Expanding the Liver Donor Pool: Promise and Peril.

Liver transplantation remains a life-saving therapy for a growing list of indications. Although 10,660 adult liver transplants were performed in the United States in 2023, a 50% increase over the preceding decade, the demand continues to far exceed the supply. Efforts to expand the liver donor pool by using donors that were previously considered unsuitable have remained an important strategy to help overcome shortages. We discuss the progress that has been made over the past decade, as well as potential future barriers that will need to be overcome to help successfully expand the liver donor p

Innovations to Expand the Liver Donor Pool: Machine Perfusion and Xenotransplantation.

The number of patients awaiting liver transplant exceeds the number of liver grafts available. However, emerging technologies offer hope. Machine perfusion enhances the preservation, graft quality, and utilization of marginal livers, thereby reducing unnecessary graft discards. Xenotransplantation provides an alternative organ source, augmenting the donor supply or serving as a bridge for critically ill patients. These innovations are described in this review, as the recent clinical applications of these technologies promise to alleviate organ scarcity, improve transplant outcomes, and save li

Heparinase I treatment to overcome RNA quantification interference in heparinized liver donor samples: One size fits all?

BACKGROUND: MicroRNAs have emerged as potential biomarkers of liver injury during organ transplantation due to their specificity, easy detection and stability in many biofluids. Heparin, which has a well-known inhibitory effect on RT-qPCR based measurements, is commonly used during organ donation. Heparinase I treatment has been used to overcome the inhibiting effect of heparin in RNA RT-qPCR analysis. However, there is a lack of evidence regarding its effective, feasible use improving specific miRNA quantification yield in the liver transplant setting. The aim of this study is to evaluate the

Haemoadsorption cartridge connected to the machine perfusion for donation after circulatory death porcine liver marginal grafts.

BACKGROUND: Marginal donation after circulatory death (DCD) liver grafts are carefully used to combat the constant shortage of donors. Clinically, the worst outcomes are mainly related to severe ischemia-reperfusion-injury and the dangerous effect of various inflammatory cytokines (CK). The machine perfusion (MP) is a promising device to rescue these grafts. AIM: To analyze the role of MP connected to a sorbent cartridge (PerSorb®) and used for very damaged DCD pig livers. METHODS: Seven grafts were procured from pigs from a slaughterhouse. Grafts were made very marginal with at least 60 min

Evaluation of Living Liver Donors Using Dual-Contrast-Agent Magnetic Resonance Imaging (MRI) and Cholangiopancreatography (MRCP)-Technical Development.

The indications for liver transplantation continue to expand to meet the growing need of patients with end-stage liver disease and select hepatic malignancies. Living donor liver transplantation allows for access to transplant with recipient outcomes superior to deceased donor liver transplantation. To ensure absolute safety of the donor and optimal outcome of the recipient, potential liver donors are subjected to an exhaustive preoperative evaluation. Radiological studies play a pivotal role in assessing the donor liver anatomy to assess suitability for donation and to facilitate surgical pla

Comparing donor selection barriers for living donor liver transplantation among Hispanic and non-Hispanic candidates in a large urban center.

Hispanics represent the largest-growing ethnic group awaiting liver transplantation. Though racial/ethnic barriers to living donor liver transplantation have been described among recipients, little is known about challenges for minority living liver donor (LLD) candidates during donor evaluation. We aimed to characterize LLD candidates and compare donor-related barriers by ethnicity through a single-center retrospective cohort study of all LLD referrals from December 2017 to August 2021. The primary outcome was LLD approval with OR estimated using multivariable logistic regression. Among 1508

The Relationship Between Preoperative Cyberchondria Levels and Death Anxiety of Liver Donors.

This descriptive and correlational study aimed to investigate the relationship between preoperative levels of cyberchondria and death anxiety in liver donors. It was conducted with 158 liver transplant donors hospitalized in the organ transplant clinics of a Liver Transplant Institute in eastern Turkey. The total score for the death anxiety scale was high (9.97 ± 2.29), while the cyberchondria scale had a moderate mean total score (44.98 ± 5.86). A moderately significant positive correlation was observed between the total scores of the death anxiety and cyberchondria scales (r: 0.427, p: 0.000

Impact of wellbeing and healthcare resources on deceased liver donation in Mexico.

In Mexico, the rates of donation after brain death (DBD) and the use of deceased donor livers (DDL) are low compared to other countries. We aimed to analyze the effect of the population well-being and healthcare resources of each State on donation trends and compare the States of Mexico to countries with similar characteristics. Using publicly available databases, we analyzed the number of DBD and DDL cases per million people (pmp), liver donor conversion rate (LDCR), and population and economic variables for each State between 2007 and 2023. Panel data multivariate regression analyses of the

Effect of CYP3A4 Methylation on Tacrolimus Pharmacokinetics.

BACKGROUND: Common genetic variants in CYP3A4 together only explain a limited amount of the variability in tacrolimus clearance. This cross-sectional study aimed to explore the extent to which pharmacokinetic variability can be explained by methylation of the CYP3A4 gene. METHODS: Residual tissue material from liver biopsies routinely collected 6 months post-transplantation was used. Inclusion criteria were tacrolimus once daily (Advagraf) in a steady state (ie, no dose change in the previous 3 days); assessment of tacrolimus pharmacokinetics within 3 weeks of the biopsy; and no documented ep

Liver Transplantation Without Systemic Antifungal Prophylaxis-An Exceptional Perspective from a Single Center Experience.

Background: Invasive fungal infections (IFIs) after liver transplantation (LT) remain a concern. No universal protocol for antifungal prophylaxis in LT exists. Antifungal prophylaxis varies across European centers. Studies suggest risk stratification for prophylaxis. This study assessed IFI frequency and outcomes in adult LT recipients without antifungal prophylaxis and evaluated risk stratification for predicting IFIs. Method: A retrospective analysis of clinical and microbiological data from 244 liver transplant patients focused on IFI within 100 days post-transplantation. Of these, 225 (92%

Liver MRI proton density fat fraction inference from contrast enhanced CT images using deep learning: A proof-of-concept study.

Metabolic dysfunction-associated steatotic liver disease (MASLD) is the most common cause of chronic liver disease worldwide, affecting over 30% of the global general population. Its progressive nature and association with other chronic diseases makes early diagnosis important. MRI Proton Density Fat Fraction (PDFF) is the most accurate noninvasive method for quantitatively assessing liver fat but is expensive and has limited availability; accurately quantifying liver fat from more accessible and affordable imaging could potentially improve patient care. This proof-of-concept study explores th

Thrombophilia in Liver Donors: Prevalence of Protein C or S Deficiency in Korean Candidates and Decision Considerations.

BACKGROUND: Donor thrombophilia poses risks to both donors and recipients. To address donor safety without unnecessarily decreasing the donor pool, we aimed to determine the incidence of protein C (PC) or protein S (PS) deficiency among living liver donor candidates and propose a decision-making algorithm for donation. METHODS: We retrospectively reviewed 1421 living donor candidates who underwent primary donor evaluation at a single center between January 2017 and February 2022. Donors with PC or PS deficiency were divided into severe (< 40%), moderate (40%-49%), and mild (50%-59%) deficienc

Evaluation of living liver donors identifying as transgender or gender diverse.

Transgender and gender diverse individuals face barriers to accessing equitable healthcare. As the liver transplant community works to expand and standardize the living donor evaluation, it is imperative that we consider the unique risk stratification considerations for this marginalized population. Herein, we review the nuances unique to psychosocial and medical considerations, including gender-affirming hormone therapy and perioperative management, with a focus on creating an inclusive environment.
