GERD management in neonatal ICU infants: study protocol for a randomized controlled trial comparing maturation, added rice starch formula and proton pump inhibitors.

BACKGROUND: The prevalence of gastroesophageal reflux disease (GERD) diagnosis among infants in the neonatal intensive care unit (NICU) is high and varies across USA NICUs. GERD diagnosis is challenging as symptoms can be physiological and association is difficult to determine without objective testing methods. Precision diagnostics using 24-hour pH-impedance (pH-Imp) studies with symptom correlation to reflux events have been developed and validated for this population. Current empiric-based, non-standardized GERD management practices are highly variable and are widely untested for safety and

Catalpol Reactivates IL-7R-STAT5 Signaling and Reverses Radiation-Induced T Helper 1 Cells Deficiency to Limit Melanoma Metastasis to Lung.

BACKGROUND: Radiation-induced quantitative deficiency of T helper 1 (Th1) cells accelerates lung tumor development. Here, we evaluated IL-7R-STAT5 signaling in Th1 recovery after irradiation and examined how catalpol contributes to Th1 reconstitution by targeting this signaling axis in mice. METHODS: The recovery characteristics of irradiated Th1 cells, compared with other IFN-γ-producing (IFN-γ+) cells, were analyzed in local radiotherapy (LRT) mice challenged with lung melanoma and single low-dose total body irradiation (SLTBI) mice without tumor. IL-7R-STAT5 signaling of Th1 from LRT mice,

Possible bronchospasm relief following flumazenil administration in a remimazolam-induced Grade III hypersensitivity reaction: a case report.

BACKGROUND: Remimazolam is an ultrashort-acting benzodiazepine for procedural sedation, with rare reports of anaphylaxis. Its clinical phenotypes and management strategies for refractory cases with suboptimal adrenaline response remain poorly defined. As remimazolam use increases globally, exploring effective interventions for its induced hypersensitivity reactions is clinically important. This report presents a rare remimazolam-induced Grade III hypersensitivity reaction dominated by bronchospasm, with flumazenil administration temporally associated with improvement of adrenaline-refractory s

Hemagglutination inhibition and alternate serologic responses following Influenza A(H3N2) virus infection.

BACKGROUND: Although the hemagglutination inhibition (HAI) titer remains the gold standard correlate of protection against influenza, it does not fully capture the broader antibody responses that contribute to immunity. METHODS: We analyzed immune responses in paired pre-infection and convalescent sera from 306 RT-PCR-confirmed A/H3N2 infections from two household studies (2014-18) in Managua, Nicaragua. Antibody responses were measured by HAI and enzyme-linked immunosorbent assays (ELISAs) against full-length hemagglutinin (HA), the HA stalk, and neuraminidase (NA). Participants were classif

Hantavirus infection: A narrative review focusing on epidemiology, diagnosis, infection control and treatment in the era of globalisation.

Hantavirus infection remains a major challenge in intensive care medicine because of its rapid progression, high mortality, and limited therapeutic options. This narrative review summarizes current evidence regarding the epidemiology, diagnosis, infection control, and critical care management of hantavirus cardiopulmonary syndrome (HCPS), with particular focus on Andes virus (ANDV) infection in South America. Hantaviruses are zoonotic RNA viruses transmitted mainly through inhalation of aerosolized rodent excreta. Unlike other hantaviruses, ANDV has demonstrated sustained person-to-person tran

Efficacy and Safety of Punch Excision Combined With Adjuvant Therapies for Hypertrophic Scars and Keloids: A Narrative Review.

BACKGROUND: Pathological scars manifest as firm, elevated, erythematous plaques, or nodules after skin injury. As challenging wound-healing complications, hypertrophic scars and keloids significantly compromise aesthetics while causing functional impairment and psychosocial distress. Consequently, their management remains a central focus in dermatology. The skin punch has undergone substantial technical advancements, emerging as a promising therapeutic modality. METHODS: To evaluate the efficacy of punch-based techniques in managing pathological scars, we conducted a comprehensive literature

Methaemoglobinaemia as a rare complication of Mycoplasma pneumoniae pneumonia.

A previously healthy female was admitted following inpatient antibiotic treatment for a lower respiratory tract infection. Since admission, her pulse oximetry revealed low oxygen saturation despite oxygen supplementation. The oxygen pressure in arterial blood remained normal with a wide gap between peripheral oxygen saturation and arterial oxygen saturation values. Suspicion of methaemoglobinaemia was raised. Methaemoglobinaemia reduction test (Brewer's test) became positive and was confirmed with a blood methaemoglobin assay value of 18.19%. High-performance liquid chromatography excluded the

A stochastic meta-population model of Ebola virus disease transmission for informing public health decisions.

