Organ procurement organization utilization of portable hypothermic oxygenated machine perfusion in liver transplantation: The first US experience.

BACKGROUND: Advanced machine perfusion techniques have increased the use of hard-to-place organs, effectively changing the liver transplantation landscape. Liver transplantation machine perfusion has been driven largely by industry or individual transplant centers. We describe the implementation of a portable, organ procurement organization-initiated hypothermic oxygenated machine perfusion strategy for liver transplantation as a proof of concept. METHODS: Surgeons and staff at a regional organ procurement organization (Indiana Donor Network) were trained to use and cannulate liver allografts

Retroperitoneoscopic Approach in Live Donor Left Nephrectomy: Insights From a Single-Center Case Series in the Sub-Himalayan Region.

Background and objective Living donor nephrectomy demands a surgical approach that ensures donor safety while preserving optimal graft function. The retroperitoneoscopic approach provides direct access to the renal hilum, potentially reducing bowel manipulation and postoperative morbidity. This study aimed to evaluate the feasibility, safety, and perioperative outcomes of laparoscopic retroperitoneoscopic donor nephrectomy (LRDN) in a small case series, with a particular emphasis on donor recovery and clinical outcomes. Methods From November 2022 to January 2023, seven patients underwent retro

AI-Driven Optimization of Kidney Allocation: Enhancing Precision in Donor-Recipient Matching.

OBJECTIVES: The growing demand for kidney transplant amid persistent organ scarcity demands improved donor -recipient compatibility assessment. Traditional allocation systems relying on rigid scoring fail to capture complex multidimensional data affecting posttransplant outcomes like graft rejection and delayed graft function. Artificial intelligence offers transformative potential through predictive analytics and adaptive learning. This study introduces OkAP, an AI -based system designed to enhance kidney transplant matching efficiency and accuracy by integrating clinical, genetic, and socioe

Graft-Versus-Host Disease After Liver Transplantation: Diagnostic and Therapeutic Challenges.

Graft-versus-host disease (GVHD) is a rare but severe complication following solid organ transplantation, with limited literature guiding diagnosis and management. We present a 55-year-old man with alcohol-associated end-stage liver disease who developed GVHD approximately one month after orthotopic liver transplantation (LT). He was initially admitted for persistent fever and concern for sepsis, with subsequent development of progressive pancytopenia and a non-pruritic maculopapular rash with mucosal involvement. Skin biopsy performed on hospital day 10 supported the diagnosis of GVHD, and do

Nanofiber-reinforced ECM-based bioink for hybrid biofabrication of adventitia-mimetic anisotropic vascular constructs.

BACKGROUND: Tissue-engineered blood vessels are essential for vascular grafting. Despite significant progress, current research predominantly focuses on the macroscopic scale of vascular grafts, neglecting the anisotropic microstructure of the vascular wall, particularly the adventitia, which plays a key role in the mechanical stability and functional performance of native vessels. Consequently, this aspect remains underexplored in current vascular tissue engineering strategies, limiting the full potential of vascular graft development. METHODS: In this study, we developed a novel hybrid biof

Cyclosporine versus placebo pretreatment of brain-dead donors and kidney graft function (Cis-A-Rein trial): a multicenter, double-blind, randomized, controlled trial.

PURPOSE: Delayed graft function is the most frequent early complication of kidney transplantation. Pretreatment of kidney donors with cyclosporine has decreased delayed graft function in animal studies by reducing ischemia-reperfusion graft injuries. No randomized clinical trials have assessed the efficacy of cyclosporine pretreatment of brain-dead donors in reducing delayed graft function. METHODS: In this multicenter randomized, double-blind, and placebo-controlled trial, brain-dead donors were randomized (1:1) to receive either 2.5 mg/kg of cyclosporine or a glucose placebo infusion. The k

Machine perfusion modulates cold-preservation injury in kidney transplantation: IDEAL stage 4 United States population cohort study.

Hypothermic machine perfusion (HMP) has surged in popularity for donor kidney preservation. Continuous HMP (cHMP) has shown clear benefits over static cold storage (SCS), whereas randomized trials on short-duration end-ischemic-HMP (eiHMP) have not. We assessed whether HMP modulates injury from increasing cold-preservation time (CPT) and analyzed the impact of HMP on short- and long-term outcomes, using Organ Procurement and Transplantation Network data (OPTN; 2010-2024) on single-kidney-transplant recipients (n = 137 835). Multivariable nonlinear (restricted cubic spline) regression with inte

Donor-Derived Cell-Free DNA in Acute and Chronic Rejection of Solid Allograft Transplantation.

Today, there have been spectacular advances in surgical techniques, organ preservation for transplantation, optimal and efficient donor and recipient selection, more efficient diagnosis of transplant complications, and major progress in pharmacological immunosuppression procedures. In this regard, survival rates after transplantation of various organs have been gaining ground, particularly in the case of lung transplants, whose average survival rate is lower than other types of transplants. In this regard, it is important to detect acute and subclinical clinical rejection, as well as chronic a

From Challenge to Opportunity: ABO-Incompatible Kidney Transplantation to Expand the Donor Pool at a Single Center.

