Risk factor analysis of bronchospasm after tracheobronchial foreign body removal: Cases report and literature review (STROBE).

This retrospective study aimed to investigate bronchospasm after tracheobronchial foreign body removal. Bronchoscopy is the main clinical treatment for removing airway foreign bodies, but postoperative airway spasm is very common. In our study, we perform a risk factor analysis of bronchospasm after tracheobronchial foreign body removal. The sample was composed of 261 children with airway foreign bodies who had undergone clinical bronchoscopy for foreign body removal under general anesthesia were enrolled from the department of otolaryngology, the First Hospital of Jilin University from 2014 t

Long-term observation of airway reconstruction using decellularized tracheal allografts in micro-miniature pigs at growing stage.

INTRODUCTION: Decellularized tissue exhibits cell matrix-like properties, along with reduced antigenicity. We explored the potential of decellularized allogeneic trachea to restore the upper respiratory tract, focusing on pediatric application. This study specifically aimed at long-term observation of tissue regeneration using a micro-miniature pig model. METHODS: Artificial defects (15 × 15 mm) in the subglottis and trachea of micro-miniature pigs were repaired by transplantation of either allogeneic decellularized or fresh (control) tracheal patches. Pigs were evaluated in situ, by bronchos

Mode of Presentation of Laryngeal Cancer: A Single Radiotherapy Institute Experience in Iraq.

PURPOSE: Laryngeal cancer is relatively common and usually presents with hoarseness due to a lesion in the glottis. We noticed that many of our patients had presented with supraglottic lesions and we could not find local literature in support of this observation. Mode of presentation and site of the lesion of these patients will be addressed in this review. MATERIALS AND METHODS: A retrospective cross-sectional descriptive study for laryngeal cancer patients who received radiotherapy at a tertiary radiotherapy center in Iraq, from January 2010 to December 2015, and who are residents in the Su

Evaluation of percutaneous dilatational tracheostomy under laryngosuspension.

PURPOSE: To describe the technique and outcomes of percutaneous tracheostomy under laryngosuspension. METHODS: A consecutive series of patients who underwent percutaneous tracheostomy under laryngosuspension were reviewed for outcomes. The procedure is performed under general anesthesia and laryngosuspension. An 8.5 oral endotracheal tube is inserted and the cuff is inflated in the supraglottis, allowing access to the whole trachea and subglottis. The tube is taped to the laryngoscope. A rigid endoscope is inserted in the endotracheal tube through a swivel connector connected to the tube and

Strategies and Controversies in the Treatment With Carbon Dioxide Laser of Laryngeal Hemangioma: A Case Series and Review of the Literature.

INTRODUCTION: The hemangioma is the most common vascular tumor, involving the head and neck in 60% of cases. It is rare in the larynx. In children, hemangiomas are more frequent on the subglottis, whereas in adults the most common site is the supraglottis. Laryngeal hemangioma with cavernous features isolated to the free edge of the vocal fold is a very rare clinical finding. We present 2 cases of glottic hemangioma. Both patients reported severe hoarseness. CASES: In the first patient, an extensive blue-purple mass was seen on the right vocal cord. The patient was posted for microlaryngeal s

Oncologic outcomes of early glottic cancers with anterior commissure involvement treated with advanced laser cordectomies.

INTRODUCTION: The contribution of transoral laser microsurgery (TLM) in the management of early glottic cancer is now well established. The anterior commissure (AC) is a delicate anatomic subsite due to its difficult access and the high risk of tumour spread from this site to the thyroid cartilage, the subglottis and the pre epiglottic space. These reasons make the endoscopic approach controversial in case of AC involvement. METHODS: This is a single centre retrospective study performed between the years 2012 and 2015. Eighteen patients treated with advanced Va laser cordectomies were include

The pediatric airway: Historical concepts, new findings, and what matters.

New observations from novel imaging techniques regarding the anatomy, dimensions, and shape of the pediatric airway have emerged and provide insight for potential changes in the clinical management of the airway in infants and children. These new findings are challenging the historical concepts of a funnel-shaped upper airway with the cricoid ring as the narrowest dimension. Although these tenets have been accepted and used to guide clinical practice in airway management, there are limited clinical investigations in children to support the validity of these concepts. Imaging modalities such as

Office-based corticosteroid injections as adjuvant therapy for subglottic stenosis.

