Allocation out-of-sequence (AOOS) refers to deviation from the match run order in deceased donor organ allocation and is intended for use in exceptional cases to expedite organ placement. Contemporary patterns of AOOS in liver transplantation are not well characterized. This study aimed to describe liver AOOS practices and examine their association with organ utilization. Liver match runs from 2019 to 2024 were analyzed using the Organ Procurement and Transplantation Network potential transplant recipient data set to identify AOOS events. Data from the Scientific Registry of Transplant Recipie

Measurement of donor-derived cell-free DNA (dd-cfDNA) enables early, non-invasive monitoring of transplanted organs, including rejection detection. We developed a method to estimate dd-cfDNA ratios using capture hybridization of 300 SNPs, next-generation sequencing (NGS), and clustering analysis. Validation was conducted using simulated mixtures of fragmented genomic DNA from two individuals (0-100%). dd-cfDNA ratios were estimated via clustering, with and without 0% mixture samples to simulate the presence or absence of pre-transplant recipient plasma. When 0% samples were included, estimatio

INTRODUCTION: There has been a surge in attention to ethical controversies associated with organ donation in the USA. The Neurocritical Care Society (NCS) Ethics Committee sought to understand member experiences and attitudes related to organ donation in the USA.
METHODS: We designed a survey of members who practice in the USA and have interacted with an organ procurement organization (OPO)/potential donor. The survey was disseminated by NCS from 1 June 2025 to 1 September 2025. Free-text responses were reviewed to identify themes.
RESULTS: Of 2204 NCS members in the USA, there were 71 respo

Human leukocyte antigen-incompatible (HLA-i) kidney transplantation enables access to transplantation for highly sensitized patients with preformed donor-specific antibodies (DSA) but remains associated with frequent early DSA rebound and alloimmune endothelial injury, resulting in a high incidence of antibody-mediated rejection (AMR) despite current desensitization and induction protocols that include imlifidase. Kidney allograft biopsy remains the diagnostic gold standard for detecting overt and subclinical AMR, yet conventional histology does not always capture early or subclinical antibody

Recently, a growing number of scientific research and clinical studies have demonstrated the potential of extracellular vesicles (EVs) secreted by cells of different types for treating various diseases. It was shown that most frequently, substances and molecules excreted by the cells exert therapeutic or other effects, and not the cells themselves. Their cargo is wrapped in membrane envelopes, allowing it to survive for a longer time and find targets in organs and tissues as well as overcome various barriers, including the blood-brain barrier. EVs from mesenchymal stromal (stem) cells (MSCs) h

IMPORTANCE: Organ procurement organizations are increasingly turning to expedited (ie, out-of-sequence) allocation of deceased donor kidneys, citing maximal utility of kidneys at risk of nonuse.
OBJECTIVE: To identify and compare kidney pairs from the same donor from whom 1 kidney was transplanted in sequence and the other out of sequence.
DESIGN, SETTING, AND PARTICIPANTS: This retrospective cohort study used US kidney transplant registry data obtained from the Scientific Registry of Transplant Recipients to identify kidney transplant donors from whom at least 1 deceased donor kidney was tr

BACKGROUND: Graft rejection following liver transplantation remains a major complication affecting long-term survival. Current monitoring strategies, which rely on liver function tests and invasive biopsies, have inherent limitations, creating a need for non-invasive biomarkers. Donor-derived cell-free DNA (dd-cfDNA) has shown promise for monitoring rejection in solid organ transplantation, but its diagnostic accuracy in liver transplantation requires systematic evaluation.
OBJECTIVE: To comprehensively assess the diagnostic accuracy of dd-cfDNA for rejection after liver transplantation via a

Post-mortem human brain banks are a key resource for researching brain diseases. The New South Wales Brain Tissue Resource Centre (BTRC) is a brain bank that uniquely focuses on the recruitment, preparation, and dissemination of tissue from patients with alcohol use disorder and controls. Our controls are prospectively followed through their lifetime via a self-reported questionnaire and yearly updates, and these standardised data allow future matching to disease cases based on individual study needs. Brain banks are expensive to run, and their sustainability is an ever-present topic of concer

IMPORTANCE: Transfers of deceased organ donors from acute care hospitals to specialized donor care units (DCUs) offer operational and outcome advantages; however, current access to DCUs is limited and geographically uneven. Expanding access to DCUs may improve donation system efficiency.
OBJECTIVE: To evaluate the geographic distribution of operating DCUs relative to acute care hospitals and explore how to most efficiently operationalize recommendations that a DCU operate in every donation region.

