Different patterns in donor-specific production of T-helper 1 and 2 cytokines by cells infiltrating the rejecting cardiac allograft.

BACKGROUND: Cytokines play an important role in allograft rejection. The local production of cytokines by T-helper 2 cells within an allograft could influence the induction of graft rejection. METHODS: Therefore we studied the in vitro production of cytokines by cells infiltrating the graft. graft-infiltrating cell cultures derived from human endomyocardial biopsy specimens more often produced interleukin-2 (p < 0.001), interferon-gamma (p < 0.001), interleukin-4 (p = 0.02), and interleukin-6 (p = 0.04) after stimulation with a B-cell line obtained from the heart donor than after stimulation

Anaesthesia for heart transplantation.

As the number of heart transplantation recipients continues to grow, more cardiac centres are performing heart transplantation and more anaesthetists need to be trained for this exciting and challenging field of anaesthesia. This articles looks at the anaesthetic management of both the heart donor and the heart recipient.

Matching the heart donor and heart transplant recipient. Clues for successful expansion of the donor pool: a multivariable, multiinstitutional report. The Cardiac Transplant Research Database Group.

Little information is available regarding donor-specific parameters that predict success or failure after heart transplantation. Furthermore, with increasing numbers of patients awaiting heart transplantation, there is tremendous pressure to expand the donor pool by stretching the margins of donor acceptability. To gain insight into donor-related and donor-recipient interrelated predictors of death after transplantation, 1719 consecutive primary transplantations performed at 27 institutions between Jan. 1, 1990, and June 30, 1992, were analyzed. Mean follow-up of survivors was 13.9 months, and

[Indications for heart transplantation].

Heart transplantation may have a considerable impact on the fate of the patient in the end stage of heart failure. A successful procedure requires the existence of a sophisticated programme combining the expertise of top departments of cardiology and cardiac surgery working in conjunction with the transplant centre directly responsible for removing the organ. One of the factors playing an important role in a favourable outcome is the correct indication for the procedure. The information on patient selection and criteria for the heart donor, presented in this paper, become increasingly importan

Why referred potential heart donors aren't used.

Heart donor demand far exceeds supply. We evaluated donor referrals to 1 organ procurement agency in an attempt to determine why many potential cardiac donors are not used. Of 430 referrals between September 1989 and August 1991, 169 hearts (39%) were harvested. In potential donors ultimately not yielding a heart, 38.7% were unavailable because the family refused to consent to organ donation, 36% were medically unsuitable, and 16.1% did not meet standard brain death criteria. Of the 94 donors not used for medical reasons, 43.6% had cardiac arrest, 17% had hypotension, 12.8% were drug abusers,

Patients who die awaiting heart transplantation.

In the United States, heart donor availability has increasingly failed to keep pace with rising demand. Transplant data were obtained from the United Network for Organ Sharing for 1988, 1989, and 1990 and by survey of 50 heart transplantation centers, which performed 1932 transplantations between 1983 and 1989. According to the United Network for Organ Sharing, 512 patients on the waiting list died in 1988; 527, in 1989; 650, in 1990, and if present trends continue more than 800 will have died in 1991. Similar numbers of patients were inactivated or removed from the list each year. Only 49% of

Expression of proliferating cell markers in normal and diseased human hearts.

Proliferating cell nuclear antigen (PCNA) myocyte expression and histopathologic features related to its occurrence were investigated in normal and diseased hearts of adult humans using both immunohistochemical and Western blotting techniques. Ki67 Western blotting was also performed in the same samples used for PCNA blotting. Two hundred seventy-one endomyocardial biopsies, and 15 adult, 1 embryonic and 2 fetal hearts were studied. The biopsies were from normal donor hearts (n = 71), patients with cardiomyopathy and myocarditis (n = 64), and patients with transplantation with (n = 106) and wi

Brain death in early infancy.

Guidelines for the determination of brain death in infants and children are now well established. Coma, absence of cranial nerve reflexes, and apnea are required in all patients. In children less than 1 year of age, supportive neurodiagnostic studies (electroencephalograms and cerebral blood flow) are recommended. Data on 52 pediatric heart donor patients were reviewed to assess whether current criteria and procedures were used in the determination of brain death. The mean age of the donors (14.3 months) was higher than that of the recipients (2.6 months). In all patients the hospital records

Brain death in the pediatric patient: historical, sociological, medical, religious, cultural, legal, and ethical considerations.

OBJECTIVE: To detail the origins of the definition of death, the development of the criterion of whole brain death as fulfilling the definition of death, and the tests used to fulfill that criterion. DATA SOURCES: A review of the literature was performed. No Institutional Review Board approval was necessary. DATA EXTRACTION: In 1959, patients were described as being in "coma dépassé" or beyond coma. In 1967, the first successful heart transplantation took place, with the organ coming from a brain-dead, beating-heart donor. However, anxiety over the definitions of death did not begin with the

The role of interventional cardiology in pediatric heart transplantation.

Hypoplastic left heart syndrome (HLHS) is the most common cause of cardiac death during the first week of life. The prognosis of this fatal malformation has changed completely since the introduction of neonatal heart transplantation. However, patients awaiting a heart donor are at high risk for closure of the ductus arteriosus and are exposed to complications from indwelling central venous catheters and long-term use of prostaglandin E1. To maintain the ductal patency, which is crucial for the survival of these patients, we performed a stenting of the ductus arteriosus in six infants with HLHS

Electrophysiological effects of atracurium and vecuronium on normal and denervated hearts.

