Public health initiative at a young offenders institute.

One in four of prisoners aged 17 to 21 are fathers, often with limited experience of positive parenting themselves. The men in Aylesbury Young Offenders Institute were found to have pride in fatherhood and during parenting classes expressed a desire to become actively involved in the care of their children. They stated that they felt they needed to learn parenting skills with their partner and family rather than in isolation from them. Health visitors were involved with parenting classes in the prison education block and helped the fathers develop joint parenting skills through a family orient

Comparison of offenders with mental illness only and offenders with dual diagnoses.

OBJECTIVE: The study compared offenders who had severe mental illness only and offenders who had severe mental illness and substance abuse problems-dual diagnoses-to determine whether these groups differed. Offenders with dual diagnoses who were involved with the criminal justice system at different levels were compared to explore their profiles and experiences after release. METHODS: Secondary data collected on offenders who had diagnoses of severe mental illness and of substance abuse in Massachusetts were used to examine sociodemographic, clinical characteristics, and criminal justice char

Gender differences in outcomes from prison-based residential treatment.

This study examines gender similarities and differences in background characteristics, the effectiveness of treatment, and the predictors of post-release outcomes among incarcerated drug-using offenders. The sample of 1,842 male and 473 female treatment and comparison subjects came from a multi-site evaluation of prison-based substance abuse treatment programs. Three-year follow-up data for recidivism and post-release drug use were analyzed using survival analysis methods. Despite the greater number of life problems among women than men, women had lower three-year recidivism rates and rates of

Evaluation of a prison outreach clinic for the diagnosis and prevention of hepatitis C: implications for the national strategy.

BACKGROUND: Hepatitis C virus (HCV) infection is a major public health problem recognised by the UK National Strategy that proposes that a care pathway for assessment, diagnosis, and treatment be established in all prisons, integrated within managed clinical networks. A prison sentence provides the opportunity to focus on traditionally hard to reach patients. AIMS: To evaluate the prevalence of HCV infection in a UK prison cluster and to assess the effectiveness of a prison outreach service for hepatitis C. SUBJECTS: Male prisoners. METHODS: A nurse specialist led clinic within a cluster of

Self-reported personality traits and disorders (DSM-IV) and risk of criminal recidivism: a prospective study.

Assessment and management of criminal offenders require valid methods to recognize personality psychopathology and other risk and protective factors for recidivism. We prospectively explored the association between dimensional and categorical measures of personality disorder (PD) measured with the DSM-IV and ICD-10 Personality Questionnaire (DIP-Q, Ottosson et al., 1995) and registered reconvictions in adult offenders. One hundred and sixty-eight offenders consecutively referred for pre-sentencing forensic psychiatric evaluation in Sweden during 1995-1996 completed DIP-Q self-reports. The subj

Treatment preparation in the context of system coordination serves inmates well.

A large percentage of jail inmates suffer from substance abuse problems; however, providing treatment in jail is difficult. Multnomah County's In Jail Intervention Program (IJIP) demonstrated an effective alternative. Finigan, Barron, and Carey (In press) and Barron and Finigan (1999) demonstrated that inmates with substance use problems, especially women, participating in IJIP experienced fewer rearrests and reincarcerations. To address the question of what led to these outcomes, quantitative data were abstracted from program, jail, and state administrative databases and were supplemented by

The relation of appraisal, coping efforts, and acuteness of trauma to PTS symptoms among former political prisoners.

We examined how trauma-specific appraisals and coping efforts mediate between traumatic experiences, acuteness of trauma, and length of imprisonment and posttraumatic stress symptoms (PTS) among 103 Palestinian former political prisoners. The findings provide support for both direct and mediated models of trauma. The acuteness of trauma (time since release), appraisal of prison experience as harmful and involving loss, and use of both emotion- and problem-focused coping efforts were associated with high levels of PTS symptoms. Torture and ill-treatment had a direct association with intrusion,

An examination of racial differences among mentally ill offenders in Massachusetts.

Data from 169 mentally ill prison inmates who completed a three-month program to help them reenter the community were analyzed to compare the demographic and clinical characteristics of white, black, and Hispanic participants. A total of 107 were Caucasian, 35 were black, and 27 were Hispanic. Caucasian participants were more likely to have affective disorders and to report a history of substance use problems. Black and Hispanic participants were more likely to have thought disorders. Black participants released from prison tended to migrate to more urban areas than Caucasians. Of those engage

Federal prison residential drug treatment reduces substance use and arrests after release.

OBJECTIVE: The effectiveness of federal prison-based residential drug and alcohol treatment programs was evaluated using event history procedures that addressed the problem of selection bias and included a wide range of control variables. METHODS: The sample comprised 760 treatment subjects and 809 comparison subjects. Treatment subjects were from 20 different prisons of medium, low, and minimum security levels. Comparison subjects were drawn from over 30 prisons. RESULTS: The results indicated that individuals who entered and completed in-prison residential treatment were less likely to exp

Jails, prisons, and the health of urban populations: a review of the impact of the correctional system on community health.

