From chemsex to a broader drug-use for sex landscape: Comparing two cross-sectional surveys on men who have sex with men in Italy.

BackgroundDrug-use in sexual settings among men who have sex with men (MSM) is increasing across Europe and is a key driver of sexually transmitted infections (STIs). Chemsex is evolving with increasing use of new psychoactive substances and diversification of context, shifting from private homes to diverse settings. Our aim was to assess changes in drug-use for sex among MSM who attended our STI/HIV reference center in two different periods over the last 8 years.MethodsWe compared two cross-sectional surveys conducted at the STI/HIV center of the San Gallicano Dermatological Institute (Rome,

Tri-use of alcohol, cannabis, and nicotine in daily life among young adults: A day-level comparison of substance use outcomes to dual-use and alcohol-only use occasions.

Alcohol, cannabis, and nicotine use are common among young adults with many "co-using" substances on the same day. Most co-use research has focused on alcohol and cannabis co-use, but many alcohol + cannabis use days involve nicotine. Nicotine is rarely considered, and such "tri-use" (same-day alcohol + cannabis + nicotine use) may increase one's risk for heavy use and harms. The current daily diary study compared substance use outcomes across tri-use, dual use, and alcohol-only use days. Participants were 99 young adult college students (Mage = 20.28, SD = 1.29) who reported weekly alcohol +

Alcohol Consumption Patterns Before and After Retirement: A Longitudinal Cohort Study Within the Survey of Health, Ageing and Retirement in Europe (SHARE).

INTRODUCTION: Retirement is a major life-course transition that can reshape health behaviors, including alcohol consumption. However, longitudinal evidence from large cross-national cohorts remains scarce. METHODS: In this longitudinal cohort study, Survey of Health, Ageing and Retirement in Europe Waves 1-8 (2004-2020) were merged, linking individual-level data from 8,998 adults aged ≥50 years who were employed at baseline and retired during follow-up. Outcomes were near-daily drinking, consumption of ≥8 alcohol units in the previous 7 days, and binge drinking (≥4-6 units on one occasion at

Two-Year Outcomes Following Delandistrogene Moxeparvovec Treatment in Ambulatory Patients with Duchenne Muscular Dystrophy: Phase 3 EMBARK Trial.

INTRODUCTION: Delandistrogene moxeparvovec is a recombinant adeno-associated virus rhesus isolate serotype 74 vector-based gene therapy that addresses the absence of functional dystrophin in Duchenne muscular dystrophy (DMD). EMBARK is a phase 3, two-part, crossover, randomized, placebo-controlled trial assessing the safety and efficacy of delandistrogene moxeparvovec (single intravenous dose 1.33 × 1014 vector genomes/kg) in ambulatory male patients with DMD aged 4 to < 8 years; N = 125. One-year results demonstrated the manageable safety of delandistrogene moxeparvovec, consistent with previ

Effectiveness of Structured Exercise Programs in Patients With Conduction System Pacing and Cardiac Resynchronization Therapy: A Systematic Review.

Following the implantation of cardiac physiological pacemakers and similar electronic cardiac devices, individuals frequently face a decline in physical activity levels, marked exercise intolerance, compromised functional capacities, and a diminished overall quality of life. These constraints are often aggravated by an underlying fear of physical exertion, pre-existing myocardial dysfunction, bodily deconditioning, and various psychosocial barriers. Consequently, structured exercise rehabilitation has gained substantial attention as a vital, non-pharmacological approach to enhancing both the p

Assessing the clinical competence of large language models for tobacco use disorder: A multi-domain expert evaluation.

BACKGROUND: Tobacco use disorder (TUD) remains the leading preventable cause of death globally, yet fewer than one-third of users receive guideline-concordant care due to workforce shortages and training gaps. Emerging artificial intelligence (AI) systems, particularly large language models (LLMs), may help expand access to evidence-based cessation support, but their clinical competence remains insufficiently characterized. OBJECTIVE: To systematically evaluate the clinical accuracy, safety, guideline adherence, and communication quality of five leading LLMs across standardized tobacco-cessat

Efficacy, Acceptability, and Feasibility of StayQuit for Sustaining Smoking Abstinence After Psychiatric Hospitalization: A Pilot Study.

PURPOSE: Veterans with mental illness have high rates of tobacco use and psychiatric hospitalization in the Veterans Health Administration (VA). VA is smoke-free, and guidelines recommend that smoking cessation medication and counseling be delivered during and one month after hospitalization to sustain abstinence. Little is known about how patients experience this intervention. This pilot study evaluated the acceptability, feasibility, and efficacy of an evidence-informed, novel smoking intervention, StayQuit, for psychiatric inpatients. METHODS: Participants were recruited from an inpatient

Electronic Cigarettes: A Comprehensive Review for Primary Care Providers.

Elecronic cigarettes (e-cigarettes), initially marketed as a safer smoking alternative, have gained widespread use, especially among adolescents and young adults who did not previously smoke. While our understanding of long-term risks is still emerging, vaping carries significant concerns for adolescents, including nicotine dependence, increased risk of substance use disorders and mood disorders, and potential cardiovascular and pulmonary harm. Primary care clinicians should routinely screen for e-cigarette use in adolescents and young adults and counsel patients on associated risks, emphasizi

Nicotine pouches: Harm reduction or initiation pathway? A pharmacy-focused review.

