Functional assessment of unilateral main bronchial stenosis is challenging because conventional pulmonary function tests and morphological imaging may not adequately reflect side-specific ventilation abnormalities. Dynamic digital radiography (DDR) enables visualization of respiratory dynamics throughout the breathing cycle. A 75-year-old woman presented with cough and dyspnea on exertion was found to have a nodular lesion causing near-complete obstruction of the left main bronchus. Bronchoscopic snare resection was performed, and histopathological examination confirmed mucoepidermoid carcinom

BACKGROUND: Congenital bronchoesophageal fistula (BEF) is a rare communicating bronchopulmonary foregut malformation (CBPFM) characterized by an abnormal connection between the esophagus and a bronchial segment or lung tissue. While esophageal atresia (EA), with or without tracheoesophageal fistula, occurs in approximately 2-3 per 10,000 live births, EA associated with CBPFM/congenital BEF is far rarer, with no established population-based incidence and only several dozen reported CBPFM cases in the literature. Most congenital BEFs present in childhood or adulthood with chronic, nonspecific re

Foreign body aspiration in children is an important cause of airway morbidity and may present with persistent cough, delayed diagnosis, atelectasis, and granulation tissue formation. Smooth hollow plastic foreign bodies may be particularly difficult to retrieve because standard forceps often fail to achieve a secure grip, and balloon-assisted techniques may be limited by repeated slippage. We report the case of a seven-year-old boy who presented with a persistent cough for 15 days following aspiration of a toy whistle. He was referred to our center after an unsuccessful retrieval attempt at an

CASE SUMMARY: A 3-year-old, spayed female domestic shorthair cat was presented for acute onset cough and dyspnoea. Thoracic radiography revealed a radiopaque foreign body proximally in the right caudal main bronchus. Initial attempts at endoscopic retrieval using a flexible bronchoscope were unsuccessful, as the foreign body migrated progressively distally into the right caudal lobe bronchus, precluding bronchoscopic removal. The cat subsequently underwent a right fifth intercostal-space lateral thoracotomy. A primary bronchotomy was performed to retrieve a small stone, and the bronchus was cl

Retained intrathoracic metallic foreign bodies (FBs) following penetrating chest trauma represent a challenging surgical emergency, particularly when situated in proximity to critical mediastinal structures. Anesthetic management in such cases demands meticulous airway planning, expertise in lung isolation techniques, and vigilant anticipation of physiological changes during one-lung ventilation. We describe two adult men with penetrating chest trauma and retained metallic FBs adjacent to critical mediastinal structures, both managed under general anesthesia with left lung isolation using a br

BACKGROUND: Neoadjuvant chemoimmunotherapy has improved resectability in locally advanced non-small cell lung cancer (NSCLC); however, its impact on complex bronchioplastic procedures remains controversial. In particular, the feasibility of extended sleeve resection (ESL) in patients with rare tracheobronchial branching anomaly after immunochemotherapy has rarely been reported. Herein, we present a rare case of an atypical type D extended sleeve resection performed to avoid pneumonectomy in a patient with a rare bronchial branching anomaly.

A 75-year-old male, former smoker (50 pack-years), with a history of squamous cell lung carcinoma treated with right lower lobectomy 20 years earlier, presented with progressive dyspnea at rest (mMRC IV) and hypoxemic respiratory failure. Chest computed tomography revealed a 14 mm nodular lesion in the left main bronchus. Bronchoscopy showed a lesion at the carina between the left upper and lower lobes, causing near-complete obstruction. Biopsy and partial endoscopic resection were performed, but follow-up demonstrated slight progression. Histopathology confirmed squamous cell carcinoma. Due t

Extrinsic airway compression secondary to vertebral osteophytes is a rare clinical entity. We report a case of an elderly patient presenting with pneumonia where a subsequent chest CT scan showed right main bronchus compression caused by lower thoracic vertebral osteoarthritis-related osteophytes. There were additional incidental findings of sclerotic skeletal lesions prompting a new diagnosis of metastatic prostate cancer. The pneumonia improved with antibiotic therapy. Interventional management of airway compression was not pursued in this patient given his poor functional state and advanced

BACKGROUND: Congenital aerodigestive fistulae (ADF) represent extremely rare developmental malformations derived from disrupted embryological separation of the primitive foregut, which can be classified by lesion location into tracheoesophageal fistulae, bronchoesophageal fistulae (BEF), and other subtypes. While typically diagnosed in neonates, isolated BEF without esophageal atresia ("H-type") may remain undetected for decades, presenting in adulthood with chronic, stereotyped symptoms. Endoscopic interventions for congenital BEF remain poorly characterized, with limited evidence regarding p

BACKGROUND: There is limited clinical data to guide surgical decisions for ACC with extensive carinal involvement in the left main bronchus. In this case, it is shown that median sternotomy and CPB are feasible in this particular anatomical setting.
CASE PRESENTATION: Cough and sputum were the presenting symptoms in a middle-aged female patient. Imaging examinations identified a left hilar mass that extended to involve the carina. The surgical intervention performed via a median sternotomy with CPB support encompassed both a left pneumonectomy and partial carinal resection along with reconstr

