Alcohol screening, counseling, and medication treatment among older adults with heart failure: A missed opportunity.

OBJECTIVE: Both alcohol use and heart failure (HF) are prevalent among older adults. Alcohol can affect heart muscle, electrical function, blood pressure, and interact with common HF medications. Alcohol screening, brief counseling, and medications for alcohol use disorder (MAUD) are evidence-based and reimbursable by Medicare, but it is unknown whether older adults with HF receive these interventions, the details of these interventions, and if clinicians are reimbursed for them. This study identified instances of claims-documented screening, brief counseling, and MAUD receipt, as well as the

Altered GABAB receptor sensitivity in heavy drinkers indexed using a TMS-based biomarker: a randomized controlled experimental medicine trial.

Metabotropic γ-aminobutyric acid type B receptors (GABABRs) are implicated in alcohol use disorder (AUD) and represent a promising therapeutic target. However, robust biomarkers of GABABRs target engagement and sensitivity in humans are lacking. We examined transcranial magnetic stimulation (TMS)-derived cortical inhibition and growth hormone (GH) release as measures of GABABR function, and whether heavy alcohol use is associated with altered responses to pharmacological GABABR activation. In a randomized, double-blind, placebo-controlled crossover study, 31 healthy volunteers (16 heavy drinke

Alcohol use is common in older patients with fall-related fractures: a secondary analysis from a prospective cohort study using PEth as an objective assessment of consumption.

BACKGROUND AND PURPOSE: Alcohol increases the risk of falls and complications, but data on alcohol intake among trauma patients is unreliable. Excessive alcohol use can contribute to excess frailty and confusion in frail individuals and increase fall risk in otherwise robust older people. Phosphatidylethanol (PEth) is a biomarker accumulating through repeated alcohol intake, i.e., suitable for screening long-term consumption. Our primary aim was to describe alcohol consumption in older individuals admitted after a fall-related fracture. Our secondary, exploratory aims were to investigate the a

Substitution, addition or stepping-stone? Exploring the temporal associations between the consumption of alcoholic and alcohol-free beverages among Dutch university students.

BACKGROUND AND AIM: Stimulating the use of alcohol-free beverages is portrayed as one strategy to reduce excessive alcohol use and alcohol-related harm among students in higher education. This strategy relies on the assumption that students drink alcohol-free to substitute alcohol (i.e. 'substitution effect'), while other possibilities like drinking alcohol-free in addition to alcohol (i.e. 'addition effect') or alcohol-free acting as a step-up to consume (more) alcohol (i.e. 'stepping-stone' effect) have largely been neglected. Consequently, this pre-registered study aimed to examine bi-direc

Psychiatric and substance use disorder polygenic scores are associated with the exposome in youth.

Gene-environment correlations (rGE) present challenges for accurately estimating the individual and joint contributions of genetic and environmental influences on risk for psychiatric and substance use disorders. The present study examined rGE between polygenic risk scores (PGS) for psychiatric and substance use disorders and environmental exposures (i.e., exposome factors) using multi-level data spanning youth- and parent-reports, as well as objective measures of the built environment. Participants included 4,797 genetically similar to European reference populations, 1,585 genetically similar

Predictors of Newly Prescribed Medications for Alcohol Use Disorder Among Hospitalized Adults: A Retrospective Cohort Study.

BACKGROUND: Alcohol use disorder (AUD) is pervasive among hospitalized adults and associated with poor post-hospitalization outcomes. Medications for alcohol use disorder (MAUD) improve clinical outcomes, but prescribing rates at hospital discharge remain low. Factors associated with inpatient MAUD initiation are incompletely understood. OBJECTIVE: Identification of patient-level and hospitalization-level factors associated with a new MAUD prescription at hospital discharge among adults with AUD. DESIGN: Retrospective cohort study. PATIENTS: Adults hospitalized with an AUD-related diagnosis

Oral Health Behavior-Cognition-Awareness education is associated with improved short-term knowledge and long-term oral hygiene in head and neck cancer patients undergoing radiotherapy: a two-phase study.

BACKGROUND: The incidence of head and neck cancer (HNC) has increased steadily, and radiotherapy remains a primary treatment modality. However, radiotherapy may induce severe oral complications, including radiation-related caries and osteoradionecrosis of the jaw (ORN). Consequently, comprehensive pre-treatment oral care and patient education are essential. This two-phase study conducted in Guangzhou, China, evaluated the effects of a structured oral health education program on short-term knowledge, behaviors, and long-term oral hygiene outcomes in HNC patients undergoing radiotherapy. Virtual Reality Therapy Assistants for Adjunctive Cue Exposure Training in the Treatment of Alcohol Use Disorder (VITAACue-AUD): Protocol for a Technical Feasibility Study.

BACKGROUND: Alcohol use disorder (AUD) is associated with a substantial individual and societal burden. Cue exposure approaches can support treatment by targeting conditioned cue reactivity and high-risk contextual associations. However, routine implementation is limited by therapist time, scalability constraints, and the difficulty of reproducing ecologically valid drinking situations in standard care settings. Virtual reality (VR) and AI have been proposed as promising tools to support scalable and contextually rich interventions in mental health care. OBJECTIVE: This study aims to develop

A comparison of recovery benefits associated with participation in SMART Recovery, AA, both SMART and AA, or neither, following a new alcohol use disorder recovery attempt.