Ebola virus disease (EVD) remains a constant international public health threat. Developing models that integrate the complex transmission dynamics of EVD is essential for informing evidence-based strategies for outbreak preparedness and response. Here, we introduce a stochastic, meta-population, compartmental model of EVD epidemics which accounts for key stages of the disease transmission including ecologically-driven zoonotic introductions, person-to-person transmission, spatial spread, and potentially complex interventions. Our model can distinguish between different transmission modes (dir

Anti-DENV IgE correlates with dengue severity and triggers FcεRI-dependent basophil activation inhibited by Omalizumab.

Severe dengue, marked by plasma leakage, is often linked to secondary heterotypic dengue virus (DENV) infection. While mast cells and basophils contribute to dengue pathogenesis, the role of anti-DENV IgE remains unclear. Here, we investigated whether anti-DENV IgE promotes FcεRI-dependent basophil activation, potentially contributing to disease severity. Plasma from dengue fever (DF, n = 42) and dengue hemorrhagic fever (DHF, n = 56) patients, collected at febrile, defervescence, and convalescent phases, were analyzed using an in-house IgE-capture ELISA developed to detect antibodies against

Cross-binding antibodies capable of neutralising diverse hantaviruses are produced in response to Puumala virus infection.

BACKGROUND: Orthohantaviruses (hantaviruses) are emerging rodent-borne pathogens that can cause severe human disease. They are present on multiple continents and are responsible for thousands of human cases per year. Despite this, no licenced therapeutics are available, vaccines for most strains are lacking, and the immunological response to infection is poorly characterised. This study aimed to analyse the humoral immune response to Puumala virus (PUUV) infection to inform future studies focussing on the production of therapeutic monoclonal antibodies and vaccination strategies. METHODS: Ser

Maternal metabolomic signatures for risk of preterm and early term birth in African American women.

BACKGROUND AND PURPOSE: Compared to healthy controls, adult patients with Sickle Cell Disease (SCD) are anemic, and therefore have higher cardiac output and Cerebral Blood Flow (CBF) to maintain brain oxygenation. They also demonstrate comparatively more cognitive deficits due to either overt strokes or silent cerebral ischemia. However, there are few correlative studies between CBF and cognitive deficits, specifically processing speed in SCD. Such studies are important to develop biomarkers of central brain processing and ischemia for diagnosis, prognosis, and evaluating the effectiveness of

Suspected Murine Typhus Associated With Neurologic Symptoms and Posterior Reversible Encephalopathy Syndrome (PRES)-Like MRI Abnormalities: A Case Report.

Murine typhus, caused by Rickettsia typhi, is a flea-borne zoonosis that typically presents with fever, headache, myalgias, and rash. Central nervous system involvement is uncommon, and neuroimaging findings that mimic posterior reversible encephalopathy syndrome (PRES) are rarely reported. This case highlights a rare neuroinfectious presentation of murine typhus with PRES-like imaging findings in a young woman. A previously healthy woman in her 30s presented with persistent retro-orbital headaches, intermittent fevers, tremors, dysarthria, photophobia, myalgias, progressive weakness, and recu

Evaluation of standard and modified two-tiered testing algorithms using well-characterized early Lyme disease samples.

UNLABELLED: Current laboratory testing for Lyme disease (LD) relies on serology. We evaluated the performance of standard two-tiered testing (STTT) and modified two-tiered testing (MTTT) algorithms using samples obtained from well-characterized patients with early LD in the U.S. East Coast and Upper Midwest. Participants with signs and symptoms of early LD (cases) and controls were enrolled by Lyme Disease Biobank. We compared the performance of four FDA-cleared STTT or MTTT algorithms using serum samples from 251 participants (107 cases, 69 with a convalescent draw; 144 endemic controls). At

Probable Leptospirosis in the Adventure Traveler with Freshwater Exposure: Narrative Review and Case Series.

Leptospirosis is an emerging zoonotic infectious disease in the adventure traveler with recreational freshwater exposure. We describe three cases of probable leptospirosis acquired during whitewater rafting trips in rural Ecuador. Rafting took place following periods of heavy rainfall and flooding of rivers, which are well documented risk factors for leptospirosis. Two of our patients presented to our rapid assessment clinic with acute febrile illness after travel to a region with a confirmed leptospirosis outbreak, while the third patient presented four weeks after a convalescent episode of a

Mechanism of activation of an ancestral TEC kinase by PIP3.

BACKGROUND: The introduction of second-generation antipsychotics (SGAs) over the past 2 decades generated considerable optimism that better antipsychotic treatments for schizophrenia and bipolar disorder were possible. SGAs offer several tolerability benefits over first-generation antipsychotics (FGAs), particularly with respect to extrapyramidal symptoms. However, SGAs can induce serious metabolic dysregulations, especially in drug-naive, first-episode, and child and adolescent populations, with olanzapine and clozapine having the highest propensity to cause these abnormalities. In this conte
