From questionnaire to conversation: a structural intervention to improve HIV test counseling.

OBJECTIVE: We describe the effects of structural intervention to enhance the quality of HIV test counseling interaction with men who have sex with men (MSM) in San Francisco. METHODS: Audio recordings of 28 rapid HIV test sessions by seven counselors were collected in two phases: before and after implementation of a waiting room intervention prior to the session. The sessions were analyzed using sequence maps to visualize and compare the sequence and distribution of four activities: counseling, information delivery, data collection, and sample collection. RESULTS: Prior to the intervention,

Can interactive skills-based seminars with standardized patients enhance clinicians' prevention skills? Measuring the impact of a CME program.

OBJECTIVE: Communication skills are crucial for high-risk behavior screening and counseling. Practicing physicians have limited opportunities to improve these skills. This paper assesses the impact of a continuing medical education (CME) program for Student Health Center clinicians that targeted communication skills, screening practices and patient satisfaction. METHODS: Program evaluation included pre- and post-objective structured clinical examinations (OSCE's), chart review, and provider and patient satisfaction surveys. Data were analyzed using paired t-tests and ranked sum tests. RESULT

From the female perspective: Long-term effects on quality of life of a program for women with asthma.

BACKGROUND: Although, among adults, asthma predominates in women, the role of sex and gender in asthma has only recently been studied. Moreover, only one study has focused on the management of asthma by women, reporting that 1 year subsequent to an intervention addressing sex and gender role factors, women's asthma status was improved. OBJECTIVE: Data from a 2-year postintervention follow-up were assessed to determine whether there were longer-term effects on the asthma status and quality of life (QoL) of the participants. METHODS: A randomized controlled design was used in which female pati

Sexual orientation and intentions to obtain breast cancer screening.

AIMS: This longitudinal study examined predictors of intentions to obtain breast cancer screening in two samples, one comprising self-identified lesbian/bisexual women (n = 150) and the other comprising heterosexual women (n = 400). We hypothesized that beliefs about mammography, cancer vulnerability, and attitudes toward the medical provider would mediate the relationship between sexual orientation and plans to obtain breast cancer screening. METHODS: Women in this study participated in one of two larger clinical trials of breast cancer risk counseling methods. All participants completed que

Relational safety and liberating training spaces: an application with a focus on sexual orientation issues.

This article describes and discusses a teaching case of a clinical training situation involving a gay marriage and family therapy student working with a same-sex affectional couple. The conceptual pillars of this teaching case, relational safety and liberating spaces, are advanced as illustrations of how the student developed his voice in the training process. Pivotal moments in this process are discussed, as are implications for training and personal and professional growth.

Sex-related knowledge, attitudes, and behaviors of U.S. medical students.

OBJECTIVE: To understand the personal and clinical safe-sex-related knowledge, attitudes, and practices of U.S. medical students. METHODS: Sixteen medical schools were selected to survey the class of 2003 based on their characteristics similar to the national average. Students were surveyed at freshman orientation, at entrance to wards, and during their senior year. The primary personal outcome was the response to the question, "Are you currently trying to practice safe sex when sexually involved? (no, not applicable/no, not trying/yes, low priority/yes, high priority)." The primary professio

Helpful and unhelpful therapy experiences of LGBT clients.

The purpose of this study was to identify a broad range of variables that characterize the helpful and unhelpful therapy experiences of lesbian, gay, bisexual, and transgender (LGBT) individuals. Interviews were completed with a diverse sample of 42 LGBT individuals who have been in therapy, and a content analysis was conducted. Results indicated that basic counseling skills and relationships were key determinants of the quality of LGBT clients' therapy experiences. Also important to the helpfulness of the therapy experience were therapist variables such as professional background and attitude

Primary care health issues among men who have sex with men.

PURPOSE: The purpose of the article is to examine "appropriate" health care for men who have sex with men (MSM), which is not to suggest "special" health care. As a group, MSM are at increased risk for sexually transmitted infections, anal cancer, and mental health disorders. Focus areas in this article will address health issues that the primary care nurse practitioner (NP) may encounter in clinical practice: anal carcinoma, sexually transmitted diseases (STDs), high-risk sexual practices, depression, and substance abuse were topics chosen for inclusion in this article. These topics were amon

Nondisclosure of sexual orientation to a physician among a sample of gay, lesbian, and bisexual youth.

BACKGROUND: The American Medical Association, the American Academy of Pediatrics, and the Society for Adolescent Medicine recommend discussing sexual orientation as part of the health supervision of all adolescents. Little is known about whether lesbian, gay, and bisexual (LGB) youth hide their orientation from health care providers, which can potentially lead to missed opportunities in identifying individual health risks and provide appropriate screening and counseling. OBJECTIVES: To describe the health care experiences of a nonclinical sample of LGB youth and identify factors associated wi

Psychosocial issues in breast cancer.

The breast cancer experience can bring with it a broad range of emotional sequelae for the woman, her spouse/partner, and her family. Often, patients bear these emotional burdens silently and miss opportunities for the physician and health care team to offer recommendations and interventions for her and her family. The proactive physician evaluating these potential difficulties and using the strength a good relationship with the patient in making appropriate referrals for counseling or support group does much to help the woman embrace as full a recovery as possible. Recommendations presented b

Adolescent substance use disorders and comorbidity.

It is imperative to know what risk factors are more likely to appear during specific developmental stages so that identification and interventions can be used to decrease the risk for future SUD. Continued surveying of risk factors that can occur at any stage in childhood are important to ensure that other risk factors are anticipated and intervened upon as well. Multiple risk factors increase the magnitude of the risk for SUD, and therefore all risk factors should be detected to convert these to protective factors. Screening instruments that can assess risk factors found to increase the risk

Do ask, do tell.

As a consequence of bias and ignorance within the medical profession, lesbians and gay men frequently receive suboptimal health care. Knowledge of each patient's sexual orientation and behaviors is critical for the development of a productive therapeutic relationship, accurate risk assessment, and the provision of pertinent preventive counseling. However, clinicians often forget to ask about this information, and many lesbians and gay men are reticent to reveal the truth. I present vignettes from my personal experiences as a lesbian patient and doctor to illustrate the importance of creating a
