Itaconate supplementation leads to improvement in donor lung function after extended hypothermic preservation.

BACKGROUND: Controlled hypothermic storage of donor organs at 10 °C is an emerging clinical standard in lung transplantation owing to its superior graft protective effects. Itaconate, a known regulator of cell immunometabolism, has been shown to be upregulated in donor lungs preserved at 10 °C for extended periods of static storage versus on ice. We hypothesized that itaconate may be an active agent in donor graft protection against ischemia-reperfusion injury rather than a byproduct of 10 °C graft metabolism. METHODS: We conducted cell-based screening of multiple formulations and concentrati

Metabolic changes and plasma glycine predict the risk of acute cellular rejection in heart transplantation.

OBJECTIVE: Brain death in organ donors may trigger tissue injury that can negatively influence transplant outcome. We analyzed plasma metabolomic profiles in heart transplant donors and tested whether any donor metabolite may predict the risk of acute cellular rejection after heart transplantation. METHODS: Plasma samples from 83 heart transplant donors and 20 healthy volunteers were profiled using quantitative targeted metabolomics of 102 metabolites. Plasma samples from another set of 48 heart transplant donors were used for validation. RESULTS: The plasma levels of 24 metabolites represen

Mitochondrial hyperoxidation contributes to warm ischemia-reperfusion injury in rat and pig livers.

BACKGROUND: Mitochondrial dysfunction is a critical factor in several diseases, but current in situ assessment methods are severely limited. Non-invasive monitoring of mitochondrial redox state using resonance Raman Spectroscopy (RRS) offers a promising solution. This study aims to demonstrate RRS utility with liver models of warm ischemia-reperfusion injury in organ transplantation. METHODS: Lewis rat (female) and Yorkshire pig (both sexes) livers were evaluated during reperfusion by subnormothermic machine perfusion, with 3-6 replicates per study group, and statistical comparisons using unp

Sex differences in cardiac recovery and ventricular gene expression in a rat model of donation after circulatory death.

BACKGROUND: Heart donation after circulatory death (DCD) is a promising strategy to increase graft supply. However, in contrast to conventional heart transplantation, in which organs are retrieved from heart-beating donors, DCD hearts are subjected to damaging conditions before and during functional, warm in-situ ischemia in the donor, leading to ischemia-reperfusion injury (IRI). Although sex differences have been identified in other contexts of cardiac IRI, such as myocardial infarction, they remain underexplored in DCD. Therefore, we aimed to investigate whether sex differences induce chang

From Patient Selection to Surveillance: Artificial Intelligence Applications in Radioiodine Therapy - A Systematic Review.

UNLABELLED: Dietary phytochemicals exhibit anticancer activity, and increasing evidence suggests that the combinations of co-occurring compounds may provide greater therapeutic efficacy than individual compounds. Eruca sativa Mill. (rocket salad) contains both erucin and kaempferol; however, their synergistic anticancer activity against non-small cell lung cancer (NSCLC) has not been systematically investigated. Network pharmacology, molecular docking, and molecular dynamics (MD) simulations were performed to identify molecular targets of erucin and kaempferol. Their synergistic anticancer act

Kidney Transplantation for Lithium Nephrotoxicity is Associated With Noninferior Graft and Patient Survival: A National Donor-Stratified Registry Analysis.

INTRODUCTION: To evaluate donor-stratified graft and patient survival following kidney transplantation (KT) for lithium-associated end-stage kidney disease (ESKD). METHODS: This retrospective analysis utilized the Organ Procurement and Transplantation Network database. Adult KT recipients (January 1, 2015-December 30, 2024) with lithium-associated nephrotoxicity as the primary listing diagnosis (n = 840) were compared with those transplanted for other etiologies. Deceased-donor (DDKT; n = 584) and living-donor (LDKT; n = 256) cohorts were propensity matched 1:1 within donor type. Gompertz par

Optimized cellular perfusate increases lung viability ex vivo and extends the duration of ex vivo lung perfusion.

BACKGROUND: Ex vivo lung perfusion (EVLP) has emerged as a strategy to assess and extend preservation of donor lungs prior to transplantation. However, due to progressive deterioration of the graft, current protocols limit EVLP duration to 10-12 h. We developed an optimized cellular perfusate, DEVOL solution, designed to support multi-day EVLP and evaluated its performance against standard TransMedics OCS perfusate. METHODS: Porcine lungs were procured, flushed with OCS solution, and perfused using either STANDARD (n=5) or DEVOL (n=5) perfusate supplemented with washed red blood cells. Physio

Alternative Preservation Strategies of Donation After Circulatory Death Hearts: Effect of Cold Ischemia Time Before Normothermic Machine Perfusion.

BACKGROUND: Donation after circulatory death (DCD) offers a promising strategy to expand the donor pool by 20% to 30%. However, DCD heart grafts experience warm ischemia injury, making effective graft viability assessment before transplantation prudent. Current methods for assessing DCD hearts are impacted by either ethical concerns and regional limitations (thoracoabdominal normothermic regional perfusion) or the need for significant resources, expertise, and equipment, with limitations in functional assessment (direct procurement and reperfusion, via the Organ Care System by TransMedics). Evaluating a Novel Cell-Free Preservation Solution for Human Cardiomyocyte Protection: A Proof-of-Concept Study.

