Effectiveness and safety of using statin therapy for the primary prevention of cardiovascular diseases in older patients with chronic kidney disease who are hypercholesterolemic: a target trial emulation study.

BACKGROUND: There remains a scarcity of evidence on initiating statin therapy for the primary prevention of cardiovascular diseases among older adults with chronic kidney disease due to the under-representation of this population in randomised controlled trials. This study aimed to evaluate the effectiveness and safety of using statin therapy for the primary prevention of cardiovascular diseases in older adults (aged 75-84 years) and very old adults (aged ≥85 years) with chronic kidney disease. METHODS: Using territory-wide public electronic health records in Hong Kong, patients older than 60

Early onset of hypertension and increased relative risks of chronic kidney disease and mortality: two population-based cohort studies in United Kingdom and Hong Kong.

This study aimed to evaluate the association between hypertension (HT) onset age and later risks of chronic kidney diseases (CKD) and mortality. Adult patients without CKD from 2008 to 2013 were identified using electronic medical records from United Kingdom (UK) and Hong Kong (HK). Patients newly diagnosed with HT and those without were included in the HT and control groups, respectively. All subjects were stratified into six age groups (18-39, 40-49, 50-59, 60-69, 70-79, ≥80). Multivariable Cox proportional hazard regression, adjusted with baseline characteristics and fine stratification wei

Development and pilot evaluation of a train-the-trainer chronic kidney disease program for community health workers.

Chronic kidney disease (CKD) affects over 37M U.S. adults, driven by type 2 diabetes and hypertension. Latino populations have 35% higher kidney failure prevalence, worse outcomes, and lower engagement in care and prevention. We co-developed and pilot tested a Train the Trainer CKD curriculum for Community Health Workers (CHWs) serving Latinos with or at risk of CKD to assess feasibility, acceptability, and perceived knowledge gains. CHWs were trained to deliver a culturally tailored one-on-one CKD education. Co designed with Caridad Awareness and Education for Kidney Disease advisory board us

Differences in Hemodialysis Claim Patterns Across Membership Types Among Patients With Renal Failure Based on National Health Insurance Data From 2017 to 2022: Cross-Sectional Analysis.

BACKGROUND: Chronic kidney disease and end-stage renal disease are major contributors to the disease burden in low- and middle-income countries, including Indonesia. Despite the expansion of universal health coverage through Badan Penyelenggara Jaminan Sosial (BPJS) Kesehatan, Indonesia's national health insurance program, disparities in access to hemodialysis persist across different socioeconomic and geographic groups. Understanding these inequities is critical to advancing equitable health care access. OBJECTIVE: This study aimed to examine disparities in hemodialysis claim patterns as a p

Crowding in or Out?National Public Pension, Inter-Generational Contract, and Family Support to Empty-Nest Older Parents in Rural China.

Evidence to date has been inconclusive on the effects of public pension on family support to older adults, and the underlying mechanism behind such effects remains unclear. This study examines the effects of the New Rural Social Pension Scheme on family support to empty-nest older parents in rural China, where family care is traditionally favored for older adults. Using data from the China Longitudinal Aging Social Survey, the findings reveal that pension receipt promotes bidirectional financial transfers and intimate relationships between generations, thus increasing (or crowding in) children

Quality, Usability, and Trust Challenges to Effective Data Use in the Deployment and Use of the Bangladesh Nutrition Information System Dashboard: Qualitative Study.

BACKGROUND: Evidence-based decision-making is essential to improve public health benefits and resources, especially in low- and middle-income countries (LMICs), but the mechanisms of its implementation remain less straightforward. The availability of high-quality, reliable, and sufficient data in LMICs can be challenging due to issues such as a lack of human resource capacity and weak digital infrastructure, among others. Health information systems (HISs) have been critical for aggregating and integrating health-related data from different sources to support evidence-based decision-making. Nut

Organisational and Ethical Challenges While Caring for Older People During the COVID-19 Pandemic in Sweden: A Focus Group Study.

INTRODUCTION: During the COVID-19 pandemic, older people in Sweden's residential care facilities and home care services experienced high mortality rates. Prior to the pandemic, the care of older people in Sweden was challenged by organisational and regulatory changes, an ageing population and insufficient increases in staff numbers. These issues led to high staff turnover, increased workloads, stress, burnout and a perceived inability to provide satisfactory care. The pandemic brought about increased job strain, stress, anxiety, depression and post-traumatic stress disorder among the staff who

Links Between Food Assistance Program Participation and Mental Health: A Systematic Review and Meta-Analysis.

Food insecurity is a mechanism through which the mental health impacts of systemic social inequities multiply across the lifespan. Federally administered food assistance programs improve food security, but their impact on mental health is less clear. We conducted a systematic review of all studies that reported an association between food assistance participation and mental health (N = 34), and a multilevel meta-analysis among studies that met inclusion criteria and reported sufficient statistical results (n = 21; k = 44). The overall effect of food assistance on mental health was not signific

The association of increased SNAP benefits during COVID-19 with food insufficiency and anxiety among US adults: a quasi-experimental study.

OBJECTIVES: The COVID-19 pandemic and subsequent policy response to mitigate disease spread had far-reaching impacts on health and social well-being. In response, the Supplemental Nutrition Assistance Program (SNAP) underwent several pandemic-era modifications, including a 15 % monthly benefit increase on January 1, 2021. Research documenting the health effects of these SNAP modifications among low-income households and minoritized groups who were most impacted by the economic fallout during the first years of the pandemic is lacking. We aimed to estimate the health effects of the 15 % SNAP be

The effects of digitalisation on health and social care work: a qualitative descriptive study of the perceptions of professionals and managers.

BACKGROUND: Today, digitalisation is strongly present in health and social care, and it increasingly affects the organisation of work, work requirements, tasks and tools. Due to the constant change in work, up-to-date knowledge is needed about these micro-level effects of digitalisation and how professionals experience the effects in their work. Furthermore, even though managers play a key role in implementing new digital services, their perceptions of the effects of digitalisation and whether they match the views of professionals remain unknown. This study examined how health and social care
