[Microvascular Reconstruction of the Laryngotracheal Junction].

BACKGROUND: The laryngotracheal junction (LTJ) is a functionally critical zone, due to its narrow anatomy, complex airflow dynamics, and proximity to the glottis and cricoid cartilage. Extensive stenoses, cricoid defects, and stomal anterior wall loss frequently exceed the possibilities of conventional resection and reconstruction. OBJECTIVE: To outline the current role of microvascular reconstruction at the LTJ, with a focus on indications, techniques, and limitations from the perspective of otolaryngology and thoracic surgery. METHODS: Narrative literature review of clinically established

High-fidelity simulation in airway management training: results from the AIR-MASTER-SIM randomised study.

INTRODUCTION: Airway management is a key competency in emergency medicine and critical care, and effective training in endotracheal intubation is essential during undergraduate medical education. Direct laryngoscopy has traditionally been considered the standard technique for airway access, although it can present technical difficulties for inexperienced operators. Videolaryngoscopy provides indirect visualisation of the glottis and may facilitate airway management during the early stages of training. This study compared the acquisition of airway management skills among medical students traine

Tramadol-Induced Persistent Singultus: A Case Report on Diagnosis, Mechanism, and Management.

Hiccups, or singultus, are involuntary diaphragmatic contractions followed by the sudden closure of the glottis, resulting in a characteristic sound. These episodes are generally transient but can be classified as acute (lasting < 48 h), persistent (lasting > 48 h), or intractable (lasting > 1 month). While most cases are self-limiting, persistent or intractable hiccups often suggest an underlying organic, psychogenic, or idiopathic cause. Among the potential causes, drug-induced singultus is a significant yet frequently underrecognized etiology. Although generally benign, persistent hiccups c

Case Report: External fixation of airway stents for management of tracheal stenosis secondary to thyroid cancer.

BACKGROUND: Thyroid cancer, in the advanced stage, often invades the airway and leads to tracheal stenosis. Airway stents can be used as a palliative treatment to restore airway and relieve dyspnea. However, stent migration remains a frequent complication, especially for lesions near the glottis. Simple fixing technology suitable for environments with limited resources is rarely reported. METHODS: A 79-year-old woman with advanced thyroid cancer developed upper tracheal stenosis due to tumor compression. To restore airway patency, the patient accepted placement of a self-expanding metallic st

Impact of Infraglottic Capture in Anterior Glottoplasty Pitch Elevation in an Ex Vivo Porcine Model.

OBJECTIVE: Endoscopic vocal fold shortening (EVFS) often focuses on the anterior-posterior (AP) web dimension as a proportion of vocal fold (VF) length. We investigated the craniocaudal dimension to determine whether thick infraglottic web geometry produces greater fundamental frequency (F0) elevation than thin webs of equivalent AP dimension. METHODS: Supraglottic structures were removed from eight porcine larynges. Compressed air passed through the glottis achieved phonation under three conditions: baseline, thin web, and thick web. Thin web sutures placed at a mean of 37% of VF length capt

Anesthetic Management of Traumatic Laryngotracheal Laceration in a Postpartum Patient: A Case Report.

Traumatic laryngotracheal lacerations are rare but immediately life-threatening injuries that require rapid, individualized airway management. The anesthesiologist faces the dual challenge of securing a compromised airway while avoiding progression of a partial tear to a complete transection. We report the case of a 35-year-old woman, 28 days postpartum, who presented to the emergency department after a self-inflicted penetrating anterior neck injury. On arrival, she had hoarseness, moderate respiratory distress, and extensive subcutaneous emphysema. Computed tomography of the neck demonstrate

Vocal-tract length estimation from vowel formants benchmarked against acoustic pharyngometry.

Estimating vocal-tract length (VTL) from vowel formants can aid speaker normalization, but few methods have been benchmarked against an anatomical reference in the same speakers. We combined acoustic pharyngometry (APh) and speech data from 42 adults to benchmark eight widely used formant-based VTL estimators against incisors-to-glottis length and to test an interpretable two-stage bias-corrected linear estimator. Across more than 400 000 central frames with valid F1-F4, traditional quarter-wave, odd-harmonic, and dispersion-type estimators correlated with VTLAPh but showed poor out-of-sample

Rostral direction virtual laryngoscopy in canine glottis under sedation with butorphanol, medetomidine, and alfaxalone.

The glottis of 26 dogs with respiratory signs was examined by virtual laryngoscopy for peering into the throat from within the trachea (RDVL: rostral direction virtual laryngoscopy) with and without sedation of butorphanol, medetomidine, and alfaxalone (BMA). These dogs are divided into two groups of with or without sedation. In 9 unsedated dogs, there were many Chihuahuas, and the primary symptom was coughing. In 17 sedated dogs, Toy Poodles and Pomeranians were the most common breeds, and the primary symptoms were coughing and worsening respiratory symptoms at excitement. RDVL provided a rel

Efficacy of two anatomically preformed supraglottic devices, the BlockBuster Laryngeal mask airway and the Ambu AuraGain, for ventilation in anesthetised paralyzed pediatric patients: a randomized trial.

