Malignant laryngeal lesions mimicking vocal process granuloma on flexible laryngoscopy: Two case reports.

Vocal process granuloma (VPG) is a common benign inflammatory lesion of the posterior glottis, typically induced by intubation injury, laryngopharyngeal reflux, or vocal abuse. Clinicians usually make an initial clinical impression of VPG merely through laryngoscopy, and routine biopsy is generally not performed. However, malignant lesions arising at the vocal process may appear highly similar to laryngeal granulomas under flexible laryngoscopy. We present two patients with a history of tracheal intubation. Their granuloma-like lesions were initially regarded as benign VPG, while postoperative

BAGLS-VF: a comprehensive dataset for glottal area and vocal fold segmentations.

OBJECTIVE: Datasets for automatic analysis of laryngeal endoscopic images are scarce, particularly those providing detailed anatomical annotations beyond the glottal area. The BAGLS dataset previously introduced a large collection of laryngoscopic images with expert annotations of the glottal area. To facilitate research on more detailed anatomical modeling and segmentation tasks, we introduce BAGLS-VF, an extension of the BAGLS dataset that includes additional annotations of the left and right vocal folds. DATA DESCRIPTION: BAGLS-VF contains 59,250 videolaryngoscopic frames with pixel-level

Vocal tract configuration and acoustic transfer characteristics in mechanisms of extreme vocal styles-A pilot study.

Nonlinear phenomenon common in extreme and traditional singing involves often coupled oscillations of glottal and supraglottic structures. Four such phonatory mechanisms-undertone, distortion, growl, and rattle-differ in supraglottic involvement and frequency ratio between glottal and supraglottic oscillators. However, the contribution of vocal tract (VT) configurations remains insufficiently understood. Using static three-dimensional magnetic resonance imaging and acoustic analysis, we investigated VT shaping in a professional singer. The VT was used to calculate volume-velocity transfer func

Validation of automated script-based proton therapy optimisation for comparative head and neck planning.

BACKGROUND AND PURPOSE: Proton and photon plan comparison is often the gateway to proton therapy for head and neck cancer (HNC). Manual planning is resource demanding, whereas automated planning may efficiently create consistent treatment plans. An automated rule-based intensity-modulated proton therapy (IMPT) plan optimisation script was developed within a commercial treatment planning system for comparative planning. This study benchmarked automated against clinical plans and evaluated whether a standard or patient-specific field set-up was necessary. MATERIALS AND METHODS: Twenty HNC patie

Single-System Laryngeal Anaphylaxis Following Gadobutrol Administration.

Gadolinium-based contrast agents (GBCAs) are widely used in magnetic resonance imaging (MRI) and are generally considered safe, with immediate hypersensitivity reactions being rare. While anaphylaxis typically presents with classic skin reactions, respiratory distress, or cardiovascular collapse, exposure to a known allergen can cause single-system respiratory or laryngeal variants. An 83-year-old male with multiple chronic medical conditions underwent a diagnostic brain MRI with contrast. Immediately after receiving 8.5 mL of gadobutrol, the patient developed hoarseness and a sporadic cough.

Difficult Airway Management of a Patient With Sturge-Weber Syndrome: A Case Report.

Sturge-Weber syndrome is a rare neurocutaneous disorder characterized by vascular malformations of the brain, eyes, and face, which can lead to difficult airway management. A 57-year-old man with Sturge-Weber syndrome presented with an oral hemangioma. Preoperative assessment, including computed tomography, raised concerns about anticipated difficult mask ventilation and tracheal intubation. Based on these findings, awake intubation was planned. After sedation and topical anesthesia, a video laryngoscope was used to visualize the glottis, and the trachea was successfully intubated. General ane

The incidence of difficult airways in an Australian adult tertiary-quaternary hospital: a retrospective observational study of 5,293 episodes of anesthesia.

PURPOSE: In this study, we aimed to evaluate the incidence and anticipation rate of difficult airways in adult nonobstetric patients undergoing general anesthesia at a single tertiary-quaternary Australian hospital, and to review the predictors of difficult airways and the impact of videolaryngoscopy (VL) on glottic visualization in unanticipated difficult airway cases. METHODS: We conducted a retrospective observational study over a 6-month period. We included 5,293 adult patients who underwent general anesthesia with airway instrumentation. Data was extracted from anesthetic records. Diffic

Endoscopic diode laser lysis of dorsal acquired glottic web and custom-made Silastic keel placement in a dog.

