Disseminated Aspergillus flavus infection presenting as spondylodiscitis: a fatal tuberculosis mimicker in an immunocompetent host - case report and literature review.

INTRODUCTION: Disseminated invasive aspergillosis is a rare but life-threatening fungal infection that predominantly affects immunocompromised individuals. Vertebral involvement is an uncommon manifestation, and diagnosis in immunocompetent patients is particularly challenging because of its nonspecific clinical and radiological features, often resulting in delayed diagnosis and inappropriate therapy. We report a fatal case of disseminated Aspergillus flavus aspergillosis presenting as spondylodiscitis in an immunocompetent host, highlighting the diagnostic challenges and therapeutic implicati

Benchmarking Vision Encoders for Image Classification in Ophthalmology.

Foundation vision encoders are rapidly emerging as the standard for retinal artificial intelligence. Yet, ophthalmology still lacks a comprehensive benchmark, leaving model selection for basic science and clinical translation as guesswork. Here, we present a large-scale comparison of 34 pretrained encoders on 39 classification tasks covering color fundus photography, optical coherence tomography, scanning laser ophthalmoscopy, and ultrawidefield imaging. Using a unified pipeline, we compare frozen-feature evaluation, linear probing, and end-to-end fine-tuning to determine which models translat

PituPhase65: An endoscopic pituitary surgery dataset for surgical phase recognition.

Surgical Phase Recognition (SPR) aims to provide the surgical team with valuable feedback while performing an intervention. In the near future, these systems will help to shorten the learning curve of residents, improve communication among professionals in the Operating Room, and obtain an automatic complete surgical report. This is an upcoming reality for surgeries such as cholecystectomy, which are highly standardized and broadly studied. Nonetheless, endoscopic pituitary surgery still has a long way to go, as it has a higher workflow variability which hinders model learning. To move forward

Isolated oncocytic sinonasal papilloma of the sphenoidal sinus.

The ectodermally derived mucosa of the nose and paranasal sinuses may give rise to three morphologically different papillomas referred to individually as exophytic papilloma, inverted papilloma (IP) and oncocytic sinonasal papilloma (OSP). OSP is the rarest of the three different types, frequently affects the maxillary sinus, ethmoid sinus and nasal cavity while sphenoidal sinus involvement is exceedingly rare. The tumour has no gender predilection and has a wide age range with the majority above 50 years of age. Clinically, its behaviour parallels IP due to local recurrence and tendency to ma

Computed Tomography, Magnetic Resonance, Cross-Sectional Anatomy, and 3D Volume Rendering of the Nasal Cavity and Paranasal Sinuses of the Alpaca (Vicugna pacos).

Understanding the anatomy of the paranasal sinuses and nasal cavity in alpacas (Vicugna pacos) is limited by scarce published data. This prospective cadaveric study aimed to enhance the interpretation of clinical cross-sectional imaging and expand knowledge of normal anatomy using computed tomography (CT), magnetic resonance imaging (MRI), macroscopic cross-sections, and 3D reconstructions (Amira v5.3.3; Thermo Fisher Scientific, Waltham, MA, USA). Seven alpacas (median age 3.9 years; five males, two females) underwent CT and MRI. Frozen anatomical slices were obtained from three heads, and si

A Y-shaped sphenoidal sinus septum: a case report.

The sphenoidal sinus septum is one of the most important landmarks during endonasal endoscopic transsphenoidal operations. During routine coronal sectioning of the face, we found a variant Y-shaped septum in the sphenoidal sinus of a female cadaver. This unusual septum was found between two sections (anterior and posterior sections) and located inferior to the pituitary gland. The sphenoid sinus was divided into three chambers by the septum, i.e., right, middle, and left. During endoscopic surgery, surgeons should be aware of these variations. To our knowledge, this report documents the first

Adaptive Treatment Strategy: Adjuvant Chemoradiotherapy for a Complex SMARCB1 (INI-1)-Deficient Sinonasal Carcinoma With Intracranial Extension.

SWI/SNF-related matrix-associated actin-dependent regulator of chromatin subfamily B member 1 (SMARCB1) (integrase interactor 1)-deficient sinonasal carcinoma (SDSNC) is a rare and aggressive malignancy of the head and neck. It is characterised by the absence of nuclear SMARCB1 expression and typically presents at advanced stages due to non-specific symptoms resembling benign conditions such as nasal polyps or sinusitis. This report describes a 64-year-old male who presented with a sphenoidal sinus mass causing bony erosion and intracranial extension, associated with headaches and diplopia. In

Ectopic dopamine agonist-resistant macroprolactinoma to the clivus masquerading as a chordoma - A case report.

