The Impoverishment Effect of Out-of-Pocket Expenditure for Hypertension in Ghana.

OBJECTIVES: The rising prevalence of hypertension in Ghana and the associated household expenditures have become major policy concerns. High out-of-pocket (OOP) payments restrict access to care and increase the risk of impoverishment. This study examines the level and effect of household impoverishment due to care-seeking for hypertension in Ghana. METHODS: We analyzed data from a catastrophic health expenditure survey conducted between December 2023 and February 2024 at 2 primary healthcare facilities: Weija Gbawe Municipal Hospital (WGMH) and Shai Osudoku District Hospital (SODH) in the Gre

Factors influencing catheter-related infections in peritoneal dialysis patients: a meta-analysis.

INTRODUCTION: Intensified poverty arising from economic decline and crisis may have contributed to reductions in HIV prevalence in Zimbabwe. OBJECTIVES: To assess the impact of the economic decline on household wealth and prevalent HIV infection using data from a population-based open cohort. METHODS: Household wealth was estimated using data from a prospective household census in Manicaland Province (1998 to 2011). Temporal trends in summed asset ownership indices for sellable, non-sellable and all assets combined were compared for households in four socio-economic strata (small towns, agri

The Prevalence of Sexual Victimization in Children and Adolescents of Indigenous and Non-Indigenous Peoples in Chile.

Chilean studies show high levels of sexual victimization among adolescents. Despite this, there is little research exploring differences between adolescents belonging to Indigenous communities and the general population. This is relevant because 12.8% of the Chilean population is from an Indigenous community. In this study, data from 13,385 adolescents all over the country (15.8% indigenous) were analyzed. The prevalence of seven forms of sexual victimization throughout life was examined. Adolescents from Indigenous communities presented a higher prevalence of statutory rape, sexual abuse by a

Cost-effectiveness of livelihood interventions for families of children with cerebral palsy in rural Bangladesh.

BACKGROUND: Families of children with Cerebral Palsy (CP) often experience extreme poverty, compounded by limited livelihood opportunities and the added demands of caregiving, which further restrict their ability to earn an income. Targeted livelihood interventions may help improve their economic well-being. This study aimed to assess the cost-effectiveness of livelihood interventions to improve household incomes of ultra-poor families of children with CP in rural Bangladesh. METHOD: This was a mixed-methods study utilising a subgroup of a pragmatic, open-label, cluster randomised controlled

Childhood diarrheal infections among the multi hazard disaster risk zones in Bangladesh: A household level analysis.

Bangladesh's geographic and climatic conditions make it highly disaster-prone, increasing the vulnerability of under-five children to diarrheal infections. This study analyzed the MICS Bangladesh 2019 dataset using a multivariable logistic regression model to assess diarrhea prevalence in low, moderate, and high multi-hazard risk zones, focusing on floods, tornadoes, cyclones, and earthquakes. Diarrhea prevalence was highest in high-risk zones (7.71 %), followed by moderate (5.98 %) and low-risk zones (4.67 %). Socioeconomic factors such as male gender, urban residence, extreme poverty, and la

Distinct Metabolic and Inflammation Signatures in Urban vs Rural Ugandan Youth With HIV on Dolutegravir.

BACKGROUND: In sub-Saharan Africa, the majority of the metabolic data are from youth living in urban areas. In youth with perinatally acquired HIV (YPHIV) and seronegative (HIV-), we examined inflammatory and metabolic signatures in urban versus rural Uganda. METHODS: YPHIV (n = 100) were enrolled from urban and rural Uganda in an observational cohort study along with age- and sex- matched, population-based HIV- (n = 99) comparators. YPHIVs were on antiretroviral with HIV-1 RNA level ≤400 copies/mL. We compared variables using Wilcoxon rank-sum tests and chi-squared tests. General linear regr

Estimating the burden of chronic stress through allostatic load in patients with chronic myeloid leukemia.

Despite advances in tyrosine kinase inhibitor (TKI) therapy, outcomes in chronic myeloid leukemia remain (CML) highly variable, underscoring the need to explore determinants beyond conventional clinical indicators. This study examined the relationship between allostatic load (AL), a biomarker-based index of cumulative chronic stress, social determinants of health (SDH), and treatment outcomes. In a retrospective cohort of 194 patients, AL was calculated using 23 biomarkers spanning cardiovascular, metabolic, hematologic, renal, and hepatic systems. Higher AL was associated with adverse SDH and

Oral absorption of semaglutide: pharmacokinetic modeling and molecular dynamics simulations.

This paper is an attempt to assess India's performance in generating the required quantity and quality of employment for its growing population since independence in 1947. But the exercise is set in a longer period that covers India's population growth since the turn of the twentieth century (1901) in relation to its ability to generate employment. The half-a-century preceding independence, despite a slow population growth, was a disaster in generating employment and any signs of structural change. Detailed analysis of the issue since independence shows that there was indeed a demographic burd

Systematic scoping review of the noma evidence landscape: current knowledge and gaps.

