Risk and determinants of tuberculosis recurrence: a 12-year population-based cohort study.

OBJECTIVES: Tuberculosis recurrence remains a challenge to elimination efforts in high-burden settings. We aimed to estimate the incidence and timing of tuberculosis recurrence and to identify its risk factors to inform post-treatment care. METHODS: We conducted a population-based cohort study of tuberculosis survivors successfully treated for a first tuberculosis episode from 2013 through 2024 in São Paulo state, Brazil. Data were obtained from the state tuberculosis surveillance system (TBweb), and deaths were identified through linkage with the national mortality database. We applied compe

New Model of Care Achieving Micro-Elimination of Hepatitis C in the United Kingdom's Largest Prison.

Wales aims to achieve the World Health Organisation goal of eliminating hepatitis C (HCV) as a public health threat by 2030, with micro-elimination in prisons identified as a key target. In January 2022, HMP Berwyn, the largest prison in the United Kingdom, introduced a multi-disciplinary blood borne virus (BBV) team and a streamlined testing and treatment pathway incorporating opt-out testing, peer support, point-of-care viral load testing and pharmacist-led prescribing. This service evaluation describes implementation of the pathway and its outcomes. Data relating to testing and treatment we

Changes in Dietary Behavior, Physical Activity, and Mental Health During a Lifestyle-Based Wellness Program for Women Re-Entering the Community After Incarceration: A Single-Arm Pre-Post Program Evaluation.

BACKGROUND: Women re-entering the community after incarceration experience substantial chronic disease and mental health burdens. Evidence regarding lifestyle medicine programming in transitional housing settings remains limited. This study evaluated changes in dietary behavior, physical activity, and mental health symptoms during a wellness program. METHODS: This single-arm, uncontrolled pre-post program evaluation retrospectively analyzed existing survey data from 17 women residing at a transitional living facility in Houston, Texas. The program included weekly yoga and meditation sessions

Obturator hernia misdiagnosed as inguinal incarceration: a case of postoperative small-bowel perforation and subcutaneous fecal emphysema.

INTRODUCTION AND IMPORTANCE: An obturator hernia is a rare condition accounting for less than 1% of abdominal wall hernias. Due to its deep pelvic location and nonspecific clinical presentation, it is frequently misdiagnosed, most often as an inguinal hernia. Delayed or incorrect diagnosis may result in bowel ischemia and perforation. CASE PRESENTATION: We report the case of a 61-year-old man who underwent emergency open surgery at another institution for a presumed strangulated right inguinal hernia with placement of prosthetic mesh. No operative report was available regarding intraoperative

Costly missed connections: Visitation and misconduct in California prisons.

Social isolation is a major public health crisis with particularly deleterious consequences for vulnerable populations including incarcerated people. Theoretically, visitation from family should reduce social isolation and improve outcomes among incarcerated people, but existing quasi-experimental research provides limited support for the relationship between visitation and in-custody outcomes. We revisit the effect of visitation on in-custody misconduct-an important but administratively defined measure of institutional safety-by exploiting the fact that fluctuations in gasoline prices create

Posterior Fibular Entrapment in a Bosworth Fracture-Dislocation: A Case Report and Review of Diagnostic and Operative Considerations.

INTRODUCTION: Bosworth fracture-dislocations are rare ankle injuries characterized by posterior entrapment of the fibula behind the tibia, making closed reduction inherently difficult. Their subtle radiographic appearance often delays diagnosis. This case highlights the diagnostic challenges and operative considerations associated with this uncommon and frequently irreducible injury pattern. CASE REPORT: A 34-year-old man presented after a fall with a posterior malleolus fracture, distal fibular fracture, and tibiotalar dislocation. Closed reduction improved alignment, but persistent mortise

Development and pilot testing of a risk assessment checklist for children of incarcerated parents in Hong Kong.

Children of incarcerated parents (CIP) are exposed to a range of biological, psychological, and social risks, yet existing risk assessment tools do not adequately capture incarceration-specific experiences. This article details the development of a comprehensive, theory-driven risk assessment checklist tailored to CIP aged 6-11, with consideration of the sociocultural context of Hong Kong. The checklist was developed using an integrative framework combining ecological systems theory and the biopsychosocial model. Items were adapted from validated instruments and supplemented with incarceration

Living with addiction in the shadow of punishment: a qualitative study of prison drug use and harm reduction in Iceland.

This qualitative study explored how people with experience of imprisonment and injecting drug use in Iceland understand drug-related harm in prison and the role of harm reduction. It adds qualitative evidence from a small prison system where social proximity, limited anonymity and concerns about trust and confidentiality shape understandings of harm, safety and support. Six participants aged 25-35 were interviewed, and the data were analysed thematically within an interpretive description framework. Participants described prison drug use as continuing dependence under hidden, unsafe and degrad

Rehabilitation through yoga: Mental health and the right to dignity in prison.

