Early United States experience with donation after circulatory death for heart retransplantation.

BACKGROUND: Donation after circulatory death (DCD) expands the heart donor pool, but outcomes after adult heart retransplantation (re-TX) are not well defined. METHODS: Using UNOS STAR data (January 1, 2019 to October 1, 2025), we studied adults undergoing orthotopic re-TX. Exposure was donor mechanism (DCD vs donation after brain death [DBD]). The primary outcome was 365-day mortality assessed with Kaplan-Meier methods and multivariable Cox regression. We also performed 2:1 propensity-score matching (PSM) using prespecified covariates. RESULTS: Among 474 re-TX recipients, 53 (11.2%) receive

Heart Donation After Controlled Circulatory Death in Noncardiac Surgery Hospitals With a Mobile Heart Team: A Case Series From an Italian Regional Program and Integrated Review of the Literature.

Controlled donation after circulatory death (cDCD) with thoracoabdominal normothermic regional perfusion (TA-NRP) has emerged worldwide as a strategy to expand the heart donor pool. In Italy, however, its implementation is constrained by a particularly restrictive setting, shaped by the mandatory 20-min asystolic no-touch period and by the absence of cardiac surgery units in most donor hospitals. Against this backdrop, the Emilia-Romagna Regional Transplant Network developed a mobile DCD Heart Team capable of performing TA-NRP in peripheral hospitals. We report a case series of 19 cDCD donors

Predicted Heart Mass and Severe Primary Graft Dysfunction in Donation After Circulatory Death Heart Transplant.

BACKGROUND: Donation after circulatory death (DCD) has expanded the US heart donor pool yet carries a higher risk of severe primary graft dysfunction (PGD) for recipients compared to donation after brain death (DBD). This study addresses an important knowledge gap of exploring the role of heart size mismatch in contributing to the risk of severe PGD in DCD-HT. METHODS: All heart-only, adult transplants from the united network for organ sharing (UNOS) database were collected from 09/2023-06/2025. Univariable (UV) and multivariable (MV) analyses were used to identify associations between variou

Development of a novel risk score to predict 1-year mortality following donation after circulatory death heart transplant.

BACKGROUND: Donation after circulatory death (DCD) has expanded the US heart donor pool and yielded similar outcomes for recipients as donation after brain death. However, little work has analyzed what factors contribute to 1-year mortality in DCD nor created a clinical risk score. This work fills these gaps. METHODS: All heart-only, adult DCD heart transplants from the United Network for Organ Sharing database were analyzed. The data was split into a training cohort and a testing cohort. Univariable and multivariable analyses were used to identify 1-year mortality risk factors in the trainin

Outcomes of donation after circulatory death heart transplantation in recipients with pulmonary hypertension.

BACKGROUND: Donation after circulatory death (DCD) has emerged as a strategy to expand the heart donor pool, but its safety in recipients with pulmonary hypertension (PH) remains uncertain. Because PH is a major risk factor for adverse outcomes after heart transplantation, clarifying the impact of DCD donation in this high-risk population is important. METHODS: We analyzed adult primary heart transplant recipients with PH in the United Network for Organ Sharing registry (2019-2024). PH was defined as mean pulmonary artery pressure >20 mm Hg and pulmonary vascular resistance >2 Wood units. Pat

Fatal early graft failure in a heart transplant recipient with occult multivessel coronary artery disease in a young donor: implications for donor evaluation.

BACKGROUND: Primary graft dysfunction (PGD) represents the leading cause of early mortality following heart transplantation. Among the associated factors, donor characteristics-particularly the presence of undiagnosed coronary artery disease-may play a relevant role. Donor evaluation is typically based on clinical assessment and echocardiographic findings, which may be insufficient to detect subclinical coronary disease, especially in young donors. CASE PRESENTATION: We report the case of a 52-year-old patient with advanced dilated cardiomyopathy who underwent orthotopic heart transplantation

Heart Allograft Utilization Rates Following Donation After Circulatory Death Compared to Donation After Brain Death in the Ideal Heart Donor.

BACKGROUND: The number of heart allografts obtained from donation after circulatory death (DCD) remains low, especially compared to donation after brain death (DBD). Our study aimed to identify factors associated with the underutilization of hearts in ideal donors. METHODS: Patient-level data were obtained from the United Network for Organ Sharing (UNOS) database for all adult deceased donors who underwent organ procurement and subsequent transplantation between January 2020 and December 2023. "Ideal" DCD donors were analyzed as a separate subset and compared with ideal DBD donors. Transplant

Superior cardiac protection in combined ex-situ perfusion of heart and liver.

BACKGROUND: Normothermic ex-situ heart perfusion has helped expand the heart donor pool however, it is not optimized yet. Experimental ex-situ organ perfusion incorporating cross-circulation with a live animal has achieved significantly longer safe ex-situ preservation times; however, the underlying mechanisms are not clear. METHODS: In this study, combined ex-situ heart-liver perfusion of porcine hearts was compared to ex-situ isolated heart perfusion. Discarded human donor hearts were also perfused ex-situ and cardiac function and metabolism was studied. RESULTS: The hearts perfused togeth

First Intraoperative Cardiac Catheterization During Donor Normothermic Regional Perfusion.

