Feasibility and acceptability of a shelter-based health empowerment program for pregnant and parenting youth experiencing homelessness.

Parenthood is common among youth experiencing homelessness (YEH), with multi-generational implications for health. Research on how to improve health among this historically marginalized population is limited. Our community-university partnership assessed the feasibility and acceptability of a shelter-based health empowerment program, which aimed to improve health among YEH and their children and prevent unplanned pregnancies. Drawing on frameworks from implementation science, we conducted semi-structured interviews with program participants (n = 11). We then conducted thematic analysis, using

Decentralized Solar-Driven Plant Factories Secure an Adequate Global Vegetable Supply beyond the Electricity Grid and Water Limitations.

Conventional open-field vegetable production is increasingly constrained by climate volatility and natural resource endowments, creating supply instabilities and shortages that threaten global food resilience and human health. Here, we integrate solar photovoltaics and atmospheric water harvesting with plant factories, creating a technically mature off-grid agricultural system using plant factories (OASIS). By decoupling production from local water and grid infrastructure, OASIS significantly expands the area suitable for cultivation. Globally, OASIS can meet the recommended upper vegetable in

The cyclical and intersecting influence of substance use, homelessness and criminal legal policy on risk among minoritized people who use drugs.

The US has seen an alarming rise in overdose mortality among people experiencing homelessness and incarceration. Drug overdose and homelessness are dual crises that disproportionately impact Black, Hispanic, and American Indian/Alaska Native communities due to historic and ongoing structural racism. Our objective was to better understand the intersection of substance use, homelessness, and criminal-legal involvement among these populations to inform harm reduction and policy. We completed in-depth semi-structured interviews with racially minoritized people who use drugs (PWUD) in Michigan, New

Help me, and I'll help myself (and others). A narrative inquiry into older women's experiences of health and homelessness.

BACKGROUND: While women's homelessness is increasingly recognized as a gendered issue, experiences of aging while homeless remain underexplored. This study examined how older women navigate intersections of aging, health, and homelessness, focusing on how these experiences are shaped by structural and social factors. METHODS: Women's lived experiences were explored and interpreted using a narrative approach. Eleven women (aged 57-65 years) took part in two in-depth interviews each, followed by participation in three sequential focus groups. Data were analyzed iteratively informed by Riessman'

Implementing AMR surveillance at a tertiary hospital in a low-income country: results from the Surveillance Partnership to Improve Data for Action on AMR (SPIDAAR) in Malawi.

BACKGROUND: Antimicrobial resistance (AMR) is a growing global health threat, particularly in low- and middle-income countries (LMICs) where infectious diseases are more prevalent. Effective surveillance in these settings is key for tracking resistance trends and informing public health interventions, yet few implementation studies have delineated the particular challenges and opportunities for antimicrobial stewardship (AMS) activities in LMICs. The objective of this study was to evaluate the impact of a multi-component AMR surveillance capacity-building intervention on institutional surveill

Neighborhood violence, visible drug activity, and substance use among urban youth: Mental health and community factors.

BACKGROUND: Adolescent substance use and exposure to neighborhood violence frequently co-occur in U.S. urban settings, particularly among racially minoritized and low-income youth. Despite shared exposure to environmental risk, some adolescents report low to no substance use. Guided by the adverse community experiences and resilience framework, this study identified classes of neighborhood risk and substance use among urban adolescents and examined whether emotional, behavioral, community, and demographic factors differentiated profile membership. METHODS: Cross-sectional survey data were col

Preconception, Prenatal and Postnatal PM2.5 Exposure and Neurodevelopmental Disorder Risk among Medicaid Recipients.

