This paper is a review of the literature on health professionals with substance abuse and it deals with the epidemiology, treatment, and results. A similar study has been carried out on health professionals in Denmark. Very few data are available for this group. The prevalence of substance abuse in health personnel seems to be equal to that in the general population, and from these data it is estimated that 3300 doctors/nurses in Denmark have an active alcohol abuse whereas the prevalence of other drug abuse is not known. OBJECTIVE: To investigate work disability among people with ankylosing spondylitis (AS) in terms of correlates and coping mechanisms.
METHODS: The sample group (n = 133) was recruited through 2 sources: 1) consecutive patients attending outpatient clinics over a 6-month period, and 2) a random sample of members of the National Ankylosing Spondylitis Society. We used a cross-sectional survey with data collected by self-administered questionnaires and telephone interviews with a randomly selected subsample (n = 6).

OBJECTIVES: To estimate the risk of bakers developing hand eczema. The importance of atopy was studied as well as change of job due to hand eczema.
METHODS: A retrospective cohort study was performed among bakers trained in Swedish trade schools in 1961-89 (n = 2923). School referents followed other programmes (n = 1258); population controls were randomly selected from the general population (n = 1258). A questionnaire on self reported hand eczema, year of onset of hand eczema, change of work due to hand eczema, childhood eczema, family atopy, and work history was posted to all participants.

The aim of this study was to assess the influence of medical and socioprofessional factors on return to work after myocardial infarction. The authors studied a continuous series of 174 patients with an average age of 51.3 years, all of whom were active before their illness. The average follow-up period was 33 months. The importance of occupational asthma and its management is usefully set in perspective by considering recent trends of increasing asthma incidence, morbidity and mortality in the population at large. We report a case of isocyanate induced asthma in a patient who had been previously exposed to isocyanate and had become a porter in a factory producing polyurethene foam. The experts judgment on an occupational dermatosis is of existential character for those concerned. The retrospective study covered the period from 1974 to 1985 and included 1,731 male members of the Swedish Transport Workers' Union in 1974. The difference between medical and social prognosis in hand eczema is stressed. Rehabilitation may be a fashionable word, but a change of job may result in a lower social standing and the eczema may persist. Job change should never be considered unless extensive patch testing has been performed. The decision also depends on age, nature of allergen, actual trade and occupation, a personal history of atopy and some personal factors. Some guidelines are given.

A retrospective study was carried out of 333 patients who had been treated by chemonucleolysis with chymopapain. The efficacy and safety of this treatment were evaluated and factors exerting a statistically significant effect on the quality of the results were selected from clinical and radiological data and the conditions of chemonucleolysis. Of participants with low back pain (LBP) at some time, about 60% said the pain had led them to consult their general practitioner, about 25% a specialist, and about 15% a chiropractor. About 30% had had an X-ray taken of their lumbar spine. Physiotherapy was the most common treatment given for the LBP. Manipulative therapy was the treatment which most often seemed to satisfy those with LBP although these cases might have been the milder acute attacks of LBP. Of the LBP complainers, 4.5% had been admitted to hospital and 1% operated on because of LBP. Work absence because of LBP occurred by 22.

Hard metals are worked with in numerous small industries. Thus hard metal fibroses are probably more frequent than has been supposed up to now. Observations on three patients have shown that a dry cough may preceed demonstrable manifestations by several months. As the full picture of the disease is hard to influence therapeutically an immediate change of job should be encouraged as soon as suspicious symptoms occur.
