Combined elevated C-reactive protein and detectable plasma RNA has predictive value for hospitalization during an outpatient COVID-19 convalescent plasma clinical trial.

BACKGROUND: Laboratory biomarkers that predict progression from mild COVID-19 to hospitalization are not well defined. OBJECTIVE: To evaluate the performance of C-reactive protein (CRP) and plasma SARS-CoV-2 RNA for predicting hospitalization among mild COVID-19 outpatients enrolled in a randomized controlled trial of COVID-19 convalescent plasma (CCP). METHODS: CRP and plasma SARS-CoV-2 RNA were measured in 487 outpatient trial participants screened before March 2021. Weighted analyses accounting for sampling probabilities were used to estimate positive predictive values (PPVs) and sensitiv

Prediction of Stroke Outcomes in a Super-aged Society: Different Effects Among Predictive Variables.

BACKGROUND AND OBJECTIVE: Population aging has progressed rapidly worldwide, particularly in developed countries such as Japan. Age is a well-established prognostic factor after stroke; however, its influence on functional outcomes may differ in increasingly older populations. This study aims to investigate whether the impact of age on post-stroke functional independence differs between patients aged ≥75 years (late elderly) and those aged <75 years. METHODS: This retrospective cohort study included 169 post-stroke patients admitted to a convalescent rehabilitation ward between 2018 and 2023.

Development of a Gait Independence Prediction Model in Patients With Stroke in a Convalescent Rehabilitation Ward: A Comparison of Decision Tree and Random Forest Models.

AIM: In this study, we aimed to examine the clinical utility of a classification and regression tree (CART) model for predicting independent ambulation at discharge based on physical function at admission to a convalescent rehabilitation ward, by comparing its performance with that of a random forest (RF) model. Seventy-three patients with stroke admitted to a convalescent rehabilitation ward were included. METHODS: Independent ambulation at discharge was defined using the Functional Independence Measure (FIM) locomotion item (walk/wheelchair): patients with a score ≥6 and ambulation as the p

Acute-phase weight change and functional recovery during convalescent rehabilitation in older stroke patients.

BACKGROUND & AIMS: Malnutrition is common among patients with stroke, and weight change is an important marker of nutritional status associated with clinical outcomes. However, the association between acute-phase weight change and functional recovery during the convalescent rehabilitation (CR) phase remains unclear, particularly in older patients. This study examined the association between acute-phase weight change and walking independence in older patients with stroke. METHODS: This retrospective cohort study included 650 patients aged ≥65 years admitted to a CR ward after acute stroke. Wei

Prediction of Discharge Functional Independence Measure Scores From Routine Admission Variables Using a ChatGPT-Assisted Workflow in Patients With Cerebral Infarction or Intracerebral Hemorrhage: A Retrospective Observational Study.

Background Predicting discharge functional status is important in stroke rehabilitation, but developing local prediction models often requires statistical and programming expertise. The primary objective of this study was to develop and internally validate prediction models for discharge Functional Independence Measure (FIM) scores using routine admission variables. The secondary objective was to describe a locally executed ChatGPT-assisted workflow used only for template generation and code scaffolding. Methods  This single-center retrospective observational study included 377 patients with c

The efficacy and safety of convalescent plasma for COVID-19 patients: a meta-analysis based on double-blinded parallel-arm randomized placebo-controlled trials.

BACKGROUND: Convalescent plasma (CP) showed promising benefits in previous coronavirus pandemics regarding efficacy and safety. However, the efficacy of CP from COVID-19 patients remains controversial and uncertain based on current randomized controlled trials (RCTs). There is an urgent need to establish the efficacy and safety of CP for COVID-19 patients as soon as possible. OBJECTIVE: To verify the efficacy and safety of CP using high-quality, double-blinded, parallel-arm, placebo-controlled randomized clinical trials, and to provide evidence-based support for the clinical application of CP

Longitudinal profiling of upper respiratory tract microbiota and metabolome in hospitalized COVID-19 convalescents: a 3-year prospective cohort study.

BACKGROUND: Long COVID is characterized by persistent, far-reaching effects in convalescent individuals, with pulmonary diffusion impairment emerging as a clinically impactful sequela affecting more than one-third of this population. The salivary microbiome and metabolome, reflecting the oral-lung axis, offer a window into the mechanisms underlying this condition. However, systematic longitudinal evidence on their long-term dynamics after infection and their predictive value for persistent pulmonary diffusion impairment remains scarce. METHODS: In this prospective cohort, we profiled the sali

Gridlock on the road to recovery: Barriers to timely pediatric trauma rehabilitation.