BACKGROUND: Noma (cancrum oris) is a severe gangrenous disease of the mouth and oro-facial structures. Noma often affects young children living in extreme poverty, malnutrition and poor sanitation. Gaps remain in understanding its aetiology, pathogenesis, prevention and treatment. METHODS AND FINDINGS: We systematically searched databases for all primary research studies (clinical trials, cohort studies, case-control, cross-sectional, other observational studies, case studies/series) reporting noma patients of any age up to 7 December 2022. The 366 publications (published between 1839 and 202

Reduction of tungiasis prevalence, intensity, and morbidity during a two-year long community-based tungiasis control project in a hyperendemic region in Karamoja, Uganda.

Tungiasis is a widespread and debilitating zoonotic Neglected Tropical Disease (NTD). Manual extraction of the sand fleas with non-sterile sharp instruments is the most common but unsafe treatment method in affected communities. Topical application of a dimeticone oil formula (NYDA) has previously been shown to be a safe and effective method of killing embedded sand fleas. The objective of this study was to evaluate a two-year long humanitarian One Health tungiasis control project in 17 villages in Napak district, Karamoja region, Northeastern Uganda. The community-based intervention included

Intestinal parasitic infections among BaAka Pygmies and Bantu people living in the rain forests in Lobaye Prefecture, Central African Republic.

BACKGROUND: Central African Republic (CAR) is one of the poorest countries in the world with nearly 65% of its inhabitants living in extreme poverty, and 87% lacking access to safely managed sanitation facilities. Poor sanitation and limited access to healthcare contribute to high transmission of intestinal parasitoses. The present study aimed to assess the occurrence of intestinal parasitic infections (IPIs) in two separate ethnic groups inhabiting the rainforests in the Lobaye Prefecture in the southern parts of the CAR: the nomadic BaAka Pygmies and the sedentary Bantu people. In addition,

Stakeholder perspectives on barriers and facilitators to hypertension control in urban Haiti: a qualitative study to inform a community-based hypertension management intervention.

BACKGROUND: Uncontrolled hypertension is the leading modifiable risk factor for cardiovascular disease mortality and remains high in low-middle income countries like Haiti. Barriers and facilitators to achieving hypertension control in urban Haiti remain poorly understood. Elucidating these factors could lead to development of successful interventions. METHODS: We conducted semi-structured interviews with healthcare providers (10) and patients with hypertension (10) from the Haiti Cardiovascular Disease Cohort, using guides developed using the Consolidated Framework for Implementation Researc

Mitigating HIV risk associated with widow cleansing and wife inheritance using combined biomedical and structural interventions in western Kenya: a mathematical modeling study.

BACKGROUND: In parts of Africa, women who become widowed lose housing, bank accounts, and other property and must re-marry to avoid extreme poverty. To re-marry, some women are required to undergo widow "cleansing"-condomless sex with a man who removes "impurities" ascribed to her from her husband's death-and are "inherited" as a wife of a brother-in-law. This study explores how HIV biomedical and structural interventions could reduce HIV-related harms associated with these practices. METHODS: We adapted EMOD-HIV, an HIV agent-based network transmission model previously calibrated and validat

Effectiveness of an Education Toolkit Delivered by Soap Operas Among Communities Living in Extreme Poverty in Improving Vaccination Confidence in the Philippines: Protocol for a Cluster Randomized Controlled Trial.

BACKGROUND: Measles and polio pose significant public health challenges globally, particularly in low-resource settings such as the Philippines, where vaccine coverage falls short of the World Health Organization's (WHO's) targets, with hard-to-reach populations contributing to the "last mile." This research addresses the "last mile" challenge in routine immunization efforts by bridging the vaccination gap in marginalized populations. OBJECTIVE: We describe the implementation of a cluster randomized controlled trial to evaluate the impact of an education toolkit aimed at improving confidence

Effects of conditional cash transfers on tuberculosis incidence and mortality according to race, ethnicity and socioeconomic factors in the 100 Million Brazilian Cohort.

Conditional cash transfer (CCT) programs have been implemented globally to alleviate poverty. Although tuberculosis (TB) is closely linked to poverty, the effects of CCT on TB outcomes among populations facing social and economic vulnerabilities remain uncertain. Here we estimated the associations between participation in the world's largest CCT program, the Brazilian Bolsa Família Program (BFP), and the reduction of TB incidence, mortality and case-fatality rates using the nationwide 100 Million Brazilian Cohort between 2004 and 2015. We also evaluated these relationships according to race, e

A 20th century case of noma infection following typhoid fever from the Morgagni Museum (Padua, Italy).

Noma is a potentially fatal, gangrenous disease that leads to tissue destruction in the face. It has been proven to develop mostly in children living in extreme poverty. There is a lack of data regarding microbiological analysis of the ulcers, making the knowledge of the bacteria involved and its etiology still unclear. Within this framework, pathological specimens from museological collections could offer relevant improvements for the comprehension of etiology of noma. The Morgagni Museum of Human Anatomy in Padua hosts a unique case of noma dating back to 1902, and two related specimens, a m