This review examined the interactions between the correctional system and the health of urban populations. Cities have more poor people, more people of color, and higher crime rates than suburban and rural areas; thus, urban populations are overrepresented in the nation's jails and prisons. As a result, US incarceration policies and programs have a disproportionate impact on urban communities, especially black and Latino ones. Health conditions that are overrepresented in incarcerated populations include substance abuse, human immunodeficiency virus (HIV) and other infectious diseases, perpetr

[Compliance of antituberculosis therapy among ex-inmates in the Madrid area].

BACKGROUND: The inmate population is not a tight compartment without communication with the community but there is a flow of persons and thus of health problems. The high incidence of tuberculosis among inmates is therefore of concern for the Public Health System. METHODS: The outcomes of antituberculosis treatment among ex-inmates released from prison in 1987 in the Madrid Area were evaluated and compared with those who remained in jail on treatment. Individuals who met the case definition of tuberculosis were included in the study. The outcome was defined as the individual status one year a

Reintegrating women leaving jail into urban communities: a description of a model program.

Women are the fastest-growing population in the criminal justice system, and jails reach more people than any other component of the correctional system. About 1 million women pass through US jails each year. Most return to their communities within a few weeks of arrest, and few receive help for the substance abuse, health, psychological or social problems that contribute to incarceration. We describe a model program, Health Link, designed to assist drug-using jailed women in New York City to return to their communities, reduce drug use and HIV risk behavior, and avoid rearrest. The program op

A collaborative effort to enhance HIV/STI screening in five county jails.

Funding from the Centers for Disease Control and Prevention and the Health Resources and Services Administration (HRSA) supports collaborations among health departments (CA, FL, GA, IL, MA, NJ, NY), correctional facilities, and community-based organizations to improve services to HIV-infected inmates, particularly as they return to the community. Additionally, HRSA funded the Evaluation and Program Support Center to guide the implementation of a multi-site evaluation of the Corrections Demonstration Project (CDP). The authors present a model approach to the problem of health disparities that i

Drug-related deaths among recently released prisoners in the Strathclyde Region of Scotland.

Drug abuse and its consequences are everyday problems encountered globally, and Scotland is no exception. During a study of drug-related deaths in the Strathclyde region of Scotland it was noted that known drug users who had recently been released from prison were at high risk of dying from a drug overdose. The majority of deaths occurred within one week of the release date and polydrug use was prevalent. Morphine was the most frequently encountered drug and this was found in combination with benzodiazepines in a significant number of cases. This paper highlights the dangers of resuming drug c

HIV infection in U.S. correctional systems: its effect on the community.

Increased rates of HIV infection and risk-taking behaviors among incarcerated men and women make the fight against HIV within the prison and jail systems an especially critical issue in community health. Overcrowded conditions impact on the rotation of inmates in and out of the correctional system. This revolving door phenomenon has implications for disease prevention and control within the community into which the inmates are released. As more people pass in and out of jails and prisons, more problems and diseases associated with incarceration pass into the community. The special needs of the

Perceptions about substance use among male adolescents in juvenile detention.

Although adolescents in juvenile detention represent a vulnerable population who are exposed to situations that foster risk-taking behaviors, few studies have been conducted with detained adolescents to determine their perceptions regarding substance use. Ethnographic interviews and observations were conducted with 20 male adolescents who resided in a large metropolitan area juvenile detention facility, to discover their substance use beliefs and the decisions they make to continue or discontinue substance use or abuse. The participants described how they initiated substance use and said that

Linking women in jail to community services: factors associated with rearrest and retention of drug-using women following release from jail.

Women in jail experience high rates of many health and social problems. This study examined the effects of preexisting social and health characteristics and the type of services received on retention in community aftercare for 193 drug-using women released from the New York City jail to two low-income communities. Rearrest rates for program participants were compared to a group of women not eligible for services because of their residence outside the target communities. Women who enrolled in residential programs with on-site drug treatment and other social services after release were compared

Neuropsychological outcomes of traumatic brain injury and substance abuse in a New Zealand prison population.

Fifty subjects with a history of traumatic brain injury (TBI) and/or substance use, completed neuropsychological measures of short and long term verbal and visual memory, information processing, motor speed and co-ordination, executive functioning, and malingering. All subjects performed below norms on tests of verbal memory and verbal abstract thinking, but overall no differences were found due to either severity of TBI or level of substance use. Maori subjects obtained the lowest scores on tests of verbal ability, but also reported higher rates of TBI and substance use, which is presumed to
