Child with mucoepidermoid carcinoma of the trachea.

Mucoepidermoid carcinoma originates from the submucosal glands of the tracheobronchial tree. It is structurally homologous with exocrine salivary glands. It is a rare tumour, consisting of 0.1% to 0.2% of primary lung malignancies.Complete surgical resection is the treatment of choice and is associated with an excellent prognosis. Here we present a case where a boy in his teenage years came with a dry cough and haemoptysis.Contrast-enhanced CT confirmed the presence of a polypoidal enhancing space-occupying lesion arising from the right anterolateral aspect of the carina, protruding into the ri

Multimodality imaging in a child with complete atrioventricular canal defect, left superior vena cava, and airway compression: Echocardiography, bronchoscopy, and lung ultrasound insights.

Congenital heart disease may coexist with airway abnormalities, increasing perioperative risk. We report a 2-year-old girl with a complete atrioventricular canal defect (CAVCD), left superior vena cava (LSVC), and severe pulmonary hypertension, in whom multimodality imaging was pivotal for diagnosis and management. Transthoracic echocardiography confirmed CAVCD, LSVC, and severe pulmonary hypertension. Preoperative computed tomography demonstrated left pulmonary artery compression of the left main bronchus and a suspected cor triatriatum. Intraoperative bronchoscopy revealed left bronchomalaci

Endobronchial Lipoma in a Young Patient: A Rare and Atypical Case Report.

Endobronchial lipoma (EL) is a rare, benign pulmonary tumor that frequently causes diagnostic delays due to its nonspecific presentation. We report a case of a 32-year-old male chronic smoker who presented with a 15-day history of dry cough and tachypnea. Chest X-ray showed left lower lobe atelectasis. Contrast-enhanced computed tomography revealed a fat-density lesion in the carina on the left side extending into the left main bronchus, causing distal obstruction and left lower lobar collapse. Diagnostic bronchoscopy confirmed a large, pedunculated mass completely occluding the left main bron

Salvage sleeve-pneumonectomy for a malignant bronchomediastinal fistula and the feasibility of a modified tritube®: a case report.

BACKGROUND: Bronchomediastinal fistula (BMF) is a rare and life-threatening complication following multimodality treatment for lung cancer. Surgical management is often the only definitive option but is technically challenging, especially in the context of radiation-induced fibrosis and the need for reliable intraoperative ventilation when the airway itself is the site of resection. We describe the surgical treatment of a large BMF in a patient with advanced non-small cell lung carcinoma (NSCLC), with an emphasis on the use of a modified narrow-bore Tritube® for airway management. CASE PRESENT

Anesthetic Management for Headscarf Pin Tracheobronchial Foreign Body Removal in a Pediatric Patient.

Foreign bodies in the airways are relatively common in preschool- and school-aged children. We report a rare case of the successful removal of a sharp metallic foreign body from the bronchus of a 12-year-old boy under combined inhalational and intravenous anesthesia without tracheal intubation. This report emphasizes the perioperative anesthetic strategies for foreign object removal. The patient presented with sudden-onset choking and hemoptysis. Chest computed tomography revealed a linear metallic foreign body measuring approximately 5 cm long in the left main bronchus. The foreign body was r

Photodynamic therapy for pulmonary mucoepidermoid carcinoma: a case report.

SIGNIFICANCE: Pulmonary mucoepidermoid carcinoma (PMEC) is a rare salivary glandtype tumor with no standard treatment. While photodynamic therapy (PDT) shows promise in early-stage central lung cancers, its role in PMEC remains underexplored. APPROACH: We report a case of a 46-year-old male with low-grade PMEC of the left main bronchus. After initial bronchoscopic electrosnare resection, recurrence occurred 2 years later. The patient was treated with PDT, consisting of three consecutive daily sessions over three days using a 630 nm light source (0.9 W). Each treatment included two light expos

Case Report: Successful repair of traumatic right main bronchial transection under veno-venous ECMO support with postoperative inhaled nitric oxide.

BACKGROUND: Traumatic bronchial transection is a rare but life-threatening consequence of blunt chest trauma. When severe hypoxemia persists despite intubation and mechanical ventilation, maintaining adequate gas exchange during airway reconstruction becomes a major challenge. CASE PRESENTATION: We describe a 52-years-old male who sustained a complete right main bronchus transection after a crush-related traffic accident. Despite intubation and maximal ventilatory support, oxygenation remained critically low. Emergency venovenous extracorporeal membrane oxygenation (VV-ECMO) was initiated by

Delayed Diagnosis of Primary Bronchial Adenoid Cystic Carcinoma Presenting as Central Airway Obstruction: A Case Report.

Bronchial adenoid cystic carcinoma (ACC) is a rare malignant tumor originating from salivary gland-type tissues of the tracheobronchial tree. It usually occurs in relatively young patients and is not strongly associated with tobacco exposure. Despite its slow-growing nature, ACC can behave aggressively locally, leading to significant airway obstruction. We report the case of a 47-year-old woman exposed to wood smoke who presented with progressive exertional dyspnea and unquantified weight loss. Chest computed tomography revealed a right hilar infiltrative lesion completely obstructing the righ

Navigating controversies in stage III NSCLC: a multidisciplinary case discussion on evolving treatment paradigms.