Heart transplantation continues to be the definitive treatment option for end-stage heart failure, but its availability remains limited by the imbalance between the supply and demand of suitable donor organs. Recent advances in the field have underscored the feasibility of expanding the pool of candidate heart donors, as illustrated by the successes of donation after circulatory death (DCD) and extended criteria donor (ECD) allografts. However, the information available to evaluating centers for coronary assessment in DCD hearts is variable and frequently limited. Here, we present an innovativ

[Heart transplantation from donation after circulatory death: current overview and future challenges].

In the treatment of end-stage heart disease, heart transplantation often represents the only viable option, particularly in terms of survival and quality of life. The evolution and refinement of medical therapy have substantially decreased mortality rates in cardiac patients, enabling them to progress to advanced stages of heart disease and consequently expanding the pool of eligible transplant candidates. A partial response to this growing demand has come from technological advancements in the development of devices designed to replace cardiac function. Meanwhile, the donor pool remains insuf

How Did Denise Darvall Die? A Contribution to the History of the First Heart Transplant.

UNLABELLED: The first heart transplantation, performed by Christiaan Barnard in 1967, was one of the most important medical events of the twentieth century. The real significance of the operation, beyond the surgical feat, was setting a precedent for the reinterpretation of death and introduction of the controversial concept of "brain death." Contemporary public acceptance of this concept was supported by the general belief that, according to the newly created criteria, Denise Darvall, the first heart donor had been brain-dead, and her heart was procured when cardiac arrest occurred after stop

Case report: ST-elevation myocardial infarction complications. How far will you go?

Herein, we review the critical role of a multi-disciplinary team approach in managing the intricate complications of ST-elevation myocardial infarction (STEMI) complicated by cardiogenic shock (CS). Mechanical circulatory support (MCS) implantation represents a potentially life-saving intervention, often serving as a bridge to heart transplantation (HTx). However, complications from prior interventions, in patients receiving MCS due to STEMI-CS, may present additional challenges to successful HTx candidacy. A 63-year-old male suffered out-of-hospital cardiac arrest and was hospitalized due to

Selective Angiography of Stimulant-Exposed Cardiac Donors Following Circulatory Death Does Not Impact Post-Transplant Outcomes.

Background/Objectives: Donation after circulatory death (DCD) has emerged to expand the heart-donor pool, but many DCD donors have risk factors such as cocaine or methamphetamine use. Stimulant use can cause coronary vasospasm and premature coronary artery disease, leading to routine donor coronary angiography (left heart catheterization, LHC) for coronary screening. However, performing LHC in DCD donors is challenging. We examined whether omitting LHC in stimulant-exposed DCD donors affects outcomes. Methods: A retrospective analysis was performed using the United Network for Organ Sharing (U

Early outcomes in patients undergoing heart re-transplantation in the era of HCV-viremic donors.

BACKGROUND: The use of hepatitis C virus (HCV) viremic donors has increased the heart donor pool and reduced waitlist time and mortality among first-time single-organ heart transplant (HT) candidates without compromising their post-transplant survival. The outcomes of heart re-transplantation (re-HT) using HCV-viremic donor grafts have not been investigated. METHODS: Using the United Network for Organ Sharing Registry, we analyzed adult patients who underwent re-HT in the USA between January 1, 2016, and December 31, 2023. HCV-viremic donors were defined by a positive nucleic acid amplificati

Risk Index Predicts Pediatric Heart Allograft Non-Utilization.

BACKGROUND: Children listed for heart transplantation face the highest waitlist mortality among all solid organ transplant patients (14%). Attempts at decreasing donor allograft non-utilization (41.5%) could potentially decrease waitlist mortality for pediatric heart transplant patients. Our aim was to quantify the non-utilization risk of pediatric donor heart allografts at the time of initial offering. METHODS: Using the United Network of Organ Sharing (UNOS) database, we retrospectively analyzed 8823 deceased donors (≤18 years old) data through univariable and multivariable analysis and log

Right ventricular dysfunction in left ventricular assist device candidates: is it time to change our prospective?

The use of left ventricular assist devices (LVAD) has significantly increased in the last years, trying to offer a therapeutic alternative to heart transplantation, in light also to the significant heart donor shortage compared to the growing advanced heart failure population. Despite technological improvements in the devices, LVAD-related mortality is still fairly high, with right heart failure being one of the predominant predictors. Therefore, many efforts have been made toward a thorough right ventricular (RV) evaluation prior to LVAD implant, considering clinical, laboratory, echocardiogr

Metabolomic insights in advanced cardiomyopathy of chronic chagasic and idiopathic patients that underwent heart transplant.

Heart failure (HF) studies typically focus on ischemic and idiopathic heart diseases. Chronic chagasic cardiomyopathy (CCC) is a progressive degenerative inflammatory condition highly prevalent in Latin America that leads to a disturbance of cardiac conduction system. Despite its clinical and epidemiological importance, CCC molecular pathogenesis is poorly understood. Here we characterize and discriminate the plasma metabolomic profile of 15 patients with advanced HF referred for heart transplantation - 8 patients with CCC and 7 with idiopathic dilated cardiomyopathy (IDC) - using gas chromato
