Beyond whole-image learning: anatomically partitioned deep learning models for superior sinonasal disease classification.

OBJECTIVES: Whole-image deep learning models for CT diagnosis of nasal cavity and paranasal sinus diseases often underperform because they disregard anatomical heterogeneity. We hypothesized that anatomically partitioned deep learning would enhance diagnostic accuracy. MATERIALS AND METHODS: A multicenter retrospective study enrolled 2947 CT examinations conducted from October 2019 to May 2022. From these, 150 patients were randomly selected for manual segmentation, covering 13 anatomical regions, including the bilateral nasal cavities, maxillary sinuses, anterior ethmoid sinuses, posterior e

Incidentally discovered asymptomatic extrasinusal mucocele of the sphenoid greater wing: A case report and review of differential diagnoses.

Extrasinusal mucoceles are exceptionally rare cystic lesions that arise outside the paranasal sinuses and may pose diagnostic challenges, particularly when incidentally detected in atypical locations. Their imaging appearance can overlap with other cystic skull-base and intracranial lesions. We report the case of a 45-year-old woman with an asymptomatic extrasinusal mucocele incidentally discovered on CT performed during follow-up of a peritonsillar abscess. Imaging revealed a well-defined expansile cystic lesion in the spheno-temporo-orbital region, causing cortical thinning without surroundi

Quantitative PCR-Based Analysis of Bacterial Profiles in Periapical Lesions and Maxillary Sinus in Odontogenic Sinusitis.

Odontogenic sinusitis (ODS) is a common cause of unilateral maxillary sinusitis arising from periapical lesions (PALs) or other dental sources. The infection is typically polymicrobial and dominated by anaerobic bacteria, which are often under detected by routine culture. Molecular approaches such as quantitative polymerase chain reaction (QPCR) and next-generation sequencing (NGS) may provide improved characterization of the microbial burden and community structure. This study aimed to compare culture-based methods, targeted quantitative PCR, and 16S rRNA sequencing in paired samples to chara

Undifferentiated pleomorphic sarcoma in the maxillary sinus: a case report.

BACKGROUND: Undifferentiated pleomorphic sarcoma, previously called malignant fibrous histiocytoma, is a type of malignant mesenchymal tumor (sarcoma) of soft tissue and sometimes bone. It is uncommon in the oral cavity and very sporadic in the maxillary sinus. Microscopic diagnosis of this malignancy in the maxillary sinus can be very challenging, because there is a range of features that may overlap with other benign and malignant tumors. CASE PRESENTATION: In this paper, we report a case of undifferentiated pleomorphic sarcoma in the maxillary sinus of a 61-year-old Iranian man who was ref

A Rare Cheek Sinus Presentation of a Type II First Branchial Cleft Anomaly: A Case Report and Literature Review.

Background: Congenital cheek sinuses are exceedingly rare craniofacial anomalies, typically present at birth and often misdiagnosed due to their resemblance to dermal pits or dimples. Only a few cases have been reported in the literature. This case is notable for its histopathological confirmation as a type II first branchial cleft anomaly containing an accessory tragus, highlighting the embryologic and clinical overlap between congenital cheek sinuses and branchial cleft anomalies. Case presentation: We report a three-year-old boy presenting with a congenital dimple along the right nasolabial

Expanding the spectrum of low-grade sinonasal adenocarcinoma with biphasic seromucinous differentiation and activating HRAS/AKT1 mutations.

AIMS: Low-grade non-intestinal-type sinonasal adenocarcinoma (LGSNAC) is a rare heterogeneous and poorly characterised group of tumours, distinct from intestinal- and salivary-type neoplasms. Therefore, further characterisation is needed for clearer biological understanding and classification. METHODS AND RESULTS: Clinical, histological and molecular characterisation of four cases of biphasic, low-grade adenocarcinomas of the sinonasal tract was performed. All patients were male, aged between 48 and 78 years, who presented with polypoid masses in the nasal cavity. Microscopically, virtually a

Development and validation of a prediction model for malignant sinonasal tumors based on MR radiomics and machine learning.

OBJECTIVES: This study aimed to utilize MR radiomics-based machine learning classifiers on a large-sample, multicenter dataset to develop an optimal model for predicting malignant sinonasal tumors and tumor-like lesions. METHODS: This study included 1711 adult patients (875 benign and 836 malignant) with sinonasal tumors or tumor-like lesions from three institutions. Patients from institution 1 (n = 1367) constituted both the training and validation cohorts, while those from institution 2 and 3 (n = 158/186) made up the test cohorts. Manual segmentation of the region of interest of the tumor

Cancer of the Nasal Cavity, Middle Ear and Accessory Sinuses - 15 Year Comparative Survival and Mortality Analysis by Age, Sex, Race, Stage, Grade, Cohort Entry Time-Period, Disease Duration and Topographic Primary Sites: A Systematic Review of 13,404 Cases for Diagnosis Years 2000-2017: (NCI SEER*Stat 8.3.8).

