[Rational assessment procedure in hypertension].

Diagnosis of hypertension is based on a classification of blood-pressure values, determined by multiple blood-pressure measurements over a period of several weeks or months. However, possibility of a so-called 'white coat effect' has to be considered, i.e. a marked increase of blood-pressure values in the presence of medical persons. Multiple control-measurements by the patient himself or an ambulatory day profile may clarify the situation in suspicious cases. In the case of pure 'white coat' hypertension, no further investigation is necessary, since cardiovascular complications only correlate

Diabetes mellitus in Bahrain: an overview.

Bahrain has faced a rapid transition in its socio-economic status, which resulted in great changes both in life-style and in patterns of health and disease. There has been a marked decline in infectious diseases and an increase in chronic diseases. Diabetes mellitus has become one of the most common public health problems in the country. In 1988 it was estimated that 3.4% of total deaths in Bahrain were due to diabetes. A community-based study among mothers aged 18 to 48 years showed that 8.5% of them had diabetes. The percentage was found to be higher among elderly Bahrainis (12.9%). Obesity,

Nutrition and high altitude exposure.

Altitude exposure leads to considerable weight loss. The different hypotheses that have been put forward to explain this phenomenon are discussed reviewing the literature: 1) a primary decrease of food intake due to loss of appetite caused, directly or indirectly, by hypoxia, changes of menus, comfort and habits, 2) a discrepancy between energy intake and energy expenditure due to an increased basal metabolic rate and/or high levels of activity which are not matched by an increased food intake, 3) a loss of body water due to increased insensible loss through increased ventilation in the mounta

[Cardiovascular risk factors in the list of patients of a general practitioner].

OBJECTIVE: Study the prevalence of cardiovascular risk factors in the list of patients of a general practitioner (GP). DESIGN: Study cardiovascular risk factors of all patients of a family doctor. SETTING: Health Center. PATIENTS: All the patients of a GP appointment (n = 1143). INTERVENTIONS: To study the clinic cards of all the patients of a GP appointment (n = 1143), in connection with the existence of cardiovascular risk factors: dyslipidemia, hypertension, smoking, obesity, diabetes and lack of physical exercise. To study these factors by age groups, sex and its association. RESULTS:

[The cardiovascular risk profile in the Glostrup region, Monica screening II, 1986/87].

Information about risk factors and cardiovascular disease was collected from 30, 40, 50, and 60 year old representatives of the suburban Danish population (n = 1,504). Reference values for systolic and diastolic blood pressure and total serum-cholesterol are given. A comprehensive risk profile is discussed on the basis of selected risk factors: arterial hypertension, total serum-cholesterol greater than 7.5 mmol/l, smoking of ten cigarettes or more per day, intake of five alcoholic drinks or more per day, lack of physical exercise, and body mass index greater than 25 kg/m2.

[Methodologic approach to the therapeutic problem of infant and childhood obesity. Review of the literature and personal experience].

Notwithstanding the high prevalence and morbidity, current knowledge of obesity is rather rudimentary. Nevertheless recent findings have enabled us to surmount anachronistic nosographic generalizations whereby excess weight was attributed only to inappropriate and excessive food intake and the lack of physical exercise. Various studies indicate that there are complex interactions in determining weight excess: genetic, psychosocial, cultural but also biochemical factors are incriminated and it is now recognised that the aetiology of obesity is multifactorial and that, consequently, there are di

[Principles and general practice of goal-oriented planning of training in patients with diabetic nephropathy and in dialysis patients].

Uremic patients on hemodialysis suffer from marked reduction of physical exercise capacity. This reduction cannot solely be explained by the underlying disease and co-existing anemia. We wondered, whether reduced exercise capacity in these patients might be due to lack of physical exercise and whether anaerobic exercise training (AET) would lead to improved work capacity. 8 patients were enrolled in this study. At the beginning and at the end of the training period, which lasted 4 to 6 weeks, a symptom-limited incremental bicycle-spiroergometry was performed. AET led to a significant increase

Coronary artery disease risk factor management in the hypertensive patient.

