Manifestations and predictors of vascular involvement in Behçet's disease.

BACKGROUND: Behçet´s disease (BD) is a variable vessel vasculitis in which vascular involvement (VBD) occurs in up to 40% of patients and represents a poor prognosis marker. Predictors of VBD have been poorly defined. AIMS: We aimed to characterise VBD patients and to identify predictors of this clinical subtype. METHODS: We performed a single-centre, retrospective study including patients fulfilling the 2013 International Criteria for BD, registered in the Rheumatic Diseases Portuguese Registry (Reuma.pt) between October 2014 and May 2023. VBD was defined as arterial or venous manifestation

Left Atrial Appendage Closure Versus Oral Anticoagulation for Prevention of Stroke or Systemic Embolism in Patients With Atrial Fibrillation: An Updated Meta-Analysis of 7004 Patients Across Six Randomized Trials.

INTRODUCTION: Left atrial appendage closure (LAAC) is an emerging alternative for stroke prevention in patients with atrial fibrillation (AF) who are unsuitable for long-term oral anticoagulants (OACs). However, its efficacy and safety relative to OACs remain uncertain. METHODS: Three databases were searched up to April 7, 2026, for randomized controlled trials comparing LAAC with OAC in patients with AF. Risk ratios with 95% confidence intervals were pooled using a random-effects model. RESULTS: Six RCTs (n = 7004 patients) were analyzed. The risk of stroke or systemic embolism (SSE) did no

Farm Trauma or Farm Drama? A Novel Approach to Farm Emergency Response Training.

OBJECTIVES: Knowledge of farm hazards is often gained through the retelling of fatal injuries and near-miss events, creating "Aha! Moments," or instances of conceptual clarity in which individuals recognize the error in a risky behavior. Unfortunately, this recognition is frequently followed by what farm populations may refer to as the "Oh, ****!" moment, when insight develops only after a severe or fatal injury has already occurred. This study examined immersive simulation using special effects (SFX) techniques and amputee role-players to simulate traumatic farm injuries, assess changes in kn

How Community Pharmacists Use Their Know-Who When Dispensing Medicines: A Qualitative Study.

RATIONAL: Recent studies indicate that physicians' and nurses' clinical reasoning partly relies on relational and contextual knowledge, known as 'Know-Who.' AIM: This study investigated whether community pharmacists also use 'Know-Who' in pharmaceutical interventions concerning antimicrobial prescriptions and how it shapes their decision-making. METHOD: This qualitative study involved face-to-face interviews with fifteen French community pharmacists. Two independent researchers analysed the data thematically using an integrative approach. RESULTS: Pharmacists reported that their decisions r

Can Umbrella Reviews Establish Consensus in Clinical Practice?

Evidence-based clinical practice is grounded in research and interpretation of findings. Individual studies capture only part of the evidence relevant to a clinical question, making synthesis important for evaluating consistency and precision. Systematic reviews bring relevant studies together, and meta-analyses use statistical methods to integrate findings. Umbrella reviews emerged to organise and contextualise findings across multiple reviews on a given topic. Convergence across umbrella reviews can support decision-making, but apparent agreement does not necessarily represent independent co

Ambivalent return: Older adults' homecoming after wartime displacement.

OBJECTIVES: Little is known about how older adults experience their return home after wartime displacement, particularly when home and community are altered or remain insecure. This longitudinal qualitative study examined continuity and change across the displacement-return transition. METHODS: Twelve participants (aged 65-92), previously interviewed during displacement (T1), were re-interviewed after returning home (T2) using a qualitative descriptive approach with thematic analysis. RESULTS: Three themes emerged: (1) the unsettled return, involving altered security, unfamiliarity within th

Yoga for Tobacco Cessation: A Randomized Controlled Trial (QUIT-SMART).