Prison environments in Latin America are overpopulated, and current policies restrict more interns' rights, which in turn leads to high rates of violence and mental health issues. All this goes against social justice in terms of equal rights, equal opportunity, and equal treatment. Following the trend of a more positive criminology, a yoga program was designed for prisoners in a penitentiary center in Valparaiso, Chile. Results of the program were measured before and after the workshop in terms of mental health and aggression. Despite no significant differences being observed in the analysis,

Telemedicine implementation in Austrian correctional facilities: a qualitative interview study with prison staff.

OBJECTIVE: Austrian correctional facilities face persistent shortages of on-site physicians challenging the principle of equivalence of care. Consequently, the Ministry of Justice launched a telemedicine (TM) pilot to support primary care in correctional facilities without round-the-clock medical cover. The goal of this study was to evaluate TM alongside its implementation in order to investigate key stakeholders' experiences with TM and its integration into routine medical practice and to assess if and how health technologies such as TM can be used to improve quality of care in underserved fa

Laparoscopic Anterior Gastropexy with a Technical Modification Using the Round Ligament for Internal Hernia of the Stomach after Sigmoid Colostomy: A Case Report.

INTRODUCTION: Internal hernia involving the stomach associated with a sigmoid colostomy is extremely rare. The hernia defect is typically located between the colostomy limb and the left abdominal wall. Because only a few cases have been reported, no standardized management strategy has been established, particularly in patients in whom immediate colostomy closure is not appropriate. CASE PRESENTATION: A woman in her 70s underwent a laparoscopic Hartmann's procedure with sigmoid colostomy for obstructive sigmoid colon cancer with para-aortic lymph node metastasis. Across the Life Course and Into Custody: Understanding Incarceration Pathways of Deaf and Hard of Hearing Individuals.

Incarceration is rarely an isolated event but reflects cumulative institutional and interpersonal disadvantage across the life course. Although life-course criminology explains these trajectories for the general population, it has rarely been applied to d/Deaf and hard-of-hearing (D/HH) individuals, a population that is both understudied and difficult to access. This study addresses that gap using Life History Calendars to reconstruct the pathways of 16 incarcerated D/HH individuals in the southeastern United States. Findings identify early communication deprivation and family isolation as dis

MRI Imaging of Uterine Incarceration: From Diagnosis to Posttreatment Follow-Up.

INTRODUCTION: Uterine incarceration is a rare and frequently underrecognized obstetric complication caused by persistent retroversion of the gravid uterus. Because clinical manifestations are often nonspecific, diagnosis may be delayed, increasing the risk of maternal and fetal complications. Imaging, particularly magnetic resonance imaging (MRI), may be essential when ultrasound findings are inconclusive. CASE PRESENTATION: A 37-year-old multigravida with a history of recurrent pregnancy loss presented at 16 weeks' gestation with acute urinary retention. Clinical examination raised suspicion

Rock Bottom Is Not a Motivator: From Opioid Addiction to Human Flourishing.

BACKGROUND: "Hitting rock bottom" is widely used in narratives of addiction and recovery. Recovery programs often assume, implicitly and explicitly, that "hitting bottom" is necessary to motivate recovery. However, a gap in the literature exists regarding the nature of experiences that characterize rock bottom, and the extent to which rock bottom experiences promote recovery. OBJECTIVES: Characterize rock bottom experiences in individuals with opioid use disorder, explore any role of these experiences in motivating recovery, and identify factors critical to sustaining recovery. DESIGN: Semi-

Adverse long-term effects of criminal legal system involvement on mental health in adulthood among a longitudinal cohort of youth in the USA.

BACKGROUND: Despite an established association of criminal legal system (CLS) involvement with poor mental health, it has been challenging to estimate a long-term causal effect among youth. This study examined the effects of CLS involvement on mental health outcomes among youth in the USA over 10 years. METHODS: We used data from 5611 youth from the National Longitudinal Survey of Youth 1997 (2000-2010). Using the parametric g-formula, we estimated the risks of (1) depressive symptoms (defined by the Mental Health Inventory 5-item version) and (2) self-reported emotional, mental health or psy

Infections and the Opioid Epidemic: Changing Epidemiology and Treatment Options.

The opioid epidemic has led to an infectious diseases syndemic, with injection drug use responsible for driving rising rates of HIV, viral hepatitis, and serious bacterial and fungal infections. Opioid use continues to cause devastating mortality, with over 80,000 opioid-involved overdose deaths in the USA in 2022. Despite the proven effectiveness of opioid agonist therapy, specifically methadone and buprenorphine, treatment coverage remains inadequate, with profound gaps in opioid use disorder (OUD) continuum of care. Injection-related infections, particularly skin and soft tissue infections,
