Prenatal substance exposure and early childhood adversity: implications for child autonomic functioning at early school-age.

INTRODUCTION: We examined associations between prenatal tobacco exposure (controlling for other substance exposures) and autonomic functioning at early school-age via infancy autonomic regulation, harsh parenting, and early childhood adversity. METHODS: 216 mothers (74% Black/African-American) and infants (51% girls) recruited at delivery were assessed repeatedly. Prenatal substance exposure was measured using multiple biomarkers and calendar-based interviews. Harsh parenting and early childhood adversity were assessed from birth to early school-age. Child autonomic nervous system functioning

Nicotine abuse and 30-day perioperative outcomes after fibula free flap reconstruction: An ACS-NSQIP study.

BACKGROUND: Fibula free flap (FFF) reconstruction is a cornerstone of maxillofacial and mandibular reconstruction, particularly for oncologic and traumatic defects. Although smoking is a well-established risk factor for impaired wound healing and microvascular complications, its specific impact on FFF outcomes remains debated. In this multicenter retrospective study, we evaluate the association between smoking and 30-day perioperative outcomes in FFF reconstruction. METHODS: A retrospective cohort study was conducted using data from the American College of Surgeons National Surgical Quality I

The burden of tobacco-free nicotine dependence in burn survivors: Acute complications and long-term morbidity.

BACKGROUND: Nicotine use from tobacco-free sources (e.g., electronic cigarettes, oral pouches, and gum) has become prevalent. While tobacco is an established detriment for wound healing, the relationship between tobacco-free nicotine dependence (TFND) and post-burn morbidities has not been defined. This study sought to assess the impact of TFND on short- and long-term complications in burn survivors. METHODS: A retrospective secondary database cohort study was conducted using electronic health records from multiple health institutions. Patients (18 + years) with burn injury and TFND were iden

The Impact of Nontobacco Nicotine Dependence on Postoperative Complications in Reconstructive and Aesthetic Breast Surgery Procedures: A Multicenter Propensity-Score Matched Analysis.

BACKGROUND: Nontobacco nicotine use, including electronic cigarettes and vaping devices, has increased substantially in recent years. While the adverse effects of combustible tobacco on wound healing are well established, less is known about postoperative risks associated with nontobacco nicotine dependence (NTND). This study evaluated the association between NTND and postoperative complications following reconstructive and aesthetic breast surgery procedures. METHODS: A retrospective cohort analysis was performed using the TriNetX research database. Patients undergoing reconstructive or aest

Development and Validation of The Skin Graft Loss Risk Estimator: An Interpretable Machine Learning-Driven Tool for Individualized Assessment in Burn Patients.

BACKGROUND: Skin graft failure is a significant complication in burn care that prolongs recovery, compromises surgical outcomes, and necessitates repeat operations. Accurate risk assessment is challenging due to a complex interplay of patient and injury factors, underscoring the need for objective, data-driven predictive tools. We developed and validated an interpretable machine learning (ML)-driven tool to provide individualized, data-driven risk assessment for skin graft loss in burn patients. METHODS: A cohort study of patients who had autologous skin grafting from the American Burn Associ

Non-tobacco nicotine dependence and incidence of pseudarthrosis and other postoperative complications in lumbar fusion surgery: a retrospective analysis.

INTRODUCTION: Lumbar fusion is a common procedure to help treat trauma, deformities, tumors, infections, and degenerative diseases of the spine by improving mechanical stability. The rate of non-tobacco nicotine, also known as e-cigarette, use among US adults has recently been increasing. This study seeks to take a collective look at fusion outcomes to address this problem by quantitatively evaluating the effects of non-tobacco nicotine dependence (NTND) on postoperative outcomes following lumbar fusion surgery. METHODS: A retrospective cohort study using the TriNetX database identified patie

Exploring the nature and relationship of tobacco cigarette craving and withdrawal.

The status of craving as a tobacco withdrawal symptom is unclear. Some withdrawal scales include it, whereas others do not. This issue is theoretically and clinically important and affects the predictive validity and interpretation of withdrawal measures. This study investigated abstinence symptoms among individuals (N = 232) undergoing unmedicated withdrawal from tobacco cigarettes during the first week following a target quit day (TQD). Participants were motivated to quit and were given counseling before and after their TQD. Analyses focused on the dynamic patterns of craving and other withd

Determining adverse childhood experiences and their association with substance use among young adults in universities and colleges in Mombasa County, Kenya.

BACKGROUND: Substance use and adverse childhood experiences (ACEs) remain major risk factors for health, education, and social relationships, with the effects of ACEs persisting into adulthood. This study aimed to investigate the association between substance use (alcohol, tobacco and tobacco products, and illicit drugs) and ACEs among young adults in higher learning institutions in Mombasa County, Kenya. METHODS: The analytical cross-sectional study collected data on having ever experienced any form of ACEs and substance use (alcohol, tobacco and tobacco products, and illicit drugs). A total

Impact of non-tobacco nicotine dependence on outcomes following carpal tunnel release: A retrospective cohort study.