Intracardiac mast cells (CMCs) have previously been shown to contribute to adverse remodeling and heart failure in animal models. As CMCs in human hearts remain unexplored, the aim of this study was to investigate the pathophysiological relevance of human CMCs through transcriptomic profiling. Biopsies were collected from the four heart chambers of heart failure patients undergoing heart transplantation surgery (n = 9), as well as from deceased organ donors without chronic heart failure (n = 5). Using flow cytometry, C-kit+CD45+ CMCs and C-kit-CD45+ hematopoietic cells were identified in all f

Vascularized composite allotransplantation (VCA) enables functional and aesthetic reconstruction after complex tissue loss but remains limited by donor scarcity and immunosuppression. Xenotransplantation, using tissues from other species, offers a potential solution. While progress in solid organ xenografts has accelerated, vascularized composite xenotransplantation (VCX) remains largely experimental. This study presents the first systematic review of VCX, tracing its development over the past century and preclinical efforts to overcome immunologic and technical barriers. A systematic review f

INTRODUCTION: Liver transplantation (LT) significantly improves survival and quality of life for patients with end-stage liver disease. Whilst it is believed that need for LT vastly exceeds organ supply, quantifying the demand accurately is challenging. The performance metrics of the transplant system, such as waitlist mortality, may be misleading as they capture only patients who are referred, evaluated and listed for LT. Although means of increasing organ availability are available, they have financial costs and may be technically and ethically challenging (such as live donation and normothe

Liver transplantation : official publication of the American Association for the Study of Liver Diseases and the International Liver Transplantation Society2026eng
Normothermic regional perfusion (NRP) adoption for donation after circulatory death (DCD) liver transplantation in the United States has been growing, but there remains a higher rate of non-utilization of NRP DCD liver grafts as compared with donation after brain death liver grafts. Six transplant centers and 1 organ procurement organization (OPO) with substantial NRP liver procurement experience performed retrospective reviews of all NRP procurement attempts from September 1, 2021, until March 1, 2024, identifying opportunities for utilization. Five of the 6 transplant centers had prospective

BACKGROUND: Tissue-specific cell-free DNA (cfDNA) offers promise as a minimally invasive biomarker of organ injury. However, current methods for cfDNA tissue-of-origin analysis often depend on genome-wide sequencing approaches, limiting their clinical scalability.
METHODS: We developed a streamlined workflow for translating publicly available DNA methylation datasets into PCR-compatible biomarker assays, using kidney-derived cfDNA as a model. DNA methylation microarray data were mined to identify kidney-specific hypermethylated regions, and candidate biomarkers were screened in silico and exp

There is little consensus on how to regulate information giving in reproductive donation - using donated gametes (eggs, sperm) to have children. Should gamete donors be anonymous or should donor-conceived individuals have access to their donor's identity, and at what age? What information should be available about donor siblings and other donor relations? And, crucially, how should this information giving be appropriately managed and regulated? Before we can answer these questions, we need to first understand what reproductive donation is. This paper sets out options for how reproductive donat

INTRODUCTION: Procurement of human spinal cord tissue for research is rare and typically occurs through deceased organ donation pathways. Existing tissue banking guidance, largely derived from surgical pathology practice, does not adequately address the ethical, anatomical, and operational constraints unique to donation-integrated spinal cord procurement.
OBJECTIVES: The objective of this article is to describe institutionally implemented governance and prioritization practices for donation-integrated procurement and research banking of human spinal cord tissue within a tertiary academic hosp

AIMS/HYPOTHESIS: Clinically actionable biomarkers that accurately reflect the health status of the beta cell are needed to improve risk stratification and optimise the timing of interventions in type 1 diabetes. We hypothesised that inflammatory stress elicits a reproducible microRNA (miRNA) program in human islets and islet-derived extracellular vesicles (EVs) that can be detected in plasma EVs to stratify diabetes risk, while also providing insight into molecular pathways linked to beta cell dysfunction.
METHODS: Human islets were exposed to IL-1β+IFN-γ, and small RNA-seq was performed on i

BACKGROUND: Among accepted US heart organs, listing centers accept organ offers earliest for White women, followed by Black women, White men, and Black men. Understanding the relationships of center- and geographic-level decisions is necessary to understand waitlist disparities. We sought to determine whether listing center and geography are associated with the first heart organ acceptance by candidate race, since race demographics vary regionally across the United States.
METHODS: Using the United Network for Organ Sharing data sets, we identified first organ offers for Black and White candi

BACKGROUND: Transplant patients may have circulating non-ABO red blood cell (RBC) antibodies, but the influence of RBC alloimmunization on solid-organ transplantation outside of the ABO blood group system is not well known. Though RBC antigens have been identified in or on cells other than RBCs, the incidence of non-ABO RBC antibodies in transplant patients has had limited investigation. Identification of non-ABO RBC antibodies in transplant patients may help clarify cases of suspected antibody-mediated rejection (AMR) when human leukocyte antigen (HLA) donor-specific antibodies (DSA) cannot b