OBJECTIVE: Subglottic stenosis (SGS) is a serious, potentially life-threatening disorder that is difficult to treat due to significant recurrence rates. While conventional treatment of SGS relies heavily on serial endoscopic dilation procedures, this study aims to characterize the efficacy of incorporating subglottic corticosteroid injections in increasing surgery-free intervals (SFIs) for a cohort of patients at a university-based medical system. STUDY DESIGN: Retrospective chart review. METHODS: All SGS patients who underwent endoscopic dilation and at least one adjuvant office-based seria

Aerodynamically driven phonation of individual vocal folds under general anesthesia in canines.

OBJECTIVES/HYPOTHESIS: We previously developed an instrument called the Aerodynamic Vocal Fold Driver (AVFD) for intraoperative magnified assessment of vocal fold (VF) vibration during microlaryngoscopy under general anesthesia. Excised larynx testing showed that the AVFD could provide useful information about the vibratory characteristics of each VF independently. The present investigation expands those findings by testing new iterations of the AVFD during microlaryngoscopy in the canine model. STUDY DESIGN: Animal model. METHODS: The AVFD is a handheld instrument that is positioned to cont

Intranasal Acetaminophen Abuse and Nasal, Pharyngeal, and Laryngotracheal Damage.

A young adult female originally presented with necrosis of the nasal cavity mucosa and septum after sniffing crushed acetaminophen. She underwent endoscopic sinus surgery and debridement but continued to use acetaminophen intranasally. Four months later, the destruction had extended to include the posterior pharyngeal wall and subglottis. The diagnosis was confirmed by polarizable talc found on biopsy of the subglottis. While nasal insufflation of cocaine and hydrocodone-acetaminophen has been well-documented, intranasal abuse of exclusively acetaminophen is not well understood. This case demo

Laryngotracheal Microbiota in Adult Laryngotracheal Stenosis.

Laryngotracheal stenosis is an obstructive respiratory disease that leads to voicing difficulties and dyspnea with potential life-threatening consequences. The majority of incidences are due to iatrogenic etiology from endotracheal tube intubation; however, airway scarring also has idiopathic causes. While recent evidence suggests a microbial contribution to mucosal inflammation, the microbiota associated with different types of stenosis has not been characterized. High-throughput sequencing of the V4 region of the16S rRNA gene was performed to characterize the microbial communities of 61 swab

Increased Expression of Estrogen Receptor Beta in Idiopathic Progressive Subglottic Stenosis.

BACKGROUND/OBJECTIVES: Idiopathic progressive subglottic stenosis (IPSS) predominantly affects females in perimenopause. It has, therefore, been hypothesized that estrogen is involved in its pathogenesis. There are two main types of estrogen receptors: ER-α and ER-β. Abnormal variants of ER-β have previously been shown to be associated with poor wound healing. Estrogen receptors have recently been identified in subglottic tissue samples, with elevated levels of ER-α and progesterone receptors, and no expression of ER-β, in stenotic specimens reported in one study. The objective of this study w

Chronic non-granulomatous supraglottitis of a male adolescent and its successful management with azathioprine.

Chronic non-granulomatous supraglottitis (CNGS) is a rare disorder of the supraglottic larynx, characterised by chronic supraglottic inflammation in the absence of granulomata, vasculitis, neoplasia, autoimmune disease or infective changes on histology. We present the case of a male adolescentwho attended with progressively worsening exertional dyspnoea, stridor and symptoms of obstructive sleep apnoea. Flexible nasendoscopy revealed marked supraglottic subepithelial thickening sparing the glottis and subglottis, confirmed on microlaryngoscopy. MRI of the head and neck demonstrated diffuse, ho

Atypical juvenile histiocytosis with novel KIF5B-ALK gene fusion mimicking subglottic hemangioma.

Juvenile Xanthograuloma (JXG) is part of a diverse set of rare histiocytic disorders marked by infiltration of tissues with neoplastic myelomonocytic-derived cells. Molecular analysis has yielded new insights into the classification and management of histiocytic diseases. A three-year-old presented with atypical croup due to a localized subglottic histiocytic lesion mimicking subglottic hemangioma. The lesion was removed via tracheofissure. Pathology revealed a JXG-like histopathology with a rare KIF5B-ALK fusion gene. This is the first isolated ALK-positive lesion to be reported in this locat