BACKGROUND: Nicotine pouches (NPs) are a rapidly growing category of tobacco-free oral nicotine products increasingly available in the community, including pharmacies. Whether NPs primarily function as harm-reduction tools for existing tobacco users or as a novel initiation pathway into nicotine and tobacco use, particularly among youth, remains a critical and unresolved clinical and public health question. OBJECTIVES: To synthesize current evidence on the pharmacology, harm-reduction potential, and initiation risk of NPs; to compare NPs with FDA-approved nicotine replacement therapy (NRT) an

Estimating the impact of out-of-home placement on health risk behavior in adolescents exposed to maltreatment: An advanced causal inference approach.

BACKGROUND: Youth who experience out-of-home placement (OOHP) engage in elevated health risk behaviors (e.g., substance use, unprotected sex), with risk potentially heightened for those placed during adolescence. Estimating causal effects is challenging because maltreated youth who are placed differ systematically from those who remain in-home. OBJECTIVE: This study examined the effects of adolescent OOHP on health risk behaviors, applying causal inference methods (g-estimation and inverse probability of treatment weighting; IPTW) to address selection bias and time-varying confounders. Psychometric properties of the malay-translated eight-item electronic cigarette dependence scale (EDS-8) among electronic cigarette users in Malaysia.

INTRODUCTION: The popularity of e-cigarettes is increasing globally. Validated instruments in local languages are needed to measure e-cigarette dependence for surveillance, diagnosis, and treatment of e-cigarette dependence. We assessed the psychometric properties of the Malay language version of the 8-item e-cigarette Dependence Scale, EDS-8(Malay), among exclusive e-cigarette users. METHODS: The original English version of the eight-item e-cigarette Dependence Scale (EDS-8) was translated into Malay to suit the local context by standard forward and backward translation methods. Nontobacco Nicotine Dependence as a Risk Factor for Poor Long-Term Outcomes After Shoulder Arthroplasty and Rotator Cuff Repair.

INTRODUCTION: Rotator cuff repair (RCR), total shoulder arthroplasty (TSA), and reverse TSA (rTSA) are common treatments for shoulder osteoarthritis and rotator cuff tears. Although tobacco use is known to worsen surgical outcomes, the effect of nontobacco nicotine dependence (ND) remains unclear. The goal of this study was to evaluate the effects of nontobacco ND on postoperative complications in TSA, rTSA, and RCR. METHODS: A retrospective cohort study using the TriNetX Research Network identified patients undergoing RCR, rTSA, or TSA. Patients with nontobacco ND were compared with those wi

Impact of gastrectomy on the risk of alcohol-associated liver disease in patients with gastric cancer.

Gastrectomy may induce physiological changes that increase susceptibility to alcohol-associated liver disease, even with low alcohol intake. This cohort study investigated the association between gastrectomy, alcohol consumption, and liver-related disease incidence using the Korean National Health Insurance Service database. We analyzed 57,893 patients diagnosed with gastric cancer between 2002 and 2015 who underwent either endoscopic resection (n = 22,078) or gastrectomy (n = 35,815). Over a median follow-up of 6.9 years, the gastrectomy group exhibited a significantly higher, dose-dependent

Impact of lab-based drug checking services on syringe service program engagement in Maryland, USA.

PURPOSE: The introduction of synthetic substances into the unregulated drug supply has driven a surge in overdose deaths, posing significant public health challenges in timely substance identification, engagement with people who use drugs (PWUD), and access to treatment. Maryland's rapid analysis of drugs (RAD) program, piloted through syringe service programs (SSPs), provides a proactive harm reduction strategy by integrating mail-in, laboratory based drug checking with broader public health services to enhance surveillance and improve engagement with PWUD. We aim to investigate the associati

Road safety threats and occupational health risk associated with psychoactive substance use and dependence risk among commercial drivers: WHO ASSIST evidence from Ghana.

BACKGROUND: Commercial drivers increasingly use psychoactive substances, raising the risk of road crashes and occupational injuries. Globally, 1.3 million people die in road crashes each year (90% in low- and middle-income countries), and Africa alone accounted for 250,000 of these deaths in 2021. Ghana as of 2026 had experienced a 18.2% increase in road traffic accidents with the National Road Safety Authority reporting 13,489 road crashes and 2,494 fatalities in 2024. In the first half of 2025 alone, the country had already recorded 7,289 crashes resulting in 1,504 deaths. In Ghana, studies

Determining the impact of cannabis use on tobacco cessation: Results from a prospective non-randomized tobacco treatment trial.

BACKGROUND AND AIMS: Tobacco-cannabis co-use is associated with increased psychosocial and health harms; however, the treatment literature assessing the impact of co-use has been mixed and suffers from critical limitations. To date, no prospective studies have evaluated the degree to which cannabis co-use affects tobacco cessation, nor are there treatment recommendations for those who co-use. The goal of this study was to evaluate and quantify the impact of cannabis co-use on tobacco cessation through a 12-week prospective tobacco cessation trial. DESIGN: This was a multi-site, non-randomized
