A profound kidney shortage compromises the effective care of renal failure patients in the United States and other nations. This article discusses the need for kidneys, the procurement of live donor and cadaver organs, and strategies to increase organ donation. It suggests that the kidney shortage can be minimized and perhaps eliminated through the use of living related donors whenever appropriate, and through more efficient procurement of volunteered cadaver kidneys. It is demonstrated that passenger lymphocytes migrate out of rat renal allografts into host spleens in a radioresistant fashion. These mobile passenger lymphocytes within BN kidney and heart transplants are immunocompetent, since they elicit a graft-versus-host (GVH) reaction in the spleens of (LEW x BN)F2 hybrid hosts. The greater GVH reaction in (LEW x BN)F1 recipients of BN kidneys reflects the greater number of mobile passenger lymphocytes in the kidney when compared to the heart. The growth promoting effects of lithium and insulin on cultures of mammary gland epithelium and CZF mouse mammary tumor cells were investigated. Lithium chloride exerts a 450-fold increase in the rate of DNA synthesis in mammary epithelium from mid-pregnant mice in organ culture or monolayer culture. We wished to see if the concentration of lysosomal enzymes in the aqueous humour had any relation to the number of dead corneal endothelial cells in stored eyes from human donors. Over four and a half years a district general hospital provided 34 cadaveric kidneys for transplantation. Mercurascan (MSC), a mercury derivative of fluorescein, was studied as to its effect on the survival of allogeneic kidney grafts in dogs. A single perfusion of the donor's kidney with saline solution with MSC added in a ratio of 1 mg/100 ml was found to have significantly extended the graft survival time from 10.8 days (SE +/- 0.6) to 15.4 days (SE +/- 1.6).