Objective. Pituitary neuroendocrine tumors (PitNETS) are common intracranial tumors, but extrasellar or ectopic PitNETS are very rare and supposed to originate from some pituitary remnants. They are mostly found in sphenoidal sinus. But particularly, ectopic clival PitNETS are highly aggressive and can cause bone invasion and can be misdiagnosed as other lesions of the skull base such as chordomas. Case Report. We report a challenging case of an ectopic prolactin-secreting PitNET arising in the clivus in a young female presenting with secondary amenorrhea and sellar mass effect symptoms. On ma

Mononostril Endoscopic Endonasal Chopstick Technique for Low Petroclival Meningioma With Sphenoidal Sinus Cranialization and Rostral Mucosal Closure.

BACKGROUND AND IMPORTANCE: Lower petroclival meningiomas (PCMs) and jugular tubercle meningiomas are among the most challenging tumors in neurosurgery, with the optimal approach still debated. Traditional posterior and lateral open approaches are still commonly employed but are invasive and carry significant risks due to brain retraction and neurovascular manipulation. The extended endonasal transclival approach has emerged as a less invasive alternative for midline lesions, offering direct access with early tumor devascularization and reduced manipulation of critical structures. However, limi

Predictive model for meningitis after pituitary tumor resection by endoscopic nasal trans-sphenoidal sinus approach.

BACKGROUND: Meningitis is a significant complication following nasal trans-sphenoidal surgery for pituitary tumor resection. Meningitis increases hospital stays and costs, posing a burden to both patients and healthcare systems. This study aimed to develop a perioperative predictive model for meningitis based on key factors, such as the operation duration, tumor diameter, and intraoperative cerebrospinal fluid (CSF) leakage. METHODS: A retrospective analysis was conducted on patients undergoing pituitary tumor resection via the nasal trans-sphenoidal approach. Predictive factors for meningiti

Arachnoid prolapse reduction with flapless reconstruction technique in pituitary adenomas treated with endoscopic transsphenoidal surgery.

BACKGROUND: Effective reconstruction of the pituitary fossa following, transsphenoidal surgery is critical in the prevention of postoperative cerebrospinal fluid, (CSF) leaks and empty sella syndrome. Therefore, reducing arachnoid prolapse into, the sphenoidal sinus is essential to avoid these and other complications. OBJECTIVE: This study aims to identify factors that are related to arachnoid prolapse into the sphenoidal sinus. As secondary objective we propose a novel flapless reconstruction technique and prove its effectiveness in avoiding postoperative CSF leak. METHODS: A case-control s

Harnessing copper: Innovative approaches to combat neurodegenerative diseases and cancer (Review).

BACKGROUND: Use of the ergot-derived dopamine receptor agonists (cabergoline and pergolide) is associated with an increased risk of cardiac valvulopathy. Pergolide was withdrawn from the US market in 2007 because of the risk of valvular heart disease, while the European Medicines Agency (EMA) required a reduction in the maximum daily dosage of cabergoline and pergolide from 6 mg/day to 3 mg/day in 2008. In Japan, the package inserts of both drugs were revised in April 2007 to request that physicians conduct periodic ultrasonic cardiography (UCG) examinations for patients taking cabergoline or

Ophthalmoplegia Associated With Extramedullary Multiple Myeloma: Case Series From a Tertiary Cancer Center.

BACKGROUND: Multiple myeloma (MM) is a malignant disorder of plasma cells that results in tumor cells replacing the bone marrow. In extramedullary MM (EMM), however, tumor cells proliferate outside the bone marrow. EMM may produce ophthalmoplegia through direct invasion of the superior orbital fissure, cavernous sinus, and/or sphenoidal sinus. Several mechanisms have been proposed including cranial nerve palsies, direct infiltration of bone, extraocular muscle metastasis, myelomatous meningitis, and parenchymal or paraneoplastic effects. METHODS: We retrospectively reviewed the medical record

Clinically relevant morphometric analysis of pterygopalatine fossa and its volumetric relationship with adjacent paranasal sinuses: a CT-based study.

OBJECTIVES: This study aimed to perform morphometric measurements of the pterygopalatine fossa (PPF), the transition zone to critical neurovascular structures. The second aim was to investigate the relationship between the volumes of the PPF and the paranasal sinuses and the effect of nasal septum deviation (NSD) types on all these measurements. METHODS: We performed PPF's morphometry and all volume measurements on the CT images of 260 patients (130 male and 130 female, age range 18-79). RESULTS: All volumetric measurements and the angle between foramen rotundum (FR) and pterygomaxillary fis

Middle meatal nasal recesses of the maxillary sinuses and dangerously modified nasal anatomy.

Pneumatisation of the maxillary sinus (MS) is variable. The archived cone-beam computed tomography file of a 54-year-old female was retrospectively evaluated anatomically. Nasal or retrobullar recesses of the MSs (NRMS) were found. The MSs were bicameral. NRMSs extended from the postero-lateral chambers of the MSs into the lateral nasal walls. The right NRMS was reached superior to the middle turbinate and the ethmoidal bulla was applied on its anterior side. The left NRMS had two medial pouch-like ends, one beneath the ethmoidal bulla and the other on the anterior side of the basal lamella of
