1840 homosexual and bisexual men were tested for human immunodeficiency virus type I (HIV) infection at the Landesinstitut für Tropenmedizin, Berlin between April 1983 and December 1987. A total of 25.1% were seropositive for HIV, but the percentage was seemingly declining. During the study period we observed also a steady decrease in the prevalence of lymphadenopathy syndrome (LAS), antibodies to hepatitis B and syphilis, and high-risk sexual behaviour in the study population. By adjusting for these changes of population characteristics we assessed the odds of HIV infection which had more tha

Information was collected from 148 homosexual men to examine their current sexual practices, knowledge of acquired immunodeficiency syndrome (AIDS), perception of risk for this disease and change in sexual behaviour. The data were analyzed to investigate the relationship of these variables to the degree of worry and concern about AIDS expressed by most respondents. Overall, a decline in high-risk sexual behaviour was reported. Measures of worry and concern were significantly related to the subjects' perception of risk for AIDS and reduction in risk behaviours. However, the measures of Health L

The sexual behaviour of 142 clients of syringe-exchange schemes was measured using a questionnaire interview based survey method. Two to four months later the questionnaire was repeated to provide measures of behavioural change. The majority of these clients were sexually active, 77% having one or more sexual partners in the 3 months prior to the first interview. Many of these clients (46% of those sexually active) had sexual partners who did not inject drugs. There is evidence that this group modified their sexual behaviour towards reducing their own risk of HIV infection, with more having no

Prevalence and incidence of HIV were studied in two cohorts of homosexual men in Amsterdam between 1980 and 1987. The cumulative incidence of HIV infection increased from a weighted 2.2% in 1980 to 39% in 1987. In that year the observed incidence was less than 1%. The estimated annual HIV incidence was 3% in 1981, rose to 8.8% in 1984 and decreased gradually to 0% in 1987. This decline was assumed to be linked to a strong reduction of high risk sexual behaviour among the men under study.

Recent Department of Health and Social Security (DHSS) guidelines on screening procedures for antibody to human T cell lymphotropic virus type III (HTLV-III) have stressed the importance of providing adequate counselling for patients before and after testing. The establishment of a national acquired immune deficiency syndrome (AIDS) counselling training unit has enabled large numbers of National Health Service staff to be trained in this work. Suggestions for the establishment of district counselling facilities are made, based on this training, and some preliminary results showing change from