Evidence linking ambient fine particulate matter (PM2.5) exposure to childhood neurodevelopmental disorders (NDDs) remains mixed. We conducted a cohort study (2001-2013) of 1,547,244 Medicaid-enrolled mother-child pairs to assess associations between zip-code-level PM2.5 exposure during preconception (12-week), prenatal (37-week), and postnatal (3-year) periods and children's NDD risk. Cox proportional hazards models estimated risks of autism spectrum disorder (ASD), attention-deficit/hyperactivity disorder (ADHD), learning difficulty (LD), speech/language disorder, developmental coordination

Breast cancer screening for persons experiencing homelessness.

BACKGROUND: Primary care providers (PCPs) care for their patients in part by helping them to follow cancer screening guidelines, including screening mammography. However, completing screening and follow-up due to abnormal results can be difficult for PCPs caring for patients with barriers to care, such as persons experiencing homelessness (PEH). The large population of PEH in the US have many unmet healthcare needs, high cancer prevalence and mortality, and therefore a need for cancer screening interventions. Hence we created and evaluated a specialized breast cancer screening program for PEH.

The Health Impacts of Extreme Weather Events: A Clinical and Equity Lens.

As clinicians, we are faced with challenges when it comes to extreme weather events that are accelerating and reshaping clinical practice; driving rising heat-related illness, worsening air quality, expanding infectious disease ranges, and increasing injuries, displacement, and mental health strain. These impacts fall hardest on communities already facing structural inequities, including older adults, low-income households, pregnant individuals, racial and ethnic minorities, outdoor workers, people with disabilities, and those experiencing homelessness. As climate-driven hazards intensify, cli

Heterogeneous impact of health insurance on children's nutritional status: insights from Instrumental Variable Quantile Regression (IV-QR) approach.

OBJECTIVE: Empirically, this study focuses on the impact assessment of health insurance uptake on the nutrition of kids under five with special attention to variation in wealth level in Pakistan. Theoretical frameworks argue that health insurance provides a safety net against poor health outcomes. METHODS: We analyzed 4,499 children from the Pakistan Demographic and Health Survey (2017-18) using Instrumental Variable Quantile Regression (IV-QR) along with logistic and probit models as bassline, with internet access and distance to health facility as instruments. This approach addresses endoge

Wireless Home Blood Pressure Monitoring With Health Coaching for Hypertension Management at a Federally Qualified Health Center: Pilot Randomized Controlled Feasibility and Acceptability Study.

BACKGROUND: Only approximately half of adults with hypertension, a leading cardiovascular risk factor, achieve blood pressure (BP) control, with persistent disparities among low-income and minority populations. Wireless home BP monitoring (HBPM) with remote patient monitoring can improve BP control, but evidence is limited in medically underserved, predominantly Hispanic populations served by federally qualified health centers (FQHCs). OBJECTIVE: The primary objective was to evaluate the feasibility and acceptability of wireless HBPM among predominantly Hispanic adults with hypertension recei

Artificial Light at Night and Hypertensive Disorders of Pregnancy: Socioeconomic Inequality in the China Birth Cohort Study.

Evidence regarding the association of outdoor artificial light at night (ALAN) with hypertensive disorders of pregnancy (HDPs) remains limited. Between February 2018 and December 2022, 241,608 participants from the China Birth Cohort Study (CBCS), which enrolled individuals from 17 provinces across China, were included in the analysis. HDP was defined by physician clinical diagnosis. We estimated outdoor ALAN within 500 m, sourced from the VIIRS/DNB, using the address of each participant, and categorized it into quartiles (lowest [Q1] as reference). We used generalized linear mixed models (GLM

Dietary Behaviors and Urinary Biomarkers in a Self-Management Support Program Among Low-Income Adults with CKD.

BACKGROUND: CKD disproportionately affects socioeconomically disadvantaged populations, among whom adoption of kidney-friendly diets is often constrained by structural barriers. Self-management support (SMS) programs can improve CKD awareness and cardiovascular risk factors; however, their ability to influence dietary behaviors and diet-related physiologic markers when nutrition content is limited remains uncertain. We evaluated whether participation in a culturally tailored SMS program with modest dietary education was associated with changes in self-reported dietary behaviors and urinary bio