BACKGROUND: Self-Management and Recovery Training (SMART Recovery or "SMART") is an internationally known "second-wave" addiction recovery mutual-help organization that has grown over several decades. Despite this, little is known empirically about who uses SMART and how they fare on important alcohol-specific outcomes over time relative to individuals using more established options such as Alcoholics Anonymous (AA), or indeed those who utilize both SMART and AA, or neither. Greater knowledge could deepen understanding of the clinical and public utility of SMART in its own right and inform cli

Exposure to Opportunities for Use Moderates the Associations of Descriptive Norms and Perceived Risk with Alcohol and Cannabis Use and Simultaneous Use.

We investigated whether exposures to opportunities for alcohol and cannabis use and simultaneous use moderate associations of descriptive norms and perceived risk with these behaviors such that substance use is lower for adolescents and young adults who report fewer opportunities regardless of their predispositions to engage in them. Method: Data were obtained using online surveys administered in 2023-24 to 1,014 adolescents and young adults (ages 15- to 20 years) in 40 California, USA, cities. They were asked how often they had an opportunity to engage in alcohol, cannabis, and simultaneous u

Safety and efficacy of psilocybin-assisted therapy for alcohol use disorder: Open-label extension of a phase II randomized controlled trial.

BACKGROUND: Psilocybin-assisted therapy (PAT) has shown promise for alcohol use disorder (AUD) in randomized controlled trials (RCTs). It remains unknown, however, whether patients derive additional benefit when psilocybin is administered open-label after completion of blinded treatment. Here, we report safety and efficacy outcomes from an open-label extension of a Phase II RCT of PAT for AUD (NCT02061293). METHODS: Fifty-nine adults with AUD received a single open-label administration of psilocybin (25-40mg/70kg) accompanied by four hours of manualized therapy. During the preceding double-bl

Impact of Donor Substance Use on Outcomes Following Pancreas Transplantation.

BACKGROUND: Donor substance use is considered by many a risk factor for poor outcomes following pancreas transplantation, yet little is known regarding the impact of donor substance use on actual outcomes. This study aimed to determine the impact of substance use, particularly cocaine, on early and late outcomes following pancreas transplantation. METHODS: This was a single center, retrospective analysis of all pancreas transplants from 2003-2023. Clinical outcomes including 30-day peak serum lipase, 7-day, 90-day, and 1-year pancreas allograft survival were assessed by donor exposure to subs

Same phenotype, different mechanisms: What five selectively bred rat lines reveal about the heterogeneity of human alcohol use disorder.

For seventy-five years, rats have been selectively bred for high voluntary alcohol consumption, yielding five major lines -UChB/UChA, P/NP, HAD/LAD, AA/ANA, and sP/sNP-msP- that share a behavioral phenotype but arose independently on three continents. This review argues that these lines are best understood as phenocopies: they reach high consumption via largely distinct biological routes rather than a single shared mechanism. Line by line, we synthesize the biochemical, genetic, and brain-circuitry explanations for the high-drinking phenotype, and compare them directly. The genetic architectur

Assessment of serum micronutrient concentration in chronic pancreatitis: insights from the nationwide COMBO randomised trial.

INTRODUCTION: In patients with chronic pancreatitis (CP), pancreatic exocrine insufficiency (PEI) and poor dietary intake may cause malnutrition. International guidelines recommend monitoring key micronutrients (MN), however clinical implications remain uncertain. This study aims to evaluate the prevalence, risk factors, complications, and longitudinal course of low serum MNs in patients with CP. METHODS: This is a predefined analysis of the COMBO trial, a nationwide stepped-wedge cluster randomised trial on evidence-based CP management. Nutritional biochemical assessment included vitamins A,

Behavior Change Interventions to Promote Healthy Lifestyles Among Adolescent and Young Adult Childhood Cancer Survivors: A Scoping Review.

BACKGROUND: Adolescent and young adult childhood cancer survivors (AYA-CCS) face elevated risks of chronic health conditions and premature mortality further exacerbated by poor lifestyle behaviors including physical inactivity, poor nutrition, and substance use. Behavioral interventions are essential for supporting long-term health in this population; however, the evidence remains fragmented and unbalanced across lifestyle domains. This scoping review aimed to identify and describe behavioral interventions promoting healthy lifestyles among AYA-CCS, focusing on four domains (physical activity,

The Event-Level Association Between Alcohol Use and Intimate Partner Violence Perpetration: A Systematic Narrative Review of the Literature.

Intimate partner violence (IPV) perpetration is a significant public health concern, with up to 90% of individuals in intimate relationships experiencing some form of physical, psychological, or sexual IPV from their partner throughout their lifetime. Over 30 years of research have concluded that alcohol use results in greater risk for IPV perpetration. Despite this known association, only recently has alcohol use been examined as a proximal predictor of IPV perpetration in daily diary-type studies. The present study aimed to provide a systematic narrative review of current research on the eve
