'Meeting people where they're at': user profile, acceptability, and impact of an integrated mobile health service for people experiencing disadvantage.

OBJECTIVES: The aim of this study was to explore the user profile, acceptability, and impact of the Pathways to Access and Treatment (PAT) mobile health service in Canberra, the capital city of Australia. METHODS: In late 2022, we conducted a mixed-methods service evaluation, integrating data from 23 service user interviews, 8 stakeholder interviews, 58 service user surveys, and operational service records from Directions Health Services (who deliver the service). Triangulating across these data, we analysed the profile of service users (e.g. demographics, health, drug use), service acceptabi

Building and Boosting Capitals for Health Care Access: A Qualitative Study of Homeless Health Peer Advocacy in London, UK.

Peer support is widely promoted as enabling health care access with people experiencing homelessness, including through enabling independence, empowering individuals and managing stigma. However, there is little understanding of these processes, including how they relate to the wide range of potential activities involved in peer support. We studied peer support within a peer advocacy service at a charity run by and for people experiencing homelessness in London, UK. We report analysis from a sub-set of qualitative interviews and focus groups gathered for a qualitative study of peer advocacy. W

Preparing Students to Care for Socially Excluded Patients: A Qualitative Study of Inclusion Health in Undergraduate Medical Education.

INTRODUCTION: Extreme health inequities are experienced by Inclusion Health groups (including people experiencing homelessness, problem substance use, Gypsy, Roma and Traveller communities, vulnerable migrants, sex workers, people in contact with the justice system and victims of modern slavery). There is evidence that undergraduate medical education is failing to prepare students to work effectively with these socially excluded groups. This research explores challenges and opportunities in teaching Inclusion Health to medical students. METHODS: Twenty-three educators involved in teaching Inc

Risk factors and circumstances associated with homelessness in older adults: a scoping review of quantitative and qualitative studies.

BACKGROUND AND OBJECTIVES: Circumstances more common with aging may challenge the ability for older adults to maintain appropriate housing. The objective of this scoping review was to identify the risk factors and conditions associated with homelessness in older adults. RESEARCH DESIGN AND METHODS: We searched OVID, CINAHL, Embase, PsycINFO, CENTRAL, and Social Work Abstracts from inception to February 2023. We included studies of older adults (defined as 50 years of age or older) or completed subgroup analyses accounting for older adults. We mapped the risk factors and conditions using an ec

Amplifying Marginalised Voices Through Advocacy: Community-Informed Curriculum Development for Cultural Safety in Homeless Health.

BACKGROUND: People experiencing homelessness face significant health inequities, yet their voices remain largely absent from medical education. Partnering with community-based advocates enables the integration of lived experience and advocacy expertise into curriculum design and delivery. This study aimed to develop and evaluate a medical education module on homeless health that embeds community-based advocacy and lived-experience expertise to advance cultural safety. APPROACH: A homelessness health module was collaboratively developed by clinical teachers, researchers, educators and communit

Outreach Models for Substance Use Disorder in Homeless Populations: A Systematic Review and Network Meta-Analysis.

OBJECTIVE: The relative effectiveness of outreach models used to address substance use disorder among homeless populations remains unclear. This systematic review and network meta-analysis aimed to compare the efficacy of various outreach interventions in improving substance use outcomes among homeless populations. METHODS: A systematic search of studies (published January 1990-January 2024) in the PubMed, Embase, PsycInfo, and Cochrane Library databases was conducted. A network meta-analysis was executed to compare outcomes of substance use frequency (self-reported days of substance use acro

Evaluating the implementation and impact of harm reduction vending machines in veterans supportive housing settings: a mixed-methods study protocol.

BACKGROUND: Lack of access to sterile supplies among people who use drugs contributes to increased rates of infectious disease transmission, including human immunodeficiency virus, hepatitis C virus, and sexually transmitted infections. People residing in California, United States Veterans, and those who have experienced homelessness are disproportionately impacted. Syringe services programs (SSPs) are vital to reducing these harms, but access may be limited by hours of operation, geographic barriers, need for in-person interaction, and stigma. Harm reduction vending machines (HRVMs) which oft

Capacity, Financing, and Structural Strain in Mental Health Care in the United States: A Narrative Review.

