Sub-glottic Diameter Measured by Ultrasonography Predicts Outer Diameter of the Clinically Best Fit Endotracheal Tube in Infants Undergoing Pediatric Cardiac Surgery.

BACKGROUND: Tracheal intubation with an oversized endotracheal tube (ETT) in a pediatric patient can lead to serious complications following extubation, including stridor, hoarseness, difficulty in breathing, increased work of breathing, respiratory failure, cardiac failure, and reintubation. Whereas undersized ETT may lead to erratic delivery of preset tidal volume during mechanical ventilation and increase the risk of aspiration of gastric contents. This study investigates the correlation between ultrasound measured subglottic airway diameter and ETT size selected by the anesthesiologists in

Comparison and Threshold Analysis of the ODET/GTD Ratio Between Patients With and Without Postoperative Sore Throat: A Retrospective Study.

AIM: This study aimed to retrospectively compare the outer diameter of the endotracheal tube to glottic transverse diameter (ODET/GTD) ratio between patients with and without postoperative sore throat (POST). An optimal ODET/GTD ratio cut-point was explored to provide a reference for endotracheal tube (ETT) selection and potentially help manage the risk of POST. METHODS: We reviewed 150 patients in the Post Anesthesia Care Unit (PACU). Among these, 68 patients developed POST and 82 did not. The ODET/GTD ratio was calculated for each patient. This ratio was compared between the two patient gro

Growth and Development of Human Laryngeal Functions.

OBJECTIVES: Human laryngeal functions are respiration (airway), local immunity, airway protection (swallowing) and phonation. Newborn larynges resemble those of non-human primates and lack mature functional structures like adults. This study summarizes the development of laryngeal functions, focusing particularly on their morphological and functional interrelationships. METHODS: Human larynges from autopsy cases were examined using whole-organ serial section studies, integrating findings from our previous studies. RESULTS: In newborns, the posterior glottis (intercartilaginous portion) occup

Bronchoscopic Internal Traction for Airway Foreign Body Removal via Tracheotomy Under ECMO.

We present a technique utilizing internal traction-assisted mobilization to effectively retrieve objects, such as pen caps, that lack an external grasping surface and resist conventional rigid bronchoscopy. In this case, tracheotomy served as an alternative extraction route for impacted foreign bodies that failed to pass across the glottis or subglottic region. Concurrently, ECMO maintains oxygenation during complex airway manipulation and reduces the risk of hypoxia.

Lateral intubation with the Macintosh laryngoscope and C-MAC D blade videolaryngoscope (LATIN): A randomized controlled trial.

BACKGROUND AND AIMS: Lateral intubation is frequently encountered outside operating room settings, particularly for endoscopic procedures. Challenges faced by anesthesiologists include a lack of training and experience in performing intubation in the lateral position, and the paucity of data regarding the ideal method. This study was aimed at comparing the glottis visualization times and intubation times with Macintosh laryngoscopes and C-MAC D blade video laryngoscopes in American Society of Anesthesiologists (ASA) 1 and 2 patients without any risk factors for difficult airway, in the right l

[Pros/Cons: Airway Management for Separate-side Ventilation: Always Use a Video Laryngoscope?].

The optimal technique for airway management during intubation with a double-lumen tube (DLT) in thoracic anesthesia remains unclear. While efforts are now being made to use a video laryngoscope for routine use with single-lumen tubes, its use with DLTs remains a topic of debate. This article presents an analysis from two perspectives in the style of a debate, in this case with a slight wink as a plea in court, comparing video laryngoscopy (VL) with direct laryngoscopy (DL) for routine use. The "defense position" highlights evidence demonstrating improved visibility of the glottis, higher succe

Standard gastroscope-guided endotracheal intubation in the lateral position for life-threatening variceal bleeding: a report of technical feasibility and airway safety.

BACKGROUND: Patients with acute upper gastrointestinal hemorrhage (UGIH) often present with airway contamination due to hematemesis and gastric reflux. Conventional endotracheal intubation (ETI) in the supine position faces challenges such as limited visualization and high aspiration risk. While securing the airway and controlling bleeding are both critical during resuscitation, simultaneous achievement of these goals remains technically difficult. CASE PRESENTATION: A 56-year-old woman with esophageal variceal rupture presented with hemorrhagic shock. After left-lateral positioning and rapid

A Real-time Scale-robust Network for Glottis Segmentation in Nasal Transnasal Intubation.

Nasotracheal intubation (NTI) is a critical clinical procedure for establishing and maintaining patient airway patency. Machine-assisted NTI has emerged as a pivotal approach for optimizing procedural efficiency and minimizing manual intervention. However, visual detection algorithms employed for NTI navigation encounter significant challenges, including complex anatomical environments and suboptimal illumination conditions surrounding the glottis. Additionally, the glottis presents considerable scale variability throughout the procedure, initially appearing as a small, difficult-to-capture st

TI-YOLO: A Lightweight and Efficient Anatomical Structure Detection Model for Tracheal Intubation.

Accurate and rapid detection of anatomical structures, such as the glottis, is critical during tracheal intubation (TI) to ensure patient safety and procedural success. However, it remains a challenge due to the limited field of view and computational resources of video laryngoscopy, especially for difficult airway situations. Existing deep learning (DL) models struggle to balance high accuracy and real-time clinical deployment. To address these issues, we propose TI-YOLO (TI-You Only Look Once), a lightweight and efficient object detection model built upon the YOLOv11 architecture. TI-YOLO in