BACKGROUND: Recovery from pediatric injury frequently requires transfer to an inpatient rehabilitation facility (IRF) for further convalescence. Waiting for disposition to such facilities can substantially prolong hospitalization. This study sought to determine the root causes of delayed discharge following medical readiness for pediatric patients. The authors hypothesized that insurance disparities and the day of week patients were deemed ready for discharge would significantly delay disposition. METHODS: A retrospective cohort study was performed by querying the authors' institutional datab

Long COVID: Deep single-cell immunophenotyping and machine learning reveal a general signature for fatigue.

BACKGROUND: The post COVID-19 condition, commonly referred to as “Long COVID” (LC), is a constellation of long-lasting and debilitating symptoms following acute SARS-CoV-2 infection, which closely resembles other post-infective fatigue states. The underlying immunological disturbances of LC are poorly understood. Multiple explanatory mechanisms, such as persisting SARS-CoV-2 reservoirs, reactivation of latent viruses, endothelial dysfunction, auto-antibodies, tissue damage, and immune abnormalities, have been proposed but remain incompletely characterized, particularly in younger populations.

Acute Febrile Multisystem Illness With Vasculitic Features in a Young Adult: A Diagnostic Challenge Between Infectious and Autoimmune Etiologies.

Acute febrile illness with rash and multisystem involvement in a young adult has a broad differential diagnosis spanning infectious and autoimmune etiologies. We report the case of a 21-year-old male who presented with fever, a mixed-morphology rash (petechiae and palpable purpura with centripetal spread), peripheral neuropathy, polyarthritis, and transaminitis following recent travel to Tennessee and Missouri. A prodromal cough two weeks prior to rash onset added diagnostic complexity. Initial serologic testing during hospitalization was negative for Rocky Mountain spotted fever (RMSF) and de

Catalpol Reactivates IL-7R-STAT5 Signaling and Reverses Radiation-Induced T Helper 1 Cells Deficiency to Limit Melanoma Metastasis to Lung.

BACKGROUND: Radiation-induced quantitative deficiency of T helper 1 (Th1) cells accelerates lung tumor development. Here, we evaluated IL-7R-STAT5 signaling in Th1 recovery after irradiation and examined how catalpol contributes to Th1 reconstitution by targeting this signaling axis in mice. METHODS: The recovery characteristics of irradiated Th1 cells, compared with other IFN-γ-producing (IFN-γ+) cells, were analyzed in local radiotherapy (LRT) mice challenged with lung melanoma and single low-dose total body irradiation (SLTBI) mice without tumor. IL-7R-STAT5 signaling of Th1 from LRT mice,

Hemagglutination inhibition and alternate serologic responses following Influenza A(H3N2) virus infection.

BACKGROUND: Although the hemagglutination inhibition (HAI) titer remains the gold standard correlate of protection against influenza, it does not fully capture the broader antibody responses that contribute to immunity. METHODS: We analyzed immune responses in paired pre-infection and convalescent sera from 306 RT-PCR-confirmed A/H3N2 infections from two household studies (2014-18) in Managua, Nicaragua. Antibody responses were measured by HAI and enzyme-linked immunosorbent assays (ELISAs) against full-length hemagglutinin (HA), the HA stalk, and neuraminidase (NA). Participants were classif

Hantavirus infection: A narrative review focusing on epidemiology, diagnosis, infection control and treatment in the era of globalisation.

Hantavirus infection remains a major challenge in intensive care medicine because of its rapid progression, high mortality, and limited therapeutic options. This narrative review summarizes current evidence regarding the epidemiology, diagnosis, infection control, and critical care management of hantavirus cardiopulmonary syndrome (HCPS), with particular focus on Andes virus (ANDV) infection in South America. Hantaviruses are zoonotic RNA viruses transmitted mainly through inhalation of aerosolized rodent excreta. Unlike other hantaviruses, ANDV has demonstrated sustained person-to-person tran

Methaemoglobinaemia as a rare complication of Mycoplasma pneumoniae pneumonia.

A previously healthy female was admitted following inpatient antibiotic treatment for a lower respiratory tract infection. Since admission, her pulse oximetry revealed low oxygen saturation despite oxygen supplementation. The oxygen pressure in arterial blood remained normal with a wide gap between peripheral oxygen saturation and arterial oxygen saturation values. Suspicion of methaemoglobinaemia was raised. Methaemoglobinaemia reduction test (Brewer's test) became positive and was confirmed with a blood methaemoglobin assay value of 18.19%. High-performance liquid chromatography excluded the

A stochastic meta-population model of Ebola virus disease transmission for informing public health decisions.

Ebola virus disease (EVD) remains a constant international public health threat. Developing models that integrate the complex transmission dynamics of EVD is essential for informing evidence-based strategies for outbreak preparedness and response. Here, we introduce a stochastic, meta-population, compartmental model of EVD epidemics which accounts for key stages of the disease transmission including ecologically-driven zoonotic introductions, person-to-person transmission, spatial spread, and potentially complex interventions. Our model can distinguish between different transmission modes (dir