The strain on the US mental health care system is structural, evidenced by a high demand for services that outstrips partial treatment coverage and limited behavioral health spending. This deficit has widespread consequences, spilling over and burdening emergency services, correctional facilities, and homelessness support systems. The narrative review examined interactions among capacity, financing, and cross-sector consequences by synthesizing surveys, claims, workforce projections, facility and crisis-service inventories, and homelessness and justice assessments identified through database a

"It's a Guy Thing": Hegemonic Masculinity and Older Men's Engagement in Shelter/Transitional Housing Programs.

Men aged 50+ are disproportionately represented among older adults in shelters. Drawing on Connell's theory of hegemonic masculinity, we conducted a secondary qualitative data analysis of interviews with older men in shelters/transitional housing aged 50+ (n = 21) and service providers (n = 15) to understand how older men negotiate support in three Canadian shelters/housing sites. Older men's experiences and behavior that limited support include: 1) choosing isolation; 2) self-reliance; 3) aggression in conflict; and 4) equating support with being less masculine. Older men negotiated support t

The Language of Bias: Labeling and Its Impact on Emergency Department Patients.

Stigmatizing language is prevalent in emergency nursing practice and has significant implications for patient care, clinical judgment, and health equity. Labels such as "drug-seeker," "frequent flyer," and "non-compliant" are often applied without comprehensive assessment and can lead to delayed treatment, inadequate pain management, misdiagnosis, and erosion of trust between patients and emergency clinicians. These terms disproportionately affect marginalized populations, including individuals with substance use disorders, individuals with mental health conditions, racialized minorities, and

Teaching undergraduates using Tracy Kidder's Rough Sleepers: students' perceptions of homelessness as a public health issue.

BACKGROUND: Homelessness is a pressing public health issue that is associated with adverse physical and mental health outcomes. Negative perceptions of individuals experiencing homelessness, including stereotyping and stigma, exacerbate health inequalities and impede access to care. Calls to address these issues include the incorporation of culturally competent education for future healthcare providers. This study explored the use of Tracy Kidder's Rough Sleepers as a teaching tool in an undergraduate health sciences course to deepen students' understanding of homelessness as a multifaceted pu

Assessing Health Literacy in Patients With Heart Failure and Potential Impact on Readmissions: A Quality Improvement Project.

PURPOSE OF STUDY: This quality improvement project was designed to close a gap in capturing health literacy in adult patients with heart failure discharged from a hospital system. An opportunity existed to investigate if health literacy levels contribute to individuals' social factors and overall health. A priority focus was evident across healthcare organizations to address the social determinants of health (SDOH) for the populations served. Throughout the literature, limited health literacy is associated with higher hospital readmission rates. Pre-implementation data included 2023 heart fail

Health-promoting housing policy in a changing climate: integrating affordability, security, and resilience.

Housing is a fundamental determinant of health, yet many housing systems fail to promote well-being or address growing challenges such as climate change, inequality, and urbanization. It is increasingly treated as an investment vehicle and as a commercial product supporting the construction industry, with substantial interactions with climate change, rather than its fundamental role providing shelter. This perspective addresses this gap by proposing an integrated framework for health-promoting housing policy that combines affordability, security, and quality, paying particular attention to the

Nursing leadership in Housing First implementation: A comparative analysis of care coordination approaches across four U.S. states.

BACKGROUND: Homelessness affects over 650,000 Americans annually, with 25% to 30% experiencing co-occurring mental health and substance use disorders. Despite evidence supporting Housing First approaches, implementation remains inconsistent across states, leading to fragmented care delivery and persistent housing instability for vulnerable populations. PURPOSE: To analyze Housing First implementation across four U.S. states and identify nursing practice and policy implications using the Minnesota Public Health Intervention Wheel framework. METHODS: Comparative case study analysis of Housing