BACKGROUND: The BlockBuster Laryngeal Mask Airway (LMA) provides a higher oropharyngeal seal pressure (OSP) than Ambu Aura Gain (AAG) in children at an intracuff pressure of 60cmH2O. Lower intracuff pressures prevent laryngopharyngeal morbidity; therefore, we compared their OSP at 40cmH2O. METHODS: Seventy children scheduled to undergo elective surgery under general anesthesia were randomly assigned to the AAG (Group A) or BlockBuster LMA (Group B) groups. After device placement, the cuff was inflated to 40cmH2O pressure. Ease of insertion, OSP, number of attempts, insertion time, glottic ali

A Closed Glottis After Shoulder Arthroscopy: A Rare Airway Complication of Interscalene Block Causing Postoperative Respiratory Failure.

Interscalene brachial plexus block (ISB) is commonly used for shoulder surgery and is associated with predictable complications such as phrenic nerve palsy due to its proximity to adjacent neural structures. While the spread of local anesthetic may involve the recurrent laryngeal nerve, resulting in vocal cord dysfunction, which is uncommon, progression to bilateral vocal cord paralysis, causing acute airway obstruction, is exceedingly rare. A 74-year-old obese female underwent elective outpatient right shoulder arthroscopy under ultrasound-guided interscalene block with bupivacaine. Within ho

Variability in manual editing of head and neck organs of interest auto-segmentations: a multi-user, longitudinal analysis.

BACKGROUND: Although auto-segmentation is widely used as a starting point for delineating organs of interest (OOIs), subsequent manual edits, and their variability over time and across users, may compromise workflow efficiency or treatment quality, limiting the intended benefits of auto-segmentation. Yet, longitudinal editing patterns are rarely assessed for clinical acceptability. AIM: To quantify editing of head-and-neck auto-segmentations over seven years, characterize variability among radiation therapy technologists (RTTs), and assess whether user-specific editing behavior aligns with pe

Case Report: Early diaphragmatic plication for combined phrenic and recurrent laryngeal nerve injury after VATS thymectomy.

Video-assisted thoracoscopic surgery (VATS) thymectomy may cause phrenic nerve injury (PNI) or recurrent laryngeal nerve (RLN) injury. Combined PNI and RLN injury is rare but can cause fatal respiratory failure. This study reported a case of VATS thymectomy in a 50-year-old man. The patient received a VATS thymectomy at an external hospital and presented to our hospital 10 days later with severe dyspnea and abdominal distention. Pre-surgery radiologic examination identified a 1.6-cm anterior mediastinal mass, and thymoma was considered. The pathology report confirmed benign reactive lymph node

Laryngeal Electromyography-Guided Injection Laryngoplasty: A Prospective Series.

OBJECTIVE: Laryngeal electromyography (LEMG) is a well-established method for botulinum toxin (Botox) injections and has also been advocated for the treatment of vocal fold paralysis. We hypothesize that LEMG may also help with the therapeutic injection of other substances. We report on 64 subjects and 118 LEMG-guided injection laryngoplasty procedures. Fiber-optic laryngoscopy was performed after injection to assess the validity of this approach. MATERIAL AND METHODS: Between 2020 and 2026, 64 subjects were selected for LEMG-guided injections, totaling 118 vocal fold injections. Inclusion cr

Effect of Left Head Rotation Maneuver on Glottic View in Pediatric Patients during Direct Laryngoscopy: A Prospective, Observational Study.

BACKGROUND: Achieving an optimal glottic view is critical in pediatric anesthesia to prevent hypoxia and bradycardia. The "Left Head Rotation" (LeHeR) maneuver has been proposed as a simple technique to improve laryngeal exposure. This study evaluated the impact of 45° LeHeR on Cormack-Lehane (CL) grading in pediatric patients compared to the classic sniffing position. METHODS: We conducted a prospective and observational study of 40 ASA I-II patients (aged 5-14 years) undergoing elective surgery. Following standardized induction, direct laryngoscopy was performed in the sniffing position usi

Comparison of anesthetic effects of different sedative drugs combined with remifentanil for general anesthesia in bronchoscopy: a randomized controlled trial.

BACKGROUND: Electronic bronchoscopy, as an important means of invasive diagnosis, exerts strong stimulation on anatomical structures such as the pharynx, glottis, carina. Therefore, selecting an appropriate anesthesia regimen not only enhances patient comfort and satisfaction but also guarantees the safety and the smooth running of the operation. This study aimed to compare the clinical anesthetic effects of different sedative drugs combined with remifentanil-based general anesthesia in patients undergoing bronchoscopy. METHODS: A total of 120 patients scheduled for elective painless electron

Evaluation of Airway Assessment Parameters: Hyomental Distance versus Ultrasound-guided Pretracheal Tissue Thickness as Predictors of Difficult Laryngoscopy.

BACKGROUND: Difficult airway remains a major cause of anesthesia-related morbidity and mortality despite advances in airway management. Conventional clinical predictors have limited predictive value. Airway ultrasound has emerged as a noninvasive tool that may improve preoperative airway assessment. AIM: The study aimed to evaluate the utility of ultrasonography as a preoperative assessment tool for predicting difficult airway and to compare its predictive accuracy with hyomental distance. MATERIALS AND METHODS: This prospective observational study included 100 adult patients undergoing elec

A novel minimally invasive intubation technique.

In clinical practice, tracheal intubation entails flexing a tracheal tube with a stylet and inserting it into the glottis under the guidance of videolaryngoscopy. Nevertheless, this approach may trigger intense sympathetic responses and elevate the risk of intubation failure. To address these issues, our research team put forward a minimally invasive intubation technique by using SEE-VL and SEEKflex, which is anticipated to enhance the first-pass success rate of intubation and mitigate stress reactions. This novel technology is expected to minimize the trauma associated with intubation and imp