Introduction ABO-incompatible kidney transplantation (ABOiKT) has emerged as a viable option whenever there is non-availability of a compatible donor. Traditionally considered a contraindication due to the high risk of antibody-mediated rejection, advancements in desensitization protocols, immunosuppressive therapy, and post-transplant monitoring have significantly improved the outcomes. In this study, we have detailed our experience of ABOiKT performed in a tertiary care hospital in North India. Methodology It was a single-center retrospective cohort study. All consecutive ABOiKT performed fr

Comparative evaluation of single-artery cannulation with passive venous drainage versus traditional dual-cannula ex vivo lung perfusion in a rat model.

BACKGROUND: Ex vivo lung perfusion (EVLP) has emerged as a critical technique for lung preservation and evaluation prior to transplantation. While conventional rat EVLP systems utilize closed-loop dual cannulation of pulmonary artery (PA) and vein, the effect of the simplified model using single PA cannulation with passive venous drainage is unknown. METHODS: We developed two EVLP models in rats: a semi-closed circuit with PA-only cannulation and left atrial incision for passive venous drainage (SC-EVLP), and a closed circuit employing both arterial and venous cannulation (C-EVLP). Donor lung

Apoptotic Colitis in a Liver Transplant Recipient: A Case of Solid Organ Transplant-Associated Graft-Versus-Host Disease.

Solid organ transplant-associated graft-versus-host disease (SOT-GVHD) is a rare but often fatal complication following liver transplantation. A 67-year-old man who underwent deceased donor liver transplantation subsequently developed progressive high-volume diarrhea and severe cytopenias with minimal initial cutaneous involvement. Colonic biopsy demonstrated apoptotic colitis with minimal lamina propria inflammation, suggestive of lower gastrointestinal (GI) GVHD. Donor-recipient chimerism analysis revealed significant donor-derived chimerism in peripheral blood and colonic tissue, confirming

Longer Duration of Donor Ventilation is not Associated With Long-term Lung Function After Lung Transplantation.

BACKGROUND: Prolonged mechanical ventilation increases the risk of developing ventilator-induced lung injury, but the relationship between the duration of donor ventilation (DDV) and post-lung transplant lung function is unknown. METHODS: We analyzed adult patients undergoing double lung transplantation in our program between January 2007 and December 2020. The risk factor of interest was DDV in days before organ retrieval. The primary outcome was forced expiratory volume in 1 s % predicted at 1-y posttransplant (1yFEV1%), and secondary outcomes, including lung function at 3 mo, short and lon

Economic benefits of T cell-depleting induction in deceased donor kidney transplant recipients.

Thymoglobulin (TMG) induction improves long-term outcomes in high-immunologic-risk kidney transplant (KTx) by reducing acute cellular rejection. However, routine use of TMG in lower-immunologic-risk KTx is controversial, given concerns about posttransplant infectious complications and added health care expenditures. United States Renal Data System data were examined to compare real-world clinical and economic outcomes-including 3-year median Medicare expenditures, incidence of key adverse events, and allograft survival-in a cohort of Medicare-insured deceased donor KTx recipients (2006-2020) t

An Unbiased Molecular Characterization of Peripartum Cardiomyopathy Hearts Identifies Mast Cell Chymase as a New Diagnostic Candidate.

Peripartum cardiomyopathy (PPCM) is a rare form of acute heart failure that develops in women toward the end of pregnancy or early postpartum. No effective, specific treatment for PPCM is available and heart transplantation or mechanical circulatory support may be required in severe cases where drug treatment for heart failure is insufficient. The mechanisms through which the disease progresses are not well understood, and despite similar clinical characteristics to dilated cardiomyopathy of other etiologies (nonperipartum cardiomyopathy; NPCM) it is not known how the molecular remodeling diff

Activation of donor-derived eosinophils during static storage contributes to primary graft dysfunction after lung transplantation.

Ischemia-reperfusion injury (IRI) contributes to deleterious outcomes after lung transplantation. Although we have shown a unique protective role for eosinophils in both establishing and maintaining lung allograft tolerance, their role in IRI remains unclear. Based on previous research demonstrating a protective role for eosinophils in liver IRI, we hypothesized that they might play a similar function in the lung. In this study, we show that donor-, but not recipient-derived, eosinophils worsen injury of syngeneic and allogeneic lung grafts rather than protect from it. Eosinophils in the lungs

Different States of Lung Allograft Injury Assessed by Plasma Donor-Derived and Total Cell-Free DNA.

Lung allografts are susceptible to myriad injury types, including acute rejection (AR), infectious disease (ID), baseline lung allograft dysfunction (BLAD), and chronic lung allograft dysfunction (CLAD), that affect outcomes. Donor-derived cell-free DNA (dd-cfDNA) is validated for detecting AR after lung transplantation (LT). However, data are limited regarding the ability of dd-cfDNA or total cell-free DNA (TcfDNA) to detect or differentiate other clinical conditions. This study stratified patients into Stable, AR, CLAD, and ID cohorts. Stable double LT recipients were further stratified into

Impact of donor and recipient sex on graft function among deceased donor kidney recipients: A paired-kidney analysis.

Females improved resilience to ischemia-reperfusion injury has been reported in animal models, a protection largely attributed to sexual hormones. Clinically, the impact of sex on short- and long-term outcomes after kidney transplantation is unclear. In this retrospective cohort study using the United Network for Organ Sharing database, we analyzed paired donor kidneys transplanted to 2 opposite sex adult recipients (N = 39 197 males and N = 39 197 females), thereby controlling for all donor-related covariates. The primary endpoint was delayed graft function. Secondary outcomes included 1-year