BACKGROUND: The management of stage III non-small cell lung cancer (NSCLC) has become increasingly complex, driven by advances in neoadjuvant and perioperative chemo-immunotherapy as well as targeted therapies. These evolving treatment paradigms have introduced new challenges for multidisciplinary teams (MDTs), regarding patient selection, treatment sequencing, surgical planning and definitions of operability and resectability. CASE PRESENTATION: We present a case of a 69-year-old male with cT3N2aM0 (single-station N2) adenocarcinoma NSCLC with a programmed death-ligand 1 (PD-L1) tumour propo

Definitive bronchoscopic photodynamic therapy for low-grade endobronchial mucoepidermoid carcinoma adjacent to the carina.

Pulmonary mucoepidermoid carcinoma (PMEC) is a rare salivary gland-type primary lung malignancy arising from the tracheobronchial submucosal glands. Although surgery remains a standard curative option, carefully selected low-grade and localized endobronchial lesions may also be amenable to definitive bronchoscopic treatment. We report a 40-year-old man with compensated liver cirrhosis (preserved liver function) who presented with a 1-year history of cough. Chest computed tomography revealed an endobronchial lesion (∼10 mm) in the left main bronchus approximately 1 cm distal to the carina. 18F-

A Case of Primary Endobronchial Hyalinizing Clear Cell Carcinoma Presenting as an Oncologic Emergency Due to Complete Left Main Bronchus Obstruction Successfully Managed with Airway Intervention.

A 66-year-old man presented with dyspnea and severe hypoxemia, requiring mechanical ventilation. Computed tomography revealed an obstruction of the left main bronchus by a bronchial tumor, thus prompting airway intervention to relieve the stenosis. A histopathological examination of the tumor revealed clear cytoplasm and hyalinized stroma, and rearrangement of EWSR1 was detected by a fluorescence in situ hybridization analysis. Comprehensive genomic profiling was performed because of the rarity of this case. GenMineTOP, capable of detecting the EWSR1::ATF1 fusion, contributed to a definitive d

Thoracoscopic right S1 + S3 segmentectomy for lung cancer with a displaced B2 bronchus: a case report.

BACKGROUND: Tracheobronchial branching anomalies are uncommon, but increase the risk of intraoperative vascular or bronchial injury if not recognized preoperatively. An independent origin of the right B2 bronchus from the bronchus intermedius is particularly rare and may be accompanied by complex pulmonary venous and arterial variations. Careful preoperative anatomical assessment is crucial when planning anatomical segmentectomy in such cases. CASE PRESENTATION: A 73-year-old woman was referred to our hospital after a 12-mm partially solid nodule with a cavity at the border between segments S

Typical carcinoid tumor originating from the tracheal carina managed with carinal resection and neo-carinal bronchoplastic reconstruction: a case report.

BACKGROUND: Primary tumors of the tracheobronchial tree are uncommon and may be overlooked because symptoms often mimic more prevalent respiratory disorders. A typical carcinoid is a well-differentiated neuroendocrine neoplasm that frequently arises in the central airways; origin from the tracheal carina is exceptional. Marked tumor vascularity may increase bleeding risk and lead to non-representative bronchoscopic biopsies, delaying diagnosis. CASE PRESENTATION: A 31-year-old Middle Eastern woman, non-smoker and previously healthy, presented with seven months of recurrent hemoptysis that grad

A unique method for one-lung ventilation in pediatric thoracic surgery: Suction catheter and single-lumen tube approach: A case report.

Effective one-lung ventilation (OLV) ensures optimal surgical exposure and isolation of the operative lung. This case describes a novel technique for achieving OLV in pediatric thoracic surgery using a suction catheter with a single-lumen endotracheal tube (SLT). An 11-year-old girl presented with a 2-day history of hemoptysis. Chest CT showed bronchial wall thickening and multifocal nodular opacities in the right upper lobe, suggesting pulmonary hemorrhage. After standard ASA monitoring, arterial and central venous lines were placed. The patient was intubated with a size 6.0 SLT and a size 10

Thoracoscopic left apicoposterior segmentectomy for bronchiectasis with a displaced B¹⁺² bronchus: a case report.

BACKGROUND: The coexistence of bronchial anatomical variations with bronchiectasis presents significant challenges for surgical intervention. Among these, an anomalous left apicoposterior segmental bronchus (B¹⁺²)—originating ectopically from the left main bronchus—is a highly uncommon variant that predisposes individuals to impaired mucociliary clearance and subsequent bronchiectasis. This case demonstrates the technical feasibility and considerations of performing a video- assisted thoracoscopic surgery (VATS) left apicoposterior segmentectomy in the presence of an displaced B1 + 2 bronchus,
