Regulations dealing with noise and vibration are examined in the light of general precepts on safeguarding the physical health of workers to be found in the Civil Code, the workers' charter and, i.e. those included in the prevention of accidents at work legislation, that dealing with health in the work environment, and the recent national health service law. A brief account is also given of the approach taken by the courts to the disputed question of the evaluation of damage to the hearing, and maximum noise acceptability levels. Reference is also made to a personal study of the risks imposed

A hearing impairment can have profound psychologic consequences. A major cause of hearing loss is exposure to excessive noise in the workplace. Stricter regulations to protect the hearing of workers became effective this year. A constructive role for the family physician is to educate the patient about the importance of hearing conservation, to reinforce compliance with personal hearing protection measures, and to support appropriate rehabilitative intervention.

The multistage theory of carcinogenesis and its implications for evaluating the effect of exposure to carcinogens in the workplace are described. This theory predicts different relationships between excess carcinogenic risk and duration of exposure, age at initial exposure, and follow-up time since exposure stopped. These relationships are shown to depend on the stage of the carcinogenic process affected by the carcinogen, i.e., action at an early stage or a later stage. The patterns of excess lung cancer mortality were examined for a cohort of copper smelter workers exposed to atmospheric ars

After several years of occupational exposure to vibration, a man developed episodic vibratory hand swelling, erythema, and pruritus. He subsequently developed symptoms compatible with carpal tunnel syndrome, but his median nerve conduction velocity remained normal. After experimental vibration of the forearm, plasma histamine levels in the ipsilateral antecubital vein rose in association with localized angioedema and slowed median nerve conduction velocity. There was no evidence of other causes of angioedema. Occupational exposure to vibration may predispose to the development of vibratory ang

A health study was carried out on 2066 workers in an aluminum smelter in Kitimat, British Columbia to study the effects of exposure to fluoride and other air contaminants encountered on the potlines on the musculoskeletal system, hemopoietic tissue, liver, and renal function. Three hundred seventy-two railway repair workers from Squamish, British Columbia served as an "external" control group. Examination of the spine and sacroiliac joints and pelvic X-ray were conducted on long-term potline workers and a number of "internal" control workers in the smelter not exposed to any air contaminants.

This review on hygienic, toxicologic and ecologic aspects of environmental burden of lead deals with the following topics: occurrence, production and use of lead; metabolism of lead and experimental toxicology of lead; toxicity of lead to man; hygienic and ecologic aspects of lead pollution. Lead ore mining and processing, production of lead-acid batteries and combustion of gasoline containing tetraethyl-lead as an anti-knock ingredient constitute at present the most important sources of lead pollution. The last named source of lead pollution in urban atmosphere is most hazardous to human heal

Exposure of nuclear medicine personnel to 133X was examined quantitatively at three area hospitals during ventilation-perfusion studies in which the technologists breathed through a specially made xenon-trapping apparatus. The accumulated mean xenon activity varied a great deal from hospital to hospital, ranging from 52 nCi (1.92 kBq) to over 5 microCi (185 kBq) during a typical 20-minute lung study. The observed difference largely depended on the xenon exhaust and trapping systems, which could make a 100-fold difference in exposure rates. The air flow and its exchange rate in the room were ad

Through the years, considerable amounts of Pb have been mobilized into our environment, and Pb may represent one of the most ubiquitous of all toxic metal contaminants. Excluding occupational exposure, the general population receives almost 70% of its total exposure to Pb from food. Other important sources of exposure include drinking water and air; minor sources include tobacco products, decorative glassware, and other types of food utensils. For young children, ingestion of Pb-containing soil or dust via normal hand-to-mouth activity or, in extreme cases, pica (the abnormal intake of soil, p

Environmental health monitoring and surveillance include activities such as collection of information on the production and use of chemicals; preparation of inventories of waste discharges; measurement of physical, chemical and biological agents in air, water and food, at work place and at home; epidemiological investigations, and collection and analysis of environmental, and health statistical data. There are two main objectives of these activities: estimation of human exposure to potentially harmful environmental factors and timely detection of adverse health effects; and the assessment of e

A follow-up study over 12 years was conducted among 556 men aged 30 to 54 in 1960 and working at that time in factories around Paris (France). Various occupational exposures were recorded at the time of the 1960 survey after a technical study of each workplace. The annual rate of decline of FEV1 during 12 years was estimated for each subject from the measurements in 1960 and 1972. This rate (the FEV1 slope) was related independently of FEV1 level (which reflects the loss since the beginning of adult life) and of smoking habits to occupational exposure to dust, gases, and heat. FEV1 slope was s

Histologic sections of liver and spleen from 99 retired coal workers and nine non-coal workers were obtained at autopsy and scored for black pigment. Pigment was minimal in the non-coal workers, with the exception of one person with silicosis. Moderate or heavy pigment was seen in 10.4 per cent of liver sections and 19.5 per cent of spleen sections from coal workers. The extrapulmonary pigment was not associated with any pathologic tissue response. Information on pulmonary pathology and occupational exposure to dust was available for most workers. Highly significant positive correlation was fo

