using the institution's electronic files and the CPSP questionnaire completed during the telephone interview. Of 123 patients assessed, 114 (92.6%) had acute postoperative pain, 15 (12.2%) reported CPSP, 11 (8.9%) reported ongoing pain, and 3 (2.4%) indicated that pain was th

Incidental intrahepatic cholangiocarcinoma detected in the explant liver after living-donor liver transplantation for polycystic liver disease.

Living-donor liver transplantation (LDLT) is considered for polycystic liver disease (PLD) with portal hypertension or quality of life deterioration. However, concomitant malignant tumors are extremely rare in PLD. The recipient was a 68-year-old female who had been followed for PLD. She developed progressive jaundice with ascites and lower leg edema. Computed tomography revealed portal hypertension with intrahepatic portal vein stenosis and splenorenal shunt. The liver was replaced by numerous cysts, but no obvious tumorous lesions were identified. Tumor markers were CEA of 7.9 ng/ml and CA19

Mechanism of activation of an ancestral TEC kinase by PIP3.

BACKGROUND: The introduction of second-generation antipsychotics (SGAs) over the past 2 decades generated considerable optimism that better antipsychotic treatments for schizophrenia and bipolar disorder were possible. SGAs offer several tolerability benefits over first-generation antipsychotics (FGAs), particularly with respect to extrapyramidal symptoms. However, SGAs can induce serious metabolic dysregulations, especially in drug-naive, first-episode, and child and adolescent populations, with olanzapine and clozapine having the highest propensity to cause these abnormalities. In this conte

Outcomes of small-size graft in highly urgent living donor liver transplantation: Korean national data.

BACKGROUND: The major concern for living donor liver transplantation (LDLT) is the selection of donor graft size because small grafts are often unable to meet metabolic demands or fail to function after implantation in highly urgent LDLT (HU-LDLT). We aim to compare the outcomes between the small graft volume group (graft-to-recipient weight ratio, GRWR <0.8%) and the non-small graft volume group (GRWR ≥0.8%) in the national data of HU-LDLT cases in Korea. METHODS: We conducted a retrospective analysis of Korean Network of Organ Sharing (KONOS) data involving consecutive HU-LDLT patients betw

Living donor liver transplantation in 2024.

Living donor liver transplantation (LDLT) has been, and continues to be, an excellent solution for many patients worldwide. Although the procedure poses significant risks for the liver donor due to its complexity, the challenge of obtaining cadaveric donors in time to prevent mortality on the waiting list makes this type of liver transplantation (LT) very common in many countries.

Shortening the recipient warm ischemia time could be a strategy for expanding the liver donor pool.

BACKGROUND: Shortening the recipient warm ischemia time (rWIT) has been proven to be effective for improving the short- and long-term outcomes after liver transplantation (LT) and offsets the negative impact of an extended cold ischemia time. However, few studies have been conducted to explore the prognostic effects of shortening the rWIT in transplantations using a liver graft from an extended-criteria donor (ECD). AIM: To investigate whether shortening the rWIT could improve the outcomes of ECD LT. METHODS: Rat ECD autologous orthotopic LT were performed with variable rWITs (0, 10, 20, and

Evaluation of Controlled Attenuation Parameter as a Tool for Assessment of Hepatic Steatosis in Living Liver Donors.

BACKGROUND: Currently, there is an absence of a standardised protocol for the preoperative detection of hepatic steatosis (HS) in living liver donors. A steatotic liver graft jeopardises the outcome of the recipient with multiple potential complications. Vibration-controlled transient elastography (Fibroscan®) provides a controlled attenuation parameter (CAP), which we have utilised in our assessment of HS in living liver donors. This approach offers a promising avenue for the advancement of preoperative evaluation protocols. METHODS: In the period spanning from October 2022 to July 2024, a c

Geospatial analysis of liver donation after death by drug intoxication.

Increases in deaths from drug intoxication in the United States could be contributing to more liver donations. This study investigates regional variation in liver donation following death by drug intoxication over a decade. The number of drug intoxication-related deaths in the continental United States (2011-2020) was collected from CDC WONDER. Reports from UNOS provided the number of liver donors who died of drug intoxication over the decade. County-level ratios of liver donors after drug intoxication to the total number of drug intoxication-related deaths were calculated. Missed donation opp
