Glottic narrowing secondary to prior mosquito repellent ingestion presenting as a difficult airway during craniovertebral junction surgery.

Airway involvement following chemical ingestion is rare but may pose life-threatening challenges to the anaesthesiologist. We report an Asian woman, in her 50s, with craniovertebral junction anomaly scheduled for posterior instrumentation comprising occiput-bilateral C1-C2 laminar screw and rod fixation along with foramen magnum decompression. Intraoperatively, she developed unexpected glottic narrowing which was later attributed to prior mosquito repellent ingestion which posed significant airway challenges. Initial intubation was uneventful with a video laryngoscope; however, attempts at tra

Pathways to nasalization of the glottal approximant /h/ in Extremaduran Spanish: an aerodynamic exploration.

Nasal spreading is often considered incompatible with oral obstruency, yet post-velopharyngeal nasal fricatives occur in several world languages. In certain Spanish varieties, the nasalized glottal approximant [ɦ̃] has been documented among rural speakers with lower educational backgrounds, but its origins remain unclear, as cited examples suggest both nasal spreading and spontaneous nasalization. These mid 20th-century descriptions are impressionistic, which is problematic given the acoustic overlap between nasality, voicing, and breathiness reported in the literature. This study explores [ɦ̃

Ultrasound characteristics of early glottic laryngeal cancer: diagnostic value and preoperative T staging.

AIM: To analyze the ultrasound characteristics of early glottic laryngeal cancer and evaluate their value in diagnosis and T-stage prediction. MATERIAL AND METHODS: A total of 154 patients with postoperative diagnosis of early glottic cancer were included. Ultrasonic findings were summarized, and factors influencing diagnosis and T staging were analyzed. RESULTS: Key findings included mucosal roughness, reduced echogenicity, vocal cord masses, and Adler grade III blood flow. Ultrasound showed 87.66% sensitivity. Adler grade and vocal cord mass were independent predictors of diagnosis. T-stag

Interpretable machine learning models using peripheral blood biomarkers for diagnosis and prognosis of glottic laryngeal squamous cell carcinoma.

To develop and validate interpretable machine learning models using routine peripheral blood biomarkers for the differential diagnosis and prognostic stratification of glottic laryngeal squamous cell carcinoma (LSCC). In this retrospective cohort study, we enrolled 124 patients with glottic LSCC, 124 patients with benign vocal cord lesions, and 124 healthy male controls. Preoperative peripheral blood parameters were utilized to develop Random Forest and XGBoost models. The clinical utility and interpretability of the models were assessed using Decision Curve Analysis (DCA) and SHapley Additive

Integrating Dynamic Representation and Multi-Priors for Transnasal Intubation via Visual Foundation Model.

Accurate and real-time glottis localization is critical for ensuring intraoperative oxygenation and patient safety during nasotracheal intubation. However, representative foundation models exemplified by the Segment Anything Model exhibit notable limitations in medical applications, stemming from their rigid attention mechanisms, feature space misalignment, and insufficient generalization to complex glottal anatomies. To address these challenges, we propose Glottis-SAM, a lightweight and task-adaptive segmentation framework that integrates dynamic representation learning with multi-prior conte

Prognostic Impact of Anterior Commissure Involvement on Treatment Failure in Early-Stage Glottic Carcinoma Treated With Definitive Radiotherapy: A Univariate and Multivariate Meta-Analysis.

BACKGROUND: Early-stage glottic carcinoma is characterized by high cure rate, but about 10% of patients still develop local recurrence after radiotherapy (RT). Although advances in local therapies have markedly improved larynx preservation, anterior commissure involvement (ACI) may worsen outcomes yet remains insufficient to fully guide personalized treatment. Here, we conducted a univariate meta-analysis and a Bayesian multivariate meta-analysis of multiple factors (MVMA-MF) to evaluate the prognostic impact of ACI and on local failure in early-stage glottic carcinoma. METHODS: We conducted

A single transoral laser microsurgery to simultaneously diagnose and treat early glottic cancer and premalignancies.

Transoral laser microsurgery (TLM) for early glottic cancer is often performed after pathological diagnosis is obtained by a biopsy, requiring two separate procedures. Single-stage TLM (SSTLM), both diagnostic and curative in one surgery, may be an alternative strategy. This retrospective cohort study enrolled patients diagnosed with early glottic cancer (T1/T2), carcinoma in situ (CIS), or severe dysplasia (SD) after TLM at a tertiary center between 2012 and 2021. The patients were classified into two groups: the two-stage TLM (TSTLM) group had first received biopsy before the definitive TLM;

Comparative effectiveness of laryngoscopes for successful intubation in difficult adult airways: A systematic review.

BACKGROUND: Endotracheal intubation (ETI) is a commonly performed emergency procedure used to secure the airways in critically ill patients. Despite its importance, ETI presents significant risks to patients with difficult airways. The availability of different types of laryngoscopes, most notably video laryngoscopes and direct laryngoscopes (DLs), has contributed to improved intubation success rates and reduced complications. While numerous studies have compared video laryngoscopes and DLs, there remains a limited synthesis of evidence evaluating the full range of all laryngoscopes across dif

Effect of Subhypnotic Dose of Propofol on Respiratory Adverse Events Following Postoperative Tonsillectomy/Adenotosillecomy: A Systematic Review and Meta-Analysis.

Background: Laryngospasm is defined as glottis closure due to reflex constriction of the laryngeal muscles. It is one of the most common complications following pediatric anesthesia that can lead to hypoxemia, bradycardia, or aspiration. Laryngospasm following tracheal extubation has different reasons: presence of secretions, foreign body in the airway, or pain at the site of surgery. Propofol is usually used as an induction or maintenance agent. However, its use with the subhypnotic dose (0.5 mg/kg) is increasing nowadays for reducing the incidence of laryngospasm. This systematic review and

Hoarseness Beyond Cancer: Laryngeal Presentation of Tuberculosis.

INTRODUCTION: Tuberculosis (TB) remains a leading infectious disease worldwide, with Ethiopia among the highest-burden countries. Laryngeal tuberculosis is rare, accounting for less than 1%-2% of TB cases and typically affects the posterior glottis and true vocal cords. However, atypical involvement of the subglottic region is uncommon and can closely resemble laryngeal carcinoma both clinically and radiologically, posing a significant diagnostic challenge. CASE PRESENTATION: We report a 64-year-old man with a 1-year history of progressive hoarseness. Neck CT demonstrated asymmetric mucosal t
