One-Lung Ventilation for Neonatal Pneumonectomy in a Hydropic Premature Infant: Fiberoptic-guided Left Mainstem Intubation.

Congenital cystic adenomatoid malformation (CCAM) is a rare lung anomaly that can cause severe respiratory distress and hydrops fetalis. One-lung ventilation (OLV) in neonates poses extreme anatomical challenges due to the lack of appropriately sized double-lumen tubes or bronchial blockers. A male neonate (2500 g, 30 weeks' gestation) presented with hydrops fetalis and a massive right-sided thoracic CCAM. Due to progressive respiratory failure and hemodynamic instability, an emergency right pneumonectomy was scheduled on postnatal day 3. To achieve OLV, a 3.5-mm cuffed endotracheal tube (with

Hybrid Airway Management for Neuromonitoring-guided Selective Phrenic Nerve Surgery in Intractable Hiccups: A Case Report.

Intractable hiccups are rare but debilitating, and may cause sleep disturbances, nutritional impairment, exhaustion, and a severe deterioration in quality of life. When pharmacological and interventional treatments fail, selected surgical procedures targeting the phrenic nerve may be considered. We report the anaesthetic management of a 32-year-old woman with idiopathic intractable hiccups who underwent right-sided selective phrenic nerve surgery guided by thoracoscopic neuromonitoring. Because thoracoscopic exposure and intraoperative neuromonitoring were required simultaneously, a hybrid air

Congenital Lung Malformations and Quantitative Assessment of Infant Tracheal-Bronchial Dimensions.

BACKGROUND: Congenital lung malformations (CLMs) are heterogeneous developmental anomalies caused by disrupted airway branching morphogenesis, but whether the central airways are also involved remains unclear. We quantified infant tracheobronchial dimensions in CLMs and compared them with published infant reference values. METHODS: This retrospective single-center study included 81 of 94 infants with CLMs assessed for eligibility. Exclusions were secretions preventing measurement (n = 3), motion artifacts (n = 7), unavailable preoperative computed tomography (CT) (n = 2), and missing diagnosi

Giant mixed vascular lymphatic malformation in a pediatric patient: a case report.

BACKGROUND: Giant Mixed vascular lymphatic malformation-is a relatively rare congenital anomaly, particularly in pediatric populations. Mediastinal vascular malformations are uncommon in children, and their exact incidence remains poorly defined in the literature. It is not a true neoplasm but rather a low-flow vascular malformation arising from aberrant development of the vascular system during embryogenesis. Due to nonspecific clinical manifestations (e.g., cough, dyspnea) and the deep location of the mediastinum-adjacent to the heart, great vessels, and airways-diagnosis and management pose

Single-lumen tube cuff tamponade and dynamic positioning for bronchoscopy-associated massive airway hemorrhage: a case series.

Bronchoscopy-associated acute massive airway hemorrhage (BAMAH) is an uncommon but life-threatening emergency, as rapid airway flooding can compromise bronchoscopic visualization, ventilation, oxygenation, and circulation. Conventional bronchoscopic and lung-isolation methods may be difficult to deploy when visualization is lost, whereas bronchial artery embolization and surgery cannot provide immediate airway stabilization. This brief report describes a rescue strategy guided by a simplified ABCDE framework that combines a cuffed single-lumen endotracheal tube with cuff tamponade, lung isolat

Concealed right upper lobe pulmonary venous variants on 3D-CTBA: morphological characterization and surgical implications.

INTRODUCTION: The surgical anatomy of the right upper lobe pulmonary vein is variable and rare anomalies often go unnoticed in the first intraoperative exposure. METHODS: 3D-CTBA reconstructions were performed on 307 right-sided contrast-enhanced chest CT data with full segmentation of the arterial, venous and bronchial systems. RESULTS: Anomalous right upper lobe vein anatomy was detected in 23 cases (7.49%) consisting of 18 right top pulmonary veins (RTPVs) and 5 cases of proposed cranially displaced central vein (CDCV). In all 18 RTPVs, a single venous trunk collecting S2 drainage followe

Left pulmonary aplasia with ipsilateral renal agenesis and double superior vena cava in a term neonate: a case report.

