[A model for the development of a reimbursement system for home health care--based on the concept of nursing cost].

The reimbursement system for nursing services needs to be set up before beginning of the home health care system. One thing we should consider as a premise in developing a reimbursement system for home health care is that the fee-schedule of home health care should be based on the cost of unit service, since the medical reimbursement system in Korea basically depends upon the concept of 'fees for service'. A model for the development of the fee schedule was designed to make some contribution to the reimbursement system for home health care, which should be prepared before the enhancement of a

Parents' evaluations of pre-school services for children with Down syndrome in two Scottish regions.

Parental evaluations of current pre-school provision for children with Down's syndrome were surveyed in two Scottish regions. Two methods were used: postal questionnaires and telephone interviews. Services covered were those provided by general practitioners, hospital specialists, health visitors, speech therapists, occupational therapists, physiotherapists, educational psychologists, home teachers, social workers and voluntary organizations. Regional differences were found in provision and in parental satisfaction with currently available services, with some of these differences being depende

[Psychological situation of home care patients in the Federal Republic of Germany].

The demographic development, advances in medicine and also the increase in AIDS and paraplegia at an early age means that we must assume approximately two million people in Germany requiring care and attention or nursing on a longterm basis. Object of the explorative pilot-study is to analyse the conditions for mental well-being in the case of homecare patients and the influence on behaviour, enjoyment of life and the quality of their hopes for the future. In the study two comparable random surveys have been examined in all their demographic and medical characteristics: (1) homecare patients (

Multiple sclerosis: health-promoting behaviors of spousal caregivers.

This pilot study examined the relationship between health-promoting behaviors of spousal caregivers of individuals with multiple sclerosis (MS) and dependency needs of the afflicted spouses. The study sample included 20 subjects who were residing with their spouses and served as primary caregivers. The data were collected through a semistructured interview of caregiving spouses. The health-promoting lifestyle profile (HPLP) was used to measure health-promoting behavior. The Index of ADL was employed to measure independence in activities of daily living (ADL). Descriptive statistics were calcul

Hispanic culture and family care of Alzheimer's patients.

Caring for Alzheimer's patients places tremendous burdens and strain on families. This study examined the ways cultural values and norms influence the experiences of Hispanic caregivers. The results revealed relationships between attitudes toward caregiving and a sense of burden and depression among the caregivers, with depression being related to stronger adherence to norms of filial support. Factors associated with increased burden include lack of time for oneself, dependency needs of the patient, and belief that one could be doing a better job as caregiver. Problems common to other groups o

Institutionalizing women's oppression: the inherent risk in health policy that fosters community participation.

Current Canadian health policy is based on the implicit assumption that women are available to provide care in the home to the dependent, the ill, the elderly, and the physically and mentally disabled. Women are socialized from birth to accept caring roles within a traditional family structure, and current societal expectations and social policy reinforce this value system. Women's health can only be understood within the context of their lived experience of social inequity, medicalization, and family caregiving. Health care professionals are complicit in sustaining women's oppression by reinf

Women and the work of caring.

In the U.S., care for chronically dependent people is done mostly by family and friends, with only sporadic community support. Though such unpaid caregiving work--the bulk of it performed by women--constitutes an enormous subsidy to the national welfare, the contribution is undervalued, and the negative practical consequences for caregivers seldom recognized.

Coordinating a multidisciplinary discharge for the technology-dependent child based on parental needs.

Primary nurses, case managers, and clinical nurse specialists in pediatric settings are continually providing care for chronically ill children. With our advancing medical knowledge and technology, the concept of chronically ill children has taken on an added dimension: technology-assisted living. Technology-dependent children pose the greatest challenge for discharge into the home environment. This article identifies the chronically ill technology-dependent child and family and their needs in order to develop a chronologic plan for a successful multidisciplinary discharge.

Caregivers of children with disabilities: a comparison of those managing "Успешно" and those needing more help.

A survey was mailed to caregivers of children with disabilities to ascertain how they were managing caregiving. Caregivers reporting they were managing "Успешно" were compared to those who reported they needed more help or could not manage much longer ("not OK"). Results showed the not OK group of caregivers had children who were more severely impaired and functionally dependent. Their mothers were in poorer physical and mental health, had greater demands placed on their time and finances, and received less emotional support from friends and family. Special programs provided some assistance but not

Supporting the elderly: workplace programs for employed caregivers.