Laryngeal web is a rare condition in dogs that can cause upper airway obstruction. Surgery is the treatment of choice to remove the glottic obstruction. In veterinary medicine, ventral laryngotomy has been described for resection of laryngeal webs. In humans, the endolaryngeal surgical approach consists of laser ablation of the glottic obstruction, followed by temporary placement of a protective keel onto the exposed arytenoid mucosa to prevent excessive granulation tissue formation and recurrence. In the present case, a dog with a dorsal acquired glottic web associated with obstructive dyspne

Spectral envelope estimation of high-pitched vowels using a shared excitation proxy and autoregressive modeling with exogenous input.

Characterizing the vocal tract spectral envelope in high-pitched vowels is challenging due to sparse harmonics and source-filter coupling. This study proposes an analytical framework using a shared glottal driving proxy to guide source-tract decoupling. Speech is decomposed into multi-band Hilbert envelopes; first-order differentiation and non-linear rectification are then applied to isolate transient energy increments during glottal closure. Non-negative matrix factorization captures the cross-band temporal dynamics of these increments, yielding a macroscopic temporal proxy of glottal excitat

Airway stenosis following immediate bilateral recurrent laryngeal nerve reconstruction: report of two cases successfully treated with laser submucosal intermediate transverse resection of the arytenoid cartilage.

BACKGROUND: While the usefulness of immediate intraoperative reconstruction of the unilateral recurrent laryngeal nerve (RLN) has been reported, there are few reports of bilateral reconstruction. Furthermore, detailed evaluation of laryngeal function before and after surgical treatment for airway compromise caused by bilateral RLN paralysis remains limited. This report aimed to highlight a preventable complication of immediate bilateral RLN reconstruction and a salvage treatment option. CASE DESCRIPTION: We report two women who underwent immediate bilateral RLN reconstruction during surgery f

Removal of Fish Bones Embedded in the Hypopharynx Using a Novel Snare Device.

BACKGROUND: Fish bones can become embedded in the throat tissues while eating and are a common emergency department (ED) symptom. They can be difficult to visualize and remove, depending on location and proximity to the glottis. We present 3 cases of fish bone removal from the pharyngeal mucosa of adult patients using a novel, simple snare device that can be assembled using widely available ED supplies. DISCUSSION: All cases failed conventional removal techniques by emergency physicians and otolaryngologists and were destined for the operating room for removal. In all cases, removal was succe

GlideRite® rigid stylet vs Flex-it® articulated stylet for intubation with videolaryngoscopy in patients with semi-rigid cervical collar: a randomized trial.

BACKGROUND: Compared with the GlideRite® rigid stylet used with the GlideScope® hyperangulated blade, the Flex-it® stylet permits dynamic orientation of the tracheal tube toward the glottis. This randomized trial compared GlideRite and Flex-it stylets regarding GlideScope intubation duration in patients wearing a semi-rigid cervical collar, simulating conditions of cervical instability. METHODS: Eighty adult patients undergoing elective surgery were randomized to tracheal intubation using the GlideScope with either the GlideRite or the Flex-it stylet. A semi-rigid cervical collar was applied

Effects of glottal and velopharyngeal port openings on the first formant frequency in natural speech.

The present study investigated the effects of the glottal opening during phonation and small openings of the velopharyngeal port on the first vowel formant frequency (F1). While previous studies confirmed that these boundary conditions increase the formant frequencies of speech sounds based on models and simulations, such a trend was so far not evident in in vivo experiments with humans. To estimate the effects of the boundary conditions on F1 in natural speech, we recorded the audio signal simultaneously with optopalatography (OPG), electroglottography, and nasal airflow of 21 speakers produc

Flexible Endoscopic Laser Surgery for Glottic Carcinoma After Radiotherapy Failure: A First Experience.

BACKGROUND: Surgery is the only salvage treatment for radiotherapy (RT) failures in glottic carcinoma management, and total laryngectomy is performed in the majority of cases, resulting in a substantial decline in the quality of life for patients. Transoral CO2 laser microsurgery (TOLMS), as a salvage treatment for RT failures, demonstrated its efficiency, albeit more inferior compared to treatment results obtained in primary glottic carcinoma, but has some limitations, predominantly associated with difficult anatomy and inadequate glottis exposure and also because of mandatory larynx suspensi

Radiological risk factors for T2-T3 glottic carcinoma treated with (chemo)radiotherapy-a multicenter analysis.

OBJECTIVES: In moderately advanced (T2-T3) glottic cancer, eligibility for organ-preserving surgery is limited by tumor extension into certain high-risk areas within the larynx that can be identified on preoperative imaging and serve as predictive factors for poorer outcomes. Their relevance in conservative treatment modalities, including (chemo)radiotherapy ([C]RT), remains unclear. The objective was to evaluate the impact of these predefined surgical high-risk zones in patients treated with (C)RT. MATERIALS AND METHODS: This retrospective study included patients with T2-3, N0-3, M0 glottic
