Forensic insights into mortality and challenges in post-mortem investigations of homeless individuals: a 10-year study (2014-2024) in Milan, Italy.

Forensic medicine represents a critical socio-health observatory for socially marginalized populations. This study provides the first systematic retrospective forensic analysis of mortality among people experiencing homelessness in Italy. We examined 104 decedents undergoing autopsy in Milan between 2014 and 2024. The annual number of autopsied homeless decedents increased over the study period, peaking in 2023-2024. The majority of deaths occurred in indoor public spaces (45/104, 43.3%), with the remainder in hospitals or green areas, reflecting extreme social marginalization and environmenta

Beyond the Clinic Walls: Nurse-Led Socialised Care and Health Equity for Vulnerable Populations in Australia.

AIM: This paper critically examines the Australian College of Nursing's proposed socialised healthcare model and the potential of nurse-led socialised care as a paradigm shift for addressing the intersection of medical need and social distress among vulnerable populations experiencing homelessness, domestic violence, poverty, and other disadvantage. APPROACH: This is a position paper. It offers a theoretical commentary and critical reading that situates the proposed socialised healthcare model within frameworks of social determinants of health, health equity, and socially responsive care deli

Probable alcohol use disorder in the U.S. Veteran population: Results from the 2025-2026 National Health and Resilience in Veterans study.

BACKGROUND: Alcohol use disorder (AUD) is a pervasive public health problem in the United States, and military veterans are at elevated risk. A prior wave of the National Health and Resilience in Veterans Study (NHRVS) estimated the prevalence of past-year probable AUD at 10.5 % in 2019-2020. Updated national surveillance data are needed given demographic shifts in the veteran population and evolving drinking patterns in the post-pandemic era. METHODS: Using data from the 2025-2026 NHRVS (N = 2,620), we estimated the weighted prevalence of probable AUD and compared it to that of the prior NHR

eHealthism in inclusion health: a qualitative longitudinal study with women experiencing homelessness.

BACKGROUND: While inclusion health approaches are increasingly recognised in research, policy, and practice as essential to addressing health inequities and social exclusion, there is limited understanding of how women experiencing homelessness engage with digital health. This represents a salient knowledge gap, given the growing interest in digital health to improve access, flexibility, and patient empowerment for marginalised populations. This study aimed to explore: (1) how women experiencing homelessness access and use digital technologies to support their health and wellbeing in everyday

Impact of a National Stepped-Care Model of Opioid Use Disorder Treatment on Access to Medication Therapies Among Homeless-Experienced Veterans.

OBJECTIVE: To determine whether the Veterans Health Administration's (VHA) Stepped Care for Opioid Use Disorder Train-the-Trainer (SCOUTT) initiative increased medication treatment for opioid use disorder (MOUD) among persons experiencing homelessness (PEH). STUDY SETTING AND DESIGN: We conducted a national retrospective cohort study of MOUD, including buprenorphine, naltrexone, or methadone, in federal fiscal years (FY) 2016-2020. In late FY2018, VHA regional directors nominated facilities to receive SCOUTT implementation support to expand prescribing of MOUD (primarily buprenorphine) in sel

Systemwide Evaluation of Field Medicine Programs in Los Angeles County: Protocol for a Mixed Methods Longitudinal Observational Study.

BACKGROUND: People experiencing homelessness face high morbidity and unmet health care needs. To address these gaps, health care providers across the United States are increasingly adopting "field medicine" models that deliver mobile health services in shelters, homeless encampments, mobile clinics, and other community settings. Despite their expanding use, systematic evaluations of field medicine programs remain limited. OBJECTIVE: This paper aims to describe a protocol for a mixed methods evaluation of field medicine for people experiencing homelessness across Los Angeles (LA) County, Calif

Oral Health and Drug Use Among People Experiencing Homelessness: A Community-Based Cross-Sectional Study.

INTRODUCTION: The health vulnerability that homeless individuals encounter, in which poor nutrition, irregular hygiene behaviours, social exclusion, and limited access to preventive dental care converge with high rates of substance use, is a major factor in the acceleration of oral disease progression. Xerostomia, bruxism, poor diet, delayed care-seeking, and trauma are other ways that drug use exacerbates oral health issues, but the extent to which these interactions occur in the homeless population is not well quantified in low- and middle-income countries. OBJECTIVE: The aim of this study

Annual Trends Among the Unsheltered in Three Los Angeles Neighborhoods: The Los Angeles Longitudinal Enumeration and Demographic Survey (LA LEADS) 2025 Annual Report.

Unsheltered homelessness remained flat between December 2024 and January 2026. While tent dwelling has fallen 55 percent since 2021, rough sleeping and vehicle dwelling have risen significantly. With 87 percent of tents now in Skid Row, tent-centric aid is less effective elsewhere. The authors recommend pivoting to a mixed approach, including centralized service hubs and low-barrier integrated behavioral health-housing options.

Where Women Veterans Seek Emergency Care: Patterns and Predictors of VA and Community Emergency Department Use.

BACKGROUND: Women using Veterans Health Administration (VA) facilities have nearly doubled over the past decade. Women Veterans have distinct emergency care needs across their lifespan; examining care setting (VA versus non-VA, hereafter Community Care) can inform service planning. Despite ongoing improvement efforts, there has not been a recent comprehensive, national assessment of emergency care utilization patterns among women Veterans. OBJECTIVE: We sought to describe women Veterans' emergency department (ED) utilization across VA-provided and VA Community Care settings, identify predicto

Detection of Acute Hepatitis C Outbreaks in California Using SaTScan: Statewide Spatiotemporal Analysis.

BACKGROUND: Acute hepatitis C virus outbreaks in California are identified by local health jurisdictions through the investigation of cases reported by laboratories and health care providers to the state's public health surveillance system. However, acute hepatitis C cases are widely underreported, limiting timely outbreak detection and early intervention. SaTScan (Space and Time Scan Statistics) has been proposed as a valuable tool to better detect disease clusters that may be missed by traditional surveillance, even when the disease is underreported, but it is not routinely used for acute he

Heat Perceptions, Behaviors, and Barriers of Unhoused Individuals in Germany.

Heat waves pose critical health risks for unhoused people. However, little is known about how this population perceives and copes with heat, particularly in Europe. Employing a sequential mixed-methods approach, we investigated dimensions of unhoused individuals' heat perceptions, coping behaviors, and barriers via semi-structured interviews (N = 9) to inform the design of a quantitative survey administered across 13 German cities in 2024 (N = 252). Results revealed that many homeless people experience fatigue, nausea, or faintness on hot days, whereas only half of respondents considered heat-

Delivering Long-Acting Injectable Cabotegravir PrEP to People Experiencing Homelessness and People Who Use Drugs.

Long-acting injectable cabotegravir (CAB-LA) may be particularly advantageous for preventing HIV among people experiencing homelessness (PEH) and people who use drugs (PWUD), populations at high HIV risk with poor oral pre-exposure prophylaxis (PrEP) outcomes. In a retrospective study using the RE-AIM framework to organize programmatic outcomes, we evaluated real-world implementation of CAB-LA at the Maria X.
