Effect of a two-hour training on physicians' skill in interpreting Pneumoconiotic chest radiographs.

OBJECTIVE: Occupational lung diseases have specific radiographic manifestations not always well known by physicians. In Japan, asbestos-related diseases became a public health concern after the "Kubota Shock", when a number of workers and residents living nearby a manufacturer of asbestos-made ducts developed mesothelioma caused by asbestos exposure. This preliminary intervention trial evaluated the effect of two-hour training on inexperienced physicians' skill in interpreting pneumoconiotic chest radiographs. METHODS: One hundred-two Japanese physicians participated in two reading-tests, usi

Hybrid Single-incision Laparoscopic Restorative Proctocolectomy with Ileal Pouch Anal Anastomosis for Ulcerative Colitis.

PLAIN SUMMARY: This article examines the overall experiences of community researchers in their involvement with the 'PROMPT' project for smoking cessation, which targeted community members who were homeless or at-risk for homelessness. More specifically, four community members, representing the study population were involved in the project as researchers. They were asked to complete surveys at both the beginning and end of each research training session to better understand their learning as it related to using a key instrument for this project, a spirometer, to measure project participants' l

[Prevalence of respiratory symptoms and associated factors: a population-based study in adults from Lages, Santa Catarina State, Brazil].

A cross-sectional population-based study was conducted in Lages, Santa Catarina State, Brazil, in order to estimate the prevalence of symptoms of chronic bronchitis, breathlessness, and wheezing and associated factors in a sample of adults 20 to 59 years of age (n = 2,051). The study employed a cluster sample design and Poisson regression analyses. Prevalence rates for chronic bronchitis, breathlessness, and wheezing were 5%, 35.7%, and 20.2%, respectively. The adjusted analysis showed chronic bronchitis associated with low schooling, smoking, and hospital admissions due to respiratory illness

Pathogenesis of COPD. Part III. Inflammation in COPD.

Chronic obstructive pulmonary disease (COPD) is mostly caused by cigarette smoking and affects up to 25% of smokers. Air pollution and occupational exposure to dust and fumes can also induce COPD. COPD is characterised by airflow limitation that is not fully reversible and chronic inflammation of the lung. Most patients with COPD also have evidence of tissue remodelling in the smaller airways. How the different pathological features are linked remains unknown. The inflammation of the COPD lung is initially caused by cigarette smoke and the increased infiltration of immune cells into the lung,

Evaluation of primary DNA damage, cytogenetic biomarkers and genetic polymorphisms for CYP1A1 and GSTM1 in road tunnel construction workers.

In tunnel construction workers, occupational exposure to dust (alpha-quartz and other particles from blasting), gases (nitrogen dioxide, NO(2)), diesel exhausts, and oil mist has been associated with lung function decline, induction of inflammatory reactions in the lungs with release of mediators that may influence blood coagulation, and increased risk of chronic obstructive pulmonary disease. The present molecular epidemiology study was designed to evaluate whether occupational exposure to indoor pollutants during road tunnel construction might result in genotoxic effects. A study group of 39

[Occupational exposure to silica dust by selected sectors of national economy in Poland based on electronic database].

BACKGROUND: To evaluate occupational exposure to dusts, the Nofer Institute of Occupational Medicine in Łódź, in collaboration with the Chief Sanitary Inspectorate, has developed the national database to store the results of routine dust exposure measurements performed by occupational hygiene and environmental protection laboratories in Poland in the years 2001-2005. It was assumed that the collected information will be useful in analyzing workers' exposure to free crystalline silica (WKK)-containing dusts in Poland, identyfing exceeded hygiene standards and showing relevant trends, which illu

Ecotoxicological screening of Kenyan tannery dust using a luminescent-based bacterial biosensor.

Ecotoxicological screening of dust sampled throughout a Kenyan tannery was conducted using a luminescence (lux)-based bacterial biosensor for both solid and liquid assays. This was complemented by chemical analysis in an attempt to identify possible causative toxic components. The biosensor results showed a highly significant (p < 0.001) difference in both solid and liquid phase toxicity in samples collected from various identified sampling points in the tannery. A positive correlation was observed between results of the solid and liquid phase techniques, for most of the sampling points indica

Reanalysis of the Harvard Six Cities Study, part I: validation and replication.