An aging population and extended longevity are increasing the number of older people needing informal and family support. At the same time, women, the traditional caregivers, have entered the work force in record numbers. Consequently, concerns about how to care for dependent family members have become workplace issues. In response to the needs of employees who care for family members, employers have produced an array of policies, benefits, and programs, including flexible work schedules and information and referral services. Although these programs are a valuable complement to community servi

Quality of life across age and family stage.

It is clear that the quality of life of the individual is intertwined with that of his or her family. Therefore, health care professionals and other professionals who work with families facing terminal illness must (1) identify those factors that put the patient and the family at risk of suffering further disruption of psychosocial quality of life and (2) seek out whatever resources are necessary to help the patient and the family improve their quality of life. Families with young children are more dependent on external resources than are other families, and they place more strains on the spou

Nursing home case-mix classification and residents suffering from cognitive impairment: RUG-II and cognition in the Texas case-mix data base.

Case-mix reimbursement systems for nursing home care are now in use or under consideration in over 20 states. Though many in the long-term care field still debate the relative merits of the various systems, those systems based on the Resource Utilization Groups-II (RUG-II) system used in New York seem to be increasingly popular. However, one of the more serious questions raised concerning RUG-like systems is whether they fully acknowledge the role played by cognitive deficits in determining nursing home residents' care needs. The results of this research indicate: 1) that resource provision an

Social support in the neonatal intensive care unit.

That having a baby in hospital undergoing intensive care is a stressful experience is beyond dispute. Every parent, no matter how well-developed their own personal resources, depends to some extent on the support of others within their social system to cope with this stress. Grandparents occupy a special place in support networks and their role was studied in families with a very low birthweight baby. Both professional and lay perspectives were investigated and 93 families participated. Questionnaires were sent to family members 1 month after the baby's birth and 1 month after discharge home.

Establishment of a state-supported, specialized home care program for children with complex health-care needs.

Establishment of a systematic approach to home care for children who require complex, technology-dependent care requires a well-thought-out process of collaboration and planning between all parties involved--the family, the professional, and the hospital. The State of Michigan Department of Public Health elected to assist families in taking their children home through a Specialized Home Care Program funded by combined sources that emphasized child care needs first rather than cost savings. This review identifies the development of roles, processes and products, and methods to enhance the disch

Medical home care services for the housebound elderly.

In our health jurisdiction the proportion of elderly people is more than double the national average, and there is a severe shortage of both home care services and long-term care beds. To help the many elderly housebound people without primary medical care we initiated a medical services home care program. The goals were patient identification, clinical assessment, medical and social stabilization, matching of the housebound patient with a nearby family physician willing and able to provide home care and provision of a backup service to the physician for consultation and help in arranging admi

New directions in nursing home ethics.

We believe that a new agenda for the ethics of long-term nursing home care could be set by seeing nursing homes as communities of caring and interdependency. The goal should be not simply to eliminate or minimize dependency whenever possible, but to make a genuinely creative and nurturing use of the dependency that is an inevitable reality for most nursing home residents. Nursing homes are rarely places of curing, but they can and should be places of healing -- of making whole -- of enabling frail or chronically ill persons to use their dependency to grow as human beings... In general, nursing

The elderly reach independence through a community stroke rehabilitation programme.

Cerebrovascular disease resulting in stroke had shown a rising trend in the last decade among the elderly population in Singapore. To prevent dependency states, a home stroke rehabilitation programme was implemented in 1988 with the help of staff nurses trained to provide rehabilitation for stroke patients at home. These nurses were specially trained by experienced physiotherapists. A simple format was used to identify patients with good rehabilitation potential, while they were in the acute hospitals. The 244 stroke patients were studied by their age, sex, the status of motivation to go throu

The burden of caring for a husband with Alzheimer's disease.

The caregiving wife needs an advocate who is concerned about her well being. The home healthcare nurse can fulfill this role by treating the needs of the wife as well as the dependent husband, advancing community programs to reduce the caregiving burden, and supporting legislation that recognizes that older women cannot be the sole caregivers to their dependent husbands.
