A scoping review of the present condition of organ donation among Chinese citizens and its influencing factors.

OBJECTIVE: This study aims to systematically analyse the current status of post-mortem organ donation among Chinese citizens and its influencing factors through a scoping review. It also seeks to identify the characteristics and gaps in existing research and provide evidence to inform policy directions for strengthening China's organ donation system and improving organ donation practices. METHODS: Guided by the scoping review methodology framework proposed by Arksey and O'Malley, a systematic search and screening of Chinese and English databases-including PubMed, Web of Science, Embase, CNKI,

Challenges in HLA and ABO typing of deceased organ donors with prior hematopoietic stem cell transplantation: two cases and practical considerations for evaluation.

BACKGROUND: Prior allogeneic hematopoietic stem cell transplantation (HSCT) can complicate HLA and ABO typing of deceased organ donors because circulating hematopoietic cells and organs may originate from different sources. CASE PRESENTATIONS: We describe two deceased organ donors with prior HSCT. In Case 1, historical records demonstrated discordance at HLA-C and HLA-DQ between the HSCT donor and the constitutional pre-HSCT typing. Peripheral-blood HLA typing reflected the HSCT donor, while a kidney blood-vessel specimen yielded four DPB1 alleles from mixed sources, permitting only a presump

Tissue-Specific Isolation of Hepatocyte Extracellular Vesicles From Human Plasma Enables Tissue-Resolved Proteomic and miRNA Profiling.

Tissue-specific resolution remains a critical barrier in liquid biopsy for precision medicine. To address this, we developed NEXPLOR (Novel EXtracellular vesicle PopuLation Omics Revealer), an immunoaffinity platform enabling selective enrichment of circulating tissue-specific extracellular vesicles (TS-EVs), demonstrated here for liver-derived hepatocyte-EVs (h-EVs). Through in silico discovery and orthogonal antibody validation across tissues, we identified a four-marker panel (ASGR1, ASGR2, TFR2, SLCO1B1) that selectively enriched hepatocyte EVs without co-capturing the dominant platelet- a

Human lung organoid modelling of tissue-resident antiviral T cell responses.

Tissue-resident immunity constitutes a first line of defence against pathogens and enables rapid innate and adaptive memory responses1-3. However, experimental studies have been hindered by a lack of holistic human in vitro models that enable epithelial infection in the context of tissue-resident immune subsets. In lung, differing notions of transient versus sustained residency of tissue-resident memory T cells (TRM cells) have challenged the extent to which anamnestic recall immunity to respiratory pathogens occurs locally or in concert with secondary lymphoid organs4-6. Here we generated lon

Contribution of donor hospital characteristics to deceased donor organ recovery rates.

The Centers for Medicare and Medicaid Services evaluates Organ Procurement Organizations (OPOs) based on donation and transplant rates within their donation service area, but individual hospital-level characteristics may also significantly impact organ recovery. We examined how hospital-level factors influence recovery rates using 2023 data from the Scientific Registry of Transplant Recipients database, the Centers for Medicare and Medicaid Services Hospital Provider Data, and Cost Reports. A hierarchical negative binomial model with OPO random intercepts was used to evaluate hospital-level or

Donor Dilemma: A National Simulation-Based Case Curriculum for Teaching Donor Call Decision-Making in Infectious Diseases Trainees.

INTRODUCTION: Donor-call decision-making during organ allocation requires clinical reasoning in the setting of uncertainty, time pressure, and evolving information. In transplant infectious diseases (TID), this skill is essential, yet exposure during training is often inconsistent and informal. Educational strategies that allow trainees to practice this type of decision-making remain limited. We aimed to evaluate the feasibility, acceptability, and perceived impact of an asynchronous electronic, case-based progressive disclosure curriculum designed to support clinical reasoning during donor-ca

Transforming Growth Factor-β1 Drives Neutrophil Chemokinesis, Not Chemotaxis.

PURPOSE: Idiopathic pulmonary fibrosis (IPF) is a devastating condition that is characterized by dysregulated repair responses to alveolar epithelial damage and progressive remodeling of the lung interstitium. Neutrophils are critical responders to tissue injury, but their role in organ fibrosis pathogenesis remains unclear. Neutrophil numbers are increased in bronchoalveolar spaces of IPF patients, and neutrophil activation markers are elevated in fibrotic lung tissue. However, the key mediators driving neutrophil movement in fibrotic lung diseases are unknown. Transforming growth factor-beta

Who should provide the uterus? The ethics of live donor recruitment for uterus transplantation.

Uterus transplantation (UTx) is an experimental surgery likely to face the issue of organ shortage. In my article, I explore how this issue might be addressed by changing the prevailing practices around live uterus donor recruitment. Currently, women with children - often the mothers of recipients - tend to be overrepresented as donors. Yet, other potentially eligible groups who may have an interest in providing their uterus - such as transgender men, or cisgender women who do not wish to gestate or to have children - tend to be excluded as potential donors. Moving forward, I recommend that do

Does normothermic regional perfusion harm donors after circulatory death?

Normothermic regional perfusion during controlled donation after circulatory death has emerged as a means to increase the number and viability of organs available for transplant. Because normothermic regional perfusion uses extracorporeal membrane oxygenation, an intervention used for resuscitation under other circumstances, critics have concluded that organ donation using normothermic regional perfusion violates the dead donor rule. As such, the debate about normothermic regional perfusion has been framed as a binary choice between normothermic regional perfusion and the dead donor rule. In t

Static cold storage at 10 °C mitigates the influence of advanced donor age on heart transplant recipient outcomes.