Because the results of the Harvard Six Cities Study played a critical role in the establishment of the current U.S. ambient air quality objective for fine particles (PM(2.5)), the U.S. Environmental Protection Agency, industry, and nongovernmental organizations called for an independent reanalysis of this study to validate the original findings reported by Dockery and colleagues in the New England Journal of Medicine (vol. 329, pp. 1753-1759) in 1993. Validation of the original findings was accomplished by a detailed statistical audit and replication of original results. With the exception of

Reanalysis of the Harvard Six Cities Study, part II: sensitivity analysis.

Following the validation and replication of the Harvard Six Cities Study (Krewski et al., this issue), we conducted a wide range of sensitivity analyses to explore the observed associations between long-term exposure to fine particle or sulfate air pollution and mortality. We examined the impact of alternative risk models on estimates of risk, taking into account covariates not included in the original analyses. These risk models provided a basis for identifying covariates that may confound or modify the association between fine particle or sulfate air pollution and mortality, and for identify

A case of intestinal-type sinonasal adenocarcinoma.

Intestinal type sinonasal adenocarcinomas are a rare malignancy of the nasal cavity and paranasal sinuses. Its association with lengthy occupational exposure to dust has been well documented, but sporadic cases have been reported. Diagnostic imaging is essential to exclude metastatic disease from the colon and may be supported by immunohistochemical studies of biopsy specimens. Treatment consists of surgery, radiation, or both, but its effect on prognosis is heavily influenced by the stage at which the disease presents.

Increased mortality in COPD among construction workers exposed to inorganic dust.

The aim of this study was to find out if occupational exposure to dust, fumes or gases, especially among never-smokers, increased the mortality from chronic obstructive pulmonary disease (COPD). A cohort of 317,629 Swedish male construction workers was followed from 1971 to 1999. Exposure to inorganic dust (asbestos, man-made mineral fibres, dust from cement, concrete and quartz), gases and irritants (epoxy resins, isocyanates and organic solvents), fumes (asphalt fumes, diesel exhaust and metal fumes), and wood dust was based on a job-exposure matrix. An internal control group with "unexposed

The Platino project: methodology of a multicenter prevalence survey of chronic obstructive pulmonary disease in major Latin American cities.

BACKGROUND: The prevalence of Chronic Obstructive Pulmonary Disease (COPD) in many developed countries appears to be increasing. There is some evidence from Latin America that COPD is a growing cause of death, but information on prevalence is scant. It is possible that, due to the high frequency of smoking in these countries, this disease may represent a major public health problem that has not yet been recognized as such. The PLATINO study is aimed at measuring COPD prevalence in major cities in Latin America. METHODS/DESIGN: A multi-country survey is being carried out in major cities in Lat

Respiratory symptoms and intensity of occupational dust exposure.

OBJECTIVES: Occupational exposure to dusts may result in chronic respiratory symptoms. METHODS: To investigate the utility of obtaining a history of occupational exposure to dust in US veterans, a respiratory health survey was conducted between 1988 and 1992 in a community-based cohort of US veterans in southeastern Massachusetts that were eligible for Veterans' Affairs (VA) healthcare benefits but were not regular users. A mail questionnaire was used to obtain a history of cough, phlegm, and wheeze, work in a dusty job, and duration, type, and intensity of dust exposure. Information on cigar

Epidemiological aspects and early detection of chronic obstructive airway diseases in the elderly.

The burdens of chronic obstructive airway diseases among the elderly in Europe, and worldwide, are increasing. Although asthma is common in all ages, the main airway disease affecting the elderly is chronic obstructive pulmonary disease (COPD). The aim of this paper is to review the prevalence and incidence of COPD on the basis of population studies. As the prevalence estimates of asthma are probably well known, only the incidence and remission of asthma will be discussed. The underdiagnosis of obstructive airway diseases is huge. A Dutch programme for early detection of obstructive airway dis

Residence near a major road and respiratory symptoms in U.S. Veterans.

BACKGROUND: There is evidence that exposure to motor vehicle exhaust is associated with respiratory disease. Studies in children have observed associations with wheeze, hospital admissions for asthma, and decrements in pulmonary function. However, a relationship of adult respiratory disease with exposure to vehicular traffic has not been established. METHODS: We studied a sample of U.S. male veterans drawn from the general population of southeastern Massachusetts. Information on respiratory symptoms and potential risk factors was collected by questionnaire. We assessed distance from residenti

INTRODUCTION: The discovery of anthracotic plaques generally suggests either a history of tuberculosis or occupational exposure to dust. Other etiologies should, however, be considered. CASE REPORTS: A 60-year-old Iranian woman presented with a history of dyspnoea and with chest radiography demonstrating calcified hilar lymph nodes and interstitial lung disease.