BACKGROUND: .-Sinonasal malignancies are rare, aggressive, deadly and challenging tumors to diagnose and treat. Since 2000, age-adjusted incidence rates average less than 1 case per 100,000 per year, male and female combined, in the United States. For the entire cohort, 2000-2017, overall median age-onset was 62.6 years. Carcinoma constitutes over 90% of these upper respiratory cancers and most cases are advanced, more than 72% (regional or distant stage) when the diagnosis is made. Composite mortality at 5 years was 108 excess deaths/1000/year with a mortality ratio of 558%, and 41% of deaths

Sinonasal Hamartomas.

A variety of hamartomatous lesions have been described in the sinonasal tract. These include respiratory epithelial adenomatoid hamartoma (REAH), seromucinous hamartoma (SH), and nasal chondromesenchymal hamartoma (NCMH). REAH and SH, demonstrate significant morphologic overlap with each other and with reactive conditions, while also being described to develop in association with other distinct tumors entities. NCMH affects a specific patient population with a specific molecular driver. The hamartomatous/neoplastic nature of REAH and SH are a topic of debate. There are emerging sinonasal hamar

Diagnostic challenges in malignant tumors of nasal cavity and paranasal sinuses.

INTRODUCTION: Malignant tumors of sinonasal tract are extremely rare and comprise 3% of all head and neck malignant tumors. They constitute 0.2% of all invasive carcinomas. Sinonasal space is a small anatomical place, but is the site of origin for tumors with diverse histological features. Many of the tumors are similar to those that occur in various parts of the body and have overlapping histological features. A panel of immunohistochemical (IHC) markers is essential to diagnose these tumors. Most of the tumors arise in the maxillary sinus followed by ethmoid sinus. History and complete head

Immunohistochemical and genetic analysis of respiratory epithelial adenomatoid hamartomas and seromucinous hamartomas: are they precursor lesions to sinonasal low-grade tubulopapillary adenocarcinomas?

Respiratory epithelial adenomatoid hamartoma (REAH) and seromucinous hamartoma (SH) are rare tumor-like lesions of the nasal cavity, paranasal sinuses, and nasopharynx. The pathogenesis of REAH/SH is still unclear. Neoplastic proliferation, chronic mechanical irritation, inflammation, or possible embryological tissue misplacement are speculated as possible mechanisms of their development. Low-grade tubulopapillary adenocarcinoma (LGTA) is a rare variant of nonsalivary, nonintestinal type sinonasal adenocarcinoma. The aim of this study was to evaluate the immunohistochemical and genetic profile

Juvenile aggressive cemento-ossifying fibroma of the sinonasal tract: case report.

Cemento-ossifying fibroma (COF) is a rare nonodontogenic tumor of the periodontal membrane that arises from the mesodermal germ layer. Clinically, these lesions are usually asymptomatic, slowly growing, and well circumscribed. However, they have demonstrated an aggressive course of development in a very small number of cases, particularly in younger patients. Surgical management via wide local excision is necessary because COF is notorious for recurrence, particularly lesions in the paranasal sinuses. We describe a case of juvenile aggressive COF of the sinonasal tract in a 12-year-old boy. Th

Low grade glandular lesions of the sinonasal tract: a focused review.

The sinonasal tract is a complex anatomic site with an exhaustive list of possible diagnoses. While most biopsies or resections encountered routinely consist of common diagnoses such as inflammatory polyps and papillomas, occasional cases are more difficult, and separating reactive or benign from malignancy can be challenging. One of the most poorly understood and daunting categories is low grade glandular or tubular proliferations, particularly on small biopsies. Possible diagnoses such as reactive lesions, respiratory epithelial adenomatoid hamartoma (REAH), seromucinous (glandular) hamartom

Review of lateral orbital wall ossifying fibroma.

Significant histological overlap exists between fibro-osseous lesions and diagnosis is made on a clinicopathological basis. Ossifying fibroma is a benign fibro-osseous neoplasm of the jaw and craniofacial complex that has generated a degree of controversy regarding diagnosis and classification, especially with respect to the psammomatoid variant. Orbital lesions mainly arise from the paranasal sinuses affecting the medial or inferior orbital wall. Lateral orbital wall ossifying fibroma is, therefore, a rare condition with only a single previous case report. We present a second case of lateral

Cemento-ossifying Fibroma Of Paranasal Sinus Presenting Acutely As Orbital Cellulitis.

Fibro-osseous lesions of the face and paranasal sinuses are relatively uncommon. These lesions have overlapping clinical, radiologic and pathologic features causing difficulty in diagnosis. Neoplastic fibro-osseous paranasal sinus lesions can be benign or malignant. The benign fibro-osseous lesions described are: ossifying fibroma (and its histologic variants) and fibrous dysplasia. The variants of ossifying fibroma differ in the nature of calcified material (i.e. cementum versus bone), in the location of the lesion (oral versus paranasal sinus or orbital), other morphologic variations (presen

Osteoid-producing orbital ethmoid tumor.

Fibro-osseus lesions of the paranasal sinuses, especially those that contain multinucleated giant cells, demonstrate a significant histopathologic overlap although they manifest themselves throughout a wide range of clinical behavior. They routinely require a critical clinical and pathologic interpretation. Often, the clinical course of the disease is the only guide to diagnosis and mode of treatment. This study presents the case of a 4-year-old boy with an osteoid-producing tumor of the ethmoid sinus. Despite an original diagnosis of fibrous dysplasia, the lesion followed a relentless course.