Urinary hydroxyproline excretion was measured in 130 chain saw operators aged 28-59 and in 31 normal control subjects aged 26-59 with no occupational exposure to vibration. The results measured were expressed as a ratio of hydroxyproline to creatinine. (1) No significant correlation between hydroxyproline/creatinine ratio and age was observed among normal subjects. (2) In chain saw operators grip and pinch strength decreased gradually with an increase of the total chain saw operating time, while the prevalence of pain in the hands or motility disturbance in the elbow joints showed a tendency t

Hearing threshold was measured with digital audiometer CASK-431 in transportable soundproof booth GIG-AU-1 in 170 women and 75 men before and at the end of the afternoon shift work in the weaving-mill with shuttle looms, where an average noise level was 100 dB(A). The average hearing losses calculated from the formula dB (1000 Hz + 2000 Hz + 4000 Hz): 3 were higher than those calculated from the formula db (500 Hz + 1000 Hz + 2000 Hz): 3. There was also a higher correlation coefficient between the occupational exposure to noise and the values of hearing loss calculated according to the former

A report by Lee and Fraumeni in 1969 linked exposure to arsenic and other contaminants to a threefold excess of respiratory cancer among 8,047 employees at the Anaconda copper smelter. We established vital status through December 1977 for a sample of 1,800 men from the original cohort. Average arsenic concentrations were estimated for each smelter department based on industrial hygiene measurements made from 1943 to 1965. Departments with similar concentrations were combined into four categories of exposure: 1) low (less than 100 micrograms/m3), 2) medium (100-499 micrograms/m3), 3) high (500-

Sample of airborne dust generated the handling of aflatoxin-contaminated corn were collected and analyzed to assess potential exposures of farmers and other agricultural workers to these mycotoxins. Using high volume total dust samplers and a high volume Andersen sampler, downwind dust samples were collected on glass filter when contaminated corn was transferred by augers from a storage bin into a wagon and back into the storage bin. The aflatoxin B1 content of the 15 dust samples ranged from 12.5 to 204.3 ppb, with an average of 138 ppb; the aflatoxin B2 content ranged from 1.1 to 41.6 ppb, w

There is increasing global concern on the upward trend of cancer attributable to environmental causes. This has found articulate expression in the intensive action programme initiated in developed countries to carry out systematic epidemiological surveillance studies on environmental based cancer, to conduct both short-term and long-term studies on experimental animals on the mechanism of induction and to search for preventive measures. Of all environmental agents, chemicals which have been introduced by man have received the maximum attention on account of the fact that besides functioning pe

The possible contribution of occupational and environmental exposures to cancer has been known for many years and is now a highly mature field of study. By the 1950s, a substantial list of agents of processes had been identified as associated with cancer of one organ or another. In the last several decades a number of additions have been made to the list. No doubt more will be found in the future. The last decade, especially, has brought increased public attention to cancer in the workplace. The Occupational Safety and Health Act of 1970 has been a significant contributor to the current increa

Four virus strains -- influenza A/Texas 1/77 (H3N2) an A/USSR 90/77 (H1N1), adenovirus and parainfluenza type 2 -- were isolated from different specimens collected in dental surgery rooms. High levels of antibodies to respiratory viruses (influenza, parainfluenza 1, 2 and 3), adeno-, cytomegalic and herpes viruses, as well as the presence of HBsAg and anti-HBs could be detected in sera from patients hospitalized for dento-maxillofacial surgery. An epidemiological survey among dentists and dental assistants confirmed their exposure to an enhanced risk of occupational virus contamination, as com

In a ceramics plant employing 225 workers, respirable dust was collected into personal samplers worn by exposed workers. Analysis by X-ray diffraction documented exposures to free silica greater than 50 micrograms/m3-the permissible exposure level or threshold limit value-time weighted average proposed by the U.S. National Institute of Occupational Safety and Health-in 15 of 25 determinations. Excessive exposures were produced by dust with quartz content less than or equal to 8% by weight. If there had been such exposures to free silica-containing dust in the past, X-ray findings compatible wi

The incidence of infections caused by opportunist mycobacteria in Wales during the years 1952--1978 was derived from the records of the Mycobacterium Reference Unit in Cardiff. Mycobacterium kansasil gave rise to 154 pulmonary and two nonpulmonary infections. Most of these patients were middle-aged men, approximately half of whom had pneumoconiosis or occupational exposure to dust. Organisms of the Mycobacterium avium-intracellulare-scrofulaceum complex caused pulmonary disease in 44 subjects and nonpulmonary disease in 20. Those with pulmonary infections had a mean age of 57 years, and the ma