BACKGROUND This case report presents a groundbreaking approach to treating a giant ruptured aortic arch aneurysm in a patient who previously underwent endovascular aneurysm repair (EVAR). CASE REPORT A 66-year-old man was admitted for surgical intervention due to a ruptured aortic arch aneurysm. He presented with shock, hypotension, hemorrhage into the left pleura, and chest and abdominal pain. He had a history of EVAR 10 years prior. An urgent computed tomography angiography revealed a ruptured aortic arch aneurysm, 12 cm in diameter, with a descending thoracic aorta segment with features of

Inflammatory myofibroblastic tumour (IMT) is a rare mesenchymal neoplasm of intermediate biological potential, most commonly affecting children and young adults, with the lung being one of the the most common sites. Its variable clinical, radiological and pathological features often mimic malignancy, making diagnosis challenging. We report a middle-aged man in his early 50s presenting with life-threatening haemoptysis and found to have a right main bronchus mass causing central airway obstruction. Emergency rigid bronchoscopy with cryo-debulking was performed for airway stabilisation and tissu

INTRODUCTION: Primary giant cell tumor of bone (GCTB) of the rib is rare. Herein, we report a rare case of posterior rib GCTB causing airway compression.
CASE PRESENTATION: A 35-year-old woman complained of back pain and dyspnea for 4 months. A bone tumor in the posterior fifth rib was suspected, and needle biopsy confirmed the diagnosis of GCTB. The tumor occupied the right thoracic cavity, compressing the main bronchus and extensively invading the vertebral body. Because complete resection was initially considered infeasible, an airway stent was placed, and denosumab therapy was initiated.

Background: No consensus guideline exists for double-lumen tube (DLT) size selection, which conventionally relies on height, sex, and chest radiography. Computed tomography (CT) has been proposed to refine sizing, particularly in women and patients of small stature. We examined whether high-resolution CT (HRCT) measurement of the trachea and main bronchi improves DLT size selection beyond a height- and sex-based estimate in patients undergoing minimally invasive coronary artery bypass (MICS-CABG). The aim of this study was to determine whether HRCT measurement of the trachea and main bronchi i

Acute hypoxemic respiratory failure in toddlers is most commonly associated with severe pneumonia, bronchiolitis, or foreign body aspiration. Complete obstruction of a main bronchus by mucus plugs is an uncommon but potentially life-threatening cause of unilateral lung collapse and severe hypoxemia. We report the case of a previously healthy 17-month-old boy who presented with severe respiratory distress after 3 days of fever and cough. Physical examination revealed marked respiratory distress with absent breath sounds over the left hemithorax. Chest radiography and computed tomography demonst

Tetralogy of Fallot (TOF) with absent pulmonary valve (APV) spectrum is a rare congenital cardiac anomaly characterized by dysplastic or APV leaflets, leading to severe pulmonary regurgitation and aneurysmal dilatation of the pulmonary arteries. We report a 4-year-old male presenting with recurrent respiratory infections. Cardiac computed tomography (CT) revealed TOF with a subaortic ventricular septal defect, overriding aorta, right ventricular hypertrophy, and right ventricular outflow tract narrowing. The pulmonary valve was dysplastic, with marked aneurysmal dilatation of the main and bran

Penetrating tracheobronchial injuries (TBIs) are highly lethal emergencies that pose a critical challenge in low-resource settings due to the lack of advanced imaging. We report the case of a male patient with a knife impaled in the right posterior thorax who presented with dyspnea, hemoptysis, and subcutaneous emphysema. Due to the clinical suspicion of an airway injury and the inability to perform a computed tomography (CT) scan, the patient was immediately transferred to the operating room guided by basic radiographs; via a right thoracotomy, the sharp object was safely extracted, and prima

Robotic left main bronchial sleeve resection for endobronchial tumors avoids pneumonectomy while preserving the entire ipsilateral lung parenchyma. Preoperative three-dimensional computed tomography reconstruction guides dissection around the pulmonary artery, pulmonary veins, and left atrium, and Firefly fluorescence imaging enables precise intraoperative tumor localization without needle puncture. We hereby describe a patient with a typical carcinoid tumor of the left main bronchus, who successfully underwent a minimally invasive lung -preserving operation to achieve negative surgical margin

Rigid bronchoscopy in patients with functional single-lung physiology presents a considerable anesthetic challenge because interruption of spontaneous ventilation may rapidly result in severe hypoxemia. Although spontaneous respiration using intravenous anesthesia and high-flow nasal oxygen (STRIVE-Hi) has been described as a technique that facilitates prolonged apneic oxygenation, evidence regarding its application to preserve spontaneous ventilation during prolonged rigid bronchoscopy remains limited. A 55-year-old man with complete obstruction of the left main bronchus secondary to post-int
