Thalassemia treatment and prevention in Uva Province, Sri Lanka: a public opinion survey.

Due to its excessive cost thalassemia management is a major health care problem in Sri Lanka. The majority of doctors are using only desferrioxamine (DFO), in grossly inadequate doses mainly because of its unavailability. Deferiprone (L1), which is more affordable, is not used due to fear of toxicity, as previously reported. Arthropathy attributed to L1 has been observed in some patients, and has led to the discontinuation of the drug in all patients, without scientific rationale. The proposed thalassemia prevention project for Uva Province is based on prevention of marriages between carriers.

Parental supervision of teenage drivers in a graduated licensing system.

OBJECTIVES: Most states now have lengthy learner periods for young, beginning drivers as part of their graduated driver licensing (GDL) systems. Although parents play a vital role during the learner stage of GDL by supervising driving practice, virtually nothing is known about the nature and quality of parental supervision. The objectives of this study were to investigate parents' supervisory behavior and parent-teen relationships during the learner stage of graduated licensing and to evaluate two approaches for assisting parents in supervising their teenager's early driving experience. Pediatric procedural pain.

Pain is subjective. The pain response is individual and is learned through social learning and experience. Early pain experiences may play a particularly important role in shaping an individual's pain responses. Painful medical procedures such as immunizations, venipunctures and dental care, and minor emergency department procedures such as laceration repair, compose a significant portion of the average child's experience with painful events. Inadequate relief of pain and distress during childhood painful medical procedures may have long-term negative effects on future pain tolerance and pain

Awareness of child sexual abuse prevention education among parents of Grade 3 elementary school pupils in Fuxin City, China.

Several recent studies on child sexual abuse (CSA) in Chinese society have shown that the problem is not uncommon, and is associated with poor mental health and health-related risk behaviors of abused youth. It is very important to understand and improve public awareness of CSA prevention, especially for the parents. However, there are few published reports on the problem of parents' awareness. To fill this gap, knowledge, attitudes and practice of CSA prevention education were explored in 385 parents of Grade 3 pupils from four schools in Fuxin City of Liaoning Province in the northeast part

Asking the correct questions to assess asthma symptoms.

A national survey of 896 parents of children with asthma was performed and responses to 2 types of inquiry were compared: global assessment versus specific assessment of symptoms. Almost all parents, 860 (96%), described their child's asthma as under "good control'' when asked a global assessment question. However, 306 (34%)-when asked specific questions-actually described poor asthma control with frequent symptoms. Medicaid insurance (OR: 1.59; 95% CI: 1.03, 2.44) and parental smoking (OR: 1.60; 95% CI: 1.06, 2.43) increased the likelihood that parents' responses would be at risk for misinter

Use of home visit and developmental clinic services by high risk Mexican-American and white non-Hispanic infants.

OBJECTIVE: To investigate whether US-born infants of mothers of Mexican descent who were enrolled in Arizona's Newborn Intensive Care Program (NICP) received follow-up services (developmental clinic and community health nurse [(CHN)] home visits) at the rates similar to White non-Hispanic (WNH) infants. Socio-economic and health status characteristics were controlled using stepped regressions in order to assess the impact of each on service use. METHODS: This population-based study used retrospective data from the NICP administrative database that were linked to birth certificates for years 1

Nighttime child care: inadequate sudden infant death syndrome risk factor knowledge, practice, and policies.

BACKGROUND: Millions of children in the US have parents who work alternative shifts. As a result, extended-hour and nighttime child care centers have increased in number to meet the needs of parents working nonstandard hours. Recognizing that 20% of sudden infant death syndrome (SIDS) occurs in child care settings and that child care providers may place infants prone, it is important to determine sleep position practices in nighttime child care centers. OBJECTIVE: To determine if nighttime child care centers 1) follow Back to Sleep recommendations; 2) are aware of the need for a safe sleep en

Culture, control and the birth environment.

These findings illustrate that women's needs are not being adequately met in many hospital birth units. Women, particularly those expecting their first baby, often know little about how the culture of hospitals varies, or the helpfulness of facilities and opportunities for control and one-to-one support that are more readily available at home or in a midwife-led unit. Nor do they know how much their opportunities for comfort and control may be compromised in a conventional hospital setting. If they are feeling anxious about whether they will be able to cope with the pain of labour and whether