BACKGROUND: Current tobacco cessation strategies, including pharmacological interventions and supportive therapies, have limitations related to adherence and relapse. Emerging evidence suggests that yoga plays a significant role in tobacco cessation through psycho-behavioral mechanisms. This study was conducted to evaluate the role of yoga among adults who used both smoking and smokeless forms (SLT) of tobacco. METHODS: A prospective, randomized, open-label, blinded endpoint trial was conducted among adult tobacco users. Participants were randomly assigned to either the yoga (n = 64) or the e

A renal cyst-clear cell renal cell carcinoma comorbidity analysis identifies a FOXM1-positive epithelial COL6A1-CD44 axis that remodels fibroblasts.

BACKGROUND: Renal cystic conditions are heterogeneous. We used a broad renal cyst-associated disease-gene framework to identify molecular programs shared with clear cell renal cell carcinoma (ccRCC), without modeling a specific cystic disorder or direct cyst-to-cancer progression. METHODS: We identified 237 shared genes between renal cyst disease and ccRCC (TCGA-KIRC), performed functional enrichment, Mendelian randomization, and built a random survival forest (RSF) risk score benchmarked against published signatures and immunotherapy cohorts. Exploring the sexual risk-taking practices of bi+ femmes: a two-wave qualitative study.

BACKGROUND: Bi+ femmes are individuals who identify as women or femmes and are attracted to more than one gender. Despite evidence that bisexual women face higher risks of unintended pregnancy, STIs, sexual violence, and mental health challenges, they are underrepresented in sexual health research.  Little is known about how partner gender, relationship context, and partner-meeting venues shape their sexual risk-taking. METHODS: We conducted a two-wave qualitative study with 15 bi+ femme participants aged 18-35 years. Participants completed a sexual agency scale and two in-depth interviews, 1

A Comprehensive Review of Wheat Noodles Science and Technology.

Wheat noodles are widely consumed staple foods, but their quality, shelf life, and sustainability remain strongly dependent on controlling interactions among ingredients and processing conditions. This review synthesizes recent scientific advances in wheat noodle production, highlights key mechanisms governing structure formation, quality improvement, preservation, and emerging manufacturing strategies. Noodle quality is primarily determined by the established three-dimensional gluten-starch network. Mechanical sheeting promotes gluten alignment and disulfide cross-linking, thereby producing a

Pulsed field ablation with Renal denervation for persistent Atrial fibrIllation: Rationale and design of the PRAISE-AF randomized trial.

BACKGROUND: Atrial fibrillation (AF) is the most common sustained cardiac arrhythmia worldwide and is associated with increased risks of stroke, heart failure, hospitalization, disability, and mortality. Catheter ablation has become an established rhythm-control strategy for symptomatic AF; however, the recurrence rate after ablation remains substantial, especially in patients with persistent AF. Pulsed field ablation (PFA) is an emerging nonthermal ablation technology with myocardial tissue selectivity and a potentially improved safety profile compared with conventional thermal ablation. Large esophageal varices predict hepatocellular carcinoma in newly diagnosed cirrhosis: insights from machine learning and propensity score analysis.

INTRODUCTION AND OBJECTIVES: Hepatocellular carcinoma (HCC) is a major complication of chronic liver disease (CLD) requiring improved risk stratification. Esophageal varices (EVs), as manifestations of severe portal hypertension, may promote HCC development through vascular remodeling and hypoxia-related pathways. We aimed to evaluate whether variceal size independently predicts HCC risk and whether specific biomarker profiles can be identified. PATIENTS AND METHODS: In this 12-year prospective cohort study, 545 patients with newly diagnosed compensated advanced chronic liver disease (cACLD)

Sputum and oral microbiota profiles are associated with long COVID severity and phenotype post-hospitalisation: a UK multicentre, prospective cohort study.

BACKGROUND: Acute COVID-19 alters the airway microbiota. Whether these changes persist post-COVID-19 and contribute to long COVID is unknown. We aimed to describe the microbiota by long COVID severity and subtype, and identify links with inflammation. METHODS: Adults were prospectively recruited to the longitudinal UK Post-hospitalisation COVID-19 study (PHOSP-COVID). Patients were grouped by previously described "severity clusters" (of ongoing health impairments) and symptom subgroups. Oral and sputum microbiota (at 6 months post-discharge) were characterised through 16S ribosomal RNA gene a
