Risk of cancer for arc welders in the Federal Republic of Germany: results of a second follow up (1983-8).

An extended follow up of 1221 chromium and nickel exposed welders in the Federal Republic of Germany confirmed an increased relative risk of 1.6 for all cancers compared with an internal reference group of 1694 turners. In an external comparison an excess of deaths from malignant tumours compared with that expected from the national mortality rates was found (standardised mortality ratio (SMR) = 109), which was clearly related to both time since first exposure and duration of exposure. Mortality from lung cancer was increased among welders (SMR = 113) but also among turners (SMR = 108). The di

[Substantiation of group MPEL for dispersive anthraquinone dyes].

The article presents results of the toxicological studies of 65 replaced anthraquinones: amino-, diamino-, haloido-, hydro- and oxianthraquinones. A similar polytropic toxic action was identified. MACs for anthraquinones, alpha-aminoanthraquinones and K-type dispersal dye-stuff were established at 5 mg/m3. The toxicity properties of 11 other dispersal dye-stuffs with anthraquinone and NF disperser were studied. Similarity in the toxic properties of the dispersal dye-stuffs and those of the earlier standardized anthraquinone derivatives were established. The group MACs for the pigment and NF di

[Free radicals in industrial medicine: their mechanisms of toxicity and biological relevance].

Several pathological processes have been attributed to or associated with free radical mechanisms, including inflammation, cardiac reperfusion injury, aging, and cancer. Moreover, free radicals (FR) produced as reactive intermediates or species during the metabolic activation of an increasing number of industrial chemicals or other toxic agents have been regarded as being primarily responsible for the toxicity of these compounds. Some of the mechanisms involved in FR-induced tissue damage are reviewed, using the following specific examples from occupational medicine: the acute hepatotoxicity o

Italian multicentre case-control study of hematolymphopoietic malignancies.

Exposures to organic solvents (including benzene), ionizing radiation and low frequency electromagnetic fields have been suggested as risk factors for leukemias in adults. Exposure to herbicides of the phenoxyacid variety (2, 4, 5-T; 2, 4-D) has been reported to be associated with an increased risk of non-Hodgkin's lymphomas. In addition several studies have suggested that agricultural workers have an excess risk of hematolymphopoietic malignancies, although no specific agent has yet been identified. Italy is particularly attractive for investigating further these associations, because: a) the

Exposure analysis and assessment for low-risk cancer agents.

The analysis of exposure is a necessary feature of epidemiological studies. In the case of low-risk cancer agents, the examination of potential exposure should follow a sequence that prioritizes the major media and routes of concern. This will minimize the misclassification of exposure and the failure to identify important co-factors, e.g. contamination in other media, etc. The manuscript describes specific components of exposure analysis and provides examples that pertain to single and multimedia exposure problems. The approach is examined by using the Total Human Environmental Exposure Study

Factors associated with human exposures to polycyclic aromatic hydrocarbons.

The number and quantities of PAH emitted and accumulated within various environmental media require the development and application of new methods for determining the major routes of exposure to individual and selected compounds. From research that has been conducted on airborne BaP and other selected PAH it appears that they can accumulate indoors as well as outdoors. Smoking seems to be a major contributor for direct and passive inhalation, but the role of cooking and wood burning related to indoor emissions needs to be investigated further. The available data indicate that many PAH will be

Sick building syndrome: acute illness among office workers--the role of building ventilation, airborne contaminants and work stress.

Outbreaks of acute illness among office workers have been reported with increasing frequency during the past 10-15 years. In the majority of cases, hazardous levels of airborne contaminants have not been found. Generally, health complaints have involved mucous membrane and respiratory tract irritation and nonspecific symptoms such as headache and fatigue. Except for rare examples of hypersensitivity pneumonitis related to microbiologic antigens, there have been no reports of serious morbidity or permanent sequelae. However, the anxiety, lost work time, decreased productivity and resources spen

Airborne endotoxin concentrations in various work areas within two cotton textile mills in the People's Republic of China.

As part of a multidisciplinary longitudinal approach to assess the roles of airborne cotton dust and endotoxins in affecting the respiratory health of cotton textile workers, this study was designed to quantify the endotoxin contamination of airborne vertically elutriated and total dusts. Yarn preparation areas (opening through fine spinning) were studied at two cotton textile mills which had been studied 5 years previously in Shanghai. People's Republic of China, Filter, with vertically elutriated (VE) or total dusts were mailed to the United States and endotoxin analyses were performed for e

The confounding of occupation and smoking and its consequences.

A strong pattern in smoking behavior can be demonstrated, in which smoking is much more prevalent among those occupational groups (and social strata) that are also more exposed to hazards in the workplace and much less prevalent among those groups less exposed to such hazards. As a consequence, comparing individuals with greater to those with lesser exposure to tobacco also compares groups that differ with respect to occupational exposure to dust, fumes and toxic substances and with respect to occupationally related lifestyle factors. Analyses of the U.S. National Health Interview Survey show

Stomach cancer and work in dusty industries.