BACKGROUND: Unilateral pulmonary agenesis/aplasia is rare and often accompanied by extrapulmonary anomalies that affect prognosis. CASE REPORT: A term male neonate delivered by cesarean section for fetal distress required brief invasive ventilation and was subsequently stabilized. Chest radiography revealed left hemithorax opacification. Contrast-enhanced CT on day 4 demonstrated a reduced left hemithorax with an absent left pulmonary parenchyma, a blind-ending left main bronchus, and an absent ipsilateral pulmonary arterial supply, consistent with left pulmonary aplasia. Evaluation also iden

Bronchial artery embolization followed by bronchoscopic electrosurgical snare resection for diagnosis and treatment of primary pulmonary osteosarcoma: case report and literature review.

Primary pulmonary osteosarcoma (PPOS) is an extremely rare extraskeletal osteosarcoma. Because its clinical and radiological features are nonspecific, establishing a diagnosis is challenging. We report the case of a 58-year-old man who presented with a 1-month history of cough and 5 days of dyspnea. Initial chest computed tomography showed destructive changes in the left lung and an endobronchial lesion in the left main bronchus. To relieve airway obstruction and obtain diagnostic tissue, a sequential strategy of bronchial artery embolization followed by bronchoscopic electrosurgical snare res

Right upper lobectomy via selective right upper lobe isolation using a left-sided double-lumen tube placed in the right bronchus intermedius: a case report.

BACKGROUND: One-lung ventilation (OLV) using a left-sided double-lumen tube (DLT) in the left main bronchus is standard for right thoracic surgery. When the contralateral left lung is severely compromised, this approach forces OLV to rely on the dysfunctional lung and risks critical hypoxemia. CASE PRESENTATION: An 84-year-old woman with a left diaphragmatic hernia causing approximately 70% compression and atelectasis of the left lung was scheduled for right upper lobectomy. Preoperative assessment indicated that standard DLT placement would necessitate complete right lung collapse, placing t

Ventilation Strategies in a Pediatric Patient Undergoing Resection and Anastomosis for a Juxta Carinal Tumor.

Tumors close to the carina pose a unique challenge to the Anesthesiologist in terms of lung isolation and ventilation strategies. We present the case of a 15-year-old patient who was diagnosed with a myofibroblastic tumor in the left main bronchus and carina, posted for a robotic assisted sleeve resection and anastomosis. The surgical approach was from the right side. Hence, the traditional strategies for right lung ventilation could not be employed. A combination of endobronchial intubation, jet ventilation via airway exchange catheter (AEC) and cross field ventilation enabled us to complete

Eleven-Year Undiagnosed Bronchial Foreign Body in an Adult: A Case Report.

Foreign body aspiration (FBA) in adults is rare, and tooth aspiration represents an exceptionally uncommon subset. Even rarer is the prolonged retention of a tooth in the left main bronchus, which is anatomically less favorable for aspiration than the right. However, individual airway variations may predispose to such occurrences. We report the case of a 56-year-old woman with an 11-year history of recurrent "pneumonia-like" episodes characterized by chronic productive cough, intermittent fever, hemoptysis, and pleuritic chest pain. She had been repeatedly treated with empirical antibiotic reg

Endoscopic Removal of an Intrabronchial Foreign Body Using Single-Use Flexible Bronchoscopy in an Older Adult: A Case Report and Review of the Literature.

Foreign body aspiration (FBA) is less common in adults than in children and occurs predominantly in older individuals with predisposing risk factors. Depending on the anatomical location of the foreign body within the tracheobronchial tree, it may cause life-threatening airway obstruction or lead to a wide range of clinical manifestations and complications, the occurrence of which largely depends on the duration of impaction. We report the case of a 74-year-old woman who was admitted with acute respiratory distress following the accidental aspiration of a solid food bolus. The initial choking

Aortic elongation with prosthetic graft followed by aortopexy for fixed left main bronchial compression in an adolescent with severe thoracic deformity: A case report.