OBJECTIVE: Advanced donor age has been associated with inferior outcomes after heart transplantation. Recent use of a 10 °C static cold storage has been shown to improve heart transplant recipient outcomes compared with traditional ice storage. This study evaluates the pertinence of donor age on posttransplant outcomes in cardiac allografts stored at 10 °C. METHODS: All adult heart transplant recipients, whose allografts were preserved with either 10 °C or ice static cold storage from a single institution from June 2023 to December 2024 were reviewed. Recipients receiving a multiorgan transpl

Performing Under Pressure: Maintenance of Donor Lung Pressure During Cabin Depressurization.

INTRODUCTION: Maintenance of donor lung pressure is recommended to maintain inflation pressures of 12-15 mm Hg. This is particularly important given graft transportation in aircraft, due to the inevitable pressure changes. CASE: We describe a case of a donor lung preservation system which maintained proper airway pressure during sudden cabin pressure loss requiring emergent landing. DISCUSSION: The donor lungs were implanted without complication. Post-transplant course was notable for primary graft dysfunction (PGD) grade 1 at 24 hours, grade 2 at 48 hours, and grade 1 at 72 hours. At 3 mont

Sensitization in organ transplantation: Assessment of Risk (STAR) 2025 meeting group report.

The Sensitization in Transplantation: Assessment of Risk initiative aimed to translate human leukocyte antibody (HLA) laboratory data into clinical practice and identify key knowledge gaps to guide future research. The Sensitization in Transplantation: Assessment of Risk working group has held 3 consensus meetings and published reports that have influenced clinical care. In 2025, the group focused on 5 evolving areas: donor-derived cell-free DNA tests, innate immunity in allograft rejection, immunogenicity and antigenicity, HLA antibody quantification, and non-HLA antibodies. Should the Current Zero HLA ABDR-Mismatch Priority in Kidney Allocation System Continue?: A Critical Appraisal.

KEY POINTS: Zero HLA ABDR mismatch priority in the Kidney Allocation System is highly biased against minority candidates. Zero HLA ABDR mismatch priority shunts 2.5% of O donors to nonidentical blood types disadvantaging O recipients. Zero HLA ABDR-mismatch priority recipients have substantially shorter dialysis exposure before transplant than nonpriority recipients. BACKGROUND: In the Kidney Allocation System (KAS), adult kidney candidates who are a zero HLA ABDR-mismatch with a potential donor are given priority in the match run over nonzero HLA ABDR-mismatch candidates. Furthermore, KAS al

Association of Organ Preservation Methods With Von Willebrand Factor Upregulation in Microvascular Endothelial Cells Posttransplantation: Ex Vivo Lung Perfusion and Static Cold Storage.

BACKGROUND: Von Willebrand factor (VWF) is a procoagulant glycoprotein expressed exclusively in endothelial cells and megakaryocytes. It mediates platelet adhesion to endothelial/subendothelial surfaces and initiates thrombogenesis. External stimuli, including hypoxia, were shown to upregulate VWF expression levels and alter its vascular tree expression pattern in the lung. Increased VWF levels are a significant risk factor for thrombus formation, a major complication in organ transplantation. Given that donor organs experience hypoxic conditions during transplantation, this study investigates

Stack: In-Context Learning of Single-Cell Biology.

Single-cell transcriptomics offers the promise of measuring the diversity of cellular phenotypes across species, diseases, and other biological conditions. Recently, foundation models have emerged to identify this variation, yet most methods represent each cell independently, despite technical limitations that reduce measurement precision at the single-cell level. Here, we present Stack, a foundation model trained on 149 million uniformly preprocessed human single cells that leverages tabular attention to generate representations for each cell informed by the cells in its context. Stack offers

Common tissue-specific expressions and regulatory factors of c-KIT isoforms with and without GNNK and GNSK sequences across five mammals.

c-KIT is a transmembrane receptor tyrosine kinase involved in various signaling pathways. Alternative pre-mRNA splicing of KIT results in isoforms that differ in the presence or absence of four amino acid sequences in the extracellular juxtamembrane region, such as, isoforms with and without the GNNK sequence (GNNK+ and GNNK - , respectively) in humans and mice, and those with and without GNSK (GNSK+ and GNSK - , respectively) in domestic dogs, cats, and sheep. These isoforms have been extensively studied as disease-associated (particularly tumors or cancer) splice variants with differing kina

Single-cell atlas of the transcriptome and chromatin accessibility in the human retina.

Single-cell sequencing has revolutionized the scale and resolution of molecular profiling of tissues and organs. Here we present an integrated dual-modal reference atlas of the most accessible portion of the mammalian central nervous system, the retina. We compiled around 3.9 million cells from 125 donors of diverse ancestral backgrounds, including 8 published studies and 2.7 million unpublished data points, to create a comprehensive human retina cell atlas (HRCA) with more than 130 cell types identified. We annotated each cluster, identified marker genes and characterized cis-regulatory eleme

[Cross-border organ exchange programs in the Nordics allow more lives to be saved].

Survival for children on the waiting list has improved significantly over time in Sweden and the Nordic countries, despite an increase in waiting list duration over time. Approximately 40% of hearts used for pediatric heart transplantation in Sweden come from donors in other Nordic countries. Outcomes following pediatric heart transplantation in Sweden and the Nordic countries are comparable to much larger regions and registries, despite a low number of yearly transplants in the region.