The city of Stoke-on-Trent, whose major industries include coalmining, iron and steel, ceramics, and rubber, has death rates from stomach cancer some 80% above the national average. To explore the hypothesis that work in these dusty industries is responsible for the local excess of stomach cancer, we compared 95 incident cases with 190 age and sex matched community controls. Lifetime occupational histories and premorbid consumption of foods suspected of causing or protecting against stomach cancer were ascertained by a self administered questionnaire, supplemented at interview. Sixty eight cas

Twenty-six workers were studied (9 chrome-platers exposed to chromium dioxide and 17 workers exposed to metallic chromium dust) in order to investigate the macroscopic and cytological changes of the nasal mucosa due to exposure to water-soluble hexavalent chromium or to metallic chromium dust in the electroplating industry and the role of different valencies in the onset of nasal disease. Experimental and epidemiological data have shown that hexavalent chromium, which has strong oxidative power, induces more noticeable toxic effects on tissues and mucous membranes than other compounds. [Lead exposure and its effects on child health].

Childhood lead poisoning is a man-made disease whose magnitude has not yet been measured in Chile. In the U.S.A. it has been estimated that 4 percent of children have elevated blood lead levels and that among poor black children, this rate is as high as 18 percent. The main sources of symptomatic lead poisoning are lead-based paint used in interior and exterior house surfaces, airborne lead from leaded gasoline and industrial emissions. Lead can be transferred from any source to soil and dust. Other sources of lead are contaminated food and water, occupational sources, and lead glazed pottery.

Health effects of organic dust exposure in dairy farmers.

Exposure to farm dust may cause asthma, rhinitis, allergic alveolitis, organic dust toxic syndrome, and chronic bronchitis. Febrile reactions to inhaled dusts are common and are usually of a toxic rather than an allergic type. Some evidence indicates that dairy farmers may have an increased prevalence of chronic bronchitis, allergic asthma, rhinitis, and chronic airflow limitation, but this has not been conclusively shown. Storage mites and cow dander are important allergens in dairy farming. Inflammation of airways may in part be caused by immediate or delayed hypersensitivity. Other mechanis

Epidemiological and experimental applications to occupational cancer prevention.

At present, there is a whole array of risk factors, such as industrial chemicals, drugs, pesticides, complex chemical mixtures, physical and biological agents, for which there is a proven causal relationship with human cancer. The epidemiological studies are essential in providing proof for a causal relationship in humans. However, the epidemiological approach to the identification of the etiology of cancer is limited for two main reasons: only relatively high risks can be detected; and, secondly, the epidemiological surveys are based on observations of the effects as a consequence of an expos

[Methodologic principles for the prognosis of chronic toxicity of substances based on results of short-term trials].

One of the most important concerns of air-hygienic research is the determination of the threshold concentration for the influence of air-contaminating substances. The threshold concentrations are the basis of the air-hygienic norms. The experimental determination of the threshold concentration with chronical influence is expensive and needs a lot of time. A new method of extrapolation of the toxicity values obtained from subacute experiments is described. Different from other methods of extrapolation the biological qualities of the substances as well as the peculiarities of the toxic process a

[Occupational exposure of the anesthesiologist to nitrous oxide and halothane. Control measures].

Anesthetic gas spillage does occur in the operating room. Anesthesiologists and other operating room personnel are likely exposed to health hazards caused by chronic exposure to anesthetic gases and vapours. The aim of the present study is to elucidate the possible relationship between the chronic exposure to anesthetic gases and their adverse effects on the health. Present data, while controversial, do not establish a cause-effect relationship. Nevertheless, until solid data refutes the existence of a health hazard for operating room personnel, the anesthesiologist must try to reduce the anes

Occupational exposures: evidence for a causal association with chronic obstructive pulmonary disease.

The increase in morbidity and mortality attributable to chronic obstructive pulmonary disease (COPD) has focused attention on environmental and host factors causally associated with the clinical entities included under the rubric of this term with a view to early preventive intervention. Despite the biologic plausibility of inhaled agents being causally implicated, only the role of tobacco smoke has been accepted beyond doubt. However, evidence implicating occupational exposures has accumulated, in particular over the last 2 yr, from: (1) community-based studies (in which larger study populati

A review of 132 consecutive patients referred for assessment of vibration white finger.

One hundred and thirty-two subjects were referred to the Institute of Occupational Health in Birmingham over a 2 year period for assessment as to whether they met the criteria for Prescribed Disease A11 (vibration white finger). A standardized questionnaire and checklist was used, and each patient with vibration white finger was staged according to the Taylor and Pelmear (1975) classification. One hundred and sixteen subjects (88 per cent) had a clinical picture consistent with a diagnosis of vibration white finger, of which 10 did not meet the DHSS criteria for prescription. For these 116 sub