Airway compression in older children is uncommon and becomes more challenging when thoracic deformities alter aortic anatomy. A 16-year-old girl with severe scoliosis and chronic bedridden status presented with progressive respiratory distress caused by compression of the left main bronchus between the ascending aorta and a right-sided descending aorta. As conventional aortopexy alone was unlikely to achieve adequate decompression, aortic elongation using a prosthetic graft was performed to reposition the descending aorta posteriorly, followed by aortopexy. Postoperative imaging confirmed comp

A central limit theorem for the Euler method for SDEs with irregular drifts.

BACKGROUND: Retinal vessel caliber is strongly associated with systemic blood pressure (BP); however, the causal relationship between retinal vascular caliber and BP remains unclear. Understanding this relationship is essential for advancing predictive, preventive, and personalized medicine (PPPM) approaches to effectively manage hypertension and its related complications. WORKING HYPOTHESIS: Microvessel morphology is causally related to blood pressure. By integrating genome-wide association studies, Mendelian randomization analysis, transcriptomic data, and multivariate genomic approaches, t

Case Report of Acute Central Airway Obstruction Induced by Anti-PD-1 Sintilimab: Clinical Presentation and Review.

Immune checkpoint inhibitors (ICIs), such as sintilimab, have revolutionized non-small cell lung cancer (NSCLC) treatment but can trigger immune-related adverse events (irAEs). While pneumonitis is well documented, central airway obstruction (CAO) due to immune-mediated necrosis is an extremely rare and life-threatening phenotype that poses significant diagnostic challenges. A patient with recurrent squamous cell carcinoma of the lung achieved partial remission after two cycles of sintilimab combined with chemotherapy. However, 48 h after the third cycle, the patient developed acute, severe dy

Surgical techniques for thoracoscopic secondary carinal reconstruction with stepwise barbed sutures.

Parenchymal-preserving surgery is the preferred technique for selected patients with centrally located airway tumors limited to the secondary carina; however, reconstruction is challenging. We present a simplified video-assisted thoracoscopic surgery technique for secondary carinal reconstruction with three barbed sutures on each side. In each patient, the first thread was used to approximate the main bronchus and lower lobe bronchus on the left side or the bronchus intermedius and upper lobe bronchus on the right side. A horizontal mattress suture was critical to secure the backward joint sec

The Diagnostic Challenge of Sharp Foreign Body Aspiration in Children: A Two-Case Series Highlighting Ingestion vs. Aspiration.

Foreign body aspiration (FBA) is a common pediatric emergency. Sharp objects, such as pins, pose a unique risk of perforation and migration. Differentiating aspiration from ingestion can be challenging when symptoms and radiological findings overlap, particularly for objects lodged in the mediastinum-a "mediastinal dilemma." We present two cases of sharp FBA in school-aged children. Case 1: A 10-year-old girl presented with acute chest pain after inhaling a headscarf pin. Chest X-ray showed a mediastinal radio-opaque shadow, creating a diagnostic dilemma. Sequential endoscopy (negative esophag

Evaluation of improved lung and airway morphology using CT 3D reconstruction after severe scoliosis correction: a retrospective cohort study.

BACKGROUND: Previous studies have not compared lung and airway morphologies before and after treatment for severe scoliosis. Herein, computed tomography (CT) 3-dimensional (3D) reconstruction of the respiratory system was performed to evaluate the changes in lung expansion and airway dilation following posterior spinal fusion for severe scoliosis. METHODS: Twenty-two patients with severe scoliosis (12 women and 10 men) who treated with posterior spinal fusion (PSF) were included. Pre- and postoperative visits included CT scans and radiographic assessments. Changes in airway dilation were asse
