The relationship between MRI invasive features and expression of EMMPRIN, galectin-3, and microvessel density in pituitary adenoma.

This study aimed to investigate the relationship between the expression of the molecular markers, extracellular matrix metalloproteinase inducer (EMMPRIN), galectin-3, and microvessel density (MVD) with MRI invasive features in invasive and noninvasive pituitary adenomas. MRI was performed preoperatively in 34 patients with histologically verified pituitary adenomas. The expression of EMMPRIN, galectin-3, and MVD was determined by using immunohistochemical techniques on excised surgical specimen from all patients. Correlative analyses between invasive MRI features and expression of EMMPRIN, ga

IgG4-related hypophysitis: a new addition to the hypophysitis spectrum.

CONTEXT: Hypophysitis is a chronic inflammation of the pituitary gland that comprises an increasingly complex clinicopathological spectrum. Within this spectrum, lymphocytic and granulomatous hypophysitis are the most common forms, but newer variants have recently been reported. OBJECTIVE: The aims of the study were to describe a new patient with IgG4-related hypophysitis, review the published literature, and provide diagnostic criteria. SETTING: A 75-yr-old man presented with a 1-yr history of frontal headache. Initial studies revealed panhypopituitarism and a mass in both the sella turcica

[Endoscopic transsphenoidal pituitary surgery].

Endoscopic surgery has been performed in our Department since 1996 as assistance in micro-neurosurgical procedures. In the same way the endoscope was used in transsphenoidal pituitary surgery, but from the beginning of 2004 we started with "pure" endoscopic surgery. We present our experience with the first ninety patients to demonstrate our way of operating. Endoscopic pituitary surgery is performed through a natural nasal air pathway without any incisions. A 4-mm endoscope is placed in front of the tumor in the sphenoidal sinus and the tumor is removed with specially designed surgical tools.

Middle turbinate vascularized flap for skull base reconstruction after an expanded endonasal approach.

INTRODUCTION: The expanded endonasal approaches to the skull base are modular approaches that arise from the sphenoidal sinus. The reconstructive techniques in these approaches are key to avoid postoperative complications. Available flaps for reconstruction include the pedicled nasoseptal flap, the transpterygoid temporoparietal fascia flap, and the posterior pedicle inferior turbinate flap (PPITF), among others. Recently, the middle turbinate flap has been described in a cadaveric study. We report our preliminary experience in the use of this middle turbinate vascularized flap for skull base

An unusual case of ectopic ACTH syndrome.

Ectopic ACTH-syndrome is a rare cause of Cushing's disease. Despite extensive diagnostic procedures the source of ACTH secretion often remains occult. This case describes a 45-year old woman with an ectopic Cushing's syndrome. Extensive imaging procedures including CT scan of chest and abdomen, octreotide scan and MRI of the chest and pituitary did not reveal the source of ACTH secretion. In consideration of an occult source of ACTH secretion we started a therapeutic trial with cabergoline (0.5 mg/d), a dopamine receptor agonist, which has been shown to be effective in ectopic Cushing's syndro

Diagnosis and treatment of pancreatic metastases of a papillary thyroid carcinoma.

BACKGROUND: Apart from regional lymph node metastases, systemic metastases occur sporadically in papillary thyroid carcinomas (PTC). The lung and bones are the most frequent localizations. Additionally known but extremely rare locations are metastases of the skeletal muscles, ovaries, submandibular gland, sphenoidal sinus, brain, adrenals, and, as shown in only two previously published cases to date, the pancreas. SUMMARY: In this article we report about two additional patients with pancreatic metastases from PTC. There is almost no prior experience about therapeutic approaches to this type o

Extended endoscopic endonasal transsphenoidal approach to the suprasellar region: anatomic study and clinical considerations.

The extended endoscopic endonasal approach to the suprasellar region was performed on 10 fresh adult cadavers to describe the anatomic landmarks and key surgical steps for safe performance of the surgical approach. The anatomic features and relationships of the sphenoidal ostia, sphenoidal sinus, and optic-carotid recess are described, as are four intradural suprasellar neurovascular structural areas, including the suprachiasmatic, subchiasmatic, retrosellar and ventricular regions. Various anatomic conditions may influence the use of the extended endoscopic endonasal approach. This approach p

An ACTH-secreting pituitary adenoma within the sphenoid sinus.

A 68-year-old woman developed Cushingoid features three months prior to admission. She was found to have a markedly elevated plasma ACTH-cortisol level. Magnetic resonance imaging (MRI) revealed a mass in the left sphenoidal sinus, which had become enlarged to a point where it could not be removed by transsphenoidal surgery. We decided to proceed with radiation therapy to shrink the tumor. However, it was ineffective. Despite a reduction in serum cortisol levels using metyrapone, she died of septic shock. We describe a rare case of an ACTH-secreting pituitary adenoma within the sphenoid sinus.

Craniotomy with endoscopic assistance in the treatment of nasopharygeal fibroangioma.

BACKGROUND: Nasopharygeal fibroangioma (NPF) can be approached through lateral rhinotomy, the middle skull fossa approach and the transcranial-facial combined approach. It is complicated and thus results in more insults, and when adopted, the total resection rate of tumor is still low. The nasal endoscope is minimally invasive, the dead angles of a craniotomy, such as sphenoid sinus, maxillary sinus, and nasopharynx are easily approached by an endoscope. Lateral rhinotomy have to make facial incision and affects maxillary bone development. We combined the craniotomy and endoscopic approach int

A new plastic model of endoscopic technique training for endonasal transsphenoidal pituitary surgery.

BACKGROUND: Endoscopic transsphenoidal approach is a minimally invasive surgical technique for the removal of sellar and parasellar lesions, which has been progressively accepted by neurosurgeons. However, frustration is often expressed by neurosurgeons when first attempting endoscopic endonasal pituitary surgery. To overcome the learning curve from microscope to endoscope in a smooth way, a new human nasal model has been developed. The present study assessed this new model of the human paranasal sinuses for endonasal surgery training, particularly for endonasal pituitary surgery training. ME

Topography of the rabbit paranasal sinuses as a prerequisite to model human sinusitis.

BACKGROUND: Anatomical studies of the rabbit paranasal cavities are impelled by the increasing interest in the rabbit model to investigate human sinusitis. Although several such studies have already been performed, the topography of the rabbit dorsal conchal and maxillary sinuses is described ambiguously and the existence of the ethmoidal, frontal and sphenoidal sinuses is controversial. METHODOLOGY: The paranasal cavities were investigated using corrosion casting, gross and histological cross-sectioning, and micro-CT scanning of rabbit noses followed by computerized three-dimensional reconst