Recent advances in understanding the relation of risk factors to coronary artery disease (CAD) have initiated a change in the approach to managing the hypertensive patient. Reduction of elevated blood pressure still remains a major therapeutic priority. However, the risk of cardiovascular morbidity is also related to hypercholesterolemia, hyperuricemia, hyperglycemia, hyperfibrinogenemia and obesity; all aggravate the risk of CAD in the patient with high blood pressure. Life-style is also important: cigarette smoking, high alcohol consumption and lack of physical exercise all predispose to pre

Obesity and hypertension: epidemiology, mechanisms, treatment.

There is a close epidemiological association between obesity and elevated blood pressure for all age groups, although not every obese individual becomes hypertensive. In populations without age-related increases in body weight, an elevation of blood pressure with age is not seen. Mechanisms included in the development of hypertension in obesity are hyperinsulinemia, insulin induced sodium retention and increased sympathetic tone. Overnutrition with over intake of sodium and lack of physical exercise contribute to the metabolic syndrome of obesity. Thus, weight reduction by decreased energy upt

[The social infrastructure of patients with urinary calculi (author's transl)].

The social infrastructure was investigated in 379 patients with urinary stones. The rates of 50% for employees and 32% for old-age pensioners were surprisingly high. 16% of the patients were employed in jobs with hard manual labour. Only 9% of the patients went in for regular physical activity in their leisure. A significant increase in urinary excretion of calcium and uric acid (p - 0.01) and inorganic phosphate (p less than 0.001) was seen in connexion with increase in net income. On the other hand, urinary pH was lowest in the highest income group (p = 0.01). According to these findings an

Plasma high density lipoproteins in coronary, cerebral and peripheral vascular disease. The influence of various risk factors.

High density lipoprotein (HDL) cholesterol and the HDL/total cholesterol ratio have been measured in 440 patients with coronary, cerebral or peripheral vascular disease and in 440 matched controls. The patients were subdivided into sex- and age-groups and according to physical activity, smoking, hypertension and non-insulin-dependent and insulin-dependent diabetes mellitus. The average HDL cholesterol level was significantly decreased in all the three groups of localization of ischaemic vascular disease (IVD). Plasma HDL concentration in men was lower than in women in every age-group. Lowest v

Factors affecting the concentration of triacylglycerols (triglycerides in plasma: reference values for adults.

We studied factors influencing concentrations of triglycerides in the blood of a presumably healthy population of about 9000 persons. Using multi-dimensional analysis, we show, in addition to various related to sex and age, the relation between triglycerides and uric acid, overweight, arterial blood pressure, use of oral contraceptives (in women), consumption of alcohol and tobacco, and lack of physical exercise. These factors were classified as a function of their discriminative power by the statistical method of segmentation. We thus chose the following factors as criteria for exclusion from

Fibrinolysis and risk factors of atherosclerotic disease, with special emphasis on diabetes mellitus.

This paper reviews and discusses the evidence supporting the involvement of defective fibrinolysis in the pathogenesis of atherosclerosis, with emphasis on diabetes mellitus. According to the literature, defective fibrinolysis has been observed in association with virtually every major "risk factor" for coronary heart disease, including diabetes mellitus, hypercholesterolemia, hypertriglyceridemia, hypertension, obesity, cigarette smoking and lack of physical exercise. The interrelationships between disturbances in carbohydrate and fat metabolism and fibrinolysis are considered. Attention is d

[Etiology and management of habitual constipation (author's transl)].

Habitual constipation is present, if defecation occurs because of prolonged retention of stool in the colon less than three times per week. Motility of the intestine is increased in these cases, not decreased as has been thought for long time. Several factors of modern civilization favor habitual constipation, e.g. nutrition devoided of poorly digestible ingredients, and lack of physical exercise. The diagnosis of habitual constipation should only be made after careful exclusion of all organic causes by means of x-ray and endoscopic examination. Therapy is conservative in most cases. The diet