INTRODUCTION: Non-tobacco nicotine dependence (NTND) from products like e-cigarettes and pouches is rapidly rising. Studies have shown that tobacco can negatively impact outcomes following carpal tunnel release (CTR), but literature focusing specifically on NTND is sparse. As such, the purpose of this study was to compare outcomes following CTR between patients with versus without a history of NTND. METHODS: The TriNetX database was queried to identify patients over 18 years old who underwent primary CTR. Patients were stratified based on their history of NTND. Cohorts underwent propensity sc

B7-H3 expression impacts outcomes in myeloma and is targetable with an NK cell engager.

B7-H3 acts as a checkpoint inhibitor, influences the tumor microenvironment, and is necessary for osteoclast (OC) development. Given its dual roles in promoting cancer survival and bone breakdown, we looked at its expression pattern in >200 multiple myeloma (MM) patients and association with cytogenetic abnormalities and outcomes. We found that most newly diagnosed patients had B7-H3+ plasma cells or stroma regardless of cytogenetic risk and expression on plasma cells increased with subsequent relapses. In marrow aspirates and core biopsies we identified B7-H3+ myeloid derived suppressor cells

Research on Eating and Adolescent Lifestyle (REAL) 2.0: 15-year follow-up study of eating disorders and weight-related trajectories, mental health and substance use health from early adolescence to early adulthood-a Canadian cohort profile.

PURPOSE: Few studies have examined how psychosocial risk and protective factors in adolescence shape mental health outcomes and other multimorbid conditions in adulthood, particularly among Canadian youth. The Research on Eating and Adolescent Lifestyle (REAL) 2.0 study was a 15-year follow-up cohort study designed to investigate how early etiological factors, including body image and disordered eating symptoms in adolescence, contribute to the development of eating, weight-related concerns, mental health and substance use health problems in early adulthood. In this paper, we describe the REAL

"Cigarettes led me back to smoking tik": lived experience perspectives on tobacco use during substance use treatment and recovery in South Africa.

INTRODUCTION: Despite the high prevalence of tobacco use among people with substance use disorder (SUD), tobacco cessation is not a routine part of SUD treatment in South Africa. No studies have explored how people receiving SUD treatment view the effects of tobacco use on SUD treatment or their barriers to tobacco cessation. Clinicians need this information to develop locally relevant, person-centred tobacco cessation interventions. To address this gap, this study aimed to explore perceptions of tobacco use and its impact on SUD recovery among people receiving SUD treatment in South Africa.

Association of under-reporting of alcohol use with HIV viral non-suppression and an all-cause mortality risk score among people with HIV on antiretroviral therapy.

BACKGROUND: Unhealthy alcohol use contributes to poor HIV-related outcomes. Alcohol under-reporting may be associated with HIV viral non-suppression and all-cause mortality risk, especially at the high end of the spectrum. We evaluated whether alcohol under-reporting was associated with these outcomes among PWH with unhealthy alcohol use who were receiving ART. METHODS: We pooled data from six studies of PWH in Uganda (ADEPT-T, DIPT, Extend), United States (MWCCS), and Russia (Russia ARCH 1, St Peter). We included PWH on ART who engaged in unhealthy drinking as determined by the alcohol bioma

Psychometric properties of MERIT: A concise tool for community health workers to screen priority mental disorders.

BACKGROUND: Task shifting is an accepted way to address the treatment gap for mental health disorders. Community Health Workers (CHWs) can play vital roles through screening, referral, and providing basic counseling. To address the dearth of a concise screening instrument for CHWs, the "Mental HEalth ScReenIng Tool for Community Health Workers in India [MERIT]" was designed and developed. In this paper, we have examined the psychometric properties of MERIT. METHODS: MERIT consists of nine stems (consisting of 11 questions) covering the following five domains: (a) Substance Abuse (alcohol, tob

Implementing Proactive Smoking Cessation Induction Interventions in a Certified Community Behavioral Health Clinic: A Pilot Type 2 Hybrid Effectiveness-Implementation Study.

BACKGROUND: People with mental health and substance use disorders have a high prevalence of tobacco use. Community mental health clinics, including certified behavioral health clinics, are a key entry point where people with mental health and substance use disorders receive care. Implementation strategies to deliver tobacco use disorder treatment in community health clinics are needed. Two promising strategies supported by clinical practice guidelines are proactive outreach (reaching out to people in a health system who smoke and providing treatment) and the 5As (mental health providers ask ab

Repeat Behavioral Counseling, With and Without Combination Nicotine Replacement Therapy, for Smoking Cessation Among People With HIV in South Africa.

Repeating an intervention strategy consistent with the chronic disease care model has been demonstrated to be efficacious in improving cessation outcomes among smokers. However, evidence is lacking for repeat treatment outcomes among people with HIV (PWH) who smoke in low-resource settings. We evaluated outcomes among participants (n = 384) who were provided a second round of treatment after initial treatment failure in a randomized controlled trial of behavioral counseling (BC) with or without combination nicotine replacement therapy (nicotine patches and gum, cNRT). The primary outcome was s
