Results of a study of the vasculature of the anterior commissure, part of a larger study of the larynx, are presented. The vasculature of the anterior commissure is dependent on the medial branch of the anteroinferior laryngeal artery. The techniques used, postmortem angiography, dissection, cleared method, microangiography and histological sections, made it possible to follow the course of the artery and determine its configuration. A retrospective review of patients treated by surgery and/or radiation for carcinoma of the supraglottic larynx and the pyriform sinus was accomplished for the period 1964-1974. This patient population reflects the 10-year period following earlier experience with conservation surgery at this institution, and, thus, updates the current status of treatment for these cancers. Endpoints examined included survival, control, pattern of failure, the influence of nodal metastasis, and voice preservation. 80 patients with recurrent carcinomas of the oropharynx, hypopharynx and supraglottis were examined. Average recurrency occured after 15,1 months. 10 patients showed first and 3 patients second recurrent carcinomas on the mucous membranes of the upper respiratory tract. All the recurrencies were located distant from the primary tumor site, within the irradiation field and most of the times contralaterally. Lymphatic vessel damage and inversion of the regular lymphatic drainage are discussed for their etiological role.

The histological structure of the supraglottic submucosa consists of a wide-mesh connective net, which builds compartments where glands are included. Somewhere, the walls of compartments are so thick that they may be considered as true ligaments. Dionosil and powdered tantalum laryngograms are compared in 18 patients with known laryngeal cancer. Dionosil more reliably defined the epiglottis, valleculae, and pyriform fossae. The glottic and subglottic regions were more clearly visualized with the use of tantalum. Both agents were equally effective in the evaluation of the supraglottis.

The consequences of immobilization error are explored through a model involving the tumor dose-response characteristics. The quantitative effect of such an error depends upon the field size, the shape of the dose-response function, and the position of the nominal dose being delivered on the dose-response function. Squamous-cell carcinoma of the supraglottis and Hodgkin's disease are discussed and values are calculated for the change in tumor control probability which might accompany immobilization error.