BACKGROUND: Heart transplantation remains the definitive therapy for end-stage heart failure; however, its clinical impact is constrained by donor availability and preservation-associated injury. Static cold storage (SCS) with cardioplegic solutions such as Custodiol is the current clinical standard of care, yet extended cold ischemia markedly increases the risk of primary graft dysfunction. There is therefore a critical need for advanced preservation strategies and solutions that maintain graft viability under both cold and normothermic conditions. AIM: This study is aimed at comparing the e

Targeting Sphingosine-1-Phosphate Receptor 1 Protects Pulmonary Vascular Endothelial Integrity During Human Ex Vivo Lung Perfusion.

BACKGROUND: Pulmonary edema due to vascular endothelial injury in donor lungs reduces organ utilization and exacerbates ischemia/reperfusion injury, resulting in poor posttransplant outcomes. Sphingosine-1-phosphate (S1P) improves vascular integrity through S1P receptor 1 (S1PR1) signaling. This study assessed whether S1PR1 agonism during ex vivo lung perfusion (EVLP) reduces vascular permeability and edema formation in human donor lungs. METHODS: S1PR1 agonist CYM5442 was administered to human donor lungs declined for transplant, during 6 h of EVLP, using a paired split-lung model with one l

Ethical issues in donation-based prosocial interventions for health: a scoping review.

BACKGROUND: Donation-based prosocial interventions for health, such as monetary donations for diagnostic tests, aim to encourage voluntary actions that benefit others while fostering public participation to promote health. These interventions are increasingly used in public health, but the associated ethical issues have not been comprehensively described. METHODS: We conducted a scoping review using the Arksey and O'Malley framework to synthesize bioethics and public health literature on the ethical issues encountered with donation-based prosocial interventions for health. We searched PubMed,

Isolation of SSEA-3-Positive Muse Cells in Canine and Feline Adipose Tissues.

Muse cells are endogenous pluripotent-like stem cells identified as stage-specific embryonic antigen-3 (SSEA-3)-positive subpopulations in the bone marrow, peripheral blood, and connective tissues of various organs. Clinical trials conducted by intravenous injection of donor-Muse cells, without the use of immunosuppressive drugs, have demonstrated safety and efficacy across multiple diseases. Since the epitope recognized by the anti-SSEA-3 antibody is a glycolipid, rather than a protein produced by a genetic code, the antibody may detect Muse cells across different species. Muse cells possess

Identifying Donor Immune Cells in the Intestinal Mucosa of Chimeric Recipient Mice by Immunofluorescence.

Murine competitive congenic bone marrow transplantation is an important method used to identify genetic contributions to immune cell abundance, activation, differentiation, and mobilization, either in the bone marrow or in the peripheral organs of the host. This enables analysis of interactions between cells from both donors in relation to each other and the host itself. Flow cytometry is the predominant approach for experimental designs such as these for its powerful quantitative capacity and ability to identify rare and complex immune cells using cell surface markers. However, information re

Bioengineering extracellular vesicles for enhanced therapeutic functionality and clinical potential in the cardiovascular system.

Cardiovascular diseases (CVDs) are a leading cause of morbidity, disability, and mortality worldwide, posing a significant threat to global health. These diseases include myocardial infarction (MI), heart failure (HF), hypertension, atherosclerosis (AS), and other pathological cardiac disorders. Despite notable advances in CVD management, such as cardiac-targeted pharmacotherapies, interventional procedures, and heart transplantation, major challenges persist, including adverse drug effects, postoperative complications, and the persistent shortage of heart donors. Extracellular vesicles (EVs)

A standardised and reproducible method for en bloc multi-visceral organ retrieval in a porcine model.

BACKGROUND: The rapidly changing landscape of abdominal transplantation including regional normothermic perfusion of abdominal organs during procurement, broadening indications for multi-visceral transplantation (MVT) in both benign and malignant pathology and the ever increasing gap between organ supply and demand have necessitated the normalisation of expanded criteria donors (ECD). Key to mitigating the increased risk of delayed or non-function of ECD organs is the development of suitable preclinical animal models that allow for the development of reliable viability assessment as well as re

Asynchronous evolution of epithelium and stroma differentiates precursor lesions from pancreatic cancer.

Pancreatic intraepithelial neoplasia (PanIN) precedes pancreatic cancer, a deadly disease characterized by an extensive tumor microenvironment. How the microenvironment evolves during cancer progression is largely unknown, as PanINs are microscopic and non-diseased pancreas samples are exceedingly rare, while adjacent normal samples are disrupted by the presence of malignancy. Leveraging donor organs and spatial technologies we mapped the evolution of PanIN to cancer. The PanIN epithelial component falls on a continuum with cancer while the PanIN microenvironment is drastically distinct. Progr

Therapeutic donor hypothermia following brain death to improve the quality of transplanted organs.

RATIONALE: Solid organ transplantation is the optimal treatment for patients living with end-stage organ failure. Globally, the majority of organ donors are deceased, most commonly following brainstem death. The process of brain death is known to cause significant injury to organs. Therefore, efforts to optimise the pre-donation care of these donors should be maximised to achieve improved recipient outcomes. OBJECTIVES: To assess the benefits and harms of therapeutic donor hypothermia in recipients of organs donated from brain-dead donors. SEARCH METHODS: We searched Cochrane Kidney and Tran

From Gene Editing to Exogenesis: Pigs as a Source of Immune-compatible Organs for Transplantation.

The persistent shortage of human organs for transplantation has intensified efforts to develop alternative sources, specifically xenotransplantation and exogenesis. Xenotransplantation uses genetically engineered pigs to provide organs, tissues, and cells for clinical use. Significant progress has occurred in developing multigene-modified pigs that lack glycan xenoantigens while expressing human complement and coagulation regulators. These modifications have successfully mitigated hyperacute, antibody-mediated, and cellular rejection in preclinical nonhuman primate models. Recent compassionate