Atracurium and vecuronium offer some advantages with respect to other nondepolarizing muscle relaxants. However, their administration has been associated with episodes of bradycardia and asystole, the mechanism of which has not yet been clarified. In an effort to analyze the effects of these drugs on automaticity, refractoriness, and conduction, 30 dogs underwent heterotopic heart transplantation to establish a model in which a normal heart (recipient) and a denervated heart (donor) could coexist. The electrophysiological studies included the determination of cycle length (CL), sinoatrial cond

Effect of fentanyl on cardiac automaticity and conduction: direct or mediated action?

The effects of fentanyl at a dose of 100 micrograms.kg-1 on cardiac automaticity, refractoriness and conduction have been studied in a model of heterotopic heart transplantation which combines a denervated heart (donor) and a non-denervated heart (recipient) in each of 20 dogs employed. Once the surgical procedure had been completed, cycle length, sinoatrial conduction time, antegrade and retrograde A-V node block points, cycle length at which 1:1 conduction ceases to exist and the antegrade effective refractory period of the A-V node and ventricles were measured. These parameters were determi

The effects of donor-recipient size disparity in infant and pediatric heart transplantation.

To determine the effect of heart donor and recipient size mismatches in infant and pediatric heart transplantation, we studied all 69 patients (age 1 day to 11 years) having 71 orthotopic heart transplants from 1985 to 1989. Patients were divided into three groups based on donor to recipient weight ratios. Group I comprised 13 heart transplants with a donor to recipient weight ratio less than 0.95 (mean 0.81, range 0.48 to 0.94); group II comprised 29 heart transplants with a weight ratio between 0.95 and 1.60 (mean 1.28); and group III had 27 heart transplants with weight ratios greater than

[Controversies in the selection of donors in heart transplants].

The shortage of ideal donor hearts has led to an increasing number of would-be heart transplant recipients who die while on the waiting list: an extension of the classical donor criteria seems necessary to expand the donor pool. In this paper, the major controversies on donor selection are presented, along with the clinical results obtained in cardiac transplantation with donor grafts not complying with standard criteria. The extension of donor age over 35 years is supported by the decreased incidence of coronary artery disease (CAD) in the Western world. In our experience, the number of donor

Successful use of the "unacceptable" heart donor.

Chronic shortage of donor organs has heightened interest in new strategies for increasing donor availability. Unacceptable hearts for transplant have previously been characterized by donor age greater than 40 years, more than 20% donor/recipient weight mismatch, ischemic time more than 4 hours, and the presence of coronary artery disease. A series of 185 consecutive orthotopic heart transplants were retrospectively examined. A significant number of donor hearts used were unacceptable by one or more of the above criteria. Our current approach is to match donors to recipients using a wide range

[The role of electrocardiographic examination in the complex evaluation of potential heart donors].

From October, 1986 to February, 1990 eighty-five potential donors with death of the brain were examined at the Scientific Research Institute of Transplantology and Artificial Organs, USSR Ministry of Public Health. Thirty of them were used as heart donors in orthotopic transplantation of the heart; 24 (80%) were males and 6 (20%) females. The cause of death of the brain was craniocerebral trauma in 13, brain tumor in 9, hemorrhage into the brain tissue in 7 cases, and gunshot injury to the skull with damage to the brain in one case. In appraisal of the donors; in 8 (26.7%) cases the ECG did no

Left Ventricular Assist Device.

Left ventricular assist device (LVADs) have been used with increasing frequency since the late 1980s. Technology has improved these devices to make them less thrombogenic and has decreased their size to allow greater application. By definition, the LVAD only assists or supports the pumping function of the left ventricle, unlike cardiopulmonary bypass which provides total circulatory and pulmonary function. LVADs support the circulation for hours to days and occasionally for weeks when the left and/or right ventricle has failed and is therefore unable to sustain the circulation. The hope is tha

[The effect of donor conditioning with cyclosporin on the survival time of allogenic heart transplants in the rat].

The aim of the study was to investigate the influence of a pretreatment of the heart donor with cyclosporin (CsA) at 25 or 50 mg kg BW on day -1 on survival time of rat heart allografts in a donor-recipient combination that was different at their major histocompatibility complex (MHC). No further immunosuppression was used. The survival of CsA-pretreated heart transplants was significantly prolonged in allogeneic recipients compared with untreated controls and 3 out of 10 grafts (25 mg kg) and 4 out of 9 grafts (50 mg kg) survived permanently. These preliminary experimental findings are new, b

Prolonged rat heart allograft survival and reduced graft immunogenicity after photochemical donor and transplant pretreatment.

The purpose of these experiments was to evaluate the influence of pretreatment of the heart donor with the photosensitizer 8-methoxypsoralen plus ex vivo longwave ultraviolet irradiation of the graft (PUVA) on survival time and immunogenicity of rat heart allografts in a donor-recipient combination that was different at the major histocompatibility complex (MHC). PUVA treatment of the donor hearts significantly prolonged their survival in allogeneic recipients. The efficacy of this therapy was dependent on the time of ultraviolet irradiation. Cardiac transplant recipients with preoperative pulmonary hypertension. Evolution of pulmonary hemodynamics and surgical options.

Among 48 consecutive patients with pretransplant pulmonary vascular resistance (PVR) greater than 4 Wood units, 38 patients underwent orthotopic heart replacement (OHT), and the remaining 10 received a graft in a heterotopic position (HHT). The OHT recipients were smaller (63 vs. 73 kg, p less than 0.05) and received a larger donor heart (donor-recipient, 109% vs. 79%, p less than 0.001) with a shorter graft ischemic time (108 vs. 139 minutes, p less than 0.05) than HHT recipients, reflecting patient selection and surgical complexity.
